Objective:To formulate the maintenance and management process of extracorporeal membrane oxygenation (ECMO) equipment for adults and explore its implementation effect, in order to provide reference for clinical nursing.Methods:A historical control study design was used. A total of 60 patients who received ECMO treatment at the Department of Critical Care Medicine of Union Hospital, Tongji Medical College, Huazhong University of Science and Technology, were selected by convenience sampling method as the research subjects. Among them, 30 patients from January to December 2022 were the control group, and 30 patients from 1 to 12 of 2023 were the observation group. The control group received ECMO treatment using routine methods, while the observation group received ECMO treatment according to the maintenance and management process for adult ECMO equipment. The charging time of the ECMO battery, the completeness of the ECMO equipment, the occurrence rate of ECMO equipment failure, the cleanliness rate of the ECMO equipment surface, and the pre-filling time of the ECMO pipeline were compared between the two groups.Results:The control group included 19 males and 11 females, with an age of (51.34 ± 6.85) years. The observation group comprised 21 males and 9 females, with an age of (50.97 ± 5.39) years. In the observation group, the compliance rate of the ECMO battery standby time was observed to be 100.00% (30/30), while the completeness rate of the equipment ready for use was 96.67% (29/30), and the qualification rate of surface cleanliness of ECMO equipment was 93.33% (28/30), all were superior to 76.67% (23/30), 73.33% (22/30), and 63.33% (19/30) in the control group, and the differences were statistically significant ( χ2 = 5.82, 4.71, 7.95, all P<0.05). The incidence rate of ECMO equipment failure in the observation group was 3.33% (1/30), a rate lower than the control group′s 16.67% (5/30), and the difference was statistically significant ( χ2 = 83.18, P<0.001). The pre-filling time of ECMO circuit in the observation group was (9.56 ± 2.20) min, which was less than the control group′s (13.30 ± 2.76) min, and the difference was statistically significant ( t = 5.79, P<0.001). Conclusions:The formulation and implementation of the process help to improve the standardized operation of ECMO in medical care, so that the ECMO equipment and instruments can be well maintained.

Objective:To explore application value of three-dimensional (3D) echocardiography in mitral insufficiency (MI). Methods:A total of 180 MI patients who admitted to The Affiliated Hospital of Xuzhou Medical University from January 2023 to November 2023 were selected,and they were divided into a severe group (n=130) and a mild group (n=50) based on the MI severity. Both groups received routine echocardiography and 3D echocardiography to compare the parameters of routine echocardiography,and analyze the diagnostic efficacy of echocardiography for left ventricular configuration and systolic function in MI patients. Results:The heart rate and QRS interval of patients in the severe group were respectively higher than those in the mild group (t=3.910,6.336,P<0.05),respectively. The left ventricular end diastolic diameter (LVEDD),LVEDD index,left ventricular (LV) length,LV length index,left ventricular end diastolic volume (LVEDV),LVEDV index,left ventricular end systolic volume (LVESV),LVESV index,LV mass index (LVMI),relative wall thickness (RWT),and two-dimensional echocardiogram sphericity index (2D-SI) in the severe group were all higher than those in the mild group (t=27.729,14.509,11.745,8.182,24.171,32.989,25.988,20.994,22.707,6.331,20.603,P<0.05). The three-dimensional left ventricular ejection fraction (3D-LVEF) of the severe group was lower than that of the mild group,and the three-dimensional end diastolic velocity (3D-EDV) of blood flow,3D-EDV index,and three-dimensional echocardiogram sphericity index (3D-SI) of the severe group were all higher than those of the mild group (t=2.855,24.320,10.409,36.056,P<0.05). The results of receiver operating characteristic (ROC) curve showed that the sensitivities of LVEDD index,LVEDV index,LVESV index,3D-LVEF,3D-EDV index,and 3D-SI were respectively 70.83％,68.47％,81.25％,71.58％,72.42％ and 90.38％ in diagnosing MI. The specificities of them were respectively 67.40％,65.06％,79.64％,68.10％,71.68％ and 90.38％. The area under curve (AUC) values of the ROC curve were respectively 0.658,0.612,0.814,0.726,0.799 and 0.852. 3D-SI had the highest sensitivity and specificity in diagnosing left ventricular remodeling of MI patients. Conclusion:3D echocardiography has higher application value in MI,which can reflect the remodeling and systolic function of left ventricle. Among of them,3D-SI can better describe the remodeling of left ventricle of patients,and can serve as a reference for the research progress of clinical observation for MI.

