Severe acute pancreatitis (SAP) is closely related to dysfunction of the spleen-stomach ascent and descent. Due to the influence of modern lifestyle and dietary factors, Qi deficiency in the spleen and stomach has become the pathological basis of SAP. Its pathogenesis is characterized by dampness, heat, pathogenic factors, stasis, stagnation, obstruction, Fu-organs Qi obstruction, pathogenic excess, and healthy Qi deficiency. At different stages of the disease course of SAP, there is a focus on both pathogenic excess and healthy Qi deficiency. It is specifically manifested as Fu-organs stagnation and heat accumulation, as well as pathogenic excess and healthy Qi deficiency, during the systemic inflammatory response phase, intermingling of blood stasis and pathogenic factors, as well as Qi deficiency and blood stasis, during the infection period, and weakness of the spleen and stomach, as well as healthy Qi deficiency and lingering pathogenic factors, during the residual infection period. Based on the theory that "the spleen and stomach are the acquired foundation", a staged treatment method centered on the core principle of "strengthening healthy Qi to eliminate pathogenic factors" was developed. The staged treatment method included "clearing the Fu-organs to expel turbidity, replenishing Qi to harmonize the stomach, activating blood circulation to expel pathogenic factors, replenishing Qi to relieve pain, promoting digestion to stimulate appetite, and replenishing Qi to invigorate the spleen". In clinical practice, Hewei Tongxie mixture, Yikang mixture, and Shiwei Jianpi Xiaoshi powder were selected for staged treatment of SAP. This article systematically summarized the theoretical basis of traditional Chinese medicine, Western medicine foundation, modern pharmacological mechanisms, and clinical application experience of the staged treatment of SAP with "strengthening the healthy Qi to eliminate pathogenic factors", providing new ideas for the treatment of SAP with traditional Chinese medicine.

AIM To optimize the extraction process for total polyphenols from Conioselinum vaginatu(Spreng.)Thell.,make component analysis, and evaluate their anti-oxidant,hypoglycemic activities.METHODS The effects of ultrasound, enzymatic hydrolysis, acid hydrolysis, alcohol extraction and hydrolysis processes on the extraction quantity of total polyphenols were investigated, respectively. With extraction temperature, extraction time, ethanol concetration and liquid-solid ratio as influencing factors, extraction quantity of total polyphenols as an evaluation index, the extraction process was optimized by response surface method.HPLC was adopted in the identification of polyphenolic composition and determination of their contents.Subsequently, total polyphenols' scavenging capacities on DPPH,ABTS,OH free radicals, total reducing power and inhibitory capacity on α-glucosidase were determined.RESULTS The highest extraction quantity of total polyphenols was observable when extraction process was employed. The optimal conditions were determined to be 62 ℃ for extraction temperature,54 min for extraction time,69%for ethanol concentration, and 50∶1 for liquid-solid ratio, the extraction quantity of total polyphenols was(9.51±0.2)mg GAE/g.Seven constituents existed in C.vaginatu, among which ferulic acid demonstrated the highest content, followed by that of myricetin, while D-tryptophan content was the lowest. At the concentration of 7.61 mg/L,total polyphenols displayed the scavenging rates on DPPH,ABTS,OH free radicals of 80.70%,85.97%,28.60%,total reducing power of 0.22,and inhibition rate on α-glucosidase of 77.23%,respectively.CONCLUSION This stable and reliable method can be used for the extraction of total polyphenols from C.vaginatum with strong anti-oxidant,hypoglycemic activities.

Background Exposure to polycyclic aromatic hydrocarbons (PAHs) is associated with the development of metabolic syndrome (MS). However, the role of amino acids in PAH-induced MS remains unclear. Objective To explore the impact of PAHs exposure on the incidence of MS among coking workers, and to determine potential modifying effect of amino acid on this relationship. Methods Unmatched nested case-control design was adopted and the baseline surveys of coking workers were conducted in two plants in Taiyuan in 2017 and 2019, followed by a 4-year follow-up. The cohort comprised 667 coking workers. A total of 362 participants were included in the study, with 84 newly diagnosed cases of MS identified as the case group and 278 as the control group. Urinary levels of 11 PAH metabolites and plasma levels of 17 amino acids were measured by ultrasensitive performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS). Logistic regression was used to estimate the association between individual PAH metabolites and MS. Stratified by the median concentration of amino acids, Bayesian kernel machine regression (BKMR) model was employed to assess the mixed effects of PAHs on MS. Due to the skewed data distribution, all PAH metabolites and amino acids in the analysis were converted by natural logarithm ln (expressed as lnv). Results The median age of the 362 participants was 37 years, and 83.2% were male. Compared to the control group, the case group exhibited higher concentrations of urinary 2-hydroxyphenanthrene (2-OHPhe), 9-hydroxyphenanthrene (9-OHPhe), and hydroxyphenanthrene (OHPhe) (P=0.005, P=0.049, and P=0.004, respectively), as well as elevated levels of plasma branched chain amino acid (BCAA) and aromatic amino acid (AAA) (P<0.05). After being adjusted for confounding factors, for every unit increase in lnv2-OHPhe in urine, the OR (95%CI) of MS was 1.57 (1.11, 2.26), and for every unit increase in lnvOHPhe, the OR (95%CI) of MS was 1.82 (1.16, 2.90). Tyrosine, leucine, and AAA all presented a significant nonlinear correlation with MS. At low levels, tyrosine, leucine, and AAA did not significantly increase the risk of MS, but at high levels, they increased the risk of MS. In the low amino acid concentration group, as well as in the low BCAA and low AAA concentration groups, it was found that compared to the PAH metabolite levels at the 50th percentile (P50), the log-odds of MS when the PAH metabolite levels was at the 75th percentile (P75) were 0.158 (95%CI: 0.150, 0.166), 0.218 (95%CI: 0.209, 0.227), and 0.262 (95% CI: 0.241, 0.282), respectively, However, no correlation between PAHs and MS was found in the high amino acid concentration group. Conclusion Amino acids modify the effect of PAHs exposure on the incidence of MS. In individuals with low plasma amino acid levels, the risk of developing MS increases with higher concentrations of mixed PAH exposure. This effect is partly due to the low concentrations of BCAA and AAA.

The development of organ transplantation has brought new hope to many patients with organ failure and their families, but it has also raised numerous ethical issues. How to balance the rights and interests between organ donors and recipients, as well as ensure the fairness and transparency of the transplantation process has become an urgent problem to be solved. Based on the latest Regulations on Organ Donation and Transplantation and the Working Rules of the Ethics Committee for Human Organ Transplantation, the current difficulties and challenges in organ transplantation ethics were deeply analyzed. Taking the ethical review practice of Shenzhen Third People’s Hospital as an example, this paper explored issues such as full informed consent of both donors and recipients, risk assessment of marginal donors, and the review of relationships between donors and recipients. It also explored and constructed a set of complete ethical review models for organ transplantation through refined management. This model improved the efficiency and quality of ethical review as well as enriched the related knowledge system. It is expected that the implementation of this model can provide a reference for promoting effective ethical review nationwide, advancing the improvement and development of ethical review work in organ transplantation. Meanwhile, more medical ethics experts and practitioners are called upon to focus on and engage in the research and practice of ethical review in organ transplantation, jointly promoting progress in this field.
