1.Clinical Pathological Features and Prognostic Analysis of Primary Osteosarcoma of the Breast

This study aimed to investigate the clinicopathological characteristics and prognostic factors of primary osteosarcoma of the breast (POB), providing evidence for optimizing diagnostic and therapeutic strategies for this rare malignancy. Clinical data were retrospectively collected from patients with pathologically confirmed breast tumors containing osteosarcomatous components. All patients were treated at the Department of Breast Surgery, Peking Union Medical College Hospital, Chinese Academy of Medical Sciences, between February 2006 and July 2025. Collected data included clinical presentation, imaging findings, pathological features, treatment modalities, and follow-up outcomes. A single-cohort analysis was performed in POB patients, and intergroup comparisons were conducted between POB patients and non-POB patients which were defined as those with other breast malignancies containing osteosarcomatous components, including malignant phyllodes tumor with osteosarcoma (MPTB-OS) and metaplastic breast carcinoma with osteosarcoma (MBC-OS). Disease-free survival (DFS) was assessed using the Kaplan-Meier method to compare prognostic differences between the two groups and evaluate the impact of surgical modalities and adjuvant therapies on DFS in POB patients. A total of 16 female patients were included, aged 37 years-76 years (median, 53 years). Among them, 8 (50.0%) were POB patients, 5 (31.3%) were MPTB-OS patients, and 3 (18.8%) were MBC-OS patients. All patients underwent surgery. Six patients received sentinel lymph node biopsy or axillary lymph node dissection, and none showed nodal metastasis. No deaths occurred during follow-up, and the 1-year and 2-year recurrence or metastasis rates were 37.5% (95% CI: 8.5%-75.5%) and 62.5% (95% CI: 24.5%-91.5%), respectively, whereas those for the non-POB patients were 0 (95% CI: 0-36.9%) and 12.5% (95% CI: 0.3%-52.7%). POB patients exhibited significantly poorer DFS than non-POB patients ( POB patients exhibited a significantly higher risk of recurrence and distant metastasis and poorer DFS than non-POB patients. Survival analysis didn't indicate that mastectomy surgery, axillary lymph node surgery, or postoperative adjuvant therapy was associated with a significant difference in DFS among POB patients.
2.Application value of metagenomic next-generation sequencing in diagnosis of spinal tuberculosis

Objective To explore the application value of metagenomic next-generation sequen-cing(mNGS)technology in the diagnosis of spinal tuberculosis.Methods A total of 129 patients with suspected spinal tuberculosis admitted from January 2021 to January 2023 were selected as study subjects.Lesion tissue samples were collected intraoperatively and subjected to conventional microbio-logical testing(CMT),Mycobacterium tuberculosis DNA(MTB-DNA)amplification testing,and mNGS testing.The diagnostic efficacy of different testing methods was compared using results of com-prehensive clinical diagnosis as the gold standard.Results Among 129 patients,101(78.29%)were confirmed to have spinal tuberculosis,and 28(21.71%)had other spinal infections.Using clinical results as the diagnostic gold standard,the sensitivity of mNGS was 94.06%(95/101),and specificity was 89.29%(25/28);the sensitivity of MTB-DNA amplification was 90.10%(91/101),and specificity was 89.29%(25/28);the sensitivity of CMT was 86.14%(87/101),and specifici-ty was 85.71%(24/28).Compared with MTB-DNA amplification and CMT,mNGS showed the highest consistency with clinical results,and its consistency in detecting different lesion sites was also optimal,with statistically significant differences(P<0.05).Conclusion mNGS testing has high diagnostic value for spinal tuberculosis and can provide a reference for clinical diagnosis,thereby guiding clinical decision-making.
3.Efficacy of combined therapy of promoting blood circulation and removing blood stasis and anisodamine hydrobromide for sepsis-induced coagulopathy

