1.Association between glycated albumin/hemoglobin A1c ratio and type 2 diabetes mellitus with metabolic associated fatty liver disease

Objective To investigate the association between glycated albumin/hemoglobin A1c(GA/HbA1c)and type 2 diabetes mellitus(T2DM)complicated with metabolic associated fatty liver disease(MAFLD).Methods A total of 502 hospitalized T2DM patients were enrolled between January 2022 and December 2023 and divided into T2DM group(n=301)and combined with MAFLD(MAFLD,n=201)group.Clinical data were collected,and Logistic regression analyses were performed to evaluate the relationship between GA/HbA1c and MAFLD.Mediation analysis was conducted to assess the role of blood lipids.Results The MAFLD group had higher levels of drinking,waist-to-hip ratio,body mass index,fasting insulin,fasting C-peptide,insulin resistance index,alanine aminotransferase,aspartate aminotransferase,triglycerides(TG),small dense low-density lipoprotein(sdLDL),and blood uric acid compared to the T2DM group(P<0.05).The age,DM duration,GA,GA/HbA1c,and high-density lipoprotein cholesterol(HDL-C)were lower in the MAFLD group than in the T2DM group(P<0.05).Logistic regression analysis showed that after adjusting for confounding factors,GA/HbA1c was an influencing factor for the occurrence of MAFLD in T2DM patients.Mediation analysis revealed that TG,sdLDL,and HDL-C had significant mediating effects,accounting for 19.3%,12.4%,and 8.5%of the risk association,respectively.Conclusions GA/HbA1c is influencing factor of MAFLD in T2DM patients,with blood lipids showing significant mediating effects,suggesting that GA/HbA1c may serve as a novel indicator for assessing MAFLD risk.
2.Improvement of sleep by Bai Ling Long Zao An Shen formula and the mechanisms in insomnia model rats induced by environmental stress

OBJECTIVE To investigate the role of Bai Ling Long Zao An Shen formula(BLLZ)in sleep improvement in an environmental stress-induced insomnia rat model and explore its underlying mechanisms.METHODS(1)Component analysis:the chemical constituents of the BLLZ extract were analyzed using ultra-high performance liquid chromatography-mass spectrometry(UPLC-MS).(2)Eval-uation of the sedative and hypnotic effect:① Mice:50 ICR mice were randomly divided into normal control group,BLLZ-L group(5,10 and 20 g·kg-1)and diazepam group(DZP,3 mg·kg-1).After five days of intragastric administration,pentobarbital sodium-induced righting reflex and locomotor activity tests were performed.② Rats:8 SD rats were implanted with electrodes and allowed to recover for seven days before baseline EEG data was collected over 24 h.A crossover design(7 d washout period)was employed,with rats randomly assigned to the DZP(3 mg·kg-1)and BLLZ(20 g·kg-1)group.After five days of treatment,24 h EEG recordings were obtained.(3)Insomnia model and interventions:①8 SD rats were allowed to recover for seven days post-surgery,followed by 6 h(14:00-20:00)baseline EEG recording.A 3×3 crossover design was used to assign rats to model(environmental stress-induced insomnia),model+DZP,or model+BLLZ groups.After five days of treatment,insomnia was induced by frequent cage changes(14:00,16:00 and 18:00),and EEG changes were monitored.(4)Mechanistic study:32 SD rats were randomly divided into the normal control group,model group,and model+DZP group.After five days of treatment,hypothalamic tissues were collected for biochemi-cal analysis.γ-aminobutyric acid(GABA),glutamate(Glu),and dopamine(DA)levels were measured using biochemical kits while γ aminobutyric acid receptor subunit alpha-1(GABAA1),core clock proteins period circadian regulator 2(PER2)and circadian locomotor output cycles(CLOCK)protein expressions were assessed by Western blotting.RESULTS(1)Compared with the normal control group,the sleep latency of BLLZ 10 and 20 g·kg-1 and DZP groups was significantly shortened,and the locomotor activity of BLLZ 20 g·kg-1 and DZP groups was significantly reduced;BLLZ 20 g·kg-1 signifi-cantly increased the total sleep time,slow-wave sleep time,and average duration of sleep in normal rats,and significantly reduced the wakefulness time.(2)The total sleep time and slow-wave sleep time of the model group significantly decreased and the wakefulness time significantly increased compared with baseline.(3)Compared with the model group,the total sleep time and slow-wave sleep time of the model+BLLZ group and the model+DZP group were significantly increased,and the wakefulness time significantly shortened.(4)Compared with the normal control group,the Glu/GABA ratio,DA content and CLOCK protein expression were significantly increased and GABAA1 and PER2 protein expres-sion were significantly decreased in the model group;compared with the model group,the Glu/GABA ratio,DA content and CLOCK protein expression were significantly decreased,and the expression of GABAA1 and PER2 were significantly increased in the model+BLLLZ group and the model+DZP group.CONCLUSION BLLZ has sedative and hypnotic effects.It can prolong the total slow-wave sleep time by increasing the average duration of slow-wave sleep episodes,thereby increasing the total sleep time and improving environmental stress-induced insomnia.The mechanism may be related to the downregulation of the Glu/GABA ratio and DA levels as well as the enhancement of GABAA1 expressions and the regulation of hypothalamic core clock protein expressions.
3.A comparative study of asymmetrical prominent veins sign in stroke based on susceptibility maps and phase maps

