1.Efficacy of stented elephant trunk procedure for right-sided aortic arch with Kommerell's diverticulum

Objective To summarize our experience and the early and midterm outcomes of stented elephant trunk procedure for right-sided aortic arch (RAA) with Kommerell's diverticulum (KD). Methods From April 2013 to July 2020, patients with RAA and KD who underwent stented elephant trunk procedure at our center were collected. Surgery was performed under moderate hypothermic circulatory arrest combined with selective antegrade cerebral perfusion via median sternotomy. Results A total of 8 patients were included, including 7 males and 1 female with a mean age of 51.88±9.61 years. All patients had an aneurysmal KD and aberrant left subclavian artery. Preoperative comorbidities included acute Stanford type B aortic dissection in 1 patient, aortic arch pseudoaneurysm in 1 patient, acute type B intramural hematoma in 2 patients, and coronary artery disease in 1 patient. Concomitant procedures included reconstruction of the left subclavian artery in all patients and coronary artery bypass grafting in 1 patient. The mean time of operation, cardiopulmonary bypass, aortic cross-clamping, and selective cerebral perfusion was 6.25±1.16 h, 157.75±40.07 min, 77.75±33.10 min, and 28.50±5.55 min, respectively. No intraoperative death occurred. There was 1 in-hospital death. Follow-up was completed in all patients with a mean period of 3.58±2.08 years. No late death occurred. A persistent anastomotic leak of the proximal arch was detected in 1 patient, but reintervention was not performed because neither aortic dilatation nor symptoms of tracheal and esophageal compression were observed during the follow-up. The remaining 6 patients showed positive aortic remodeling with complete thrombosis of the aneurysmal KD, and neither aortic event nor tracheal and esophageal compression occurred. Conclusion Stented elephant trunk procedure is a safe and feasible technique for selected patients with RAA and KD, which can achieve favorable early and midterm outcomes.
2.Influential factors and interventions of aortic remodeling in residual aortic dissection after surgical repairs

The aortic dissection is a critical emergency in cardiovascular surgery and can be classified as Stanford type A and type B aortic dissection depending on whether the ascending aorta is involved. Currently, the mainstream treatment option for type A aortic dissection is open surgery, while type B dissection can be treated by interventional therapy, open surgery, or optimal medical therapy depending on the situation. However, regardless of the treatment option, it is difficult to resolve all the problems at once. As a result, residual aortic dissection is commonly found in patients with aortic dissection. This article will review the hot issues of aortic remodeling in terms of the definition, prognosis, and risk factors.
3.A randomized controlled clinical study of plica aryepiglottica movement for guiding painless gastroscopic entry

Objective To explore the effectiveness and safety of guiding painless gastroscopic entry with plica aryepiglottica movement as the mark.Methods A total of 200 patients with elective painless gas-troscopic examination from January 2022 to August 2022 were selected,92 males and 108 female,aged 18-60 years old,BMI 18-30 kg/m2 and ASA grade Ⅰ-Ⅲ.The patients were divided into the two groups:the con-trol group (group C) and observation group (group O) by adopting the random number table method,100 ca-ses in each group.The disappearance of eyelash reflex in the group C served as the entry timing of gastrosco-py,while the plica aryepiglottica non-movement under the gastroscopic view in the group O served as the en-try timing of gastroscopy.The bispectral index (BIS) value during gastroscopic operation persistently main-tained 40-60.The main observation indicators were the adverse reactions caused by gastroscopic operation (body movement reactions,bucking,hiccup,laryngeal spasm,etc.),and the secondary observation indicators were:general data of the two groups,SpO2,HR,SBP and DBP values after entering the room (T0),at the en-try time of gastroscopy (T1),3 min after gastroscopic entry (T2),6 min after gastroscopic entry (T3) and at the time of gastroscopic retraction (T4),and the time of medication to gastroscopy,gastroscopic operation time and intraoperative medication supplementation.Results A total of 192 cases were ultimately included,99 cases in the group C and 93 cases in the group O.There was no statistically significant difference in general da-ta between the two groups (P>0.05);compared with the group C,the use amount of cyclophosphamide dur-ing anesthesia induction in the group O was increased (P<0.05).Compared with the group C,the incidence rates of body movement reaction,bucking,hiccup and total adverse reactions in the group O were decreased (P<0.05);SpO2,HR,SBP and DBP at different time points had no statistically significant differences be-tween the two groups (P>0.05).Conclusion Using the glottic movement as the marker to guide the pain-less gastroscopy entry is effective and safe,and could reduce the adverse reactions such as body movement re-action,bucking and hiccup caused by gastroscopic entry.Observing the plica aryepiglottica movement in the gastrocopic entry operation is simple and easy to perform,which could serve an objective gastroscopic entry timing mark and is worth promoting and using in clinic.
4.Impact of the adhesive status of perinephric fat on the selection of dissection routes in retroperitoneal laparoscopic adrenalectomy

