Tetralogy of Fallot (TOF) is the most common cyanotic congenital heart disease. As a critical component of surgical repair, reconstruction of the right ventricular outflow tract (RVOT) serves as a primary determinant of postoperative hemodynamics and long-term prognosis. With advancements in surgical techniques, early postoperative survival rates for TOF have significantly improved, shifting clinical focus toward addressing long-term complications such as right ventricular dysfunction and reduced exercise tolerance. However, challenges persist regarding optimal timing of surgery in relation to adverse long-term outcomes, balancing RVOT reconstruction strategies, and selecting appropriate repair materials for RVOT reconstruction, with a lack of unified high-level evidence-based guidelines. To address these issues and standardize the application of radical corrective surgery for TOF to better serve patients, the National Society for Congenital Heart Diseases has systematically reviewed existing high-level evidence. Consequently, we have registered and formulated the "Protocol for Chinese expert consensus on right ventricular outflow tract reconstruction in tetralogy of Fallot (2026 edition)", which details the methodology and development process of this upcoming consensus.

Introduction:
Pheochromocytoma is a catecholamine-secreting adrenal
tumor associated with major perioperative hemodynamic
instability. When concurrent major pelvic pathology
requires surgery, operative planning becomes particularly
challenging. We describe the successful single-stage
management of pheochromocytoma and a giant uterine
fibroid, highlighting the importance of multidisciplinary
coordination and perioperative optimization.
Case:
A 49-year-old female with symptomatic uterine fibroid
was found to have proliferative endometrium on a pipelle
biopsy. Computed tomography (CT) abdomen incidentally
detected a right adrenal mass alongside a large posterior
uterine fibroid (11.9 × 17.4 × 14.6 cm). CT adrenal protocol
demonstrated a heterogeneously enhancing right adrenal
mass (6.9 × 6.9 × 9.6 cm) with high unenhanced attenuation.
Biochemical evaluation revealed markedly elevated
24-hour urinary metanephrine (4.7× upper limit) and
normetanephrine (2.4× upper limit).
Following multidisciplinary discussions, a single-stage
surgical approach was planned after careful assessment of
feasibility and perioperative risk in view of the uncertain malignant potential of the pelvic mass and to minimize
repeated exposure to anesthesia. Preoperative optimization
included transitioning from terazosin to phenoxybenzamine, with subsequent addition of bisoprolol for
hemodynamic control. The operative strategy prioritized
pheochromocytoma resection first, given its potential
for significant hemodynamic instability. Progression to
hysterectomy was contingent upon achieving adequate
intraoperative hemodynamic stability following adrenalectomy, with continuous reassessment by the anesthetic
and surgical teams.
Right adrenalectomy was performed first, followed by
total abdominal hysterectomy with bilateral salpingooophorectomy. Significant hemodynamic lability occurred
during tumor manipulation, with hypertensive surges
managed using sodium nitroprusside and remifentanil
infusions. Following adrenal vein ligation and tumor
removal, hypotension was managed with noradrenaline
and additional adrenaline support as required. Total
operative time was approximately 6 hours. Postoperatively,
transient noradrenaline support was required but was
rapidly weaned as hemodynamic stability was achieved.
Conclusion
Single-stage adrenalectomy and major pelvic surgery
can be safely performed in selected patients with pheochromocytoma when guided by meticulous preoperative
optimization, clear intraoperative sequencing, and close
multidisciplinary coordination.

Objective This study conducted molecular biological identification of the viruses carried by Culex pipiens pallens specimens collected in Shunyi District, Beijing in 2023, and observed the changes in the infection rate of the viruses carried by Cx. pipiens pallens at different collection times. Methods Cx. pipiens pallens were collected using carbon dioxide mosquito traps. The mosquito samples were ground in batches and analyzed by molecular biology technologies. The virus infection rate at different collection times was analyzed statistically. Results 17 strains of Culex pipiens pallens densovirus(CppDNV)were identified from Cx. pipiens pallens samples collected in Shunyi District, Beijing, in 2023. The nucleotide sequence analysis of the virus genome coding region showed that CppDNV was a single-stranded DNA virus with a total length of 3 335 nt, encoding 2 non-structural proteins(NS1, NS2)and 1 capsid protein(VP). The nucleotide(amino acid)sequence lengths of the three proteins were 2 376 nt(791 aa),1 092 nt(363 aa)and 1 071 nt(356 aa), respectively. Phylogenetic analysis showed that CppDNV was located in genus Brevihamaparvovirus. Statistical analysis showed no significant difference in infection rates across collection times(χ2=4.429, P=0.194). Conclusions CppDNV was identified in Cx. pipiens pallens in Beijing, and it was stably maintained in this natural population.

