1.Annual review of global liver transplantation research in 2024: technological breakthroughs, precision management and future challenges

In recent years, significant progress has been made in the field of liver transplantation in terms of donor expansion, technological innovation and perioperative management. Machine perfusion technology, through dynamic repair and assessment of donor liver quality, can effectively reduce postoperative complications and increase the utilization rate of marginal donor livers. The optimization of split liver transplantation technology combined with normothermic perfusion further alleviates the shortage of donors, but its promotion is still limited by technical barriers. Xenotransplantation has achieved preclinical breakthroughs in the field of genetically modified pig livers, but ethical and immune barrier issues need to be urgently resolved. In the field of liver cancer liver transplantation, the focus is on neoadjuvant treatment with immune checkpoint inhibitors and the development of recurrence prediction models, which promotes precise treatment. For perioperative management, the optimization of individualized immunosuppressive regimens, artificial liver support, and strategies for the prevention and control of vascular complications has significantly improved patients’ survival rates. Personalized treatment for children, elderly recipients, and recipients with multiple comorbidities provides new ideas for liver transplantation in special populations. In the future, liver transplantation research may focus on the integration of multidisciplinary approaches, individualized treatment and emerging technologies to advance the global liver transplantation cause to new heights.
2.The effects of lncRNA EBLN3P on the proliferation, migration and epithelial-mesenchymal transition (EMT) of thyroid cancer B-CPAP cells by regulating the miR-369-3p/CCND1 axis

[摘 要] 目的:探究长链非编码RNA(lncRNA)内源性博尔纳病毒样核蛋白3假基因(EBLN3P)调控miR-369-3p/CCND1轴对甲状腺癌B-CPAP细胞增殖、迁移和EMT进程的影响。方法:收集2020年5月至2021年5月间在海南省肿瘤医院手术切除的20例甲状腺癌及相应癌旁组织标本,以及甲状腺癌B-CPAP细胞,qPCR法和WB法检测癌组织和细胞中EBLN3P、miR-369-3p、CCND1 mRNA水平,双萤光素酶报告基因实验验证lncRNA EBLN3P、CCND1与miR-369-3p之间的靶向关系。随机将B-CPAP细胞分为对照组、sh-NC组、sh-EBLN3P组、sh-EBLN3P + anti-NC组、sh-EBLN3P + anti-miR-369-3p组,通过克隆形成实验、划痕愈合实验、Transwell实验分别检测各组细胞的增殖和迁移能力,WB法检测各组细胞中EMT相关蛋白的表达。构建B-CPAP细胞裸鼠移植瘤模型,观察沉默EBLN3P对移植瘤生长的影响。结果:在甲状腺癌组织和B-CPAP细胞中EBLN3P、CCND1 mRNA表达上调,miR-369-3p表达下调(均P < 0.05);EBLN3P与miR-369-3p、CCND1与miR-369-3p之间有结合位点,存在靶向关系。与sh-NC组比较,sh-EBLN3P组克隆形成数、划痕愈合率、细胞迁移数降低(均P < 0.05),EBLN3P、CCND1、Ki67、MMP-2、N-cadherin、vimentin表达下调,miR-369-3p、E-cadherin表达上调(均P < 0.05);与sh-EBLN3P + anti-NC组比较,sh-EBLN3P + anti-miR-369-3p组miR-369-3p表达下调,克隆形成数、划痕愈合率、细胞迁移数均升高(均P < 0.05),CCND1、Ki67、MMP-2、N-cadherin、vimentin表达均上调,E-cadherin表达下调(均P < 0.05)。与sh-NC组比较,sh-EBLN3P组B-CPAP细胞裸鼠移植瘤的体积和质量均显著降低(均P < 0.05)。结论:在甲状腺癌组织和B-CPAP细胞中lncRNA EBLN3P表达上调,沉默EBLN3P可靶向调控miR-369-3p/CCND1轴抑制甲状腺癌B-CPAP细胞的增殖、迁移、EMT进程。
3.Does Vertebral Cement Augmentation Reduce Postoperative Proximal Junction Complications in Spinal Deformity Corrective Surgery: A Systematic Review and Meta-analysis