Objective To explore the current status and relevance of job burnout and social support among medical staff in the community of Hospitals under the control of Epidemic prevention and control and to provide scientific basis for targeted intervention measures. Methods The job burnout scale and social support scale were used to investigate and analyze the job burnout and social support of medical staff in Dongxihu District, Wuhan. Results Among 294 medical staff, 163 cases (55.44%) had occupational burnout, among which, mild to moderate and severe occupational burnout accounted for 46.26% and 9.18% respectively. The results of partial correlation analysis showed that the total score and various dimensions of the subject's job burnout were negatively correlated with the total score of the subject's social support, subjective support, and support utilization ( P<0.05). Conclusion In the context of normalized epidemic prevention and control of Epidemic prevention and control, medical staff suffered from severe job burnout; the social support of medical staff may affect their job burnout.

Objective:To quantitatively evaluate the change of the long axis angle and the relative displacement of the crown feature points of the posterior teeth under normal bite force utilizing an intraoral scanner, and to provide clinical reference.Methods:From May to December 2019, fifteen graduate volunteers (5 males and 10 females, aged from 22 to 30, with an average age of 25.7 years) from Peking University School and Hospital of Stomatology were recruited to participate in the study. The surface data (U1, L1) of the maxillary and mandibular posterior teeth were scanned by an intraoral scanner i500, and saved as original data. The volunteers were guided to bite in the intercuspal position with normal bite force. The buccal bite data of the posterior teeth were scanned as the basis for registration. The digital casts were imported into Geomagic studio 2013 software and the boundary lines along the gingival margin and mesial and distal contact area of posterior teeth of data (U1, L1) were determined. Long axis of the crown, crown centroid and mesial functional cusp vertex were establishd. The data (U1, L1) were segmented into single tooth. Single tooth was aligned to buccal bite data separately using best-fit alignment command based on the buccal common area of the crown and new casts data (U2, L2) were obtained as the data under bite force. The long axis angle and centroid distance between adjacent teeth (second premolar and first molar, second molar and first molar) were measured and the deviation between data obtained at mouth-open state and that at biting state was calculated. Negative value meant centroid distance became shorter under bite force. The first molar was set as the common area and registrate the U1, L1 to U2, L2. The angle of long axes, and displacement of centroid and of functional cusp vertex between second premolar of two casts were calculated, as well as between second molar of two casts. Wilcoxon signed rank test was used to analyze the differences of teeth displacement between second premolar and second molar, and between maxillary and mandibular jaws with SPSS 26.0 statistical software.Results:The result of second premolar and second molar in the same jaw had no statistical difference ( P>0.05). The centroid distance deviation of mandibular second premolar-first molar [-0.022(0.046) mm] was larger than that of maxilla [-0.006 (0.040) mm] ( P<0.05). The long axis angle of second premolar itself [0.913°(0.647°)] and centroid distance of second molar itself [0.102 (0.106) mm] on the mandibular jaw were different from that on the maxillary jaw, which were 0.590°(0.550°) and 0.074(0.060) mm respectively ( P<0.05). Conclusions:Under bite force, displacement of the second premolar and second molar was present, including the displacement of centroids and deflection of long axes. The mandibular posterior teeth have larger displacement than the maxillary teeth.

