Sarcopenia is a major geriatric syndrome characterized by progressive loss of muscle mass and strength, resulting in disability and mortality. This narrative review synthesizes international consensus recommendations and Korean evidence to guide context-specific sarcopenia management strategies. PubMed, Embase, Cochrane Library, and KoreaMed (January 2000–November 2025) were searched, focusing on randomized trials, meta-analyses, systematic reviews, clinical practice guidelines, and large observational studies. Global diagnostic frameworks have evolved from muscle mass-based definitions toward multidimensional models that incorporate muscle strength and physical performance. Exercise and nutrition remain the mainstay treatments, with resistance-based training and adequate protein intake. Currently, pharmacologic options with proven clinical benefit are limited. In Korea, growing evidence supports the effectiveness of community-based sarcopenia interventions, underscoring the need for standardized, integrated delivery models that bridge the fragmented healthcare system and enable sustainable implementation.
2.Evidence appraisal and updates for the 2025 Dietary Reference Intakes for Koreans for calcium

Purpose:
The Dietary Reference Intakes for Koreans (KDRIs) serve as a foundation for nutrition policy, nutrition education, and dietary intake evaluation among healthy Koreans.The KDRIs are revised every five years to ensure that the guidelines are based on a robust and up-to-date scientific foundation. For the 2025 KDRIs for calcium, the 2020 KDRIs were revisited, and references were critically evaluated.
Methods:
All references cited in the 2020 KDRIs were extracted, reviewed, and organized according to the framework for the estimated average requirement (EAR). Each reference was evaluated based on the participant characteristics, including age, race/ethnicity, and health status, as well as study design, exposure, and outcomes. In addition, calculations for the EAR and Recommended Nutritient Intakes were re-examined for each sex and age group and compared with the current literature.
Results:
Major revisions in the 2025 KDRIs were driven by updates in the reference bodyweight, improved consistency in rounding and coefficients of variation, inclusion of studies in East Asian populations, and clarification of the use of calcium balance studies.Nevertheless, further studies in Koreans, particularly in pregnant and lactating women, are needed to strengthen the scientific evidence for the calcium DRI.
Conclusion
The 2025 KDRIs for calcium were revised to be based on more robust logical reasoning and calculation methods. Future research conducted on Korean populations is necessary to define the calcium requirements for Koreans more accurately.
3.Postpartum bone mineral density in Korean women: associations with lactation status and calcium intake

BACKGROUND/OBJECTIVES:
Postpartum bone metabolism undergoes substantial alterations based on breastfeeding status. Research on maternal calcium intake and bone recovery, particularly among Asian women, remains limited. This study investigated postpartum change in bone mineral density (BMD) and its association with calcium intake during late pregnancy and post-delivery according to breastfeeding status.
SUBJECTS/METHODS:
Thirty-six women (22 breastfeeding, 14 non-breastfeeding) underwent dual-energy X-ray absorptiometry 3 times: 1) within 3 days of delivery, 2) at 6 mon postpartum for non-breastfeeding women or within 1 mon of weaning for breastfeeding women, and 3) at 12 mon postpartum. Dietary calcium intake was evaluated using a food frequency questionnaire and dietary supplement survey at all visits. Association between calcium intake with percentage change in BMD (%ΔBMD) was assessed by linear regression.
RESULTS:
The BMD at the lumbar spine, total hip, femoral neck, and trochanter significantly decreased during breastfeeding, whereas non-breastfeeding women displayed increased BMD at the femoral neck at 6 mon postpartum. In non-breastfeeding women, postpartum calcium intake was positively associated with %ΔBMD at the total hip, femoral neck, and intertrochanter (P < 0.05). Few significant associations were observed between %ΔBMD and late pregnancy intakes across all women, regardless of breastfeeding status. At 12 mon, BMD in breastfeeding women recovered to delivery levels, while non-breastfeeding women maintained their BMD observed at 6 mon postpartum.
CONCLUSION
Postpartum calcium intake is positively associated with bone recovery in non-breastfeeding women, suggesting that its effects on bone recovery potentially vary with lactation status.