Objective:
To assess the effectiveness of vertebral cement augmentation (VCA) at upper instrumented vertebra (UIV) and UIV+1 in preventing proximal junction complications in correction surgery for adult spinal deformity patients.
Methods:
A literature search was conducted on Web of Science, PubMed, and Cochrane Library databases for comparative studies published before December 30th, 2024. Two reviewers independently screened eligible articles based on the inclusion and exclusion criteria, assessed study quality with Newcastle-Ottawa scale, and extracted data like study characteristics, surgical details, primary and secondary outcomes. Data analysis was performed using Review Manager 5.4 and Stata software.
Results:
Of all 513 papers screened, a meta-analysis was conducted on 7 articles, which included 333 cases in the VCA group and 827 cases in the control group. Patients in the VCA group had significantly older age and lower T score than patients in the control group. Although there was no statistically significant difference in the incidence of proximal junctional failure between the 2 groups, the results of the meta-analysis showed that the incidence of proximal junctional failure and the need for revision surgery were reduced by 36% and 71%, respectively, in the VCA group. One study reported 2 clinically silent pulmonary cement embolism and 1 patient requiring surgical decompression for cement leak into the spinal canal.
Conclusion
This meta-analysis supported the use of VCA in corrective surgery for spinal deformities patients, especially in patients with advanced age and osteoporosis.
4.Structural Validity of the Diagnostic Interview for Internet Addiction Scale for Clinical Samples in Korean Children and Adolescents: Exploratory and Confirmatory Factor Analysis

Objective:
This study aimed to validate the reliability and validity of the Diagnostic Interview for the Internet Addiction Scale (DIA) among Korean children and adolescents in the clinical setting.
Methods:
We collected the clinical data from university hospitals in South Korea and 194 children and adolescents (aged 7–18 years) completed the questionnaire. The content validity was conducted on 10 items of the DIA and an internal consistency test was performed for the verification of reliability.
Results:
Participants on average, aged 13.17 years (standard deviation=2.46), and 75.3% (n=146) were boys. The DIA was highly correlated with the scores of the Korean scale for Internet addiction for adolescents, Young’s Internet Addiction Test, Internet addiction proneness scale for children and adolescents. The overall sampling suitability of the 10-item scale was tested using the Kaiser–Meyer–Olkin, resulting in a high value of 0.861. The DIA revealed a two-factor structure and the Cronbach’s alpha correlation coefficient for the total scale was 0.806. Confirmatory factor analysis showed an acceptable model fit (root-mean square error of approximation=0.058, comparative fit index=0.950, and Tucker-Lewis Index=0.919).
Conclusion
The DIA may suggest in-depth-scale examinations of the factors that influence Internet addiction. We may expect that DIA would be used efficiently for the diagnosing of Internet addiction and further studies for the assessment.

6.Job Analysis of Nurses Working at Dementia Care Centers Using DACUM

Purpose:
The purpose of this study is to conduct job analysis of nurses at dementia care centers and to identify the importance, frequency, and difficulty of each duty and task.
Methods:
Through Developing a Curriculum (DACUM) Committee workshop, the committee members developed a job analysis tool using DACUM, and the nurses working at dementia care centers evaluated the importance, frequency, and difficulty of each duty and task.
Results:
The jobs of the nurses were derived from 10 duties and 66 tasks, and each duty consisted of 3 to 10 tasks. The important duties were ‘public guardianship project for dementia’ and ‘dementia diagnosis screening,’ the most frequent duties were ‘consultation and registration management,’ and ‘dementia diagnosis screening,’ and the most difficult duties were ‘public guardianship project for dementia’ and ‘project planning and evaluation.’ Based on these results, the core duties and tasks were derived, and the top priority duties were ‘consultation and registration management,’ ‘case management,’ and ‘support for families and carers of dementia patients’.
Conclusion
The most recent duties of nurses, who have the largest proportion of workers at dementia care centers, were identified, and the core duties that should be given priority in selecting the direction of education for job performance and professional improvement were presented. Based on the application method of education and training presented in this study, it is important to detail education and training that is appropriate for and applicable to each duty to support the professionalism of nurses at dementia care centers.

For patients with a few remaining abutment teeth, traditional removable partial dentures and implant-supported fixed prostheses are common treatment options.However, removable dentures often struggle to provide stability, especially as bone resorption occurs over time. Implant-supported fixed prostheses offer longterm stability but are costly and affected by anatomical and medical factors. A newer option is implant-assisted removable partial dentures, which use a minimal number of implants combined with a surveyed crown. This approach enhances support, retention, and stability while reducing financial and surgical burdens. It also improves the prognosis of the remaining teeth, increases patient satisfaction, and enhances masticatory function, making it a promising alternative to conventional removable dentures.
8.Radial Type Low-Intensity Extracorporeal Shockwave Therapy Enhances Penile Microvascular Perfusion in an Aging Rat Model: A Novel Interventional Strategy to Treat Erectile Dysfunction