Objective To investigate the clinical efficacy of the combined therapy of promoting blood circulation and removing blood stasis and anisodamine hydrobromide in the treatment of sepsis-induced coagulopathy(SIC).Methods A total of 102 SIC patients treated in our hospital from Sep-tember 2021 to September 2024 were selected as study subjects.They were divided into control group(n=51)and experimental group(n=51)according to different treatment methods.The control group received conventional treatment,while the experimental group received an additional combined therapy of promoting blood circulation and removing blood stasis and anisodamine hydrobromide on the basis of the control group.Coagulation function and thrombotic risk were assessed in both groups before treatment and at 24,48,and 72 h after treatment.Clinical efficacy and treatment safety were com-pared,and lactate clearance rate was measured before treatment and at 2,6,and 24 h after treat-ment.Results After treatment,the levels of thrombin time(TT),prothrombin time(PT),acti-vated partial thromboplastin time(APTT),and D-dimer(D-D)decreased,while the platelet(PLT)count and fibrinogen(FIB)levels increased compared with treatment before,with all differ-ences being statistically significant(P<0.05).At 24 hours post-treatment,the TT,PT,and D-D levels in the experimental group were lower than those in the control group(P<0.05),whereas no significant differences were observed in the remaining indicators(P>0.05).At 48 hours post-treat-ment,the TT,PT,APTT,and D-D levels in the experimental group were lower than those in the control group,while the PLT level was higher(P<0.05).At 72 hours post-treatment,the TT,PT,and APTT levels in the experimental group were lower than those in the control group,whereas the PLT and FIB levels were higher(P<0.05).The levels of the four new thrombotic indicators,namely thrombomodulin(TM),thrombin-antithrombin complex(TAT),plasmin-α2-antiplasmin complex(PIC),and tissue plasminogen activator-plasminogen activator inhibitor complex(t-PAIC),decreased with prolonged treatment duration in both groups,with differences in different time points being statistically significant(P<0.05).At 24 hours post-treatment,the TM and TAT levels in the experimental group were lower than those in the control group,with both differences being statistical-ly significant(P<0.05).At 48 hours post-treatment,the TM,PIC,and t-PAIC levels in the ex-perimental group were lower than those in the control group(P<0.05).At 72 hours post-treat-ment,the levels of four indicators thrombosis in the experimental group were lower than those in the control group,but only the differences in TM and PIC levels were statistically significant(P<0.05).The thromboelastography indicators,including R value,K value,and maximum amplitude(MA)decreased,while α angle increased with prolonged treatment duration in both groups(P<0.05).At 24 and 48 hours post-treatment,the R value in the experimental group was lower than that in the control group,with both differences being statistically significant(P<0.05).At 72 hours post-treatment,the R value,and MA in the experimental group were lower than those in the control group(P<0.05).The Sequential Organ Failure Assessment(SOFA)score,SIC score,and Acute Physiology and Chronic Health Evaluation Ⅱ score decreased post-treatment in both groups compared with pretreatment levels,and were lower in the experimental group than in the con-trol group(P<0.05).The lactate clearance rate increased with prolonged treatment duration in both groups(P<0.05).At 2,6,and 24 hours post-treatment,the lactate clearance rate in the ex-perimental group was higher than that in the control group(P<0.05).The 28-day survival rate was 100.00%in the experimental group,which was higher than 92.16%in the control group(P<0.05).Conclusion The combined therapy of promoting blood circulation and removing blood sta-sis and anisodamine hydrobromide has good clinical efficacy in the treatment of SIC.It can improve patients' coagulation function,reduce thrombotic risk,and has high treatment safety.
4.Relationship between minimal residual disease on the 19th day and prognosis of children with acute B-lymphoblastic leukemia

Objective To explore the relationship between minimal residual disease(MRD)on the 19th day(D19)and prognosis of children with acute B-lymphoblastic leukemia(B-ALL),as well as the correlation with related biological changes.Methods A total of 88 children with B-ALL newly diagnosed in this hospital from April 2016 to April 2020 who met the enrollment conditions were analyzed for induction therapy D19 MRD,overall survival(OS)rate,event-free survival(EFS)rate,chromosome karyotype,fusion gene and mu-tation gene.MRD≥ 0.01%was considered positive,and they were divided into MRD positive group and MRD negative group.The characteristics of OS rate,EFS rate,immunophenotype and molecular biology/cytogenet-ics were compared between the two groups over a period of 3 years.Results The 3-year OS rate and EFS rate of 88 pediatric patients were 92.0%and 86.4%,respectively.The rates of OS rate and EFS rate in MRD posi-tive group were lower than those in MRD negative group,with statistical significance(P<0.05).The detec-tion rate of CD10 in MRD positive group was lower than that in MRD negative group,and the difference was statistically significant(P<0.05).Thirty-two patients(36.4%)detected 8 types of 35 fusion genes.The de-tection rates of BCR-ABL1 and E2A-PBX1 in MRD positive group were higher than those in MRD negative group,and the differences were statistically significant(P<0.05).Among 48 cases(54.5%)of pediatric pa-tients,41 types of 91 mutated genes were detected,and the remaining mutated genes were less than 5 cases.Abnormal karyotype was detected in 18 cases(20.5%),and no mitotic phase was detected in 17 cases.There was no difference in MRD between normal and abnormal karyotype.Binary Logistic regression analysis showed that BCR-ABL1 and E2A-PBX1 were prognostic factors of children with B-ALL.Conclusion The positive D19 MRD is the influential factor of adverse OS and EFS in children with B-ALL.Both E2A-PBX1 and BCR-ABL1 have adverse effects on the prognosis of children with B-ALL.
5.Diagnostic value of serum PDCD5 and ADAMTS13 for myocardial injury after percutaneous coronary intervention in patients with coronary heart disease

