The collateral disease theory stands as one of the most instructive frameworks for elucidating the pathology of Mycoplasma pneumoniae pneumonia (MPP). Its characteristic pathological features—toxin damaging collaterals, dysfunction of Qi collaterals, and protracted illness involving collaterals—closely align with the manifestations of cytokine storms, immune dysregulation, and chronic damage induced by M. pneumoniae (MP). Drug-resistant MP further exacerbates these manifestations and poses challenges in clinical treatment, making it essential to clarify the intrinsic relationship between the collateral disease theory and MPP. However, existing studies still lack relevant elaboration, resulting in a gap in guidance by traditional Chinese medicine (TCM) theory in clinical practice. Considering the instructive, scientific, and necessary role of the collateral disease theory in MPP, this paper thoroughly elucidates the pathological manifestations of MPP and the core evolutionary characteristics of collateral damage, and further provides a scientific interpretation from the perspective of collateral disease on the clinical efficacy and mechanism of TCM treatment of MPP over the past five years. This review aims to reveal the intrinsic relationship between the collateral disease theory and MPP, enrich the scientific connotation of TCM theory, and bridge the gap in TCM theoretical guidance, thereby providing scientific research clues for TCM treatment of MPP.
2.Changes in the body shape and ergonomic compatibility for functional dimensions of desks and chairs for students in Harbin during 2010-2024
Chinese Journal of School Health 2025;46(3):315-320
Objective:
To analyze the change trends in the body shape indicators and proportions of students in Harbin from 2010 to 2024, and to investigate ergonomic compatibility of functional dimensions of school desks and chairs with current student shape indicators, so as to provide a reference for revising furniture standards of desks and chairs.
Methods:
Between September and November of both 2010 and 2024, a combination of convenience sampling and stratified cluster random sampling was conducted across three districts in Harbin, yielding samples of 6 590 and 6 252 students, respectively. Anthropometric shape indicators cluding height, sitting height, crus length, and thigh length-and their proportional changes were compared over the 15-year period. The 2024 data were compared with current standard functional dimensions of school furniture. The statistical analysis incorporated t-test and Mann-Whitney U- test.
Results:
From 2010 to 2024, average height increased by 1.8 cm for boys and 1.5 cm for girls; sitting height increased by 1.5 cm for both genders; crus length increased by 0.3 cm for boys and 0.4 cm for girls; and thigh length increased by 0.5 cm for both genders. The ratios of sitting height to height, and sitting height to leg length increased by less than 0.1 . The difference between desk chair height and 1/3 sitting height ranged from 0.4-0.8 cm. Among students matched with size 0 desks and chairs, 22.0% had a desk to chair height difference less than 0, indicating that the desk to chair height difference might be insufficient for taller students. The differences between seat height and fibular height ranged from -1.4 to 1.1 cm; and the differences between seat depth and buttock popliteal length ranged from -9.8 to 3.4 cm. Among obese students, the differences between seat width and 1/2 hip circumference ranged from -20.5 to -8.7 cm, while it ranged from -12.2 to -3.8 cm among non obese students.
Conclusion
Current furniture standards basically satisfy hygienic requirements; however, in the case of exceptionally tall and obese students, ergonomic accommodations such as adaptive seating allocation or personalized adjustments are recommended to meet hygienic requirements.
3.Application of multimodal weakly-supervised learning in image synthesis and segmentation of liver cancer
Chinese Journal of Medical Physics 2024;41(1):8-17
Although it has high resolution for soft tissues, magnetic resonance imaging(MRI)is not the standard for chest imaging, which results in an insufficient amount of expert-annotated MRI data. Therefore,CT image is usually converted into MRI image. To overcome the difficulty of obtaining the corresponding modal CT and MRI images,a CSCGAN model with CycleGAN as the framework is proposed based on the structural characteristics of generative adversarial networks.Considering the possibility of mode collapse in CycleGAN,StyleGan2 which can control the style and feature details of the synthetic image and realize the synthesis of high-resolution images is integrated into CycleGAN for reconstructing the generator.A noise module is introduced to reduce external interference. In addition,in order to prevent the loss of tumors during conversion, the discriminator structure of the network is modified, and a mixed attention mechanism is added.Experimental results show that compared with the images generated by other methods, those generated by the proposed model are improved in Dice similarity coefficient,Hausdorff distance, volume ratio and mean intersection over union, indicating that the proposed method can effectively realize the mode conversion of liver tumor images, and that the generated data can improve the segmentation accuracy.

