1.Effects of thumbtack-needle embedding therapy of auricular acupuncture on gastrointestinal function and intestinal microflora in patients with gastric cancer after operation.

OBJECTIVE:
To observe the effects of thumbtack-needle embedding therapy of auricular acupuncture on gastrointestinal function and intestinal microflora in the patients with gastric cancer after operation, and to explore its mechanism.
METHODS:
A total of 80 patients with gastric cancer after radical operation were randomly divided into an observation group (40 cases, 3 cases discontinued) and a control group (40 cases, 3 cases discontinued). The patients of both groups received the perioperative care for accelerating recovery. Additionally, in the observation group, the thumbtack-needle embedding therapy of auricular acupuncture was delivered at the auricular points of unilateral side, including Wei (CO4), Pi (CO13), Dachang (CO7), Xiaochang (CO6), Yuanzhong (AT2,3,4i), Erzhong (HX1), Sanjiao (CO17) and Jiaowozhong (TF3), and the needles were embedded and retained for 72 h. The postoperative recovery time of gastrointestinal function (the postoperative bowel sound recovery time, the first exhaust time, the first defecation time), the postoperative hospital stay and pain visual analogue scale (VAS) score were observed in the two groups. Before operation and on day 5 after operation, the serum gastrin level was detected in the two groups. The third-generation 16S rRNA sequencing technology was used to detect the composition and relative abundance of intestinal flora in the two groups before and after operation.
RESULTS:
Compared with the control group, the postoperative bowel sound recovery time, the first exhaust time and the first defecation time were shortened in the observation group (P<0.05). In the observation group, the VAS scores at 24 h, 48 h, and 72 h after surgery were lower than those of the control group, respectively (P<0.05). There was no significant differences in postoperative hospital stay and serum gastrin level between the two groups (P>0.05). The alpha diversity analysis showed that the differences in Shannon index, Simpson index, Pielou_J index and Pd_fath index were not significant statistically after intervention between the two groups (P>0.05). After intervention, the community structure of the fecal sample was similar at each taxonomic level between the two groups, and although the proportion between species was various, the difference was not significant (P>0.05). After intervention, there were 55 species with the differences between the two groups, 17 species of them presented significant difference in relative abundance in the observation group and 38 species in the control group. Regarding the level of genus, the levels of Klebsiell and Enterobacter increased (P<0.05) and the level of Streptococcus decreased (P<0.05) in the observation group. The main microbial groups that played an important role were Coprobacillaceae, Sutterellaceae and Yersiniaceae in the observation group. KEGG function prediction indicated that the function of intestinal microflora was mainly associated with the cofactor and vitamin metabolism, carbohydrate metabolism, and amino acid metabolism.
CONCLUSION
The thumbtack-needle embedding therapy of auricular acupuncture improves the postoperative gastrointestinal function of the patients with gastric cancer probably through regulating the structure and relative abundance of intestinal microflora and affecting the energy metabolism.

2.Tongnao Huoluo Liyan acupuncture combined with rehabilitation training for post-stroke dysphagia: a randomized controlled trial.

OBJECTIVE:
To observe the efficacy of the Tongnao Huoluo Liyan (unblocking brain, activating collaterals and relaxing throat) acupuncture combined with rehabilitation training for post-stroke dysphagia (PSD).
METHODS:
A total of 92 PSD patients were randomly assigned to an observation group (46 cases, 1 case was discontinued) and a control group (46 cases, 1 case was discontinued, 1 case dropped out). The patients in the control group received rehabilitation training, including low-frequency neuromuscular electrical stimulation for swallowing, lip and tongue movement training, and oral sensory function training, once daily, five times per week, for four weeks. The patients in the observation group received the Tongnao Huoluo Liyan acupuncture method in addition to the same rehabilitation training. Acupoints included Lianquan (CV23), bilateral Jia Lianquan, Jinjin (EX-HN12), Yuye (EX-HN13), Baihui (GV20), Shuigou (GV26), and bilateral Neiguan (PC6), once daily, five times per week, for four weeks. Before and after treatment, Kubota water stvallowing test grading, standardized swallowing assessment (SSA) scores, Fujishima Ichiro swallowing efficacy scores, and swallowing quality of life questionnaire (SWAL-QOL) scores were assessed in both groups. Surface electromyography (sEMG) was used to evaluate the average amplitude (AEMG) and mean swallowing time of the suprahyoid and infrahyoid muscle groups.
RESULTS:
After treatment, the proportion of patients classified as Grade Ⅰ or Ⅱ in the Kubota water stvallowing test was increased in both groups (P<0.05), with better results in the observation group compared to the control group (P<0.05). Compare before treatment, SSA scores were decreased in both groups after treatment (P<0.05), with lower scores in the observation group than in the control group (P<0.05). Compare before treatment, Fujishima Ichiro swallowing efficacy scores and SWAL-QOL scores were improved in both groups after treatment (P<0.05), with significantly higher scores in the observation group than those in the control group (P<0.05). Compare before treatment, AEMG values of the Submental musckes and infrahyoid muscles groups were increased (P<0.05), and mean swallowing time was decreased (P<0.05) in both groups after treatment. The observation group showed greater increases in AEMG values and shorter mean swallowing times compared to the control group (P<0.05). The total effective rate was 97.8% (44/45) in the observation group, higher than 84.1% (37/44) in the control group (P<0.05).
CONCLUSION
The Tongnao Huoluo Liyan acupuncture combined with rehabilitation training could enhance the contraction function in swallowing-related muscle groups, facilitate hyoid and laryngeal elevation, restore swallowing function, and improve patients' quality of life. This combined treatment approach is superior to rehabilitation training alone.