Objective To investigate the diagnostic value of serum programmed cell death 5(PDCD5)com-bined with a disintegrin and metalloproteinase with a thrombospondin type 1 motif member 13(ADAMTS13)for myocardial injury after percutaneous coronary intervention(PCI)in patients with coronary heart disease.Methods Totally 150 patients with coronary heart disease in the First Affiliated Hospital of Xi'an Jiaotong University from January 2021 to October 2023 were selected and divided into uninjured group(108 cases)and injured group(42 cases)based on whether myocardial injury occurred after PCI.Fully automated biochemical analyzer was applied to detect levels of creatine kinase(CK),creatine kinase isoenzyme(CK-MB),cardiac tro-ponin I(cTnI),enzyme-linked immunosorbent assay(ELISA)was applied to detect serum levels of PDCD5 and ADAMTS13,and Doppler ultrasound detector was applied to detect left ventricular end diastolic diameter(LVEDD),left ventricular end systolic diameter(LVESD),and left ventricular ejection fraction(LVEF).In addition,SYNergy between percutaneous coronary intervention with TAXus and cardiac surgery(SYNTAX)score was used to evaluate the severity of coronary artery disease.Multivariate Logistic regression was applied to analyze the influencing factors of myocardial injury after PCI.Pearson correlation was used to analyze the correlation between PDCD5,ADAMTS13 levels with myocardial enzyme and cardiac function indicators.Re-ceiver operating characteristic(ROC)curve was applied to analyze the diagnostic value of PDCD5 and AD-AMTS13 in patients with myocardial injury after PCI.Results Compared with the non injury group,the inju-ry group had higher levels of cTnI、CK-MB、LVEDD、LVESD、SYNTAX score and PDCD5(P<0.05),and lower LVEF、ADAMTS13(P<0.05).Multivariate Logistic regression analysis showed that elevated cTnI(OR=1.862)and PDCD5(OR=1.615)were independent risk factors for myocardial injury(P<O.05),while elevated LVEF(OR=0.652)and ADAMTS13(OR=0.426)were independent protective factors for myocardial injury(P<0.05).Correlation analysis shows that level of PDCD5 was positively correlated with cTnI,CK,CK-MB,LVEDD,LVESD and SYNTAX score,and negatively correlated with LVEF,while level of ADAMTS13 was negatively correlated with cTnI,CK,CK-MB,LVEDD,LVESD and SYNTAX score,and positively correlated with LVEF(P<0.05).ROC curve analysis showed the area under the curve(AUC)for diagnosing myocardial injury with PDCD5 and ADAMTS13 was larger than single diagnosis(P<0.05).Con-clusion Patients with myocardial injury have elevated level of PDCD5 and decreased level of ADAMTS13,which are correlated with myocardial enzyme and cardiac function indicators.The combined diagnosis of myo-cardial injury in patients with coronary heart disease after PCI has certain value.
6.Association between serum PAGln,SCFAs levels and ventricular remodeling and prognosis in patients with chronic heart failure

