Objective:To establish a prognostic prediction model for hepatocellular carcinoma (HCC) using bioinformatics approaches to guide personalized therapy.Methods:Based on bioinformatics, the differential analysis was carried out on the GSE19665 data set of The Cancer Genome Atlas-Liver Hepatocellular Carcinoma (TCGA-LIHC) and Gene Expression Omnibus (GEO), and the same differentially expressed genes were obtained by means of Wayne diagram. Functional enrichment analyses using Gene Ontology, Kyoto Encyclopedia of Genes and Genome, and Gene Set Enrichment Analysis were conducted on co-expressed genes. Based on clinicopathological and transcriptomic profiles, TCGA-LIHC patients were stratified into training ( n=246) and internal validation ( n=116) cohorts, with external validation using Japanese liver cancer data ( n=231) from the International Cancer Genome Consortium. A LASSO-Cox regression-derived risk scoring model was established and visualized as a nomogram. The clinical utility of the risk score was evaluated through multiple analytical approaches.A nomogram incorporating the risk score was developed, and its predictive performance was validated using the concordance index (C-index) and calibration curves. The measurement data of normal distribution were expressed as mean±standard deviation( ± s), and the t-test was used for comparison between groups. The measurement data with non-normal distribution were expressed as M( Q1, Q3), and the Wilcoxon test was used for comparison between groups. The Kruskal-Wallis test was applied to evaluate the significance of the differences among multiple groups. The prognostic value of the risk score was estimated using Kaplan-Meier analysis and ROC curve. Multivariate Cox regression clarified the independent prognostic value of the risk score. Results:Differential analysis identified 457 commonly expressed differentially expressed genes (DEGs). Enrichment analysis revealed that these common DEGs were significantly enriched in pathways related to the cell cycle of tumor cells.The LASSO-Cox regression model selected eight candidate genes ( CENPA, NDC80, ANXA10, NEIL3, G6 PD, MCM10, SOCS2, MMP1). The predictive risk score generated using these eight genes demonstrated a strong association with the overall survival of HCC patients.The nomogram combining the predictive risk score with clinicopathological features exhibited high predictive performance in both the training and validation cohorts. Furthermore, the prognostic value of this risk score was successfully validated in the external validation cohort. Conclusion:This study successfully developed a new predictive model that accurately predicts the 1-year, 3-year and 5-year survival rates of patients with liver cancer. This can serve as a potential tool to help guide patients in personalized treatment.

Objective To investigate the relationship between the risk perception and nursing working environment with the mental resilience in the emergency department nurses.Methods The convenience sam-pling method was adopted.A total of 577 emergency department nurses in 45 hospitals of Guangdong Province from October to November 2023 were selected as the respondents.The CD-RISC scale,nursing staff risk per-ception questionnaire and nursing work environment scale were used to conduct the survey.The multiple line-ar regression was adopted to analyze the influencing factors of mental resilience in the emergency department nurses.The Pearson correlation was adopted to analyze the correlation between the risk perception and nursing working environment with the mental resilience.Results The gender,age,specialist nurse or not,post,hospi-tal type and hospital level had the effect on the mental resilience in the emergency department nurses (P<0.05).The scores of mental resilience scale in 577 nurses were (86.07±15.95) points and the scoring rate was 68.86％;the scores of nursing staff risk perception questionnaire were (79.38±16.32) points and the scoring rate was 56.70％;the scores of the nursing work environment scale were (93.51±13.25) points and the scoring rate was 75.41％.The multiple linear regression analysis showed that the post was the influencing factor of mental resilience in the emergency department nurses.The results of Pearson correlation analysis showed that the nursing work environment was positively correlated with the mental resilience (r=0.476),and negatively correlated with the risk perception (r=-0.252).Conclusion The hospital nursing managers should pay attention to the influencing factors of mental resilience in the emergency department nurses and fo-cus on three major types of risks affecting physical function,time and occupational exposure of the emergency department nurses to improve the working environment of emergency nursing.

