ObjectiveTo observe the clinical efficacy and safety of Yuyin Lidan granules (YYLD) in preventing the recurrence of common bile duct stones (CBDS) in patients with liver and gallbladder dampness-heat syndrome following endoscopic retrograde cholangiopancreatography (ERCP). MethodsThis randomized, parallel, controlled trial enrolled postoperative CBDS-ERCP patients who met the inclusion and exclusion criteria. Sixty-four patients were randomly assigned to an observation group or a control group, with 32 cases in each. Both groups received conventional Western medical treatment after ERCP, while the observation group additionally received YYLD for 8 weeks. The follow-up period lasted for 1 year. The efficacy indicators included bile bilirubin levels, traditional Chinese medicine (TCM) syndrome scores, clinical efficacy rate, pancreatitis and inflammation markers, postoperative liver function, and CBDS recurrence rate at 1-year follow-up, which were used to jointly evaluate the clinical efficacy and safety of both groups. ResultsA total of 56 patients completed the study and were included in the final analysis, i.e., 29 in the observation group and 27 in the control group. Baseline characteristics were comparable between the two groups. Compared with pre-treatment and with the control group after treatment, the bile bilirubin level in the observation group significantly decreased (P<0.05). After treatment, the clinical cure and marked improvement rates were higher in the observation group than in the control group, showing a statistically significant difference in overall clinical efficacy (P<0.05). Compared with pre-treatment, the primary and secondary symptoms in the observation group, as well as the primary symptom and the secondary symptom of nausea and vomiting in the control group (weeks 4 and 8), were significantly reduced (P<0.05). Compared with the control group after treatment, the observation group showed significant reductions in the primary symptom of loose stools/constipation (day 5 and week 4) and in three secondary symptoms, i.e., bitter taste and sticky dry mouth, abdominal distension and poor appetite (throughout the treatment period), and general heaviness and fatigue (day 5 and week 4), with statistical differences (P<0.05). Compared with pre-treatment, both groups showed decreased lipase and urinary amylase levels (P<0.05). However, no significant between-group differences were observed in pancreatitis or inflammation-related indices after treatment. Compared with pre-treatment, all liver function indicators in the observation group and alanine aminotransferase ( ALT ), γ-glutamyl transferase ( γ-GT ), alkaline phosphatase (ALP), and conjugated bilirubin in the control group significantly decreased at weeks 4 and 8 (P<0.05). Compared with the control group after treatment, only serum total bilirubin and unconjugated bilirubin were significantly reduced in the observation group during the treatment period (P<0.05). ConclusionYYLD combined with conventional Western medical treatment can effectively regulate bilirubin metabolism (in bile and serum), improve TCM clinical symptoms, and prevent CBDS recurrence after ERCP in patients with liver and gallbladder dampness-heat syndrome. This regimen is safe and effective and is worthy of further clinical research and promotion.

Objective To investigate the effect of baseline atherogenic index of plasma(AIP)on the long-term prognosis of patients with acute myocardial infarction(AMI).Methods A total of 712 AMI patients admitted to the hospital from January 2018 to December 2019 were continuously included as subjects and divided into a low-value group(AIP<0.280,n=237),a median-value group(AIP 0.280~0.852,n=238)and a high-value group(AIP>0.852,n=237)according to the baseline AIP tertiles. The primary endpoint was defined as the occurrence of major cardiovascular adverse events(MACEs).Multivariate Cox regression was used to analyze the independent influencing factors of MACEs.The nonlinear relationship between AIP and the risk of MACEs was analyzed with restricted cubic spline plots.Kaplan-Meier curve was used to analyze survival differences between groups.Subgroup analysis assesses the consistency of AIP's predictive value to MACEs.Results With the increase of AIP tertile groups, the proportion of dyslipidemia and MACEs increased, white blood cell count, fasting blood glucose, triglyceride, total cholesterol, low density lipoprotein and AIP increased, and high-density lipoprotein decreased, with statistical significance(P<0.05).Multivariate Cox regression analysis showed that AIP was an independent risk factor for MACEs(HR=2.024,95%CI:1.211~3.381,P=0.007).The results of restricted cubic spline analysis show that there is an L-shaped nonlinear effect relationship between AIP and the risk of MACEs(P-nonlinear=0.008).When AIP>0.613,the risk of MACEs in AMI patients increases with the increase of AIP.Kaplan-Meier survival curve analysis results show:With the increase of AIP,the cumulative incidence of MACEs in AMI patients increased significantly(Log-rank test,P=0.032).Compared with the low-value group, the risk of MACEs in the high-value group increased by 131%(HR=2.311,95%CI:1.261~4.234,P=0.007).The results of subgroup analysis showed that the P value of interaction within each subgroup was not significant, and the ability of AIP to predict MACEs was applicable to all subgroups. Conclusion Increased AIP at baseline is an independent predictor of poor long-term prognosis in patients with AMI.

Aim To explore the biological mechanism of ligustilide in the prevention and treatment of osteo-porosis by regulating H-type blood vessels, combined with animal experiments for verification, based on net-work pharmacology and molecular docking technology Methods The possible mechanism of ligustilide regu-lating H-type blood vessels to prevent osteoporosis was predicted by network pharmacology.Molecular docking technology was used to verify the binding ability of the core target EGFR to ligustilide. The rat model of osteo-porosis was established and divided into the sham group, model group, ligustilide high, medium and low dose(80,40,20 mg·kg-1)groups.The pathological changes of femur were observed by HE staining. The expressions of CD31,EMCN,OSX+and RUNX2+pro-tein in tibial metaphysis were detected by immunofluo-rescence.The expression of p-EGFR,p-PI3K and p-Akt protein was detected by Western blot. Results The results of network pharmacology showed that a total of 20 intersection targets were obtained.EGFR,PTGS2,ESR1 and ICAM1 were core targets, and mo-lecular docking showed that EGFR had a strong bind-ing ability with ligustilide. The signaling pathways of ligustilide in the prevention and treatment of osteoporo-sis by regulating the expression of H-type blood vessels were mainly enriched in PI3K-Akt,TNF,etc.Com-pared with the model group, ligustilide could signifi-cantly increase the number of trabecular bone and im-prove the destruction of bone microstructure. The ex-pression of CD31,EMCN,OSX+and RUNX2+signifi-cantly increased(P<0.01,P<0.05),the formation of H-type blood vessels were promoted, and the expres-sion of p-EGFR,p-PI3K and p-Akt significantly in-creased(P<0.01,P<0.05).Conclusions Ligusti-lide can increase the expression of H-type blood vessels in bone tissue of osteoporosis model rats, reduce the damage of bone trabecula and improve bone micro-structure effectively.EGFR-mediated PI3K/Akt signa-ling pathway may be the key way to exert its biological effects.
