2.Precision medicine for advanced biliary tract cancer in China: current status and future perspectives.

Biliary tract cancer (BTC) is a rare group of malignancies that develop from the epithelial lining of the biliary tree and have a poor prognosis. Although chemotherapy is the standard of care for patients with advanced BTC in China, its clinical benefits are moderate. In recent years, the approval of targeted therapies and immunotherapies has provided new avenues for the management of advanced BTC. Nonetheless, the increasing number of personalized medicine approaches has created a challenge for clinicians choosing individualized treatment strategies based on tumor characteristics. In this article, we discuss recent progress in implementing precision medicine approaches for advanced BTC in China and examine genomic profiling studies in Chinese patients with advanced BTC. We also discuss the challenges and opportunities of using precision medicine approaches, as well as the importance of considering population-specific factors and tailoring treatment approaches to improve outcomes for patients with BTC. In addition to providing a comprehensive overview of current and emerging precision medicine approaches for the management of advanced BTC in China, this review article will support clinicians outside of China by serving as a reference regarding the role of patient- and population-specific factors in clinical decision-making for patients with this rare malignancy.

The superiority of magnetic resonance(MR)images in soft tissue imaging makes them indispensable for medical diagnosis and radiotherapy,but factors such as acquisition cost and contraindications limit their widespread application.In contrast,computed tomography(CT)scanning has the advantages of fast imaging speed and low cost.Herein,this review summarizes the research progress of generative deep learning models in the field of medical CT to MR image synthesis,and especially analyzes the technical characteristics,performance advantages,and challenges of various MR image synthesis methods from clinical scenarios such as spinal lesions,acute ischemic stroke,and tumor segmentation.Furthermore,the application value and future research prospects of medical image synthesis are discussed.

BACKGROUND:Antioxidants can alleviate cellular damage and matrix degradation caused by oxidative stress,protect the structure and function of intervertebral discs,and thus delay the occurrence and development of intervertebral disc degeneration.OBJECTIVE:To review the research status of antioxidants in the treatment of intervertebral disc degeneration.METHODS:The first author searched the articles included in CNKI and PubMed databases.The literature search time was from inception to June 2024.Chinese search terms were"intervertebral disc degeneration,antioxidants,oxidative stress,signaling pathways."English search terms were"antioxidants,oxidative stresses,oxidative DNA damage,intervertebral disc degenerations,degenerative disc disease*,disc degeneration*,signal pathway."Finally,92 articles that met the criteria were selected for review.RESULTS AND CONCLUSION:(1)Antioxidants have multiple roles in the treatment of intervertebral disc degeneration,including reducing oxidative stress,inhibiting inflammatory responses,promoting autophagy,inhibiting apoptosis,and protecting the extracellular matrix.Through the combined effect of multiple pathways,antioxidants are expected to become an important means in the treatment of intervertebral disc degeneration.(2)The nano-hydrogel system can quickly and stably target the delivery of antioxidants to the inside of the intervertebral disc and improve the bioavailability of antioxidants.Therefore,the development of new antioxidants and antioxidant combination treatment strategies carried by nano-hydrogel systems will be the focus of future research.

7.Analysis of factors influencing the efficacy and prognosis of surgical treatment for primary malignant pelvic bone tumors

Objective:To analyze the prognostic factors and the influence of surgical margin to prognosis.Methods:A retrospective analysis was performed for 208 pelvic tumors who received surgical treatment from January 2000 to December 2017 in our instituition. Survival analysis was performed using the Kaplan-Meier method and Log rank test, and impact factor analysis was performed using Cox regression models.Results:There were 183 initial patients and 25 recurrent cases. According to Enneking staging, 110 cases were stage ⅠB and 98 cases were stage ⅡB. 19 lesions were in zone Ⅰ, 1 in zone Ⅱ, 15 in zone Ⅲ, 29 in zone Ⅰ+Ⅱ, 71 in zone Ⅱ+Ⅲ, 29 in zone Ⅰ+Ⅳ, 35 in zone Ⅰ+Ⅱ+Ⅲ, 3 in zone Ⅰ+Ⅱ+Ⅳ, and 6 in zone Ⅰ+Ⅱ+Ⅲ+Ⅳ. Surgical margins including Intralesional excision in 7 cases, contaminated margin in 21 cases, marginal resection in 67 cases, and wide resection in 113 cases. Local recurrence occurred in 37 cases (17.8%), 25 cases were performed by reoperation and 12 cases received amputation finally. The 5-year recurrence rate of marginal resection was higher than wide resection ( P<0.05), and the recurrence-free survival rate of marginal resection was lower than wide resection ( P<0.05). There was significant differences in recurrence rate and recurrence-free survival rate between R0 and R1 resection ( P<0.05). 92 cases were not reconstructed and 116 cases were reconstructed after pelvic surgery. At the last follow-up, 63 patients (30.3%) died, and the 5-year, 10-year and 15-year survival rates were 70.4%, 66.8% and 61.3%, respectively. The 5-year survival rate of stage ⅠB and ⅡB tumor was 90.4% and 46.8%, respectively. There were 29 cases had postoperative wound complications (13.8%), 1 case with pelvic organ injury. The final function was evaluated in 132 patients, with an average MSTS score of 25.1±3.6. Cox multivariate analysis showed that surgical staging, R0/R1 margin and metastasis were independent prognostic factors for pelvic tumors. Conclusions:The safe surgical margin is the key factor for recurrence-free of pelvic tumor. The survival rate of stage ⅡB pelvic tumors was significantly lower than that of stage ⅠB tumors. Wound infection is the main postoperative complication. Surgical staging, R0/R1 margin and metastasis were independent prognostic factors of pelvic tumors.
8.Analysis of Vitamin D in Children with Henoch-Schonlein Purpura

Objective To analyze the changes of vitamin D in children with Henoch-Schonlein purpura(HSP).Methods 130 children with HSP from Kunming Children's Hospital between July 2022-July 2023 were selected as the study subjects and 100 healthy children were selected during the same period as the control group.The blood samples were collected from the children with HSP and the healthy children.The content of vitamin D was measured by Kunming Kingmed Institute for Clinical Laboratory.Results The content of 25(OH)D in children with HSP was lower than that in healthy children,and the difference was statistically significant(P<0.01).The proportion of vitamin D insufficiency in children with HSP was higher than that in healthy children,and the difference was statistically significant(P<0.01).Conclusion The children with HSP are prone to vitamin D insufficiency.Vitamin D supplementation may provide a new method for the treatment of HSP.

Following the principles of evidence-based medicine,in accordance with the structure and drafting rules of standardized documents,based on literature research,according to the characteristics of chronic cough in children and issues that need to form a consensus,the TCM Guidelines for Diagnosis and Treatment of Chronic Cough in Children was formulated based on the Delphi method,expert discussion meetings,and public solicitation of opinions.The guideline includes scope of application,terms and definitions,eti-ology and diagnosis,auxiliary examination,treatment,prevention and care.The aim is to clarify the optimal treatment plan of Chinese medicine in the diagnosis and treatment of this disease,and to provide guidance for improving the clinical diagnosis and treatment of chronic cough in children with Chinese medicine.