Objective: To investigate the clinical effect of restoration of posterior teeth with machinable CAD/CAM resin hybrid ceramic (VITA Enamic) onlays after root canal treatment. Methods : 136 posterior teeth restored after root canal treatment in our hospital were selected as the research subjects. The posterior teeth were restored chairside with CAD / CAM ceramic onlays to recover occlusion relation and protect residual dental tissues. With 68 teeth in each group, one group was restored with a machinable CAD / CAM resin hybrid ceramic (VITA Enamic) while the control group received zirconia-reinforced lithium silicate ceramic (VITA Suprinity). Before bonding, all the onlays were examined for good adjacency, fitness and occlusion, and then the adhesive surface was etched with hydrofluoric acids. After isolating the rubber dams in the mouths, the onlays were bonded with dual-cure resin cements. The patients were followed up for 24 months to compare the degree of prosthesis integrity, marginal fitness, gingival health, dental integrity, color matching and secondary caries. Results : After restoration, all the onlays were in harmony with the surrounding teeth with good aesthetic effect and suitable marginal fit. Two years later, there were no significant differences in the marginal fitness, gingival health, dental integrity or secondary cavities between the two groups (P > 0.05). In terms of restoration integrity, the VITA Enamic onlays (100%) were intact and displayed good retention. The group that received VITA Suprinity had 6 onlay (9.23%) fractures and a success rate of 90.77%; however, the residual dental tissues did not break. The difference of prosthesis integrity between the two groups was statistically significant (χ2= 4.45, P < 0.05). Conclusion Chairside CAD/CAM resin hybrid ceramic (VITA Enamic) onlays can quickly restore posterior teeth after root canal therapy and better protect the residual dental tissue.

Objective To observe the effect of Shenmai injection combined with enteral nutrition (EN) on immune function in patients with severe cardiac insufficiency. Methods Fifty-seven patients with severe cardiac insufficiency admitted to the Department of Critical Care Medicine of Taizhou Hospital of Zhejiang Province from June 2015 to June 2018 were divided into an EN group (31 cases) and an EN group combined with Shenmai injection group (26 cases). The EN group was given EN on the basis of routine western medicine treatment, while in the EN combined with Shenmai injection group was treated additionally by intravenous drip of Shenmai injection 100 mL/d on the basis of above EN group treatment. The efficacies of the two groups were evaluated after consecutive 7-day treatment in the two groups. The changes in levels of subsets of T-lymphocytes (CD3+, CD4+, CD8+, CD4+/CD8+) and immunosuppressive cells CD14+ monocyte human leukocyte antigen DR (HLA-DR) were observed before and after treatment. Results After treatment, the levels of T-cell subsets CD3+, CD4+, CD4+/CD8+ and CD14+ monocytes HLA-DR in the peripheral blood of the two groups were significantly higher than those before treatment [CD3+: EN group was 0.539±0.126 vs. 0.379±0.093,Shenmai injection group was 0.652±0.185 vs. 0.393±0.091; CD4+: EN group was 0.402±0.121 vs. 0.275±0.066,Shenmai injection group was 0.524±0.168 vs. 0.281±0.077; CD4+/CD8+:EN group was 1.83±0.70 vs. 1.11±0.70,Shenmai injection group was 2.81±0.91 vs. 1.19±0.58; CD14+HLA-DR:EN group was (43.3±7.1)% vs. (35.4±5.7)%,Shenmai injection group was (54.9±6.2)% vs. (36.1±8.3)%]; After treatment, CD8+ in EN group decreased (0.223±0.052 vs. 0.253±0.081), while CD8+ in shenmai injection group increased (0.288±0.051 vs. 0.259±0.078), and the increase degrees of the above-mentioned indexes in EN combined with Shenmai injection group were more obvious than those in the EN group after treatment [CD3+: 0.652±0.185 vs. 0.539±0.126, CD4+: 0.524±0.168 vs. 0.402±0.121, CD8+: 0.288±0.051 vs. 0.223±0.052, CD4+/CD8+: 2.81±0.91 vs. 1.83±0.70, CD14+HLA-DR: (54.9±6.2)%, (43.3±7.1)%, all P < 0.05]. Conclusion The combined use of Shenmai injection and early EN can improve the immune function of T-lymphocytes in patients with severe cardiac insufficiency. The mechanism may be related to the enhancement of the activation of T lymphocytes and promotion of the CD14+ monocytes increase and immune function.