Objective To assess the clinical significance of extra-adipose capsule route and intra-adipose capsule route for the resection of benign adrenal tumors with retroperitoneal laparoscopic adrenalectomy(RLA),and to explore the selection of route based on the mayo adhesive probability(MAP)scoring system.Methods Clinical data of 102 patients who received RLA and pathologically diagnosed as benign adrenal tumors during Feb.2015 and Dec.2020 in the First Affiliated Hospital of Xi'an Jiaotong University were retrospectively reviewed,and MAP scores were assessed with the preoperative tomography images.The gross and MAP score stratified perioperative outcomes between extra-adipose capsule route(classical group,n=56)and intra-adipose capsule route(modified group,n=46)were compared respectively.Results All procedures were successfully completed with no conversion to open surgery and with no need for transfusion.There were no significant differences in operation time[(102.1±26.3)min vs.(110.2±32.1)min,P=0.17]and intraoperative blood loss[(53.5±34.0)mL vs.(61.1±48.4)mL,P=0.35]between the two groups.Subgroup analysis based on MAP score showed that for low risk patients(MAP score 0-2),operative results were comparable between the two groups,but for high risk patients(MAP score 3-5),the operation time was significantly shorter[(114.7±20.7)min vs.(137.2±23.0)min,P<0.01],and blood loss was significantly less[(52.7±33.1)mL vs.(92.8±49.7)mL,P=0.01]in the classical group than in the modified group.Conclusion RLA could be performed with either surgical routes safely and effectively.MAP scoring system could be an effective tool for preoperative surgical route planning.Compared with the modified route,classical route is more suitable for patients with high MAP score to achieve better operative outcomes.
5.Long-term outcomes of laparoscopic gastrectomy for locally advanced gastric cancer with serosa-invasion

Objective:To evaluate the long-term outcomes and prognostic factors of locally advanced gastric cancer with serosa-invasion.Methods:This study is a retrospective cohort study. The clinical and pathological data of 495 patients with locally advanced gastric cancer with serosa-invasion who underwent laparoscopic radical gastrectomy in Department of General Surgery, the First Hospital Affiliated to Army Medical University from October 2012 to October 2018 was analyzed retrospectively. There were 356 males and 139 females with an age ( M(IQR)) of 59 (16) years (range: 18 to 75 years). Observation indicators included postoperative results and long-term prognosis. The survival curve was drawn by the Kaplan-Meier method. Univariate and multivariate prognostic analysis was performed using the Cox proportional hazards model. Results:Among the 495 patients, a total of 57 patients (11.5%) were lost to follow-up, with a follow-up time of 89 (40) months (range: 23 to 134 months). The 5-year disease-free survival rate (DFS) and the 5-year overall survival rate (OS) were 56.0% and 58.2%, respectively. The 5-year DFS for patients with stage ⅡB, ⅢA, ⅢB, ⅢC were 71.2%, 60.5%, 51.6%, 33.3%, respectively. The 5-year OS for patients with stage ⅡB, ⅢA, ⅢB, ⅢC were 71.2%, 62.2%, 54.1%, 39.3%, respectively. Multivariate analysis showed that age >65 years (DFS: HR=1.402, 95% CI: 1.022 to 1.922, P=0.036; OS: HR=1.461, 95% CI: 1.057 to 2.019, P=0.022), lymph node dissection number less than 25 (DFS: HR=1.348, 95% CI: 1.019 to 1.779, P=0.036; OS: HR=1.376, 95% CI: 1.035 to 1.825, P=0.028), pathological stage Ⅲ (DFS: HR=2.131, 95% CI: 1.444 to 3.144, P<0.01; OS: HR=2.079, 95% CI: 1.406 to 3.074, P<0.01), and no postoperative chemotherapy (DFS: HR=3.127, 95% CI: 2.377 to 4.113, P<0.01; OS: HR=3.768, 95% CI: 2.828 to 5.020, P<0.01) were independent prognostic factors for the decrease in DFS and OS rates. Conclusions:Laparoscopic radical gastrectomy for locally advanced gastric cancer with serosa-invasion could achieve satisfactory long-term oncological outcomes. More lymph node dissection and standardized postoperative adjuvant chemotherapy are expected to further improve the prognosis of patients with locally advanced gastric cancer with serous invasion after laparoscopic radical surgery.

