1.Evaluation of Effect of Tongnaoyin on Blood-brain Barrier Injury in Acute Ischemic Stroke Patients Based on Dynamic Contrast-enhanced Magnetic Resonance Imaging

ObjectiveTo evaluate the effects of Tongnaoyin on the blood-brain barrier status and neurological impairment in acute ischemic stroke (AIS) patients with the syndrome of phlegm-stasis blocking collaterals by dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI). MethodsA total of 63 patients diagnosed with AIS in the Jiangsu Province Hospital of Chinese Medicine from October 2022 to December 2023 were enrolled in this study. According to random number table method,the patients were assigned into a control group (32 cases) and an observation group (31 cases). The control group received conventional Western medical treatment,and the observation group took 200 mL Tongnaoyin after meals,twice a day from day 2 of admission on the basis of the treatment in the control group. After 7 days of treatment,the patients were examined by DCE-MRI. The baseline data for two groups of patients before treatment were compared. The National Institute of Health Stroke Scale (NIHSS) score and modified Rankin Scale (mRS) score were recorded before treatment and after 90 days of treatment for both groups. The rKtrans,rKep,and rVe values were obtained from the region of interest (ROI) of the infarct zone/mirror area and compared between the two groups. ResultsThere was no significant difference in the NIHSS or mRS score between the two groups before treatment. After 90 days of treatment,the NIHSS and mRS scores declined in both groups,and the observation group had lower scores than the control group (P<0.05). After treatment,the rKtrans and rVe in the observation group were lower than those in the control group (P<0.01). ConclusionCompared with conventional Western medical treatment alone,conventional Western medical treatment combined with Tongnaoyin accelerates the repair of the blood-brain barrier in AIS patients,thereby ameliorating neurological impairment after AIS to improve the prognosis.
2.Effect of Tongnaoyin on Cerebral Hemodynamics in Patients with Acute Cerebral Infarction of Phlegm and Blood Stasis Syndrome Based on CTA/CTP

ObjectiveTo investigate the changes in cerebral blood perfusion in patients with acute cerebral infarction after taking Tongnaoyin, a traditional Chinese medicine, based on head and neck computed tomography (CT) angiography (CTA) combined with brain CT perfusion imaging (CTP). MethodA total of 240 patients with cerebral infarction of phlegm and blood stasis syndrome treated in Jiangsu Province Hospital of Traditional Chinese Medicine from March 2018 to September 2023 were randomly divided into a control group (99 cases) and a Tongnaoyin group (141 cases). Based on the guidelines, the control group was treated with conventional treatment such as anti-aggregation, anticoagulation, lipid-lowering and plaque stabilization, brain protection, and supportive treatment. The Tongnaoyin group was treated with Tongnaoyin of 200 mL in warm conditions in the morning and evening on the basis of the control group. Both groups underwent CTA combined with CTP within 24 hours after admission, and they were reexamined by CTA and CTP in the sixth month after admission. The degree of intracranial artery stenosis was determined according to the North American Symptomatic Carotid Endarterectomy Trial (NASCET) method. The relative cerebral blood volume (rCBV), relative cerebral blood flow (rCBF), mean transit time (MTT), and time to peak (TTP) of the lesion area before and after treatment were compared. The adverse outcomes of the two groups within six months after discharge were compared. ResultCompared with the group before treatment, the degree of vascular stenosis in the Tongnaoyin group was significantly reduced, and the difference was statistically significant (Z=105.369,P<0.05). Compared with the control group after treatment, the improvement rate of vascular stenosis in the Tongnaoyin group was higher (χ2=84.179,P<0.01), and the curative effect was better.After treatment, the rCBV and rCBF of patients in the Tongnaoyin group were significantly increased, and the difference was statistically significant (P<0.01). MTT and TTP showed a trend of shortening, but the difference was not statistically significant. There was no statistically significant difference in rCBV, rCBF, MTT, and TTP in the control group. Compared with those in the control group after treatment, the rCBV and rCBF in the Tongnaoyin group were significantly increased, while MTT and TTP were significantly reduced (P<0.01). After six months of discharge, the risk of poor prognosis in the Tongnaoyin group was significantly reduced compared with the control group (P<0.05). ConclusionTongnaoyin has a good effect on improving cerebral blood perfusion in patients with acute cerebral infarction. It can be used as an effective supplement for the conventional treatment of ischemic stroke to improve clinical efficacy.

