1.Correlation between newly formed bone and the progression of experimental peri-implantitis with or without alveolar ridge preservation in infected and non-infected teeth: a secondary analysis of a preclinical study

Purpose:
We examined the progression of experimental peri-implantitis in sites that underwent either alveolar ridge preservation (ARP) or spontaneous healing (SH), comparing infected teeth (IT) and non-infected teeth (NIT). This investigation is a secondary analysis of a preclinical study aimed at exploring the correlation between newly formed bone and implant stability quotient (ISQ), as well as the association between newly formed bone and the progression of experimental peri-implantitis.
Methods:
The bilateral mandibular third or fourth premolars of 6 beagle dogs were randomly assigned to 4 groups: IT/SH, IT/ARP, NIT/SH, and NIT/ARP. Following implant placement, core biopsies were retrieved from each site, and the ISQ value was measured. A 3-month period was allowed for peri-implantitis induction, followed by an additional 3 months for the spontaneous progression of peri-implantitis, with radiographs taken at each time point.
Results:
During the spontaneous progression of peri-implantitis, no statistically significant differences were observed among the groups in terms of mean ISQ values and radiographic marginal bone loss. Similarly, the percentages of bone substitute, newly formed bone, and fibrovascular connective tissue in core biopsies did not differ significantly among the groups.Linear regression analysis revealed no significant linear correlation between newly formed bone and ISQ in any group (P>0.05). However, a weak linear correlation between newly formed bone and marginal bone loss during the spontaneous progression of peri-implantitis was noted in the IT/SH group alone (P=0.036).
Conclusions
Within the limitations of this study, we were unable to demonstrate that ARP could improve newly formed bone or primary implant stability. Furthermore, neither ARP nor SH significantly influenced the spontaneous progression of experimental peri-implantitis.
2.Clinical evaluation of implants placed within or beyond the boundaries of the alveolar ridge preservation:a retrospective case series with 10weeks of observations in 28 patients

Purpose:
This study investigated the effect of implant vertical positioning within alveolar ridge preservation (ARP) sites on implant stability quotient (ISQ) values, which were measured 10 weeks post-implantation.
Methods:
Patients who underwent ARP using collagenized deproteinized bovine bone mineral, followed by implant placement in the posterior area, were divided into 2 groups:the within-ARP group and the beyond-ARP group. In the within-ARP group, osteotomy and implant placement occurred within the ARP boundary. In contrast, in the beyond-ARP group, these procedures were performed beyond the ARP boundary, incorporating 3 mm of pristine bone at the implant’s apex. Bone quality was assessed by tactile sense, and both insertion torque during implant surgery and ISQ values at 10 weeks post-implant surgery were measured. Multiple linear regression analysis and Pearson correlation analysis were used to explore the relationship between insertion torque and ISQ values.
Results:
In total, 30 ARP sites in 28 patients were analyzed. There was no significant difference in bone quality, as determined by tactile sense, between the within-ARP and beyond-ARP groups. At the time of implant placement, the beyond-ARP group exhibited a higher insertion torque (33.33±13.39 Ncm) compared to the within-ARP group (17.08±11.17 Ncm). However, the ISQ values were similar between the 2 groups 10 weeks after implant placement. A positive correlation between insertion torque and ISQ values was confirmed at 10 weeks post-implant.
Conclusions
The engagement of pristine bone may facilitate high insertion torque during the placement of implants in ARP sites. Nevertheless, by 10 weeks post-implantation, the ISQ values were found to be comparable, irrespective of the implant’s position.
3.Does defect configuration affect the outcomes of alveolar ridge preservation? An experimental in vivo study