3.The reduction of reproductive function of male zebrafish caused by high-fat diet:A molecular basis study

Objective To investigate the effects and mechanisms of high-fat diet(HFD)-induced obesity on male ze-brafish reproductive function.Methods Adult male zebrafish were divided into normal diet(ND)group and HFD group.Growth and metabolic conditions were evaluated by measuring body weight,body length,BMI,organ index,and glucose/lipid lev-els.Reproductive capacity was assessed via sperm concentration,motility,fertilization rate,and testosterone levels.Testicular tissues from both of groups were subjected to transeriptomic sequencing(RNA-seq).And qRT-PCR was used to validate the expression of genes.Results Compared to male zebrafish in ND group,the ones in HFD group exhibited hepatic steatosis and glucose/lipid meta-bolic disorders(P<0.05).Testicular structural disorganization,along with reduced testosterone levels,decreased gonadosomatic in-dex,and impaired sperm concentration and motility occurred in HFD group(P<0.05).GO analysis revealed that differentially ex-pressed genes were enriched in spermatogenesis and ciliary system,while KEGG analysis highlighted metabolic related pathways(pu-rine metabolism,thyroid hormone synthesis,mTOR signaling)and cell adhesion molecules.Twenty key differentially expressed genes were validated by qRT-PCR,which confirmed the reliability of RNA-Seq results.Conclusion Impairment of reproductive function induced by HFD in zebrafish may be associated with three regulatory mechanisms including ciliary system,metabolic dysregulation,and aberrant cell adhesion molecule signaling.This study provides mechanistic insights and identifies potential therapeutic targets for clinical management of diet-associated infertility.
4.Role and potential mechanisms of lactylation modification in ischemic stroke

Lactylation, a newly discovered post-translational modification, has been reported to be involved in various physiological and pathological processes. Recent studies have found that lactylation is potentially linked to the pathological processes and repair mechanisms of ischemic stroke. This article explores the impact and role of lactylation after the occurrence of ischemic stroke on neuroinflammation, energy metabolism regulation, oxidative stress, signaling pathway modulation, and angiogenesis. It may serve as a bridge connecting lactylation and ischemic stroke, offering insights and guidance for future research and clinical strategies in ischemic stroke.
5.Clinical analysis of endoscopic diagnosis and treatment for 5 cases of duodenal variceal bleeding

To investigate the clinical outcomes of endoscopic diagnosis and treatment in patients with duodenal variceal bleeding, a retrospective analysis was conducted on clinical data of patients with ectopic duodenal variceal bleeding diagnosed and treated endoscopically at the Digestive Endoscopy Center of Beijing Friendship Hospital, Capital Medical University, from August 2011 to December 2024. Five male patients (mean age 50.4 years) were included. Etiologies included alcohol-related cirrhosis (1 case), hepatitis B virus-related cirrhosis (2 cases), autoimmune disease-associated portal hypertension (1 case), and thrombophilia-related portal hypertension (1 case). Immediate hemostasis was achieved endoscopically in all cases. Two patients (1 alcohol-related and 1 hepatitis B virus-related cirrhosis) exhibited persistent decline in hemoglobin levels post-procedure and received sequential interventional therapy before discharge. The remaining 3 patients recovered uneventfully. Duodenal variceal bleeding, though rare and life-threatening, can be effectively managed through multidisciplinary collaboration. Endoscopic hemostasis should be prioritized when technically feasible.
6.Analysis of health-related quality of life and prognostic factors in pediatric patients with brain tumors