Objective To explore the value of the minimum intensity projection(minIP)images generated by post-processing of susceptibility weighted imaging(SWI)and corrected phase image(CPI)in evaluating the asymmetrical prominent veins sign(APVS)in acute ischemic stroke.Methods A retrospective analysis was conducted on 86 patients with acute ischemic stroke.Group A underwent conventional SWI reconstruction to generate minIP images,while group B used CPI for re-reconstruction to produce minIP images.Both groups used the same scanning method but different post-processing techniques to generate two sets of images,with each group consisted of 86 patients.Two deputy chief physicians of imaging diagnostics scored subjectively with a double-blind 5-point method to compare the ability of the two groups to display APVS and analyze the display rate of APVS.Results The subjective scores of group B were significantly higher than those of group A,with a statistically significant difference(P<0.05).The display rates of APVS in groups A and B were 67.44%and 73.26%respectively.Group B had a higher display rate of APVS below the tentorium cerebelli than above it.Conclusion The minIP images generated by CPI post-processing can achieve the effects similar to phase difference enhanced imaging(PADRE),and is superior to SWI reconstruction method in displaying APVS.It can be used as a supplementary post-processing method when acute stroke shows poor APVS,which has practical clinical application value and can provide more imaging basis for clinical practice.

7.Feasibility analysis and application of ovarian tissue oocyte- in vitro maturation in the patients with malignant tumors undergoing fertility preservation

Methods:OTO-IVM outcomes from 27 malignant tumor patients who underwent fertility preservation in Reproductive Medicine Research Center of the Sixth Affiliated Hospital of Sun Yat-Sen University from March 2017 to August 2022 were analyzed, including the number of mature oocytes (M II oocytes), maturation rate, and maturation time. The fertilization rate, the cleavage rate, and the embryonic development potential of IVM-derived M II were also analyzed. Further, the short-term use of gonadotropin (Gn) in OTO-IVM before ovarian tissue acquisition was initially explored. Results:After OTO-IVM, 81.48% (22/27) of patients had at least one M II oocyte, with a mean number of M II oocytes of 3.00 (1.50, 7.00) and a maturation rate of 38.81%. About 4.85% (13/268) of oocytes matured within 24 h; 14.93% (40/268) matured between 24 h and 36 h; 16.79% (45/268) matured between 36 h and 48 h, and only 2.24% (6/268) matured after 48 h. A total of 41 M II oocytes from 4 patients were fertilized by intracytoplasmic sperm injection (ICSI), with a fertilization rate of 85.37% (35/41), cleavage rate of 94.29% (33/35), variable embryo rate of 54.29% (19/35), and high-quality embryo rate of 34.29% (12/35). The IVM rate was significantly higher in patients who used Gn than in those who did not [63.16% (48/76) vs. 29.17% (56/192), P<0.001]. Conclusion:OTO-IVM can be used as a complementary method for fertility preservation in oncology patients and obtain a certain number of oocytes and embryos. Short-term use of Gn before ovarian tissue acquisition can improve oocyte maturation rate, but further studies are needed to verify the value of Gn in OTO-IVM.