Objective To investigate the relationship between serum phenylacetylglutamine(PAGln),short-chain fatty acids(SCFAs)levels and ventricular remodeling and prognosis in patients with chronic heart failure(CHF).Methods A total of 267 CHF patients treated at Jiulongpo District People's Hospital of Chongqing from January 2019 to December 2022 were selected as the CHF group,and 100 healthy individuals underwent physical examination during the same period were selected as the control group.CHF patients were further divided into a poor prognosis group(n=63)and a good prognosis group(n=204)based on 1-year outcomes.Serum levels of PAGln and SCFAs(acetic acid,propionic acid,butyric acid)were measured using liquid chromatography-tandem mass spectrometry.Echocardiography was used to assess ventricular remode-ling indicators including left ventricular mass index(LVMI),interventricular septal thickness in diastole(IVST),left ventricular posterior wall thickness(LVPWT),and left ventricular end-diastolic diameter(LVEDD).Pearson correlation was used to analyze the associations between serum PAGln,SCFAs levels and LVMI,IVST,LVPWT,LVEDD.Multivariate logistic regression was used to identify risk factors for poor prognosis in CHF patients,and receiver operating characteristic(ROC)curves were used to assess the predic-tive value of PAGln and SCFAs levels.Results Serum PAGln levels were significantly higher in the CHF group than in the control group,while acetic acid,propionic acid,and butyric acid levels were significantly low-er(P<0.05).LVMI,IVST,LVPWT,and LVEDD were significantly elevated in the CHF group compared to the control group(P<0.05).Serum PAGln levels were positively correlated with LVMI,IVST,LVPWT,and LVEDD(P<0.05),while SCFAs levels were negatively correlated with these parameters(P<0.05).Univa-riate analysis showed that age,duration of CHF,NYHA functional classification,atrial fibrillation,LVEF,NT-proBNP,PAGln,acetic acid,propionic acid,and butyric acid were associated with poor prognosis(P<0.05).Multivariate analysis identified NYHA class Ⅲ-Ⅳ,atrial fibrillation,elevated NT-proBNP,and elevated PA-Gln as independent risk factors for poor prognosis,while higher LVEF and increased levels of acetic acid,pro-pionic acid,and butyric acid were independent protective factors(P<0.05).ROC curve analysis showed that the area under the curve(AUC)for the combined prediction using PAGln and SCFAs was 0.881,which was greater than that for PAGln(0.800)or SCFAs(0.789)alone(P<0.05).Conclusion Elevated serum PAGln and decreased SCFAs levels are closely associated with ventricular remodeling and poor prognosis in CHF pa-tients.The combination of serum PAGln and SCFAs levels has a high predictive value for poor prognosis in CHF.
7.A scoping review of research on toxic leadership behavior among nursing managers

Objective:To conduct a comprehensive global review of research on toxic leadership behavior among nursing managers and to analyze the current research status.Methods:Literature related to toxic leadership behavior among nursing managers was retrieved from PubMed, Web of Science, Cochrane Library, China National Knowledge Infrastructure, Wanfang Database, VIP Chinese Science and Technology Journal Database, and China Biomedical Literature Database. The search period extended from the inception of each database to February 15, 2025. The included literature was then organized, analyzed, and summarized.Results:A total of 28 studies were included. There were 23 studies reported the occurrence of toxic leadership behavior among nursing managers, the incidence of toxic leadership behavior was almost at a moderately low level. There were 8 studies reported the influencing factors of toxic leadership behavior, its influencing factors encompassed three primary aspects: the organizational level, the personal characteristics of nursing managers, and characteristics of the nursing staff group. Assessment tools of toxic leadership behavior included the Toxic Leadership Behaviors of Nurse Managers, the Abuse Management Questionnaire, etc. The adverse consequences included increased nurse turnover intention, decreased nursing quality, and reduced organizational trust, etc. Interventions were planned and implemented from three perspectives: the hospital managers, nursing managers, and the nurses.Conclusions:The factors influencing toxic leadership behavior among nursing managers and its adverse consequences require further exploration. Therefore, more intervention studies are warranted to provide new insights and methods for preventing and reducing toxic leadership behavior.
8.Analysis of factors affecting long-term survival in patients with anaplastic thyroid carcinoma and the efficacy of immunotherapy