Participants (n = 20) recorded videos of their meals, which were subsequently analyzed for food and nutrient intake and compared with intakes of participants of the Korea National Health and Nutrition Examination Survey (KNHANES) aged ≥ 80 yrs (n = 1,769), which were assessed via 24-h recall.
RESULTS:
Most participants (85.2%) consumed similar amounts of food at meals; however, only 65.1% ingested meals at regular times. The mean NQ-E score was 48.0 ± 11.9 and did not differ among regions. In Gwangju participants, subjective income was positively associated with diet quality. The mean energy and grain intakes per meal were lower, whereas the percent energy intake from protein and intakes of seaweed, meat and poultry, and iron were higher in GuGokSun and HwaDam participants with meal recordings than in KNHANES participants.
CONCLUSION
Among Korean adults aged ≥ 95 yrs, few regional variations exist in dietary quality and habits, although associations with diet quality vary within regions. 6.Food and nutrient intake in pregnant women with singletons or multiples and post-delivery changes in intake in Korea: an observational study

Objectives:
Nutrient intake during pregnancy and lactation is crucial for the health of both mother and offspring. Diet and nutrient metabolism potentially vary according to ethnicity and fetal number; nevertheless, recent studies validating this are inadequate. Furthermore, few studies have tracked changes in intake after delivery. We compared the food and nutrient intakes between pregnant women in Korea carrying singletons and multiples during late pregnancy and assessed their changes through postpartum.
Methods:
Ninety-eight pregnant women were recruited from Chonnam National University Hospital between January 2019 and December 2023, and 48 responded to follow-up. Third trimester and postpartum intake were assessed via food frequency questionnaires and supplement questionnaires. Student’s t-test, Mann–Whitney U test, chi-square test, paired t-test or Wilcoxon signed-rank test was performed and adjustments were made for covariates.
Results:
Nutrient intake was generally adequate relative to the Dietary Reference Intakes for Koreans, with no differences between singleton- and multiple-pregnancy women. Sixty-six of 98 (67%) pregnant women consumed meat, fish, vegetables, and fruit daily. Dairy intake was low, while the mean iron intake during pregnancy reached 54.2 ± 34.0 mg/d, exceeding the tolerable upper intake level, mainly owing to supplements. Postpartum fruit and vitamin C intake decreased, with no significant differences between breastfeeding and non-breastfeeding women.
Conclusion
Dietary intake did not significantly differ between Korean singleton- and multiple-pregnancy women. Dairy intake was low and iron intake was excessive. Fruit intake decreased after delivery; however, difference in dietary intake according to breastfeeding status was minimal. Nutritional education may be necessary to promote a balanced diet in pregnant and postpartum women. Trial Registration: Clinical Research Information Service Identifier KCT0005118.

10.Risk of all-cause mortality is associated with multiple healthrelated lifestyle behaviors and does not differ between urban and rural areas in Korea

BACKGROUND/OBJECTIVES:
Urban−rural inequities in health and mortality exist in Korea, a highly centralized developed country. The potential impact of multiple health-related lifestyle behaviors on mortality and difference between urban and rural areas is not fully understood.This study aimed to investigate the effect of high-risk health behaviors on all-cause mortality among residents living in urban and rural in Korea.
SUBJECTS/METHODS:
Cross-sectional analyses were conducted on 8,298 adults aged 40 yrs and older from the Korea National Health and Nutrition Examination Survey 2013–2015.High-risk behaviors were defined as having poor diet quality, current smoking, high-risk drinking, or insufficient physical activity. Mortality status was linked to the Cause of Death data followed up to December 31, 2019. The associations between all-cause mortality and high-risk behaviors were evaluated using Cox proportional hazard regression models adjusted for age, sex, education, income, and survey year. Population attributable fractions (PAFs) were calculated, and effect modification analysis was conducted. Participants were stratified by residential area (urban or rural).
RESULTS:
During the follow-up (median: 5.4 yrs), 313 deaths occurred. A higher proportion of rural residents than urban residents engaged in multiple high-risk behaviors (28.9% vs.22.6%; P < 0.0001). As individual factors, a greater risk of mortality was associated with poor diet quality, current smoking, and inadequate physical activity, and these tendencies persisted in rural residents, especially for diet quality. Multiple high-risk behaviors were positively associated with a higher risk of mortality in Koreans living in urban and rural areas. PAF (95% confidence interval) was 18.5% (7.35–27.9%) and 29.8% (16.1–40.2%) in urban and rural residents, respectively. No additive or multiplicative effect of the region was observed.
CONCLUSION
The higher prevalence of multiple high-risk lifestyle behaviors in rural residents may explain the higher mortality in rural areas compared to urban areas.Comprehensive public health policies to improve health-related behaviors in rural populations may be needed.