Chinese Journal of Medical Physics 2024;41(5):571-578
In view of complex pathogenic factors and diagnostic difficulties of benign paroxysmal positional vertigo,a novel method for diagnosing nystagmus in vertigo based on non-local convolutional and convolutional block attention module(CBAM)is proposed. An object detection model is constructed to locate the pupil, thereby tracking eye movement and extract temporal data of horizontal and vertical motion trajectories.Subsequently,a classification model is employed for detection and classification, utilizing a non-local convolutional module to capture remote dependency relationships in nystagmus data, and introducing CBAM to extract high-and low-level semantic information in the feature layer for enhancing the detection performance.Experiments were conducted on a video nystagmus dataset provided by the Eye,Ear,Nose and Throat Hospital. The results show that compared with the best mainstream method, the proposed method improves precision, recall rate, accuracy, and average F1 score by 1.82%,2.09%,1.62%,and 1.96%,respectively,demonstrating its superiority.

Due to the low contrast of CT images, irregular shape of the liver, and blurred boundaries with adjacent organs, the existing methods based on convolutional neural network underperform in liver segmentation tasks, especially for boundary recognition and small object detection.A novel liver segmentation method is proposed based on multi-scale feature fusion and attention, namely MFFA UNet.Multi-scale feature fusion is firstly employed to acquire abundant segmentation details, while spatial and channel attention mechanisms are utilized to capture global spatial and inter-channel relationships.Additionally,a deep supervision module fully leverages the output of intermediate hidden layers, enhancing the learning capability of the network, which in turn accelerates the network's convergence speed. Moreover,a hybrid loss function is adopted to address the issue of class imbalance, further boosting the model's segmentation efficacy.Experimental results demonstrate that the proposed MFFA UNet outperforms the prevailing segmentation networks on the public LITS dataset, producing results that are closer to the ground truth.

A composite nystagmus classification framework enhanced by dual attention mechanism is proposed to address the problem that the existing researches only identify whether nystagmus occurs in a horizontal, vertical,or axial direction, but fail to consider the issue of composite nystagmus composed of multiple directions with various intensities in clinical practice.A spatiotemporal concentration algorithm for nystagmus videos is presented, and it combines convolutional neural networks and Hough transform to remove interference from invalid frames and regions and to improve the quality of nystagmus videos. Then,a dense optical flow algorithm is used to extract the optical flow field of eye movement. Finally,a composite nystagmus classification network based on dual attention mechanism enhancement is constructed. An improved efficient channel attention module is used to effectively obtain the direction and intensity of nystagmus in different channels of the optical flow map;and a temporal attention module is added at the end of the bidirectional long short-term memory network to achieve significant expression of classification results based on different temporal features. On the nystagmus dataset provided by the cooperating hospital, the proposed method has an accuracy rate of 83.17%for composite nystagmus classification, and achieved accuracy rates of 91.03%,89.74%,and 86.05%for individual horizontal, vertical, and axial nystagmus classifications. The proposed method realizes the intelligent classification of composite nystagmus and is valuable in clinic.

Hepatocellular carcinoma(HCC),one of the most prevalent malignant tumors causing the highest mortality globally, imposes an especially heavy burden of disease in China.Individuals living in high-altitude areas have a lower incidence of and mortality resulting from HCC compared with those in low-altitude regions do,potentially due to adaptive evolution in responses to hypoxic stress.Notably,high-altitude hypoxic stress is associated with the development and progression of HCC.Hypoxic stress may be involved in the development and progression of HCC by modulating the senescence, apoptosis, metabolism, tumor microenvironment, and tumor immunity of HCC cells.Additionally, the latest clinical findings indicate that high-altitude hypoxic environment has a significant impact on liver regeneration after HCC resection surgery. However, there is still a debate going on regarding whether high-altitude hypoxic stress promotes or inhibits the progression of HCC.This review covers three main aspects, the impact of adaptive evolution to high-altitude hypoxic stress on the development and progression of HCC in long-term residents of high-altitude areas, the effects of high-altitude hypoxic stress on the senescence, apoptosis, metabolism, tumor microenvironment, tumor metabolism, and tumor immunity of HCC cells, and the effect of high-altitude hypoxic stress on liver regeneration after HCC resection. We discussed the effect of changes in oxygen concentrations, cellular context, and tissue microenvironment on HCC development and progression. Moreover,we highlighted the potential for using research findings on mechanisms underlying high-altitude hypoxic stress to optimize HCC treatment strategies.