This article summarizes the unique viewpoints and experience application of the famous and veteran Chinese medicine practitioner,Professor Wang Xinzhi,in treating emotional diseases from the perspective of gallbladder theory.Based on the physiologi-cal functions and characteristics of the gallbladder in Chinese medicine,it is proposed that the"heart mind-gallbladder-viscera"axis dominates the generation and changes of emotions,and it is believed that gallbladder failure is the key pathogenesis of emotional disor-ders.The treatment of clinical syndromes should be based on the type of gallbladder,and emotional diseases can be divided into types of insufficient gallbladder qi,unfavorable Shaoyang,gallbladder and heat excess,timidity-deficiency,and heart-gallbladder indeci-sion,according to clinical manifestations;based on the basic principle of adjusting the functions of the heart,spleen,liver,gallblad-der,kidney and other organs,treatment methods such as tonifying the spleen and kidneys,increasing gallbladder qi,resolving Shaoy-ang,clearing gallbladder heat,warming yang and replenishing qi,calming the mind,resolving phlegm and removing blood stasis should be used,highlighting the joint treatment of the heart and the gallbladder,and the simultaneous regulation of the liver and gall-bladder,so that the mind can be at ease,the gallbladder can be decisive,and the emotions can be harmonious.
4.Research progress in cognition, attitude and willingness for organ donation among ICU staff in China

In recent years, organ donation and transplantation have entered a stage of steady development in China. Nevertheless, the shortage of transplant organs and the contradiction between supply and demand of organs are still the bottlenecks to achieve the strategy of "self-sufficiency in organ transplantation" advocated by the World Health Organization (WHO). The key reasons for donor loss described in the "critical pathway of organ donation" defined by the WHO include the identification and referral of potential donors and the maintenance and repair of organs. Smooth development, high efficiency and high-quality development of organ donation cannot be achieved without the support of intensive care medicine, which are highly associated with the cognition, recognition and participation of intensive care unit(ICU) staff. In this article, research progress in ICU staff’s cognition, attitude and willingness for organ donation were reviewed and relevant influencing factors were discussed, aiming to offer targeted suggestions on how to resolve these difficulties.
5.Application of distraction lengthening for correcting the short nail deformity in unilateral congenital brachydactyly type D

Objective:To describe the effect of distraction lengthening to correct nail deformity in unilateral congenital brachydactyly type D (BDD).Methods:Patients with unilateral BDD who were treated by distraction lengthening between January 2020 and December 2021 in the Department of Plastic and Reconstructive Surgery of Shanghai 9th People’s Hospital were retrospectively reviewed. A transverse osteotomy was applied on the middle diaphysis of distal phalanx, and one pin was inserted percutaneously on both sides of the osteotomy site, then a mini external fixator was installed. Distraction lengthening was performed with a slow and sustained procedure. The aesthetic and functional results were compared preoperatively and postoperatively. Patient satisfaction questionnaires were recorded.Results:35 patients were enrolled, including 12 males and 23 females, with a mean age of 29.5 years. 17 thumbs were left and 18 thumbs were right. The time for distraction lengthening was 10-17 days, and the mean time was 12.3 days. The total time for external fixator maintenance was 55-70 days, with a mean time of 61.4 days. The mean follow-up was 18.6 months. The mean thumbnail length was increased from (9.2±0.6) mm to (14.2±0.4) mm [lengthening ratio, (54.4±7.5)%] after distraction lengthening ( P<0.05). The ratio of nail length to width changed from 0.63±0.05 to 1.05±0.04 [increase ratio, (68.2±10.5)%, P<0.05], which was equal to the normal value (1.07±0.03) ( P>0.05). Meanwhile, the distal thumb length increased from (22.6±0.7) mm to (29.6±1.0) mm ( P<0.05), with a mean increase of (6.9±1.0) mm, and the increase ratio was (30.8±4.8)%. The ratio of nail length to distal thumb length changed from 0.41±0.03 to 0.51±0.02 [increase ratio, (23.6±8.3) %, P<0.05]. The thumb function was not significantly influenced ( P>0.05). 33 patients (94.3%) were very satisfied with the results. Conclusion:Short nail deformity in BDD can be corrected by distraction lengthening with a good aesthetic appearance and without functional impairment.

