1.Study on the flavonoids from Epimedium multiflorum T. S. Ying

Objective To investigate the chemical constituents of Epimedium multiflorum T. S. Ying. Methods The ethanol extract of E. multiflorum was separated and purified by silica gel column chromatography, preparative thin-layer chromatography, and semi-preparative reversed-phase high-performance liquid chromatography. The structures of the compounds were identified by nuclear magnetic resonance spectroscopy and comparison with literature data. Results Ten compounds were isolated and purified, which structures were identified as baohuoside Ⅱ(1),2′′-O-rhamnosyl-icariside Ⅱ(2), icariin(3), epimedoside A(4), ikarisoside B(5), epimedin C(6), baohuoside Ⅴ(7), epimedin A(8), epimedin B(9), and ikarisoside C(10). Conclusion All the isolated flavonoid compounds were obtained from E. multiflorum for the first time.
2.-75 G/A Polymorphism of Apolipoprotein A1 Gene Promoter Region in Normal Pregnant Women and Patients With Gestational Diabetes Mellitus

Objective To investigate the-75 G/A single-nucleotide polymorphism in the promoter region of apolipoprotein A1 gene(apoA1)and its association with gestational diabetes mellitus(GDM)in pregnant women and to provide references for the exploration in the molecular genetic basis of GDM.Methods A total of 626 GDM patients and 1022 normal pregnant women,ie,the controls,were included in the study.The genotyping of apoA1-75 G/A polymorphism was performed by polymerase chain reaction and restriction fragment length polymorphism(PCR-RFLP)analysis.Total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),and glucose(Glu)were measured by enzymatic methods.Plasma insulin(INS)was measured by chemiluminescence immunoassay.The protein levels of apoA1 and apoB were measured by the turbidimetric immunoassay.Results Allele frequencies of G and A were 0.718 and 0.282 in the GDM group and 0.713 and 0.287 in the control group,respectively.Distribution of the genotype frequencies was found to be in Hardy-Weinberg equilibrium in both the GDM and control groups.There was no significant difference in the frequencies of alleles G and A and the genotypes of apoA1-75 G/A polymorphism between the GDM and the control group(P>0.05).In the GDM group,the carriers with the genotype AA were associated with significantly higher levels of TC,HDL-C,and apoA1 than those with genotypes GG and GA did(all P<0.05).After the GDM patients were divided into obese and non-obese subgroups,the genotype-related apoA1 variation was observed only in obese patients,while the genotype-related TC and HDL-C variations were evident in non-obese patients(P<0.05).In the control group,carriers of genotypes AA and GA had higher systolic blood pressure(SBP)and HDL-C than the carriers of genotype GG did(all P<0.05).Carriers of genotypes AA had significantly lower Glu levels than carriers of genotypes GG and GA did(P<0.05).The control subjects were further divided into subgroups according to their body mass index(BMI).Analysis of the subgroups showed that AA carriers were associated with higher SBP levels in the obese control women only,while lower Glu levels were evident in both obese and non-obese control women.Conclusion These results suggest that-75 G/A polymorphism in the apoA1 gene is not associated with GDM.However,the genetic variation is closed associated with the plasma apoA1,HDL-C,and TC levels in GDM patients and plasma HDL-C,Glu,and SBP levels in the control subjects.The apoA1 variant-associated lipids and SBP variation is BMI dependent in both groups.
3.Recompensation of liver cirrhosis: Current status and challenges

Traditionally, the progression from compensated liver cirrhosis to decompensated liver cirrhosis has been considered an irreversible point in the natural history of the disease; however, with the suppression of underlying etiology, cure, and disease regression, this view is challenged by an increasing number of new evidence, and the idea of “recompensation of liver cirrhosis” is gradually being accepted. In recent years, scholars in China and globally have been exploring the specific definition of recompensation of liver cirrhosis and the clinical features of patients. By summarizing the recent studies on recompensation of liver cirrhosis in China and globally, integrating existing views, and analyzing related research evidence, this article points out the main challenges in the field of recompensation at this stage, including the lack of in-depth clinical and basic research, the need to define recompensation in the context of NAFLD, and related ethical issues, in order to provide new directions for future research in this field.
4.Bidirectional relationship between nonalcoholic fatty liver disease and sarcopenia: New insights and perspectives

