1.A novel gamma-ray cone-beam focused stereotactic radiotherapy system

Stereotactic radiotherapy is widely favored because of its high treatment precision and less fractionations.ZND-A is a new domestic gamma-ray cone-beam focused stereotactic radiotherapy system.Herein the technical characteristics of ZND-A system are described in detail from the aspects of the treatment frame,gamma-ray module,collimator module,six-dimensional treatment couch module and image-guided system module,and the main parameters are compared with the mainstream gamma knife equipments at home and abroad.With reference to Response Evaluation Criteria in Solid Tumors(RECIST 1.1),the initial efficacy of the patients treated by the ZND-A system is analyzed to evaluate the advantages and disadvantages of the ZND-A system for providing a reference for the hospital clinical use of this type of gamma knife.
2.Comparison of the efficacy of modified rivet double pulley fixation and Kirschner wire tension band fixation for inferior pole patella fractures

Objective To compare the clinical effects of improved rivet double pulley fixation method and traditional Kirschner wire tension band in the treatment of patellar lower pole fractures.Methods 78 patients with patellar inferior pole fractures who underwent surgery in our department between January 2020 and January 2024 and met the inclusion criteria were collected.Divided into two groups according to different fixing methods,the observation group(39 cases)were fixed by modified rivet double pulley,the control group(39 cases)were fixed by steel wire tension band.The operation time,hospital stay,medical expenses,knee motion of the two groups at 1.5 months,3 months and 6 months after surgery,and knee lyshom function score at 3 months and 6 months after surgery were compared between the two groups.Results The observation group had a surgery time of(62.58±6.37)minutes compared to the control group[(60.20±11.18)minutes],and the observation group had a hospital stay of(6.45±1.35)days compared to the control group[(6.40±1.30)days].There was no significant difference in surgical time and hospitalization time between the two group(P>0.05).The total medical expenses of the observation group were significantly lower than those of the control group(P<0.05).Knee joint activity in observation group was higher than that in control group at 1.5 months,3 months and 6 months after surgery(P<0.05),and knee lyshom function scores in observation group at 3 months and 6 months after surgery were significantly higher than those in control group(P<0.0 5).Conclusion Compared with Kirschner wire tension band fixation for the treatment of patellar lower pole fractures,the improved riveting double pulley surgery does not require secondary surgery to remove internal fixation,resulting in lower overall medical costs,better postoperative knee joint mobility,and better postoperative knee joint function scores.The improved riveting double pulley surgery has more advantages in treating patellar lower pole fractures compared to traditional Kirschner wire tension band fixation.

5.Huangqin Decoction alleviates ulcerative colitis in mice by reducing endoplasmic reticulum stress

Objective To evaluate the therapeutic effect of Huangqin Decoction(HQD)on ulcerative colitis(UC)in mice and explore its mechanism.Methods Male Balb/c mice were randomly divided into normal control group,model group,mesalazine group(5-ASA,200 mg/kg),and low-,medium-and high-dose HQD groups(2.275,4.55 and 9.1 g/kg,respectively).With the exception of those in the normal control group,all the mice were exposed to 3%DSS solution in drinking water for 7 days to establish UC models.After treatment with the indicated drugs,the mice were assessed for colon injury and apoptosis using HE,AB-PAS and TUNEL staining,and the expression levels of inflammatory factors were detected with ELISA.Western blotting,immunohistochemistry and qRT-PCR were used to detect the changes in protein expressions associated with the intestinal chemical barrier,mechanical barrier and endoplasmic reticulum stress(ERS).Results HQD treatment significantly reduced DAI score and macro score of UC mice,decreased colonic epithelial cell apoptosis,lowered expressions of IL-6,TNF-α,IL-1β and IL-8,and enhanced the expressions of MUC2 and TFF3.HQD treatment also upregulated the protein expressions of claudin-1,occludin and E-cadherin,reduced the expressions of GRP78,CHOP,caspase-12 and caspase-3,decreased the phosphorylation levels of PERK,eIF2α and IRE1α,and increased the Bcl-2/Bax ratio in the colon tissues of UC mice.Conclusion HQD inhibits colonic epithelial cell apoptosis and improves intestinal barrier function in UC mice possibly by reducing ERS mediated by the PERK and IRE1α signaling pathways.

7.RRM2 promotes tumor progression by interfering with G2/M cycle of prostate cancer cells

【Objective】 To investigate the role of RRM2 in prostate cancer and the mechanism. 【Methods】 The data of prostate cancer expression profile were downloaded from The Cancer Genome Atlas (TCGA). The correlation between RRM2 expression and clinicopathological features and prognosis of prostate cancer was analyzed. The protein expressions of RRM2 in 55 cases of prostate cancer and 38 benign tissues were determined with immunohistochemistry (IHC). The effects of RRM2 on the biological process of prostate cancer were assessed with bioinformatic analysis. The biological process of RRM2 affecting the progression of prostate cancer was verified with Western blot and flow cytometry. 【Results】 RRM2 was highly expressed in prostate cancer, and the expression was positively correlated with the clinical stage, pathological grade and metastasis of prostate cancer (P<0.05). Higher RRM2 expression predicted poorer survival. RRM2 co-expression positively correlated genes were involved in cell cycle pathways, pyrimidine nucleotide metabolism, and biological processes such as RNA transport. Cell cycle pathways were significantly enriched. RRM2 was highly correlated with CDK1 and PCNA molecules. RRM2 knockdown reduced the protein expressions of CDK1 and PCNA in DU145 and LNCap cell lines, which were arrested in the G2/M phase. 【Conclusion】 RRM2 promotes tumor progression by interfering with G2/M cycle of prostate cancer cells.
8.Exploration of an energy consumption control strategy for hospitals based on the energy consumption characteristics of medical service units