Purpose:
The purpose of this study was to compare the bone healing potential of 1-, 2-, and 3-wall defects following alveolar ridge preservation (ARP) treatment, as well as to evaluate the efficacy of ARP as a treatment option for destructive sites.
Methods:
Three groups, characterized by 1-, 2-, and 3-wall defects, were randomly assigned to the maxillary second, third, and fourth premolars in each of 8 beagle dogs. Each defect was created at either the mesial or distal root site of the tooth, which was hemi-sectioned and extracted. The contralateral root was preserved to superimpose with the experimental site for histomorphometric analysis. For each site, either spontaneous healing (SH; control) or ARP (test intervention) was randomly applied. Each group was divided in half and underwent a healing period of either 4 or 12 weeks. The Mann-Whitney U test and Kruskal-Wallis test were used for histomorphometric analyses. Statistical significance was set at P<0.05.
Results:
Qualitative analysis revealed a higher percentage of new bone in the apical area compared to the coronal area, regardless of defect type and healing period. In quantitative analysis, the 3-wall defect exhibited a significantly higher percentage of mineralization in the ARP group after 12 weeks of healing (ARP: 61.73%±7.52%; SH: 48.84%±3.06%; P=0.029).An increased percentage of mineralization was observed with a greater number of remaining bony walls, although this finding did not reach statistical significance.
Conclusions
Within the limitations of this study, ARP treatment for compromised sockets appears to yield a higher percentage of mineralization compared to SH. Although the effectiveness of the remaining bony walls was limited, their presence appeared to improve the percentage of mineralization in ARP treatment.
4.Bone regeneration and biosorption patterns of different bone substitutes:an in vivo study in rabbit skulls

Purpose:
This study aimed to analyze the bone regeneration and biosorption patterns of different bone substitutes in a rabbit skull defect model.
Methods:
Four circular 8 mm-defects were created in the cranium of 12 New Zealand white rabbits, each weighing approximately 3 kg. Each defect was randomly assigned to one of 4 treatment groups: cortical deproteinized porcine bone mineral (DPBM), cancellous DPBM, biphasic calcium phosphate (BCP) with a 6:4 ratio of hydroxyapatite (HA) to β-tricalcium phosphate (β-TCP) (TCP4), and BCP with a 2:8 ratio of HA to β-TCP (TCP8). The rabbits were euthanized at either 6 weeks (n=6) or 12 weeks (n=6) post-surgery. The harvested specimens were then analyzed both radiographically and histomorphometrically.
Results:
In the micro-computed tomography analysis, no statistically significant differences were observed among the 4 groups, except in the bone graft volume/tissue volume (GV/TV) at 12 weeks. Cortical DBPM exhibited a higher GV/TV ratio than cancellous DBPM at the same time point. The histomorphometric analysis revealed increased biosorption in cancellous DBPM compared to cortical DBPM at 12 weeks. However, the percentage of newly formed bone did not significantly differ among the 4 groups.
Conclusions
All types of bone substitutes demonstrated similar patterns of bone regeneration at both 6 and 12 weeks of observation. However, cancellous DPBM exhibited a higher rate of bioabsorption compared to other bone substitutes, suggesting that it may have different indications or applications in guided bone regeneration protocols.

Multiple myeloma (MM), a hematological malignancy, is characterized by malignant plasma cell proliferation in the bone marrow. Recent treatment advances have significantly improved patient outcomes associated with MM.In this study, we aimed to develop comprehensive, evidence-based guidelines for the diagnosis, prognosis, and treat‑ ment of MM. We identified 12 key clinical questions essential for MM management, guiding the extensive literature review and meta-analysis of the study. Our guidelines provide evidence-based recommendations by integrating patient preferences with survey data. These recommendations include current and emerging diagnostic tools, thera‑ peutic agents, and treatment strategies. By prioritizing a patient-centered approach and rigorous data analysis, these guidelines were developed to enhance MM management, both in Korea and globally.
6.Therapeutic Effect of Anti-inflammatory Tripeptide Cream in Hand-Foot Syndrome/Skin Reaction Related to Anticancer Drugs: A Randomized, Double-Blind, Placebo-Controlled Pilot Trial