Nonalcoholic fatty liver disease (NAFLD) is a group of highly heterogeneous diseases closely associated with metabolic dysfunction. Sarcopenia is a syndrome caused by a continuous decline in muscle mass, strength, and function, and it is often accompanied by NAFLD. Insulin resistance is the main pathological mechanism for sarcopenia and NAFLD, and in addition, factors such as changes in proteins and branched-chain amino acid, hyperammonemia, intestinal flora, and endocrine dysfunction can also lead to sarcopenia and NAFLD. With the deepening of clinical research, many published prospective studies have confirmed the existence of a bidirectional and complex pathophysiological relationship between sarcopenia and NAFLD. This article reviews the bidirectional relationship between sarcopenia and NAFLD, discusses the common pathogenesis of sarcopenia and NAFLD, summarizes the challenges faced in this field, and proposes new directions for the research on the bidirectional relationship between NAFLD and sarcopenia.
5.Analysis of pediatric hemoptysis recurrence related to vascular malformations after transcatheter occlusion

Objective To investigate pediatric hemoptysis recurrence related to vascular malforma-tions after transcatheter occlusion and offer suitable preventive actions for reducing the rate of hemoptysis recurrence.Methods The clinical data of 27 children,collected form Department of Cardiology,Children's Hospital of Chongqing Medical University between June 2012 and April 2017,with hemoptysis related to vascular malformations were retrospectively analyzed.The clinical manifestation,image feature and occlusion program of children with hemoptysis recurrence were re-analyzed and evaluated. Results All 27 children with hemoptysis received transcatheter occlusion, whose vascular malformations included bronchial-pulmonary artery fistula (24 cases,88.9%) and pulmonary arteriovenous fistulas (3 cases,11.1%) by angiography. Six cases,approximately 26.1%,suffered from recurrent hemoptysis after therapy,and the interval time of hemoptysis recurrence was roughly (5.6 ± 2.3)months.It indicated mycoplasma pneumonia infection in all children with hemoptysis recurrence, and re-angiography showed that more abnormal vessels were found, other minor vessels except for vessels occluded grew thick and large,vessels except for vessels occluded were recanalization in children with recurrent hemoptysis.Conclusion Recurrence is the common complication of hemoptysis related to vascular malformations in children,and bronchial-pulmonary artery fistula is the most common type with hemoptysis recurrence. The main causes of hemoptysis recurrence include mycoplasma pneumonia infection,vessels without occlusion enlargement and recanalization.
6.Correlation Analysis on Cross Section Color of Licorice and Its Active Ingredients

This study was aimed to establish an objective and convenient method to evaluate the quality of licorice through the study on correlation between the cross section color and contents of active ingredients of licorice.Therefore,colorimeter was introduced and applied to determinate cross section color of licorice.Meanwhile,contents of five active ingredients of licorice were also determined.HPLC was used to determine liquiritin and glycyrrhizic acid.Colorimetric method was used to determine total saponins.Ultraviolet spectrophotometry was used to determine total flavonoids.Sulphuric acid-phenol colorimetry was used to determine polysaccharides.Correlation between the cross section color and content determination result was analyzed.The results showed that the correlation coefficient of glycyrrhizic acid content and L* was-0.578,P < 0.001,the correlation coefficient with b* was 0.596,P < 0.001;the correlation coefficient of liquiritin content and L* was-0.503,P =0.002,the correlation coefficient with b* was 0.890,P < 0.001;the correlation coefficient of total flavonoids content and L* was-0.729,P < 0.001,the correlation coefficient with b* was 0.724,P < 0.001;the correlation coefficient of polysaccharides content and L* was 0.230,P =0.190,the correlation coefficient with b* was-0.390,P =0.023;the correlation coefficient of total saponins content and L* was-0.411,P =0.016,the correlation coefficient with b* was 0.738,P < 0.001.It was concluded that the cross section color index of licorice has significant correlation with contents of glycyrrhizic acid,liquiritin,total flavonoids and total saponins.There was no significant correlation with content of polysaccharides.It illustrated the close correlation between cross section color of licorice and its active ingredients.Through the digitalized determination on color,contents of chemical composition in licorice can be initially determined or predicted objectively.It provided a new idea and method for the quality evaluation of Chinese herbal medicine.