7.Effects of herbal tea residue supplement on growth performance,serum biochemi-cal and intestinal immune indexes in white feather broilers

This experiment aims to investigate the effects of a dietary supplement of herbal tea resi-due on white feather broilers in the prospectives of growth performance,serum indexes and intesti-nal immune indexes.A total of 280 1-day-old white feather broilers with similar body mass and good health were randomly divided into 4 groups with 5 replicates per group and 14 birds per repli-cate.Group Ⅰ(control group)was fed a basal diet.Groups Ⅱ,Ⅲ and Ⅳ were fed a diet supple-mented with 2%,4%and 6%herbal tea residue powder,respectively.The feeding lasted for 49 days,and was divided into 2 phases from 1 to 21 days of age and 22 to 49 days of age.Blood,tissue and mucosa of the duodenum and jejunum were collected on 21 d and 49 d.Total cholesterol(T-CHO),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C)and the activities of aspartate aminotransferase(AST)and alanine amin-otransferase(ALT)in the serum were examined.The villus height and crypt depth of the jejunum were observed and measured by morphology.The mRNA expression levels of intestinal interleukin-1β(IL-1β),interleukin-2(IL-2),interleukin-4(IL-4),interleukin-10(IL-10)and tumor necrosis factor-α(TNF-α)were determined by qPCR.The results showed that compared with group Ⅰ,the mass of group Ⅲ on 21 d and 49 d significantly increased(P<0.05).The average daily feed intake of 22-49 d and 1-49 d significantly increased(P<0.05).The average daily mass gain of 1-21 d,22-49 d and 1-49 d significantly increased(P<0.05).In the serum of 21 d,compared with group Ⅰ,T-CHO in group Ⅱ significantly decreased(P<0.05).TG in groups Ⅱ,m and Ⅳ significantly de-creased(P<0.05).LDL-C and AST activity in group Ⅳ significantly decreased(P<0.05).In the serum of 49 d,T-CHO in group Ⅲ,LDL-C in group Ⅳ,HDL-C of groups Ⅲ and Ⅳ significantly increased(P<0.05),and the ALT activity of these two groups decreased(P<0.05).AST activity in group Ⅱ significantly decreased(P<0.05).In the duodenum of 21 d,compared with group Ⅰ,the expressions of TNF-α in group Ⅱ was significantly decreased(P<0.05).In the duodenum of 49 d,the expression of IL-10 in group Ⅱ was significantly increased(P<0.05),and the expression of IL-2 in groups Ⅲ and Ⅳ was significantly decreased(P<0.05).In the jejunum of 49 d,the ex-pression of IL-10 in group Ⅱ was increased(P<0.05),and the expression of IL-2 in groups Ⅲand Ⅳ was significantly decreased(P<0.05).In conclusion,dietary herbal tea residue can regulate circulating lipid level and enhance intestinal immunity of white feather broilers without affecting their normal growth performance.This experiment also suggested that the a 2%-4%supplementa-tion of herbal tea residues to white feather broilers was of good effect.
8.Effect of porcine GHRH(1-44)expression plasmid on mouse growth and per-formance of pregnant sows