Objective To investigate the species composition of mosquitoes and breeding conditions of larvae in the rice planting areas of Beijing, and to provide a scientific basis for mosquito control in rice fields. Methods Adult mosquitoes were collected using CO2-baited light traps, and mosquito larvae(including pupae)were sampled via the dipping method. Results A total of 4 genera and 6 species of adult mosquitoes were captured in the study area. The dominant species were Culex pipiens pallens, Cx. tritaeniorhynchus, and Anopheles sinensis. The average adult mosquito density was 5.11 specimens per trap-hour, with slightly lower density in July compared to August. For larvae and pupae,3 genera and 4 species were identified, dominated by Cx. tritaeniorhynchus, An. sinensis, and Aedes vexans. Conclusions The mosquito species composition in Beijing′s rice planting areas is diverse. Mosquito control strategies should prioritize the management of adult mosquitoes, while density control of mosquito larvae in paddy fields can be achieved through intermittent irrigation techniques.

ObjectiveTo construct a group-based trajectory model (GBTM) for early medication adherence check-in, and to analyze the relationship between different trajectories and treatment outcomes in tuberculosis patients using data that were generated from smart tools for monitoring their medication adherence and check-in. MethodsFrom October 1, 2022 to September 30, 2023, a total of 163 pulmonary tuberculosis patients diagnosed in Fengxian District were selected as the study subjects. The GBTM was utilized to analyze the weekly active check-in trajectories of the subjects during the first 4 weeks and establish different trajectory groups. The χ² tests were employed to compare the differences between groups and logistic regression analysis was conducted to explore the relationship between different trajectory groups and treatment outcomes. ResultsA total of four groups were generated by GBTM analyses, of which a low level of punch card was maintained in group A, 6% of the drug users increased rapidly from a low level in group B, 17% of drug users increased gradually from a low level in group C, and 18% of drug users maintained a high level of punch card in group D. The trajectory group was divided into two groups according to homogeneity, namely the low level medication punch card group (group A) and the high level medication punch card group (group B, group C, and group D). The results of multivariate logistic regression analyses revealed that low-level medication check-in (OR=3.250, 95%CI: 1.089‒9.696), increasing age (OR=1.030, 95%CI: 1.004‒1.056), and not undergoing sputum examination at the end of the fifth month (OR=2.746, 95%CI: 1.090‒7.009) were significantly associated with poor treatment outcomes. ConclusionThe medication check-in trajectory of pulmonary tuberculosis patients within the first 4 weeks is correlated with adverse outcomes, or namely consistent low-level medication adherence check-ins are associated with poor treatment outcomes, while high-level medication adherence check-ins are associated with a lower incidence of adverse outcomes.
6.A network analysis of depression and autonomic symptoms in Parkinson disease
Journal of Apoplexy and Nervous Diseases 2025;42(2):115-120
Objective To investigate the association between depression and autonomic nervous function in Parkinson disease (PD), and to provide a basis for clinical treatment. Methods Clinical and neurocirculation data were collected from 168 PD patients who attended Department of Neurology, The Second Affiliated Hospital of Hainan Medical College, from July 2022 to July 2023, and according to the score of Beck Depression Inventory, the patients were divided into depression in PD (dPD) group with 57 patients and non-dPD (nPD) group with 111 patients. General clinical data were collected from all patients. The supine-to-standing TCD test was performed for all patients to record systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR), and the mean velocity (Vm), pulsatility index (PI), and resistance index (RI) of the middle cerebral artery (MCA) at 1, 3, and 5 minutes in both the supine and standing positions. A network was constructed for depression symptoms in PD. Results In the network of non-motor symptoms in PD, depression showed the highest centrality and the strongest predictability and was strongly correlated with sleep/fatigue and mood/cognition, with a strength centrality stability coefficient (CS strength) of 0.440. Compared with the nPD group, the dPD group had significantly lower supine HR, ∆HR, Vm in the standing position, and ∆Vm%, a significantly greater ∆DBP, and a significantly higher proportion of patients with dizziness with orthostatic hypotension or orthostatic cerebral hypoperfusion (P<0.05). Depression was positively correlated with ∆SBP, ∆DBP, Vm in the supine position, and RI in the standing position, and it was negatively correlated with ∆HR, DBP in the supine position, HR in the supine position, and ∆PI (CS strength=0.375 and 0.222). Conclusion Impairment of cardiovascular and cerebral autonomic nervous function might be involved in the pathogenesis of depression in PD, and intervention of depression can help improve the overall non-motor symptoms of PD, with sleep, fatigue, and cognition as the effective targets for improving depression in PD.

