Helicobacter pylori(H.pylori)infection is the most significant controllable risk factor for gastric cancer,and its eradication is a critical component of gastric cancer prevention in China.A family-based screening and treatment strategy for H.pylori aligns well with China's public health needs.It demonstrates higher eradication success rate,lower recurrence rate,and superior cost-effectiveness,making it suitable for both high-and low-prevalence regions.Implementation of this family-based strategy can lead to greater precision,higher efficiency,and overall coverage in gastric cancer prevention in China,offering a scalable"China model"for global gastric cancer control.

Objective:To investigate the value of multi-parametric analysis based on dual-layer detector spectral CT (DLCT) in predicting the initial recurrence risk for papillary thyroid carcinoma (PTC).Methods:From November 2021 to October 2022, 102 PTC patients confirmed by pathology were retrospectively collected at the First Affiliated Hospital of Nanjing Medical University in this cross-sectional study. There were 25 males and 77 females, with an age of (42±13) years old. The initial recurrence risk assessment for PTC patients was categorized into a low-risk group (75 cases) and an intermediate-high-risk group (27 cases). Clinical data, including age, gender, body mass index, history of nodular goiter, history of Hashimoto thyroiditis, and preoperative thyroid function, were collected. Tumor morphological features, including size, location, shape, aspect ratio, the degree of thyroid capsule contact, calcification, and cystic change, were evaluated. Quantitative DLCT parameters, including iodine concentration (IC), standardized iodine concentration (NIC), effective atomic number (Z eff), standardized effective atomic number (NZ eff), electronic density (ED), CT values under different energy levels (40-200 keV, 30 keV intervals) and slope of energy spectrum curve (λ HU) both in the arterial and venous phase were measured. The differences in clinical, morphological features, and spectral CT quantitative parameters between the two groups were compared using independent sample ttest, Mann-Whitney U test, or χ2 test. Multivariate logistic regression analyses were used to construct three models based on clinical and morphological features, quantitative DLCT parameters and their combination, respectively. The receiver operating characteristic curve was used to evaluate the predictive performance of these models for the initial recurrence risk of PTC patients, and the area under the curve (AUC) was compared using the DeLong test. Results:Significant differences were found in gender, lesion long diameter, lesion short diameter and calcification between the low-risk group and intermediate-high-risk groups ( P<0.05). The arterial phase IC, arterial phase Z eff, arterial phase λ HU, arterial phase CT 40 keV, venous phase NIC and venous phase NZ eff in intermediate-high-risk group were significantly lower than those in the low-risk group ( P<0.05). The logistic regression analysis revealed that the clinical model included gender ( OR=2.895, 95% CI 1.047-8.002, P=0.040) and lesion long diameter ( OR=1.142, 95% CI 1.042-1.251, P=0.004), with an AUC of 0.720, sensitivity of 63.0%, and specificity of 78.7% in predicting the initial recurrence risk of PTC patients. The DLCT quantitative parameter model included arterial phase IC ( OR=0.580, 95% CI 0.370-0.908, P=0.017), venous phase NIC ( OR=0.077, 95% CI 0.011-0.536, P=0.010), and venous phase NZ eff ( OR=0.002, 95% CI 0.001-0.103, P=0.009), with an AUC of 0.774, sensitivity of 71.9%, and specificity of 70.0%. The AUC of the combined model was 0.857, with a sensitivity of 74.1%, and specificity of 88.0%, outperforming the clinical model ( Z=2.92, P=0.004) and the DLCT quantitative parameter model ( Z=2.07, P=0.046). Conclusion:Multi-parametric analysis based on DLCT can help predict the initial recurrence risk for PTC, and combining it with clinical and morphological features, the predictive accuracy can be improved.