7.A novel mutation in the ABCD1 gene causes adrenomyeloneuropathy by impairing autophagy level

Objective:To report a Chinese family with a novel ABCD1 gene mutation at c.332T>G (p.V111G) site and discuss its clinical characteristics and molecular mechanism.Methods:The clinical data, laboratory examination, and imaging examination results were analyzed to make the clinical diagnosis of a middle-aged onset patient from the First Affiliated Hospital of Zhengzhou University in May 2017. High-throughput sequencing was used to discover a novel ABCD1 gene mutation. Sanger sequencing was used to find out whether other family members contain the same ABCD1 gene mutation. The pathogenicity of this mutation was explored by protein structure prediction and pathogenicity analysis. Adrenoleukodystrophy protein-green fluorescent protein (ALDP-GFP) and ALDP-GFP (V111G) plasmids were constructed and human embryonic kidney 293 cells were transfected, then immunofluorescence and Western blotting were used to explore the molecular mechanism of this mutation (completed in Henan Provincial People′s Hospital).Results:The proband (a 39-year-old male) was diagnosed as adrenomyeloneuropathy, a subset of X-linked adrenoleukodystrophy, with a novel heterozygous missense mutation in the ABCD1 gene at c.332T>G (p.V111G) site, and his mother and two daughters were all carriers. Protein structure prediction and pathogenicity results suggested that this mutation is pathogenic. Overexpression of ALDP-GFP (V111G) in the human embryonic kidney 293 cells resulted in a significant decrease in the expression levels of ALDP and the abnormal localization from the peroxisomal membrane to the cytoplasm, accompanied by significant down-regulation of LC3-Ⅱ/LC3-Ⅰ and beclin-1.Conclusion:c.332T>G (p.V111G) is a novel pathogenic mutation in the ABCD1 gene, which causes adrenomyeloneuropathy by impairing autophagy.
8.Overview of global organ donation and transplantation in 2020

Objective At present, the novel coronavirus pneumonia (COVID-19) pandemic is still raging in certain regions around the globe, and the prevention and control of the pandemic should be strengthened. Under the challenges of respective social environment and allocation of medical resources, and support from the inertia and inherent productivity of the system on which the industry depends, extensive attempts are being delivered to push forward the work of organ donation and transplantation in each country. Under the guidance of national experts and committee members, Shanxi Provincial Human Organ Procurement and Allocation Service Center was established on August 28, 2018 approved by the former Shanxi Provincial Health and Family Planning Commission. It is the only independent non-profit medical institution in Shanxi Province. In this article, the system construction of citizen's organ donation and transplantation fitting national and provincial conditions was further explored according to the data analysis of organ donation and transplantation in the United States and Spain during the COVID-19 pandemic combined with the implementation of organ donation work in Shanxi Provincial Human Organ Procurement and Allocation Service Center.
9.Visible light emitted quantum dots targeting EGFR in triple-negative breast cancer