Objective:To explore the long-term survival outcomes of patients with anaplastic thyroid cancer (ATC) and analyze key factors influencing the prognosis.Methods:A retrospective analysis was conducted on the clinical and follow-up data of 77 ATC patients treated at the Sun Yat-sen University Cancer Center from March 2000 to July 2022, with tumor-specific survival as the primary endpoint. The Kaplan-Meier method was used to plot the survival curves, and univariate and multivariate Cox regression analyses were performed to identify the prognostic factors.Results:Among the 77 patients, 64 underwent surgical treatment, with 33 receiving surgery alone, 8 undergoing surgery combined with chemotherapy, 13 undergoing surgery with radiotherapy, 1 undergoing surgery with chemotherapy and radiotherapy, 2 receiving surgery combined with chemotherapy and targeted therapy, 3 receiving surgery with targeted therapy, and 4 receiving surgery with immunotherapy and targeted therapy. Among the 13 patients who did not undergo surgery, 2 received chemotherapy alone, 3 received targeted therapy alone, 1 received immunotherapy alone, 1 received chemoradiotherapy, 5 received chemotherapy combined with immunotherapy, and 1 received immunotherapy combined with targeted therapy. The median follow-up time was 8.4 months, with 58 patients (75.3%) died, and the median survival time was 6.63 months. Univariate Cox regression analysis showed that C-reactive protein, monocyte count, lymphocyte count, abnormal albumin levels, the maximum diameter of the primary tumor, BMI, and whether immunotherapy was administered were significantly associated with survival in ATC patients (all P<0.05). Multivariate Cox regression analysis indicated that immunotherapy was an independent factor for survival in ATC patients ( HR=0.18, 95% CI: 0.05-0.62, P=0.007). Among the 40 patients admitted after 2015, the 11 patients who received immunotherapy had a median survival time of 17.2 months, which was superior to the 29 patients who did not receive this treatment (median survival time 6.2 months, P=0.03). Conclusions:ATC patients receiving immunotherapy had a better prognosis and longer survival. Additionally, elevated C-reactive protein, abnormal albumin, monocyte count, lymphocyte count, and BMI might be associated with poorer prognosis in ATC. Tailoring treatment based on the individual characteristics of ATC patients may be beneficial for their long-term survival.
9.Expression levels and clinical significance of serum lncRNA CASC11 and lncRNA THOR in patients with esophageal cancer

Objective To investigate the expression levels and clinical significance of serum long chain non coding RNA cancer susceptibility candidate 11(lncRNA CASC11)and Testis-associated highly-conserved oncogenic long non-coding RNA(lncRNA THOR)in esophageal cancer patients.Methods Esophageal cancer patients diagnosed in our hospital from January 2017 to January 2021 were selected as the observation group(n=129),and healthy individuals who underwent physical examination in our hospital(n=129)were as the control group.Serum lncRNA CASC11 and lncRNA THOR expression levels were detected by RT-qPCR;the 3-year survival rate was analyzed by Kaplan-Meier method;and the diagnostic value was analyzed by ROC curve.Results The expression levels of serum lncRNA CASC11(1.48±0.35 vs 1.01±0.20)and lncRNA THOR(1.54±0.28 vs 1.11±0.15)in the observation group were obviously higher than those in the control group(P<0.05).The proportions of high expression of serum lncRNA CASC11 and lncRNA THOR in patients with infiltration depth of T3~T4,tumor diameter>4 cm,TNM staging of Ⅲ-Ⅳ,and lymph node metastasis were obviously higher than those in patients with infiltration depth T1~T2,tumor diameter ≤ 4 cm,TNM staging Ⅰ-Ⅱ,and no lymph node metastasis(P<0.05).After 3-year of follow-up,52 out of 129 esophageal cancer patients died and 77 survived.The 3-year survival rate of patients with high expression of lncRNA CASC11(32.20%vs 82.86%)and lncRNA THOR(36.07%vs 80.88%)was obviously lower than that of patients with low expression of lncRNA CASC11 and lncRNA THOR(P<0.05).The area under the curve(AUC)of serum lncRNA CASC11 and lncRNA THOR levels for diagnosing esophageal cancer was 0.888 and 0.914,respectively,with cutoff value of 1.29 and 1.32.The AUC of the combined diagnosis of the two was 0.946,and the combined diagnosis of the two was superior to their individual diagnosis(Zcombination-lncRNA CASC11=2.410,Zcombination-lncRNA THOR=2.167,P=0.010,0.040).Conclusion The serum levels of lncRNA CASC11 and lncRNA THOR in patients with esophageal cancer are upregulated,they are obviously associated with the 3-year survival rate of patients,and the combination of the two has higher efficacy in the diagnosis of esophageal cancer.