With the increasing variety and quantity of energy consuming equipment in hospitals, the phenomenon of high energy consumption in hospitals is becoming increasingly prominent. In June 2021, this study established an energy consumption control strategy for hospitals based on the energy consumption characteristics of medical service units, targeting the current situation and shortcomings of hospital energy consumption management, and then conducted exploration of strategy application. The strategy was designed to deepen the analysis of the energy consumption hierarchy and energy consumption characteristics of medical service units in the hospital′s energy consumption system, extract personalized energy consumption control indexes, and optimize or build energy consumption control systems with the help of self-control and information technology, through collaboration among multiple departments. These measures above aimed to reduce ineffective energy consumption in the energy consumption terminal of medical service units, promote the deep integration of energy consumption system operation status and energy consumption demand characteristics of medical service units, and realize precise energy consumption control.In June 2021 and March 2022, two tertiary hospitals carried out energy consumption control practices based on the energy consumption characteristics of operating rooms and wards respectively. After practices, the energy-saving rates of operating rooms and wards were both>10.0%, achieving good application results. This strategy can provide references for hospitals in China to promote energy conservation and emission reduction.
9.Application and experience of laparoscopy for gastric cancer after transformation therapy (report of 7 cases)

Objective:To investigate the application value of laparoscopy for stage Ⅳ gastric cancer after transformation therapy.Methods:The clinical data of 7 patients with advanced gastric cancer who underwent laparoscopic surgery after transformation therapy in the First Affiliated Hospital of Hebei North University from January 2016 to December 2020 were retrospectively analyzed. Seven patients were diagnosed as stage Ⅳ gastric cancer by computed tomography (CT), positron emission tomography (PET-CT) and laparoscopy, and underwent paclitaxel based conversion therapy. After evaluating the curative effect, they underwent surgical resection. The intraoperative and postoperative conditions and survival rate of the patients were analyzed.Results:After transformation therapy, 6 patients (85.7%) had partial remission, 1 patient (14.3%) had stable disease, and the overall effective rate was 6/7. Laparoscopic-assisted surgery was performed in 3 patients, and 4 patients were converted to laparotomy. The operation time was (297.9±35.6)min, the intraoperative blood loss was (257.0±106.0)ml, the number of dissected lymph nodes was (38.4±9.1), the gastric tube indwelling time was (72.4±9.6)h, the jejunal feeding tube indwelling time was (15.4±5.6)d, and the liquid diet time was (8.6±3.4)d, the length of hospital stay was (17.1±5.5)d. The postoperative complication rate was 3/7, and there were no unplanned secondary operations and deaths. R0 resection was performed in 6 cases, and R1 resection in 1 case. Tumor regression grade (TRG) classification: 5 cases were grade 2 and 2 cases were grade 3. The median progression free survival of the 7 patients was 15.3 months, the median overall survival was 21.6 months, and the 1-year survival rate was 6/7.Conclusions:Laparoscopy has irreplaceable value in the staging and efficacy judgment of transformation therapy for gastric cancer. However, after transformation therapy, the tissue edema increases, the tumor boundary becomes more unclear, and the surgical operation becomes more difficult. Therefore, it is not necessary to force the laparoscopic operation, and switch to laparotomy according to the situation.
10.Emerging vaccine nanotechnology: From defense against infection to sniping cancer.

Looking retrospectively at the development of humanity, vaccination is an unprecedented medical landmark that saves lives by harnessing the human immune system. During the ongoing coronavirus disease 2019 (COVID-19) pandemic, vaccination is still the most effective defense modality. The successful clinical application of the lipid nanoparticle-based Pfizer/BioNTech and Moderna mRNA COVID-19 vaccines highlights promising future of nanotechnology in vaccine development. Compared with conventional vaccines, nanovaccines are supposed to have advantages in lymph node accumulation, antigen assembly, and antigen presentation; they also have, unique pathogen biomimicry properties because of well-organized combination of multiple immune factors. Beyond infectious diseases, vaccine nanotechnology also exhibits considerable potential for cancer treatment. The ultimate goal of cancer vaccines is to fully mobilize the potency of the immune system as a living therapeutic to recognize tumor antigens and eliminate tumor cells, and nanotechnologies have the requisite properties to realize this goal. In this review, we summarize the recent advances in vaccine nanotechnology from infectious disease prevention to cancer immunotherapy and highlight the different types of materials, mechanisms, administration methods, as well as future perspectives.