Objective:To explore the feasibility and conditions of in vitro and in vivo imaging of triple-negative breast cancer using visible light emitted quantum dots(QDs) as the carrier to target epidermal growth factor receptor (EGFR). Methods:The water-soluble QDs reacted with Cetuximab to synthesize the probe QD-Cetuximab. The morphology, particle size, stability and luminescence properties of the probe were examined. Human breast cancer cells MDA-MB-468 (EGFR+ ) and MDA-MB-453 (EGFR-) were cultured. Cytotoxicity assays, in vitro imaging and fluorescence intensity quantification were performed after cells incubation with QD-Cetuximab and QDs. Eight MDA-MB-468 tumor-bearing mice models were constructed, 100 μl QD-Cetuximab and QDs were injected through the tail vein. In vivo imaging and probe distribution were obtained at different time points. Independent-sample t test was used to analyze the data. Results:QD-Cetuximab had a particle size of (40.34±2.44) nm detected by transmission electron microscope (TEM), a hydrated particle size of (57.85±4.69) nm detected by dynamic light scattering (DLS), and a stable structure. When the concentration of QD-Cetuximab was ≤50 nmol/L, the relative survival rate of cells was more than 90%, and when the concentration exceeded 100 nmol/L, the relative survival rate of cells was reduced to (72.52±4.91)% ( P<0.05). The red fluorescence of MDA-MB-468 incubated with QD-Cetuximab was stronger than that of MDA-MB-468 incubated with QDs and MDA-MB-453 incubated with QD-Cetuximab or QDs. The confocal fluorescent intensity quantitative determination showed that the ratio of QD-Cetuximab group/QDs group was 5.1 (863.36/169.97). Flow cytometry showed that the uptake of QD-Cetuximab and QDs by MDA-MB-468 increased with incremental incubating concentration, and the former was more significantly( t values: 12.25-38.11, all P<0.05). When the incubating concentration was 25, 50, 100, and 200 nmol/L, the quantitative average fluorescent intensity ratio of QD-Cetuximab group/QDs group was 5.4, 6.9, 7.4 and 6.2, respectively. The QD-Cetuximab and QDs probes mainly accumulated in the liver in vivo. The fluorescence emitted by tumor was not obvious under the high fluorescence of liver as a background. However, the fluorescence was visible in the isolated tumor tissue, and the quantitative fluorescence intensity of experimental group and control group were (2.46±0.60)×10 4 and (1.29±0.05)×10 4, respectively ( t=3.392, P=0.015). Conclusions:Cetuximab can increase the targeting ability of QDs and promote cell uptake. Although the isolated tumor imaging results are acceptable, further modification of QDs should be considered to reduce the liver uptake and improving in vivo fluorescence imaging efficiency.
10.Rivaroxaban and interventional therapy for acute pulmonary embolism

Objective To explore the safety and efficacy of rivaroxaban after interventional treatment of pulmonary embolism.Methods Patients with acute pulmonary embolism undergoing pulmonary indwelling catheter thrombolysis at Chifeng Municipal Hospital from Jun 2016 to Jun 2017 were divided into a group of 23 patients,who afterwards receiving rivaroxaban as a long-term anticoagulant,and that of 45 patients treated with traditional anticoagulant therapy (low molecular weight heparin bridged warfarin).Results The short and mid-term follow-up for the cure rate in the rivaroxaban group was higher than that in the traditional anticoagulation group (60.9% vs.35.6%,P <0.05,and 73.9% vs.44.4%,P <0.05).The incidence of adverse reactions such as bleeding (4.3%) was significantly lower than that of the heparin warfarin group (28.9%),there was no recurrence of venous thromboembolism (VTE) in the rivaroxaban group,and 3 cases in the traditional anticoagulant group;there was no death in either group.Conclusions In the interventional treatment of acute pulmonary embolism,single-agent oral rivaroxaban is equivalent to low-molecular-weight heparin-bridged warfarin;rivaroxaban can effectively reduce pulmonary embolism and the occurrence of bleeding complications after endovascular intervention.