Objective:To explore the regulatory effect of miR-873-5p micro-RNA targeting voltage-dependent anion channel protein 1 (VDAC1) in neurons and its mechanism. Methods:Murine nerve cells were randomly divided in vitro into a control group, a model group, a mimetic negative carrier (miR-con) group and an miR-873-5p group. The epileptiform hippocampal nerve cell model was induced in all of the cells except those in the control group using magnesium-free medium. The control group was normally cultured, while the miR-con and miR-873-5p groups were transfected with miR control and miR-873-5p RNA respectively. Real-time fluorescent quantitative polymerase chain reactions were used to detect the expression of miR-873-5p and VDAC1 mRNA. Western blotting was employed to detect VDAC1, B-cell lymphoma/leukemia-2 protein (Bcl-2), Bcl-2 associated X protein (Bax) and cleared caspase-3 in the neurons. The levels of reactive oxygen species (ROS), malondialdehyde (MDA) and glutathione (GSH) were measured using the DCFH-DA fluorescent probe, the thiobarbituric acid method and enzyme-linked immunosorbent assay respectively. Any apoptosis was detected using flow cytometry, while the targeting of miR-873-5p on VDAC1 was verified using the double fluorescence zymase reporter gene method. Results:Compared with the control group, a significant decrease in the average expression of miR-873-5p, Bcl-2 and in GSH and MDA levels was observed in the model group, but there was a significant increase in the average level of VDAC1, Bax, cleaved caspase-3 and ROS and in the rate of apoptosis. Compared with the miR-con group, a significant decrease in the average expression of Bax, cleaved caspase-3, ROS and in the apoptosis rate was observed in the miR-873-5p group, but there was a significant increase in the average level of Bcl-2, GSH and MDA. Moreover, it was verified that miR-873-5p reduced the expression of VDAC1.Conclusion:miR-873-5p protects damaged neurons by inhibiting their apoptosis through negatively regulating the target gene VDAC1 and the oxidative stress response.

Objective:To assess the relationship between serum total bilirubin and fundus arteriosclerosis in different genders. Methods:The physical examination data of Huadong Sanatorium in 2018 were analyzed, and a total of 26 275 people were included in this retrospective cross-sectional study. The age of this study was 18-86 (47.7±11.1) years old. Among them, there were 15 244 males (58.02%) and 11 031 females (41.98%). Participants were divided into 4 groups according to total bilirubin quartile values: Q1<11.50 μmol/L, Q2∶11.50-13.93 μmol/L, Q3∶13.94-17.14 μmol/L and Q4>17.14 μmol/L. The relationship between total serum bilirubin and fundus arteriosclerosis is determined using univariate and multivariate logistic regression analysis methods. The restricted cubic spline method was used to detect the dose-response relationship between total bilirubin and fundus arteriosclerosis. Results:In males, univariate analysis showed that high level of total bilirubin was a protective factor for fundus arteriosclerosis ( OR=0.87, 95% CI 0.78-0.97, P=0.012). After adjusting for other confounding factors, multivariate analysis showed that high level of total bilirubin remained as an independent protective factor for fundus arteriosclerosis ( OR=0.86, 95% CI 0.74-0.99, P=0.047). There was a linear dose-response relationship between total bilirubin level and fundus arteriosclerosis ( P=0.012). In females, univariate analysis showed that there were no statistically significant association between high level of total bilirubin and fundus arteriosclerosis ( OR=0.96, 95% CI 0.80-1.17, P=0.709). After adjusting for other confounding factors, multivariate analysis showed no statistically significant association between high level of total bilirubin and fundus arteriosclerosis ( OR=0.98, 95% CI 0.76-1.27, P=0.888). No linear dose-response relationship between total bilirubin level and fundus arteriosclerosis was found in females ( P=0.253). Conclusion:There are gender differences in the relationship between total bilirubin and fundus arteriosclerosis in this cohort. Elevated levels of total bilirubin are associated with fundus arteriosclerosis in males but not in females.