Purpose:
Physiological aging is associated with microvascular dysfunction, including in the penis, and this may contribute to age-related erectile dysfunction (ED). Low-intensity extracorporeal shockwave therapy (Li-ESWT) is a non-invasive intervention for ED, but its effect on penile microvascular function, remains unclear. Our objectives are to (i) evaluate the effect of Li-ESWT (specifically radial type ESWT [rESWT]) on penile microvascular perfusion (PMP) in aging rats, (ii) elucidate a possible mechanism, and (iii) evaluate its impact on angiogenic and smooth muscle biomarkers in cavernosal tissue.
Materials and Methods:
Male rats (n=9; 15–18 months) were anesthetized and subjected to rESWT while monitoring PMP. The nitric oxide (NO) pathway involvement was assessed by measuring the effect of rESWT on PMP following an intracavernosal injection of N(G)-nitroarginine methyl ester (L-NAME) (NO synthase inhibitor). To elucidate the cellular mechanism, another group of rats received repeated rESWT (n=4) or no treatment (n=4) three times/week for two weeks. Rats were euthanized at the end of the study and penile tissues were analyzed for angiogenic markers (vascular endothelial growth factor-A [VEGF-A], endothelial nitric oxide synthase [eNOS]) and smooth muscle content (α-actin) using immunostaining, Western blot, and quantitative polymerase chain reaction (qPCR).
Results:
rESWT resulted in more than a 2-fold increase in PMP (from 68.5 arbitrary units; 163.7 AU). L-NAME injection produced a <40%–50% decrease (185.3 to 101.0 AU) in rESWT-induced PMP response. Immunostaining revealed increased α-actin, eNOS, and VEGF-A in the cavernosum and these findings were confirmed by qPCR and Western blot results.
Conclusions
rESWT improved PMP, which may be mediated via increased VEGF expression, which stimulates the NO/cyclic guanosine monophosphate pathway, resulting in sustained PMP. rESWT devices could offer a safe, non-invasive treatment for age-related ED.
9.Korean Guidelines for Diagnosis and Management of Idiopathic Nonspecific Interstitial Pneumonia

Idiopathic nonspecific interstitial pneumonia (iNSIP) is recognized as a distinct entity among various types of idiopathic interstitial pneumonias. It is identified histologically by the nonspecific interstitial pneumonia pattern. A diagnosis of iNSIP is feasible once secondary causes or underlying diseases are ruled out. Usually presenting with respiratory symptoms such as shortness of breath and cough, iNSIP has a subacute or chronic course. It predominantly affects females aged 50 to 60 years who are non-smokers. Key imaging findings on chest high-resolution computed tomography include bilateral reticular opacities in lower lungs, traction bronchiectasis, reduced lung volumes and, ground-glass opacities. Abnormalities are typically diffuse across both lungs with subpleural distributions. Treatment often involves systemic steroids, either alone or in combination with other immunosuppressants, although evidence supporting effectiveness of these treatments is limited. Prognosis is generally more favorable for iNSIP than for idiopathic pulmonary fibrosis, with many studies reporting a 5-year survival rate above 70%. Antifibrotic agents should be considered in a condition, termed progressive pulmonary fibrosis, where pulmonary fibrosis progressively worsens.
10.Development and Application of New Risk-Adjustment Models to Improve the Current Model for Hospital Standardized Mortality Ratio in South Korea

Purpose:
This study assessed the validity of the hospital standardized mortality ratio (HSMR) risk-adjusted model by comparing models that include clinical information and the current model based on administrative information in South Korea.
Materials and Methods:
The data of 53976 inpatients were analyzed. The current HSMR risk-adjusted model (Model 1) adjusts for sex, age, health coverage, emergency hospitalization status, main diagnosis, surgery status, and Charlson Comorbidity Index (CCI) using administrative data. As candidate variables, among clinical information, the American Society of Anesthesiologists score, Acute Physiology and Chronic Health Evaluation (APACHE) II, Simplified Acute Physiology Score (SAPS) 3, present on admission CCI, and cancer stage were collected. Surgery status, intensive care in the intensive care unit, and CCI were selected as proxy variables among administrative data. In-hospital death was defined as the dependent variable, and a logistic regression analysis was performed. The statistical performance of each model was compared using C-index values.
Results:
There was a strong correlation between variables in the administrative data and those in the medical records. The C-index of the existing model (Model 1) was 0.785; Model 2, which included all clinical data, had a higher C-index of 0.857. In Model 4, in which APACHE II and SAPS 3 were replaced with variables recorded in the administrative data from Model 2, the C-index further increased to 0.863.
Conclusion
The HSMR assessment model improved when clinical data were adjusted. Simultaneously, the validity of the evaluation method could be secured even if some of the clinical information was replaced with the information in the administrative data.
