Radical resection of mid-and low-rectal cancer requires not only oncologic safety but also preservation of organs and postoperative bowel function. While a 1-2 cm distal resection margin has been largely accepted, the optimal length of the proximal margin remains highly controversial.Clinically,the"10-cm rule"derived from colon cancer is often referenced, yet its applicability to rectal cancer lacks consistent supporting evidence.Previous studies have shown that an excessively long proximal margin may increase anastomotic tension and lead to anastomotic leakage, whereas insufficient resection heightens the risk of positive margins and local recurrence. In addition, the extent of lymph node metastasis, vascular perfusion of the proximal bowel,radiation-induced injury after neoadjuvant chemoradiotherapy, and postoperative bowel function-particularly low anterior resection syndrome-are all important factors influencing the selection of the proximal margin. In recent years, the application of indocyanine green fluorescence imaging has provided new evidence for intraoperative assessment of bowel perfusion;for patients receiving neoadjuvant chemoradiotherapy, radiation injury presents a gradient pattern, and resecting approximately≥20 cm proximal to the tumor may reduce the incidence of anastomosis-related complications.Based on current literature, this review provides a systematic overview of the historical evolution, influencing factors, and clinical evidence regarding proximal resection margins in rectal cancer surgery, with the aim of informing individualized margin selection and optimizing surgical strategies.

Objective To explore the prognostic value of neuroimaging score in patients with acute vertebrobasilar artery occlusion(VBAO)with successful revascularization. Methods A total of 83 patients with acute VBAO who underwent endovascular treatment with successful recanalization, modified thrombolysis for cerebral infarction grade 2b or 3 were retrospectively selected.Based on baseline diffusion weighted imaging(DWI)sequences, posterior circulation infarcts were assessed using the novel posterior circulation score(N-PCS),brainstem score(BSS),and posterior circulation Alberta stroke program early CT score(pc-ASPECTS).A 90 d modified Rankin scale(mRS)score ≤2 points was defined as functional independence. The predictive value of the three scores was assessed by the receiver operating characteristic(ROC)curve analysis. Results The N-PCS[median(interquartile range):2(1-2)vs 4(2-5)]was statistically significant between the functional independent group(50 cases)and the poor group(33 cases)(P<0.001).Multifactorial logistic regression analysis showed that age[adjusted odds ratio(OR)1.08;95%confidence interval(CI)1.02-1.14;P=0.006],baseline National Institutes of Health Stroke Scale(NIHSS)(adjusted OR 1.12;95%CI 1.03-1.20;P=0.005),and N-PCS(adjusted OR 2.84;95%CI 1.30-6.19;P=0.009)were independent prognostic factors for a favorable prognosis. Conclusion The N-PCS based on DWI can predict the prognosis of acute VBAO patients with successful revascularization.
5.Progress of morphine in neoplasms
Cancer Research and Clinic 2024;36(8):637-640
Pain is one of the most common symptoms of tumors for cancer patients, especially those with advanced tumors. As a classic opioid that can relieve pain, morphine is widely applied in cancer patients. However, morphine has certain limitations in clinical application, for example, the long-term usage of morphine can result in analgesic tolerance and hyperalgesia, cause resistance to other chemotherapy drugs, and weaken the efficacy of chemotherapy drugs. When morphine is combined with some drugs, the adverse effects of morphine can be reduced. And the effect of morphine on tumor immunity, growth, metastasis, angiogenesis and apoptosis is also complex, and different concentrations and experimental models will have an impact on the efficacy of morphine, and the specific mechanism of these effects needs to be further explored. This paper reviews the characteristics, clinical use and the influence of morphine on cancer development.

The increasing number of pulmonary nodules being detected by computed tomography scans significantly increase the workload of the radiologists for scan interpretation. Limitations of traditional methods for differential diagnosis of pulmonary nodules have been increasingly prominent. Artificial intelligence (AI) has the potential to increase the efficiency of discrimination and invasiveness classification for pulmonary nodules and lead to effective nodule management. Chinese Experts Consensus on Artificial Intelligence Assisted Management for Pulmonary Nodule (2022 Version) has been officially released recently. This article closely follows the context, significance, core implications, and the impact of future AI-assisted management on the diagnosis and treatment of pulmonary nodules. It is hoped that through our joint efforts, we can promote the standardization of management for pulmonary nodules and strive to improve the long-term survival and postoperative life quality of patients with lung cancer.

The revised revision of the Shenzhen Special Economic Zone Medical Regulations passed in 2022 is an important institutional innovation in China, while many problems still need to be solved in the specific practice and application. This paper compared the practical development characteristics of the living will system between China and Japan, and analyzed the relationship between doctor-patients relationship under the intervention of the living will system and the symbolic representation mechanism of living will by using the semiotics theory. According to the links of "addresser-symbolic text" and "symbolic text-addressee", the six types of problems faced by current living will system in China were discussed, and the relevant experience and enlightenment were summarized to provided reference for the development of the living will system in China.
9.Progress of guanylate binding protein 5 in tumors
Cancer Research and Clinic 2023;35(9):709-712
Malignant tumor ranks as a leading cause of human death. Guanylate binding protein 5 (GBP5) belongs to the family of interferon-γ-inducible large GTPase superfamily and participates in various pathological and physiological processes. A growing number of studies have indicated that GBP5 is involved in the malignant progression of tumors. This article mainly reviews the biological characteristics of GBP5 and its expression, regulation and function in tumors, in the hope of providing a theoretical basis for the diagnosis and treatment of tumors.

Based on polyketide syntheses gene (PKS) and non-ribosomal peptide synthetases gene (NRPS), one strain with high anti-pathogenic activity was screened from 77 strains isolated from Arctic marine sediments and identified. By optimizing the composition of culture medium and fermentation conditions, the production of this strain's active metabolites was improved and the main metabolites were identified by HRMS, 1H NMR and 13C NMR. The antibacterial spectrum of the main metabolites and the effect of the metabolites on cucumber Fusarium wilt were also determined. The results showed that the strain was Bacillus velezensis and it showed growth promoting effect on plants. When the strain was cultured in 5 g/L maltose, 10 g/L tryptone, 10 g/L sodium chloride, at 30 ℃, 150 r/min for 60 h, the diameter of the inhibition zone increased from (16.23±0.42) to (24.42±0.57) mm. The metabolites of this strain mainly contain macrolide compound macrolactin A, which has antagonistic effect on a variety of pathogenic bacteria and fungi. Cucumber seedling experiments showed that the metabolites of this strain had a protective effect on cucumber Fusarium wilt, and showed a good potential for development and application as a biocontrol agent.
