1.Evaluation of Effect of Tongnaoyin on Blood-brain Barrier Injury in Acute Ischemic Stroke Patients Based on Dynamic Contrast-enhanced Magnetic Resonance Imaging

ObjectiveTo evaluate the effects of Tongnaoyin on the blood-brain barrier status and neurological impairment in acute ischemic stroke (AIS) patients with the syndrome of phlegm-stasis blocking collaterals by dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI). MethodsA total of 63 patients diagnosed with AIS in the Jiangsu Province Hospital of Chinese Medicine from October 2022 to December 2023 were enrolled in this study. According to random number table method,the patients were assigned into a control group (32 cases) and an observation group (31 cases). The control group received conventional Western medical treatment,and the observation group took 200 mL Tongnaoyin after meals,twice a day from day 2 of admission on the basis of the treatment in the control group. After 7 days of treatment,the patients were examined by DCE-MRI. The baseline data for two groups of patients before treatment were compared. The National Institute of Health Stroke Scale (NIHSS) score and modified Rankin Scale (mRS) score were recorded before treatment and after 90 days of treatment for both groups. The rKtrans,rKep,and rVe values were obtained from the region of interest (ROI) of the infarct zone/mirror area and compared between the two groups. ResultsThere was no significant difference in the NIHSS or mRS score between the two groups before treatment. After 90 days of treatment,the NIHSS and mRS scores declined in both groups,and the observation group had lower scores than the control group (P<0.05). After treatment,the rKtrans and rVe in the observation group were lower than those in the control group (P<0.01). ConclusionCompared with conventional Western medical treatment alone,conventional Western medical treatment combined with Tongnaoyin accelerates the repair of the blood-brain barrier in AIS patients,thereby ameliorating neurological impairment after AIS to improve the prognosis.
2.Multiple neurofibromatosis type 1 in the right maxillofacial region: a case report and literature review

Objective:
To explore the clinical characteristics and diagnosis and treatment plans of neurofibromatosis type 1 (NF1), and to provide references for clinical diagnosis and treatment.
Methods :
The clinical manifestations and treatment of an 8-year-old female patient with NF1 was reported. A literature review was conducted to summarize the clinical characteristics and therapeutic strategies of NF1. Multiple NF1s occurred on the right cheek, orbit, and eyelid, and recurred after surgical resection. The tumor caused ptosis, incomplete closure, and vision loss in the upper eyelid of the right eye. After a multidisciplinary assessment determined that radical resection was not feasible, selumetinib sulfate targeted therapy was adopted (25 mg, Po, bid), 28 days constitute one treatment course, and 14 courses have been completed, combined with symptomatic ocular treatments, such as Befusu.
Result:
The follow-up showed that the tumor volume did not continue to increase (stable disease), the uncorrected vision of the right eye improved (0.05 vs 0.1), and no drug-related adverse reactions occurred during the treatment period. The literature review summarizes the diverse clinical manifestations of NF1, with café-au-lait macules, multiple neurofibromas, and Lisch nodules being hallmark features. Currently, surgical intervention remains the most commonly employed and primary therapeutic approach for NF1; however, for patients who do not meet the criteria for surgery, alternative treatment strategies should be considered. MEK inhibitors, such as selumetinib, demonstrate significant efficacy in inhibiting the growth of NF1-associated plexiform neurofibromas, with tumor volume reductions of at least 20% observed in 70% of pediatric patients in the SPRINT clinical trial. Furthermore, these inhibitors exhibit favorable long-term safety profiles.
Conclusion
Café-au-lait macules, multiple neurofibromas, and Lisch nodules are hallmark features of NF1. Selumetinib is safe and effective for NF1 in the head and neck of children, and it is the preferred treatment option for patients who are not suitable for surgery. Long-term follow-up monitoring of tumor changes and drug safety is required.
3.Classical Sweet syndrome with multiple organ lesions by 18F-FDG PET/CT: A case report.