Abstract
Skin, the largest organ of the human body, serves as the primary barrier against environmental hazards. Skin aging is a complex process influenced by both intrinsic factors, such as endogenous physiological changes, and extrinsic factors, including environmental exposures, reflecting the internal health status of the body. During skin aging, various cell types exhibit diverse responses to aging-related stimulations. This paper systematically reviews the roles of cells in the pathogenesis of skin aging and explores potential therapeutic strategies for mitigating this process. Furthermore, based on the biological activities of cells during skin aging, we elucidate the interaction mechanisms among cells within the skin microenvironment.

Annals of Coloproctology 2025;41(1):27-39
Purpose:
Diagnosing sarcopenia necessitates the measurement of skeletal muscle mass. However, guidelines lack a standardized imaging modality with thresholds validated among Asians. This systematic review compared ultrasonography, computed tomography (CT), magnetic resonance imaging (MRI), and bioelectrical impedance analysis (BIA)/body composition monitoring in the detection of sarcopenia within Asian populations.
Methods:
PubMed and Embase were systematically searched for studies analyzing ultrasonography, CT, MRI, and BIA in diagnosing sarcopenia among Asians. Study quality was assessed using the Newcastle-Ottawa scale.
Results:
Pooled findings from 21,598 patients across 25 studies were examined. In receiver operating characteristic analysis, ultrasound displayed a pooled mean area under the curve (AUC) of 0.767 (95% confidence interval [CI], 0.709–0.806), with mean sensitivity of 81.1% (95% CI, 0.744–0.846) and specificity of 73.1% (95% CI, 0.648–0.774), for detecting sarcopenia in Asian populations. CT exhibited an AUC of 0.720 (sensitivity, 54.0%; specificity, 92.0%). MRI demonstrated an AUC of 0.839 (sensitivity, 67.0%; specificity, 66.0%). BIA displayed an AUC of 0.905 (95% CI, 0.842–0.968), 80.7% sensitivity (95% CI, 0.129–0.679), and 82.4% specificity (95% CI, 0.191–0.633).
Conclusion
Various modalities aid in diagnosing sarcopenia, and selection should be individualized. Although only BIA and dual-energy x-ray absorptiometry are recommended by the Asian Working Group for Sarcopenia and the European Working Group on Sarcopenia in Older People, ultrasound imaging may hold diagnostic value for sarcopenia in the Asian population. In certain groups, diagnostic use of CT and MRI is warranted. Future research can standardize and validate modality-specific thresholds and protocols within Asian populations.

Objective:To analyse the effect of ultrasound-guided percutaneous liver biopsy and the coaxial biopsy needle tract radiofrequency ablation on patients diagnosed with hepatocellular carcinoma who are considered to be at risk of bleeding. Methods:The data of 117 patients with hepatocellular carcinoma who underwent coaxial biopsy needle tract radiofrequency ablation after ultrasound-guided percutaneous liver biopsy in Peking University First Hospital from March 2019 to April 2023 were retrospectively analysed. There were 95 males and 22 females, with the age of (62.0±11.8) years. A comprehensive analysis was conducted on the following variables: the pre-puncture platelet count, the international standardised ratio, anticoagulation therapy, the haemoglobin (Hb) level, the success rate of the liver puncture, the qualified rate of liver puncture specimens, the number of puncture samples, the length of hospital stay, the Hb level after puncture, bleeding within 10 days post-operation, and complications after ablation, including biliary fistula, hemothorax and organ perforation. Results:Among the 117 patients, 60 cases (51.3%) had an international normalized ratio >1.1, 40 cases (34.2%) had thrombocytopenia, that is, <150×10 9/L, and 17 cases (14.5%) received continuous anticoagulation therapy before the operation. It is evident that all 117 patients successfully completed the ultrasound-guided percutaneous liver biopsy, and that all liver biopsy specimens were qualified. The absence of biliary fistula, hemothorax, organ perforation or death in the patients post-ablation was noted. According to the adverse event evaluation criteria, version 5.0, 113 cases (96.6%) were classified as grade 1 and 4 cases (3.4%) were classified as grade 3. The Hb concentration of patients with minor bleeding (grade 1) prior to puncture was (119.7±22.2) g/L, which was significantly higher than the Hb concentration of patients with severe bleeding (grade 3), (76.0±10.4) g/L ( t=3.92, P=0.010). A meticulous examination of the data revealed that there were no statistically significant differences between the two groups with regard to pre-puncture platelet count, pre-puncture international standardised ratio, pre-puncture proportion of receiving anticoagulant drugs, length of hospital stay and number of puncture samples (all P>0.05). Conclusion:For patients with hepatocellular carcinoma who are at risk of bleeding, ultrasound-guided percutaneous liver biopsy followed by coaxial biopsy needle tract radiofrequency ablation can obtain satisfactory liver tissue samples and is relatively safe. There were differences in hemoglobin levels before puncture among patients with different bleeding after puncture.