Objective:To explore the clinical features and genetic etiology of a patient with adult-onset globoid cell leukodystrophy/Krabbe disease (KD).Methods:A patient who was admitted to the Tongji Hospital Affiliated to Tongji Medical College, Huazhong University of Science and Technology on February 15, 2022 due to exacerbation of right leg weakness for over 4 years was selected as the study subject. Clinical data and results of medical imaging and genetic analysis were analyzed. Candidate variants were verified by family analysis.Results:The patient, a 36-year-old woman, had spasmodic gait as the primary presentation. Cranial magnetic resonance imaging (MRI) revealed symmetrical abnormalities in the bilateral corticospinal tracts, and the activity of β-galactocerebrosidase (GALC) in her white blood cells was significantly decreased. The patient was found to harbor compound heterozygous variants of the GALC gene, namely c.461C>A (p.Pro154His) and c. 1901T>C (p.Leu634Ser). Her mother, sister and nephew were heterozygous carriers of the c. 461C>A (p.Pro154His) variant, whilst her father was heterozygous for the c.1901T>C (p.Leu634Ser) variant. Conclusion:The patient was ultimately diagnosed with adult-onset KD, for which the compound heterozygous variants of the GALC gene may be accountable.

The O-RADS US and vascular index from MV-Flow (VIMV) were measured and compared with the reference standard. O-RADS US and MV-Flow were tested for consistency. Results: Receiver operating characteristic curves were drawn for O-RADS US, MV-Flow, and their combination. The combined methods had the largest area under the curve (0.955), followed by O-RADS US (0.929) and MV-Flow (0.923). A mass was considered malignant when the O-RADS US classification was 5 and VIMV was ≥7.15. With this definition, MV-Flow had the highest sensitivity (87.10%), with consistent findings for the combined diagnostic methods and O-RADS US (83.87%). The specificity of the combined diagnostic methods (93.83%) was higher than that of MV-Flow (91.36%). O-RADS US had the lowest specificity (90.12%). The combined diagnostic methods had the highest coincidence rate (91.07%), and MV-Flow (90.18%) had a significantly higher coincidence rate than O-RADS US (88.39%). Both O-RADS US and MV-Flow showed good consistency among different physicians (former kappa, 0.974; latter intraclass correlation coefficient [ICC], 0.986). MV-Flow had a high consistency for the same physician (ICC, 1). Conclusion O-RADS US and MV-Flow exhibited good diagnostic efficacy, and their combined diagnostic efficacy was higher than that of each individually. O-RADS US and MV-Flow can improve the diagnosis of benign and malignant ovarian-adnexal masses.

Objective To analyze the basic epidemiological characteristics, drug resistance profiles, molecular characterization features, and phylogenetic relationships of mcr-1 (polymyxin resistance gene, mediated colistin resistance)-positive Salmonella typhimurium collected by Guangzhou Center for Disease Control and Prevention (GZCDC) from 2013 to 2020. Methods Strain identification was performed using a VITEK 2 Compact fully automated microbial biochemical identifier. The mcr-1-positive strains were screened using PCR and agarose gel electrophoresis. The mcr-1-positive strains were subjected to drug sensitivity tests and molecular typing experiments by micro broth dilution and pulse-field gel electrophoresis (PFGE). Whole genome sequencing and phylogenetic analysis were performed on the mcr-1-positive strains. Results From 2013 to 2020, 64 mcr-1-positive strains were screened out of 1 783 Salmonella typhimurium strains, and the mcr-1 positivity rate ranged from 0.37% to 6.84%. The drug sensitivity results showed that all 64 mcr-1 positive strains were resistant to polymyxin and exhibited resistance to ≥3 classes of antibiotics, with 90.63% (58/64) of strains resistant to ≥7 classes. The PFGE results indicated that the 64 positive strains could be grouped into 5 clusters with approximately 70% homology. The phylogenetic tree further suggested the diversity and complexity of transmission pathways of mcr-1-positive Salmonella typhimurium from different sources. Conclusions There has been a high detection rate of mcr-1-positive Salmonella typhimurium in Guangzhou City in recent years, along with a diverse drug resistance spectrum and high genetic diversity among different strains. Continued monitoring and tracing of drug resistance should be strengthened.

Smartphones have made great strides in neonatology.With the advantages of portability, low cost, ease of operation, and powerful application processing, smartphones, when combined with appropriate medical applications, enable rapid and accurate monitoring of neonatal diseases and are important for the early prevention and treatment of various neonatal conditions.In this review, we discussed the application of various smartphone-based assistive technologies in the neonatal field.