Sweet syndrome (acute febrile neutrophilic dermatosis) is a relatively rare inflammatory di-sease, which is characterized by the sudden appearance of tender erythematous skin lesions, often accompanied by pyrexia and elevated neutrophil count. The pathogenesis is not clear yet. Recently, multiple studies have found the association between Sweet syndrome and infections, autoimmune diseases, malignant tumors and the application of multiple drugs. According to different causes, Sweet syndrome can be divided into three types: classical (or idiopathic) Sweet syndrome, malignancy-associated Sweet syndrome and drug-induced Sweet syndrome. Classical Sweet syndrome usually presents in women between the age of 30 to 50 years and may be related to infection, inflammatory bowel disease, or pregnancy. The clinical symptoms typically respond promptly after corticosteroid therapy. The major diagnostic criteria of classical Sweet syndrome include sudden painful erythematous skin lesions, histopathologic evidence of a dense neutrophilic infiltrate without evidence of leukocytoclastic vasculitis; minor criteria include pyrexia over 38 ℃, association with hematologic or visceral malignancies, inflammatory diseases, pregnancy or preceded by infection, prompt response to systemic glucocorticoid or potassium iodide treatment, abnormal laboratory values (three of four: erythrocyte sedimentation rate >20 mm/h, positive C-reactive protein, >8.0×109/L leukocytes, >70% neutrophils). The presence of both major criteria and two of the four minor criteria are required to diagnose classical Sweet syndrome. As for the malignancy-associated Sweet syndrome, skin lesions can be found precede, follow, or at the same time with the diagnosis of hematologic malignancy or a solid tumor. At present, 18F-fluorodeoxyglucose (18F-FDG) is commonly used as a positron emission tomography computed tomography (PET/CT) imaging agent for diagnosing and screening malignant tumors. Therefore, most of the case reports on the 18F-FDG PET/CT manifestations of Sweet syndrome are malignancy-associated. Even classical Sweet syndrome is often accompanied by inflammatory bowel disease, autoimmune diseases, etc. Therefore, for patients with suspected or confirmed Sweet syndrome, it is necessary to take 18F-FDG PET/CT examination to clarify the general condition, whether it is for patients with malignant signs such as elevated tumor markers values and weight loss, or for patients with classical Sweet syndrome to exclude underlying inflammatory diseases. 18F-FDG PET/CT is often able to detect the solid tumor early, and assess the degree of hematologic malignancy and inflammatory disease. This study reported a classical Sweet syndrome case associated with inflammatory bowel disease, which was confirmed with skin and intestinal histological examination. The clinical manifestations, laboratory values, 18F-FDG PET/CT manifestations of the patient related diseases were reported, which was to improve nuclear medicine physicians' understanding of Sweet syndrome. Early diagnosis and treatment can often achieve excellent clinical effect.

4.YOD1 regulates microglial homeostasis by deubiquitinating MYH9 to promote the pathogenesis of Alzheimer's disease.

Alzheimer's disease (AD) is the major form of dementia in the elderly and is closely related to the toxic effects of microglia sustained activation. In AD, sustained microglial activation triggers impaired synaptic pruning, neuroinflammation, neurotoxicity, and cognitive deficits. Accumulating evidence has demonstrated that aberrant expression of deubiquitinating enzymes is associated with regulating microglia function. Here, we use RNA sequencing to identify a deubiquitinase YOD1 as a regulator of microglial function and AD pathology. Further study showed that YOD1 knockout significantly improved the migration, phagocytosis, and inflammatory response of microglia, thereby improving the cognitive impairment of AD model mice. Through LC-MS/MS analysis combined with Co-IP, we found that Myosin heavy chain 9 (MYH9), a key regulator maintaining microglia homeostasis, is an interacting protein of YOD1. Mechanistically, YOD1 binds to MYH9 and maintains its stability by removing the K48 ubiquitin chain from MYH9, thereby mediating the microglia polarization signaling pathway to mediate microglia homeostasis. Taken together, our study reveals a specific role of microglial YOD1 in mediating microglia homeostasis and AD pathology, which provides a potential strategy for targeting microglia to treat AD.
5.Finite element analysis of osteoporosis in proximal femur after cannulated screw fixation for femoral neck fracture