The study adopted muscle injection of pVAX1-SP-GHRH(1-44)expression plasmid and electrostimulation to determine its effects on mouse growth and sow production performance.One hundred and fifty four-week-old C57 BL/6 female mice were randomly divided into 6 groups of 5 replicates each.Muscle single-injection followed by electrostimulation was performed.The con-trol group received an empty plasmid injection(80 μg/kg),while the treatment groups received pVAX1-SP-GHRH(1-44)plasmid injections(20,40,80,120,160 μg/kg).Twenty healthy preg-nant sows were randomly divided into 2 groups,each with 10 sows.Electrostimulation treatment was applied to the semimembranosus muscle of the pregnant sows after a single injection.The con-trol group received physiological saline injection,while the plasmid group received a 2 mg pVAX1-SP-GHRH(1-44)expression plasmid injection.Mouse weight,feed intake,and serum GHRH and IGF-1 levels were measured at days 0,7,14,21,and 28 after injection.Pregnant sows were bled via the tail vein at days 0,14,28,and 42 after injection,and their serum was separated to measure serum GHRH and IGF-1 levels.The birth weight,placental weight,number of piglets born,number of healthy piglets,number of weak piglets,number of deformed piglets,number of stillborn piglets,and number of mummified piglets were recorded at day 14.The mouse study re-sults showed that muscle injection of pVAX1-SP-GHRH(1-44)plasmid followed by electrostim-ulation could promote mouse feeding and increase weight gain(P<0.05),significantly increase mouse serum GHRH and IGF-1 levels(P<0.05),and maintain its effects until day 21.The results of the pregnant sow study showed that the average birth weight of the piglets in the plasmid group was significantly increased(P<0.01),and the placenta weight was significantly increased(P<0.05).The serum GHRH and IGF-1 concentrations in the plasmid group sows were significantly increased(P<0.01).The study results showed that muscle injection of pVAX1-SP-GHRH(1-44)expression plasmid followed by electrostimulation could promote mouse feeding and increase weight gain,and also significantly improve the average birth weight and placental weight of the piglets in pregnant sows.
9.Application of the Traditional Chinese Medicine Circular Motion Theory in Tinnitus Patients with Sleep Disorders and Anxiety State

[Objective]To summarize Professor XIA Yongliang's clinical experience in treating tinnitus with sleep disorder and anxiety by using the circular motion theory of traditional Chinese medicine.[Methods]Through the way of clinical study with teacher,reviewing the literature of traditional Chinese medicine and collecting and sorting typical medical records,taking one medical case of tinnitus with sleep disorder and anxiety as example to expound Professor XIA's opinion and application experience.[Results]Professor XIA believes that the etiology of tinnitus with sleep disorder and anxiety is clear Yang does not rise,the turbid Yin dose not pour down,leading to the stagnation of five senses organs and nine orifices.As days going on,hyperactivity of liver wood restrains spleen earth,making the condition difficult to cure.Patients often present with symptoms such as irritability and anxiety.Therefore,Professor XIA uses Buzhong Yiqi Decoction to replenish the middle Qi and ascend clear Yang,with the middle axis in operation and the four dimensions attached,leading to fall of minister fire,rise of monarch fire.In addition,ascending and descending Qi,transportation up and down,and assisting Qi mechanism are used to alleviate various symptoms.In the medical case,the patient visited the hospital for the first time.Professor XIA analyzed the symptoms based on differentiation,and the medication was prescribed according to the symptoms,Buzhong Yiqi Decoction was used as the main prescription,which was adjusted according to the symptoms,and the patient's symptoms were relieved.[Conclusion]Professor XIA treats the disease according to the circular motion theory of traditional Chinese medicine,the clinical effect is remarkable.This has important clinical significance in the treatment of this disease with traditional Chinese medicine.
10.Association between Metabolically Healthy Status and Risk of Gastrointestinal Cancer

Purpose:
Although obesity is associated with numerous diseases, the risks of disease may depend on metabolically healthy status. Nevertheless, it is unclear to whether metabolically healthy status affects risk of gastrointestinal (GI) cancer in general Chinese population.
Materials and Methods:
A total of 114,995 participants who met the criteria were included from the Kailuan Study. The study participants were divided into four groups according to body mass index (BMI)/waist circumference (WC) and metabolic status. Incident of GI cancer (esophageal cancer, gastric cancer, liver cancer, biliary cancer, pancreatic cancer, and colorectal cancer) during 2006-2020 were confirmed by review of medical records. The Cox proportional hazard regression models were used to assess the association metabolically healthy status with the risk of GI cancer by calculating the hazard ratios (HR) and 95% confidence interval (CI).
Results:
During a mean 13.76 years of follow-up, we documented 2,311 GI cancers. Multivariate Cox regression analysis showed that compared with the metabolically healthy normal-weight group, metabolically healthy obese (MHO) participants demonstrated an increased risk of developing GI cancer (HR, 1.54; 95% CI, 1.11 to 2.13) by BMI categories. However, such associations were not found for WC category. These associations were moderated by age, sex, and anatomical site of the tumor. Individuals with metabolic unhealthy normal-weight or metabolic unhealthy obesity phenotype also have an increased risk of GI cancer.
Conclusion
MHO phenotype was associated with increased risk of GI cancer. Moreover, individuals who complicated by metabolic unhealthy status have an increased risk of developing GI cancer. Hence, clinicians should consider the risk of incident GI cancer in people with abnormal metabolically healthy status and counsel them about metabolic fitness and weight control.