Objective To investigate the effects of celastrol on the cell growth and apoptosis of human gallbladder cancer NOZ cells,and explore its potential molecular mechanism.Methods NOZ cell were cultured in vitro.And CCK-8 assay,Annexin V-FITC/PI staining method,cell cycle analysis were conducted to investigate the effects of celastrol on the growth and apoptosis of NOZ cells after being treated with drugs.The mitochondrial membrane potential and Bax and Bcl-2 protein expression level were determined by Rhodamine 123 and Western blot,respectively.Results Celastrol could inhibit NOZ cell growth,and the IC50 value was 5.3 μmol/L.Annexin-V/PI staining showed that cell apoptosis of NOZ cells were induced as the celastrol concentration increased,and the apoptosis ratio of control group was 4.4％,while the apoptosis rates of the test groups (2,5,10 p mol/L) were 7.4％,27.1％ and 43.4％,respectively.In addition,cell cycle analysis revealed that celastrol could induce G1-phase arrest.The G1-phase rate of control group was 25.6％,while the G1-phase rates of the test groups (2,5,10 μmol/L) were 36.5％,45.7％ and 92.5％,respectively.The mitochondrial membrane potential was measured after treatment with celastrol and the results indicated that the mitochondrial membrane potential was significantly decreased.Western Blot showed that the protein expression of Bax increased and Bcl-2 decreased in a time-dependent manner after treatment with celastrol.Conclusions Celastrol may inhibit cell proliferation of human gallbladder cancer NOZ cells and induce cell apoptosis partly by inducing the loss of mitochondrial membrane potential.

Objective To investigate the clinical effect of cryosurgery in the preoperative preparation of liver transplantation in treating liver cancer.Method This study reviewed retrospectively clinical data from 74 patients who underwent cryosurgery of liver cancer before liver transplantation.According to the differences between transplantation programs,74 patients were divided into 2 groups:26 patients in Argon-helium cryoablation group (AHC group) and 48 patients in transcatheter arterial chemoembolization group (TACE group).Whether the patients in two groups met the standard of Milan after treatment,as well as the incidence rate of complication,waiting time for transplantation and MELD score before transplantation were compared between two groups after preoperative therapy.What's more,operation time,no liver time,amount of bleeding,PT and serum level of aminotransferase at 1st,3rd,and 5th day after transplantation were analyzed.Abdominal drainage fluid volume,the incidence of infection,acute rejection,kidney failure,biliary complication,and vascular complication in two groups were also compared.Disease free survival rate was counted after two years by outpatient follow-up.Result Only 3 cases in two groups exceeded Milan standard after treatment,one in AHC group and two in TACE group.The complication incidence and waiting time in two groups had no statistically significant differences.The MELD score in AHC group was significantly lower than in TACE group before transplantation.The operation time,amount of bleeding and transfusion requirements in AHC group were also significantly lower than in TACE group.The time without liver in two groups had no statistically significant difference.The speed of liver function recover was faster in ACH group,and the abdominal drainage fluid volume was less.There were no significant differences in incidence of postoperative complications between two groups (P＞0.05).Conclusion Cryosurgery therapy has little effect on liver functions after treatment.It is a good therapy for liver cancer patients before liver transplantation.

Objective To investigate the effect of total glucosides of paeong (TGP) on the liver lipid infiltration and fibrosis in rats with non-alcoholic fatty liver disease (NAFLD) induced by fructose and high-fat diet. Methods Fructose-high-fatty induced NAFLD rat model was established. Metformin ( MET,200 mg · kg-1 ) and TGP (200,100 mg · kg-1 ) was intragastrically given to the rats in the treatment group,TGP high dose and low dose group,respectively. Normal control group and model control group was intragastrically treated with equivalent distilled water (10 mL·kg-1 ). At the fourth week after the treatment,all the rats were sacrificed and the indices such as serum fasting blood glucose(FBG),INS,insulin sensitivity index (ISI),triglycerides(TG),apelin-36,visfatin,alanine aminotransferase(ALT),aspartate aminotransferase(AST),free fatty acid (FFA),collagen Ⅲ(COLⅢ),collagen Ⅳ(COLⅣ) were determined. Hepatic content of TG was determined and the pathological changes in the liver tissues were observed under the microscope. Results As compared with the model control group,TGP effectively decreased FBG,INS,TG in serum and liver tissues,activity of ALT and AST in serum and content of FAA,Apelin, Visfatin,COLⅢ and COLⅣ,with significant differences (P<0. 05 or P<0. 01). TGP alleviated lipid infiltration and fibrosis in rat liver tissues. Conclusion TGP can inhibit effectively lipid infiltration and fibrosis of NAFLD rats,probably through improving glucolipid metabolism and antogonizing insulin resistance.