BACKGROUND:After the internal fixation of cannulated screws in femoral neck fractures,because the affected limb is often unable to bear weight in the short term and the implants with high stiffness have a stress shielding effect on the fracture end,it is easy to cause osteoporosis of the affected limb and changes in the biomechanical distribution of the proximal femur,the incidence of osteonecrosis of the femoral head is high after surgery.At present,few studies have been conducted on the biomechanical effects of osteoporosis at the proximal end of the femur occurring after femoral neck fracture surgery on femoral neck fracture treated with cannulated screws. OBJECTIVE:Using finite element analysis,to investigate the biomechanical effects of osteoporosis occurring after femoral neck fracture surgery on femoral neck fracture treated with cannulated screws and explore the role of biomechanical factors in osteonecrosis of the femoral head. METHODS:Based on the obtained CT scan data of the femur in a patient with a femoral neck fracture,a proximal femoral model for internal fixation for femoral neck fracture was established by Mimics 19.0,3-Matic,UG 11.0,Hypermesh 14.0,and Abaqus software.One finite element model of the proximal femur without osteoporosis and three finite element models of the proximal femur with osteoporosis were analyzed using Abaqus software.The stress,contact pressure,displacement peak and cloud map under different components of the four models were measured and analyzed,and the internal stress changes and distribution of the femoral head were compared and analyzed. RESULTS AND CONCLUSION:The stresses and contact pressures of the femoral head and lower anterior cannulated screws varied more with the degree of osteoporosis.The peak displacement of the four models increased slowly with the degree of osteoporosis.By one-way analysis of variance,there was no significant effect of the degree of osteoporosis on the peak stress,contact pressure,and displacement of the different components.The internal stress distribution of the femoral head changed with the degree of osteoporosis.Changes in the biomechanical environment of the proximal femur have an important impact on osteonecrosis of the femoral head.
6.Investigation on acute chromium poisoning accident occurred in process of electroplating tank cleaning

Background Electroplating technology is widely used in the mechanical manufacturing industry. Electroplating workers may be exposed to occupational hazards such as chromium, zinc, cyanides, and sulfuric acid. With the continuous improvement of protective conditions, occupational hazards have been effectively controlled, but there is less focus on the potential acute occupational poisoning that may occur during the electroplating process. Objective To identify causes of an acute chromium poisoning accident occurred during the cleaning process of electroplating tank in a hardware processing plant, and to formulate relevant prevention and control measures to avoid similar poisoning accidents. Methods Occupational history, medical history, and treatment process of the poisoned worker were inquired in detail, his inpatient medical records were consulted , and his blood samples were tested. Occupational health investigation and testing were carried out at the scene of the poisoning accident. 7.Prognostic value of serum levels of HMGB1,IL-34 and ANGPTL4 in patients with acute respiratory distress syndrome

Objective To investigate the prognostic value of serum levels of high mobility group protein B1(HMGB1),interleukin-34(IL-34)and angiopoietin like protein 4(ANGPTL4)in patients with acute respira-tory distress syndrome(ARDS).Methods A total of 97 patients with acute respiratory distress syndrome from June 2021 to June 2023 were selected as the observation group,and 97 healthy patients from the hospital during the same period were selected as the control group.The levels of HMGB1,IL-34 and ANGPTL4 were compared between the two groups.The clinical data and serum levels of HMGB1,IL-34 and ANGPTL4 of pa-tients with different prognosis were compared and analyzed,and the influencing factors of prognosis were ana-lyzed.The predictive value of prognostic factors on prognosis was analyzed,and the predictive value of serum HMGB1,IL-34 and ANGPTL4 combined detection on prognosis was evaluated.Results The serum levels of HMGB1,IL-34 and ANGPTL4 in observation group were higher than those in control group,with statistical significance(P<0.05).Oxygenation index(PaO2/FiO2)in death group was lower than that in survival group,acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)score,Murray lung injury score(MLIS),HMGB1,IL-34 and ANGPTL4 levels were higher than those in survival group,and the differences were statistically significant(P<0.05).PaO2/FiO2,APACHE Ⅱ score,MLIS,HMGB1,IL-34 and AN-GPTL4 levels were all prognostic factors of ARDS patients(P<0.05).The area under the curve(AUC)of PaO2/FiO2,APACHE Ⅱ score,MLIS and HMGB1,IL-34 and ANGPTL4 for predicting prognosis were 0.779,0.799,0.808,0.844,0.772 and 0.822,respectively.There was no significant difference in AUC among prognostic factors(P>0.05).The analysis of receiver operating characteristic curve showed that the AUC of se-rum HMGB1,IL-34 and ANGPTL4 combined to predict the prognosis of ARDS patients was 0.915(95%CI:0.841-0.962),which was significantly higher than that predicted by each indicator alone.Conclusion The combined detection of serum HMGB1,IL-34 and ANGPTL4 levels in ARDS patients has certain prognostic value.