Objective:To explore the key factors affecting health-related quality of life in children with brain tumors following initial diagnosis and to analyze its impact on survival and prognosis.Methods:Seventy-eight pediatric brain tumor patients who participated in a prospective cohort study between June 1st,2016 and June 30th,2021 were included for health-related quality of life assessment and long-term follow-up(median follow-up duration:52 months).Results:The male-to-female ratio among the 78 children was 1.1:1,with a median age of 7.0(4.0,10.0)years. The scores of the Pediatric Quality of Life Inventory? 4.0 Generic Core Scales(PedsQL? 4.0)were(67.40±18.26)for parent proxy reports and(67.87±20.40)for child self-reports. Cronbach's α coefficients ranged from 0.790 to 0.927,with the intraclass correlation coefficient(ICC)was 0.673. According to the PedsQL? 4.0,impaired quality of life was observed in 50.0% of children by parent proxy report and 52.8% by child self-report,primarily affecting physical and role functioning. In addition,70% of caregivers reported impaired quality of life,with worry being the most prominent issue. Key factors affecting children's quality of life included radiotherapy,tumor stage,annual family income,and parents' marital status,while caregivers' quality of life was influenced by radiotherapy and the child's IgA levels(all P<0.05). Children with decreased total scores,impaired physical functioning,or impaired emotional functioning on the PedsQL? 4.0 parent proxy report exhibited an increased risk of mortality(all P<0.05). In multivariate Cox regression analysis,independent prognostic factors included a decrease in the total score on the PedsQL? 4.0 parent proxy report( HR=6.702,95% CI:1.442-31.151, P<0.05),presence of hydrocephalus( HR=33.602,95% CI:4.354-259.333, P<0.05),tumor recurrence( HR=16.846,95% CI:3.158-89.852, P<0.05),and absence of hydrocephalus shunt surgery( HR=13.428,95% CI:1.761-102.394, P<0.05). Conclusion:The quality of life of newly diagnosed children with brain tumors is lower than that of healthy children,and quality of life is an important prognostic factor. Quality of life assessment should be an integral component of a comprehensive management program for children with brain tumors.
7.Analysis on the Clinical Application Rules of Baihui Acupoint in Ancient Literature Based on Data Mining

Objective To provide reference for the clinical use of Baihui acupoint(GV20)in treating diseases,the rules of clinical application of GV20 in ancient literature through data mining technology was analyzed.Methods"Zhong Hua Yi Dian"was searched by computer for relevant papers involving GV20 in the main treatment of illnesses,the main treatment of illnesses by combining other acupoints,and the treatment methods.After organizing the included articles into a database,Microsoft Excel 2021 was used to analyze the frequency of diseases and the use of acupoint pairing,attribution to meridians and special points,and IBM SPSS STATISTIC 27 software was used to analyze the cluster analysis of high-frequency compatible acupoints.Results Valid prescriptions from 209 medical books,including Zhen Jiu Da Cheng(The Great Compendium of Acupuncture and Moxibustion),containing 311 compatible acupoints,were screened and summarized.The main treatment conditions of GV20 mainly included internal medicine,surgery,gynecology,pediatrics and other disciplines,with the brain and liver and gallbladder systems of internal medicine as the main diseases.The top 5 frequency acupoints were Quchi(LI11),Hegu(LI4),Shenting(GV24),Zusanli(ST36),and Shangxing(GV23),and the most frequently involved meridians were Bladder Meridian of Foot-Taiyang,Governor Vessel,and Gallbladder Meridian of Foot-Shaoyang.The compatible acupoints mainly distributed in the head and neck.The special points with the highest frequency of collocation were Five-Shu acupoints,confluent acupoints,and Back-Shu acupoints.In the cluster analysis,the icicle diagram showed 2 effective clusters and the dendrogram 4 clusters of acupoints.Conclusion The main treatment range of GV20 in ancient medical texts is wide,and the main treatment range can be enlarged by pairing,emphasizing the use of moxibustion,which can provide a reference for modern clinical treatment.

Based on the underlying mechanism of acupuncture therapy for intracerebral hemorrhage(ICH),we discussed the research on the pathological mechanism of ICH and the effect of acupuncture therapy in recent 20 years,from primary and secondary brain injury.It was concluded that acupuncture had an intervention effect on multiple links such as hematoma compression,brain edema,oxidative stress,nerve cell death and cerebral blood flow changes.However,there were still problems such as less in vivo research,lack of thinking on syndrome differentiation and insufficient research on the quantity effect and overall regulation of acupuncture.
