Background/Aims:
Liver transplantation (LT) following total hepatectomy is a life-saving treatment for hepatocellular carcinoma (HCC). The HCC recurrence after LT hinders the effectiveness of the procedure. The objective of this study is to develop a pre-operative risk stratification model based on a liquid biopsy.
Methods:
We conducted a comprehensive multi-omics study of 260 HCC patients from three centers, including clinical data, low-coverage whole-genome sequencing of cell-free DNA (cfDNA) from plasma, as well as whole-exome, single-nucleus RNA, and spatial transcriptomics from matched tumor and non-tumor tissues.
Results:
We identified cfDNA-derived copy number alteration (CNA) signatures associated with post-transplant recurrence. By integrating cfDNA-derived CNA profiles with single-cell transcriptomic data, we traced recurrence-associated cfDNA to a distinct subpopulation of malignant cells within the primary tumor. These cells were embedded in a pro-metastatic microenvironment of specialized endothelial subtypes and cancer-associated fibroblasts. Notably, most recurrence-associated lesions were detectable in cfDNA prior to liver transplantation (LT). Building on these insights, we developed the ZJU Criteria based on CNA fragments and tumor markers, a pre-LT risk prediction tool that integrates conventional clinical factors with cfDNA-derived CNA signatures, and validated it using internal and independent external cohorts.
Conclusion
Our findings suggest that post-transplant recurrence commonly originates from advanced subclones that emerge late during tumor evolution. The ZJU Criteria provides an accurate, non-invasive strategy that significantly improves pre-LT risk stratification and clinical decision-making for patients with HCC.

Objective This study aimed to investigate the genetic diversity, differentiation, and population structure of Aedes albopictus across different geographical regions in Shandong Province, and to explore the relationship between genetic diversity and geographical distribution. Methods Between July and August 2024, ten Ae. albopictus populations were sampled from seven cities in Shandong Province. Genomic DNA was extracted from individual mosquitoes, and the mitochondrial cytochrome c oxidase subunit I(COⅠ)gene was amplified using PCR and sequenced. The obtained sequences were verified using BLAST and analyzed with MAFFT, MEGA 11, DnaSP v6.12, Arlequin 3.5, PopART 1.7, STRUCTURE 2.3.4 and the R packages adegenet and vegan to assess genetic diversity and population structure. Results A total of 229 COⅠ sequences(662 bp) was obtained, revealing ten variable sites with no insertions or deletions. The overall base composition showed an AT bias of 67.7%. Haplotype analysis identified 11 haplotypes, with Hap2 being the dominant and most widely distributed haplotype across all populations. Neutrality tests showed significant population expansion only in the Rizhao population. Mantel testing result revealed that geographical distance does not significantly impede gene flow. Overall, genetic differentiation among populations was low, indicating frequent gene flow. When combined with additional samples from Shanghai, Fujian, Guangdong, Yunnan, Hainan and Guangxi, STRUCTURE, UPGMA and DAPC analyses revealed two primary genetic clusters of Ae. albopictus. Conclusions Ae. albopictus populations in Shandong Province exhibit frequent gene flow, low genetic differentiation, and relatively low overall genetic diversity. However, the Rizhao population showed signs of recent expansion, highlighting the need for enhanced surveillance and targeted control measures in this area.

RNA-binding protein human antigen R(HuR)is a protein product of the embryonic lethal abnormal vision gene(ELAVL).It is widely expressed in human cells and primarily regulates mRNA stability through post-transcriptional mecha-nisms,particularly by binding to AU-enriched elements(AR-Es).Recent studies have indicated that HuR interacts with non-coding RNAs to participate in the regulation of gene expression, including long non-coding RNAs,circular RNAs,microRNAs,and vault RNAs.The interactions between HuR and these ncR-NAs play a crucial role in the occurrence and development of va-rious diseases, including tumors.Since there are already reviews summarizing the research on tumors, this review mainly focuses on summarizing the role of HuR-ncRNA interactions in diseases other than tumors.

Objective To investigate the appropriate indicators for monitoring pediatric nociceptive stimu-lation,this study compared the SPI and NOX,two dual-parameter nociceptive stimulation monitors based on different principles,in terms of their effects on remifentanil consumption and postoperative recovery in pediatric adenotonsil-lectomy.Methods Children aged 3~12 years who were scheduled to undergo adenotonsillectomy under general anesthesia with endotracheal intubation were randomly assigned to the conventional group(Group R,n=19),the SPI group(Group S,n=19),and the NOX group(Group N,n=18)according to the type of nociceptive stimu-lation monitor used. All children were subjected to routine fasting. The depth of anesthesia was monitored using a BIS monitor, and the remifentanil infusion rate was adjusted according to heart rate,SPI,or NOX values to maintain the index within the range of 30~50.After surgery, all children were transferred to the Post-Anesthesia Care Unit(PACU)with the tracheal catheter in place until they recovered.During the operation, the consumption of anes-thetics such as remifentanil was recorded.Postoperatively, pain and agitation scores, the incidence of agitation at different time points, the duration of anesthesia, the surgical time, the time to extubation, and the length of stay in the recovery room were measured.Additionally, postoperative adverse reactions and perioperative vital signs were documented. Results In comparison with Group R,in Group N,the intraoperative consumption of remifentanil and the agitation score during the recovery period were significantly reduced.Conversely,in Group S,both of(P<0.05).There were no significant disparities in the FLACC score, the incidence of agitation, and the extubation time among the three groups. Conclusions The NOX index can serve as a quantitative metric for monitoring nonci-ceptive stimulation during pediatric adenotonsillectomy. This index has the potential to decrease the intraoperative consumption of opioids and the residence time in the recovery room.

Objective To examine the changing prevalence and antimicrobial resistance profiles of Burkholderia cepacia in 52 hospitals across China from 2015 to 2021.Methods A total of 9 261 strains of B.cepacia were collected from 52 hospitals between January 1,2015 and December 31,2021.Antimicrobial susceptibility of the strains was tested using Kirby-Bauer method or automated antimicrobial susceptibility testing systems according to a unified protocol. The results were interpreted according to the breakpoints released in the Clinical & Laboratory Standards Institute(CLSI)guidelines(2023 edition). Results A total of 9 261 strains of B.cepacia were isolated from all age groups, especially elderly patients. The proportion was 11.1%(1 032 strains)in children, significantly lower than the proportion in adults.About half(46.5%,4 310/9 261)of the strains were isolated from patients at least 60 years old and 42.3%(3 919/9 261)of the strains were isolated from young adults. Most isolates(71.1%)were isolated from sputum and respiratory secretions, followed by urine(10.7%)and blood samples(8.1%).B.cepacia isolates were highly susceptible to the five antimicrobial agents recommended in the CLSI M100 document(33rd edition,2023).B.cepacia isolates showed relatively higher resistance rates to meropenem and levofloxacin. However, the resistance rates to ceftazidime,trimethoprim-sulfamethoxazole,and minocycline remained below 8.1%.The percentage of B.cepacia strains resistant to levofloxacin was the highest compared to other antibiotics in any of the three age groups(from 12.4%in the patients<18 years old to 20.6%in the patients aged 60 years or older). Conclusions B.cepacia is one of the clinically important non-fermenting gram-negative bacteria.Accurate and timely reporting of antimicrobial susceptibility test results and ongoing antimicrobial resistance surveillance are helpful for rational prescription of antimicrobial agents and proper prevention and control of nosocomial infections.