Despite growing prevalence and incidence, the management of gout remains suboptimal. The intermittent nature of the gout makes the long-term urate-lowering therapy (ULT) particularly important for gout management. However, patients are reluctant to take medication day after day to manage incurable occasional gout flares, and suffer from possible long-term toxicity. Therefore, a safe and easy-to-operate drug delivery system with simple preparation for the long-term management of gout is very necessary. Here, a chitosan-containing sustained-release microneedle system co-loaded with colchicine and uricase liposomes were fabricated to achieve this goal. This microneedle system was confirmed to successfully deliver the drug to the skin and maintain a one-week drug retention. Furthermore, its powerful therapeutic potency to manage gout was investigated in both acute gouty and chronic gouty models. Besides, the drug co-delivery system could help avoid long-term daily oral colchicine, a drug with a narrow therapeutic index. This system also avoids mass injection of uricase by improving its stability, enhancing the clinical application value of uricase. In general, this two-drug system reduces the dosage of uricase and colchicine and improves the patient's compliance, which has a strong clinical translation.
9.The application value of tibial transverse transport microvascular reconstruction combined with antibiotic bone cement in the limb salvage of diabetic foot

Objective:To analyze the application value of tibial transverse transport (TTT) microvascular reconstruction combined with antibiotic bone cement in the limb salvage of diabetic foot.Methods:Seventy-seven patients with diabetic foot admitted to the Traditional Chinese Medicine Hospital of Zhongshan from January 2018 to January 2021 were enrolled retrospectively, and they were divided into the control group (performed TTT microvascular reconstruction, 47 cases) and the combined group (performed TTT microvascular reconstruction combined with antibiotic bone cement, 30 cases) according to the surgical method. Wound healing rate, limb salvage rate, the ulcer healing time, bone window healing time, wound infection control status, affected side lower limb vascular blood supply situation, and peripheral nerve function recovery were compared between the two groups.Results:The amputation rate between the two groups had no significant difference ( P>0.05), but the healing rate in the combined group was higher than that in the control group: 90.00%(27/30) vs. 68.08%(32/47); and the ulcer healing time, bone window healing time in the combined group were shorter than those in the control group: (12.55 ± 2.14) weeks vs. (15.01 ± 3.17) weeks, (17.47 ± 2.09) weeks vs. (19.00 ± 2.58) weeks, there were statistical differences ( P<0.05). The positive percentage of bacteria culture in wound secretions in the combined group after the surgery for 6 d was lower than that in the control group: 20.00%(6/30) vs. 44.68%(21/47), there was statistical difference ( P<0.05). After the surgery for 3 months, the dorsal skin temperature and ankle brachial index in the combined group were higher than those in the control group: (31.05 ± 1.74) ℃ vs. (29.27 ± 1.66) ℃, 0.93 ± 0.29 vs. 0.80 ± 0.19, there were statistical differences ( P<0.05), but the scores of visual analog scale (VAS) and 10 g nylon line test values between the two groups had no significant differences ( P>0.05). Conclusions:TTT combined with antibiotic bone cement can achieve better wound healing and infection control effect in the limb salvage of diabetic foot, and the blood supply improvement of the affected limb is more significant.
10.Application of MRI-related indicators of posterior cruciate ligament in the diagnosis of anterior cruciate ligament injury: an update

The main function of anterior cruciate ligament (ACL) is to maintain stability of the knee joint and prevent anterior displacement of the tibial plateau. ACL injury accounts for more than 50% of the knee joint injuries. If not timely handled, it will increase the risk of secondary injuries to structures such as the meniscus and cartilage, causing chronic pain and degeneration of the knee joint. Although most ACL injuries can be determined by their direct signs on MRI, the identification of complex situations and partial tears of ACL are still not satisfactory, which subsequently affects treatment strategies. After ACL injury, changes in anatomical relationship of the knee joint can also lead to morphological changes in other structures such as the posterior cruciate ligament (PCL) on MRI, and these indirect signs can assist in the diagnosis of ACL injury. The authors reviewed the application of MRI-related indicators of PCL in diagnosing ACL injury, hoping to provide references and new ideas for clinical decision-making.
