OBJECTIVE To evaluate the cost-effectiveness of dapagliflozin and empagliflozin in the treatment of type 2 diabetic kidney disease from the perspective of healthcare system in China. METHODS Based on the data from the two multicenter clinical trials, DECLARE-TIMI 58 and EMPA-REG OUTCOME, a Markov model was constructed according to the urinary albumin-to-creatinine ratio (UACR) of the patients with type 2 diabetic kidney disease with a cycle of 1 year, simulating until 99% of patients died. The model outputs were total costs and quality-adjusted life year (QALY). The cost-effectiveness of the two treatment regimens was assessed by comparing their incremental cost-effectiveness ratio (ICER) and the willingness-to-pay threshold (WTP,set at three times China’s 2023 per capita gross domestic product, i.e., 268 074 yuan/QALY). Additionally, oneway sensitivity analysis and probabilistic sensitivity analysis were performed to test the robustness of the base analysis results. RESULTS Compared with dapagliflozin, the ICER for empagliflozin regimen was 44 334.82 yuan/QALY, which was below the WTP , indicating its cost-effectiveness. The results of the oneway sensitivity analysis indicated that the incidence of non-fatal myocardial infarction in both groups and the utility values associated with the microalbuminuria state had the most significant impact on the outcomes, but did not change the base-case conclusion. The probabilistic sensitivity analysis indicated that the results of the base-case analysis were robust. CONCLUSIONS With a WTP of three times China’s per capita gross domestic product in 2023, empagliflozin is more cost-effective than dapagliflozin in treating type 2 diabetic kidney disease.

Traditional Chinese medicine of Polygonum cuspidatum has been utilized in China for thousands of years. Its primary active compound, polydatin, exhibits a variety of pharmacological effects including the regulation of glucose and lipid metabolism, suppression of cough and asthma, as well as antibacterial and anti-inflammatory properties. However, conventional methods for polydatin production are inadequate to satisfy the market demand. This study aims to explore the green and efficient preparation of polydatin. With resveratrol as the substrate, we efficiently synthesized polydatin by using the triple mutant IGW (Y14I/I62G/M315W) of the glycosyltransferase UGTBS based on a strategy of two-enzyme coupled with one-pot and realized the recycling of uridine diphosphate-glucose (UDPG). The conditions of the two-enzyme reaction were optimized. Under the conditions of 35 ℃, pH 8.0, IGW: AtSuSy1 activity ratio of 3:4, dimethyl sulfoxide (DMSO) volume fraction of 5%, uridine diphosphate (UDP) concentration of 0.10 mmol/L, and sucrose concentration of 0.6 mol/L, the conversion of 2 mmol/L resveratrol reached 80.6% within 1 h, and the proportion of polydatin was over 90%. This study achieved the recycling of UDPG via a two-enzyme coupling system and shortened the reaction time. At the same time, the fed-batch strategy was adopted, and the yield of polydatin reached 6.28 g/L after 24 h in the one-pot coupling reaction, which provided a new strategy for green and efficient preparation of polydatin.

Severe open tibiofibular fractures account for approximately 28.1% of all open fractures. Among them, Gustilo-Anderson type IIIB/C fractures present significant clinical challenges due to associated bone and soft tissue defects, high infection rates, and risk of amputation. Inadequate preoperative assessment may lead to suboptimal emergency surgical planning or intraoperative complications. Historically, external fixation was often preferred, but this approach has been associated with limitations such as restricted joint mobility, delayed bone union, joint stiffness, and disuse osteoporosis, resulting in poor functional recovery. With advancements of debridement techniques, standardization of antibiotic use, and popularization of early soft tissue coverage, early internal fixation has gained broader acceptance. Nevertheless, controversies persist regarding the choice of fixation method, timing of definitive fixation, use of reamed versus unreamed intramedullary nailing, and necessity of fibular fixation. To standardize the diagnosis and early management of severe open tibiofibular fractures, reduce complication rates, and improve functional recovery, the Society of Microsurgery of the Chinese Medical Association organized a panel of domestic experts to develop the Evidence-based guideline for the diagnosis and early fixation of severe open tibiofibular fractures ( version 2025), using evidence-based methodology. The guidelines provided 12 recommendations covering diagnostic and early fixation strategies of severe open tibiofibular fractures, aiming to provide clinicians with scientifically grounded and standardized guidance.

Objective:To summarize the safety and efficacy of thoracoscopic minimally invasive " one-stop" ablation for the treatment of atrial fibrillation(AF).Methods:A retrospective study was conducted on all patients with isolated atrial fibrillation who underwent thoracoscopic radiofrequency ablation combined with left atrial appendage clipping(LAAC) at West China Hospital of Sichuan University from September 2019 to October 2023. Preoperative baseline data, perioperative complications, and 3-month, 6-month, and 12-month postoperative follow-up data were collected and analyzed. Results:A total of 87 patients were included, with a mean age of(60.5±9.0) years old. Among them, 47 were males and 40 were females. Of these patients, 12 had paroxysmal AF, and 75 had persistent AF. Fourteen patients had a prior history of catheter-based radiofrequency ablation, and 11 had a history of transient ischemic attack(TIA) or stroke. All procedures were successfully completed without conversion to open thoracotomy, perioperative mortality, or perioperative stroke events. During the follow-up period, one patient died, no strokes or left atrial appendage reconnection events were observed. The sinus rhythm maintenance rates at 3、6 and 12 months postoperatively were 89.6%(78/87)、82.8%(72/87) and 75.9%(66/87), respectively. Multivariate logistic regression analysis identified a preoperative left atrial anteroposterior diameter>40 mm as an independent risk factor for postoperative AF recurrence. Conclusion:Thoracoscopic minimally invasive ablation combined with left atrial appendage clipping as a " one-stop" procedure is a safe and effective method for the treatment of isolated atrial fibrillation, achieving satisfactory surgical ablation success rates while effectively preventing stroke.

Objective:To explore the value of contrast-enhaoced echocardiography for measuring pulmonary transit time(PTT)in assessing heart failure after percutaneous coronary intervention(PCI)in acute ST-segment elevation myocardial infarction(STEMI)patients. Methods:From September 2023 to September 2024,120 patients with STEMI undergoing PCI at Hunan Provincial People's Hospital were prospectively selected and divided into a heart failure group( n=42)and a non-heart failure group( n=78)according to the guidelines. The differences in general clinical data, laboratory parameters, and echocardiographic parameters between the two groups were compared. The diagnostic efficacies of PTT,normalized PTT(nPTT),and N-terminal pro-B-type natriuretic peptide(NT-proBNP)were analyzed. Consistency between them and New York Heart Association(NYHA)heart function classification was tested. Results:Compared to the non-heart failure group, the NT-proBNP,PTT,and nPTT values in the heart failure group were significantly increased(all P<0.05). The area under the curve(AUC)of nPTT was 0.944,better than that of PTT and NT-proBNP(AUC=0.871,0.887). After K-means clustering reclassified patients into four levels based on nPTT values,nPTT classification showed moderate consistency with NYHA classification(Kappa=0.580, P<0.001),and nPTT differed significantly across NYHA classifications( P<0.05). Conclusions:PTT,as an echocardiographic index for assessing cardiac function, has similar diagnostic efficacy to NT-proBNP,the nPTT is even better. It shows moderate consistency with the NYHA classification and holds potential for differentiating overlapping NYHA grades. Importantly,it offers a fresh objective way to evaluate cardiac dysfunction after PCI in STEMI patients.