BACKGROUND: Thoracoscopic surgery has gradually become the major procedure for lung cancer surgery in our department. Its characteristics are minimal trauma and quick recovery, which make approximately 90% of patients discharge from the hospital after surgery. However, the postoperative complications still happen now and then. We analyzed the patients who had been hospitalized for longer than 7 days after thoracoscopic lung cancer surgery, aiming to summarize the types and risk factors of complications, and improve postoperative safety of patients. METHODS: The data were come from the prospective database of Thoracic Surgery Unit One in Peking Cancer Hospital, and patients that underwent thoracoscopic pulmonary surgery between Jan. 2010 and Dec. 2014 with length of stay more than 7 days were included in the study. The classifications of the complications were investigated and graded as mild or severe complications according to modified Claviengrading, the relationship between clinical factors and degrees of complications was also analyzed. RESULTS: The hospitalization of 115 cases were longer than 7 days after surgery, accounting for 10.3% (115/1,112) of the whole patients that underwent surgery during the same period. Eighty-one cases had mild complications, accounting for 7.3% (81/1,112) of the whole cases that underwent surgery during the same period and 70.4% (81/115) of the cases with prolonged length of stay; the proportions of severe complications in both groups were 3.1% (34/1,112) and 29.6% (34/115), respectively; and the proportions of complications that caused perioperative deaths were 0.18% (2/1112) and 1.7% (2/115), respectively. Among all the postoperative complications, the most common was air leakage for more than 5 days after surgery, with a total of 20 cases (1.8% and 17.4%). The other common complications were: atelectasis (19 cases, 1.7% and 16.5%), pulmonary infection (18 cases, 1.6% and 15.7%), etc. The less common complications was bronchopleural fistula (4 cases, 0.36% and 3.5%) with very high risk, and 2 cases died perioperatively due to the combination of acute respiratory distresssyndrome (ARDS). In the clinical factors, only preoperative low pulmonary function (FEV1%<70%) was the potential risk factor for postoperative severe complications (45.8% vs 23.6%, P=0.038). There was no significant difference either regarding the 5 year disease free survival or the 5 year overall survival between mild complication group and severe complication group, with 5 year DFS being 52.2% and 51.9%, respectively (P=0.894) , and 5 year overall survival being 64.0% and 53.5%, respectively (P=0.673). CONCLUSIONS Continuous postoperative air leakage, atelectasis and pulmonary infections were the major causes for prolonged hospitalization after thoracoscopic surgery for lung cancer, and bronchopleural fistula was the most perilous complications. Patients with low preoperative pulmonary function were more likely to have severe postoperative complication, however, this would not influence the long term survival of the patients.

BACKGROUND: Currently, there is no consensus on the follow-up strategy (follow-up time interval and content) of non-small cell lung cancer (NSCLC) in the world, and the relevant clinical evidence is also very limited. In this study, we aimed to summarize the recurrence/metastasis sites and timings of stage I NSCLC patients based on their follow-up data, aiming to provide a basis of follow-up time interval and content for this group of patients. METHODS: We retrospectively analyzed the 416 stage I NSCLC patients that underwent continuous anatomic lobectomy between Jan. 2000 to Oct. 2013 in our prospective lung cancer database. According to the recurrence/metastasis sites and timings, the long term follow-up time interval and content were explored. RESULTS: The 5-yr disease free survival (DFS) and overall survival (OS) in the whole group were 82.4% and 85.4%, respectively. There were 76 cases (18.3%) had recurrence/metastasis during follow-up, among which the most frequent site was pulmonary metastasis (21 cases, 5.0%), followed by brain metastasis (20 cases, 4.8%), bone metastasis (12 cases, 2.9%), and mediastinal lymph node metastasis (12 cases, 2.9%). Among the factors that could influence recurrence/metastasis, patients with pT2a suffered from a higher recurrence/metastasis rate compared to patients with pT1 (P=0.006), with 5-yr DFS being 73.8% and 87.3%, respectively (P=0.002), and the 5-yr OS being 77.7% and 90.3%, respectively (P=0.011). CONCLUSIONS The commonest recurrence/metastasis sites of stage I NSCLC after anatomic lobectomy are lung, brain and mediastinal lymph nodes, the risk of recurrence/metastasis within 2 years were equal to that between 3 years and 5 years. The follow-up frequencies and content within 2 years could be adjusted according to T stages.