9.Phase I clinical trial of pharmacokinetics and safety comparison between the generic and the original bevacizumab

Objective:To compare the pharmacokinetics and safety of single intravenous injection of the generic bevacizumab injection WBP264 and the original bevacizumab injection Avastin ? in healthy male volunteers. Methods:The study was designed as a randomized, double-blind, single dose, parallel, and controlled phase I clinical trial. Healthy male volunteers who were recruited publicly were randomized into the trial group (intravenous infusion of WBP264) and the control group (intravenous infusion of Avastin ?), and the dose was 3 mg/kg. Peripheral venous blood was collected within 30 minutes before administration, 45 minutes after onset of the administration, immediately after finishing the administration, 2.5, 3.5, 5.5, 9.5, 13.5, 24, 48 hours and on the 5th, 8th, 15th, 22nd, 29th, 36th, 43rd, 57th, 71st, 85th, and 99th days after the administration. The plasma concentration was measured by enzyme-linked immunosorbent assay, the plasma concentration-time curve and its semilogarithmic plot were plotted, and the pharmacokinetic parameters such as the area under the plasma concentration-time curve [including AUC from time zero to the time of the last quantifiable concentration (AUC 0-t) and AUC from time zero to infinity (AUC 0-∞)], peak concentration ( Cmax), time to peak ( Tmax), plasma elimination half-life ( t1/2), clearance rate (CL), and apparent volume of distribution (Vd) were calculated. When the 90% confidence intervals ( CI) of the geometric mean ratio of AUC 0-t, AUC 0-∞, and Cmax between the trial group and the control group were between 0.80-1.25, it indicated that pharmacokinetics of WBP264 and Avastin ? were similar. The physical examination, vital signs detection, electrocardiogram, and laboratory tests were performed on the subjects, the occurrence of adverse events (AEs) and the severity classification were recorded, and correlation between the AEs and the trial drug was evaluated. The anti-drug antibody and its neutralizing antibody were detected before administration and on the 8th, 15th, 29th, 43rd, 71st, and 99th days after administration to evaluate the immunogenicity of the drug. Results:A total of 78 subjects were recruited, 39 in the trial group and 39 in the control group. In the trial group, 2 cases withdrew from the trial (1 case did not take the drug and 1 case withdrew for personal reason after taking the drug). Seventy-seven cases were in the safety analysis set and 76 cases in the pharmacokinetic analysis set. The differences in age, height, weight, and body mass index between the 2 groups were not significant (all P>0.05). The plasma concentration-time curves of bevacizumab between the trial group and the control group were similar. The geometric mean ratios (90% CI) of AUC 0-t, AUC 0-∞, and Cmax were 1.04 (0.98-1.10), 1.03 (0.98-1.10), and 1.09 (1.03-1.14), respectively. The differences in the incidence of overall AEs [89.5% (34/38) vs. 87.2% (34/39)] and the incidence of AEs possibly related to the trial drug [86.8% (33/38) vs. 79.5% (31/39)] between the trial group and the control group were not significant (all P>0.05). Only one case of AE in the trial group was grade 3 in severity and was assessed as being not related to the drug, and the rest were grade 1-2, with grade 1 AEs accounting for the vast majority. The difference in the positive rate of anti-drug antibody between the trial group and the control group was not significant [10.5% (4/38) vs. 10.3% (4/39), P>0.05]. The neutralizing antibody test was negative in the patients with positive anti-drug antibody. Conclusion:The pharmacokinetics and safety of WBP264 and Avastin ? are similar.
10.Puberty induction by autograft of cryopreserved ovarian tissue in a patient with β-thalassemia after hematopoietic stem cell transplantation: a case report and literature review

Objective:To explore the feasibility of autologous transplantation of frozen-thawed ovarian tissue to induce pubertal development in adolescent females.Methods:Before hematopoietic stem cell transplantation in patient with severe β-thalassemia, 11 pieces of ovarian tissue were frozen in the Center of Reproductive Medicine, the Sixth Affiliated Hospital of Sun Yat-sen University in 2019. The patient was diagnosed as premature ovarian failure after hematopoietic stem cell transplantation. There were no signs of puberty development and menarche. Orthotopic ovarian tissue transplantation was performed for the patient through laparoscopy, and a total of 5 pieces of ovarian tissue were transplanted on January 20, 2022. Postoperatively, we followed up the sex hormone levels, growth and development of the patients and menarche.Results:The patient developed menarche 5 months after ovarian transplantation. The levels of sex hormones showed that follicle-stimulating hormone and luteinizing hormone were significantly decreased, and estradiol levels were significantly increased, indicating that ovarian tissue transplantation was successful, and follicles had begun to recruit and develop. The patient's ultrasonography revealed a markedly enlarged uterus and a thickened endometrium. Antral follicles were detected in the left implantation site of pelvic cavity.Conclusion:Cryopreservation of ovarian tissue is recommended for fertility preservation in prepubertal children. Autologous frozen-thawed ovarian tissue transplantation can induce natural puberty development and restore the reproductive endocrine function in children with ovarian failure, delayed puberty development or even stagnation.