Purpose:
Hand-foot syndrome (HFS) and hand-foot skin reaction (HFSR) are relatively common toxicities that interfere with the quality of life (QoL) of patients with cancer. Anti-inflammatory tripeptide cream (ATPC) is a complex formulation of anti-inflammatory tripeptides, the CD99-agonist Binterin and the Wnt-antagonist Winhibin. The present study aimed to assess the therapeutic effects of ATPC in HFS/HFSR associated with anticancer drugs.
Materials and Methods:
This was a single-center, randomized, double-blind, placebo-controlled trial. Patients who developed grade 1 HFS/HFSR after systemic anticancer treatments were enrolled, and randomly assigned to receive either ATPC or placebo cream (PC) and followed up at 3-week intervals for up to 9 weeks. Primary endpoint was the development of grade ≥ 2 HFS/HFSR.
Results:
Between April 2019 and July 2022, 60 patients (31 in the ATPC and 29 in the PC group) completed the study. The incidence of grade ≥ 2 HFS/HFSR was significantly lower in the ATPC than in the PC group (25.8% vs. 51.7%, p=0.039). The ATPC showed trends towards a better QoL score, assessed by a HFSR and QoL questionnaire at 9 weeks (26.0 vs. 29.9, p=0.574), and a lower frequency of discontinuation, interruption, or dose reduction of anticancer drugs (51.6% vs. 58.6%, p=0.586) than the PC group over 9 weeks, though without statistical significance.
Conclusion
Our results showed that ATPC significantly decreased the development of grade ≥ 2 HFS/HFSR in patients already with HFS/HFSR. Therefore, ATPC may be an effective treatment for HFS/HFSR associated with anticancer drugs.
7.Change of Sacral Slope according to the Surgical Position in Total Hip Arthroplasty

Purpose:
Pelvis tilting in sagittal plane influences the acetabular cup position. Majority of total hip arthroplasty (THA) are performed in lateral decubitus surgical position. This study is to assess whether there is any difference in sacral slope between standing and lateral decubitus position and influence of this variation in planning acetabular cup anteversion.
Materials and Methods:
This is a prospective study including 50 patients operated between January 2020 to March 2022. Preoperative radiograph included lumbosacral spine lateral X-ray in standing, supine and lateral decubitus positions to calculate the sacral slope for assessment of anterior or posterior pelvic tilting. In our study, we determined the position of the acetabular cup based on changes in sacral slope between standing and lateral decubitus postures. For patients whose sacral slope increased from lateral decubitus to standing, we implanted the acetabular component with a higher degree of anteversion. Conversely, for patients with reverse phenomenon, the cup was inserted at lower anteversion.
Results:
Twenty-four patients (48.0%) had increase in sacral slope from lateral decubitus to standing whereas 26 patients (52.0%) had decrease in sacral slope. There was linear correlation between difference in preoperative sacral slope and postoperative cross table lateral cup anteversion. Harris hip scores improved from 40.78 to 85.43. There was no subluxation or dislocation in any patient at minimum 2-year follow-up.
Conclusion
Individualized acetabular cup placement is important for better functional outcome in THA. Evaluation of pelvic tilting in lateral decubitus position is necessary for better positioning of acetabular cup and avoid postoperative complications.
8.The cumulative survival rate of dental implants with micro-threads:a long-term retrospective study

Purpose:
This study aimed to evaluate the long-term cumulative survival rate (CSR) of dental implants with micro-threads in the neck over a 10-year follow-up period and to examine the factors influencing the survival rate of dental implants.
Methods:
This retrospective study was based on radiographic and dental records. In total, 151 patients received 490 Oneplant ® dental implants with an implant neck micro-thread design during 2006–2010 in the Department of Periodontology of Seoul National University Dental Hospital. Implant survival was evaluated using Kaplan–Meier analysis. Cox proportional hazard regression analysis was used to identify the factors influencing implant failure.
Results:
Ten out of 490 implants (2.04%) failed due to fixture fracture. The CSR of the implants was 97.9%, and no significant difference was observed in the CSR between externaland internal-implant types (98.2% and 97.6%, respectively,P=0.670). In Cox regression analysis, 2-stage surgery significantly increased the risk of implant failure (hazard ratio: 4.769, P=0.039). There were no significant differences in influencing factors, including sex, age, implant diameter, length, fixture type, location, surgical procedure, bone grafting, and restoration type.
Conclusions
Within the limitations of this retrospective study, the micro-thread design of the implant neck was found to be favorable for implant survival, with stable clinical outcomes.
9.Adjunctive effect of 470-nm and 630-nm light-emitting diode irradiation in experimental periodontitis treatment: a preclinical study

Purpose:
This study investigated the adjunctive effect of light-emitting diodes (LEDs) in the treatment of experimental periodontitis.
Methods:
Experimental periodontitis was induced by placing ligatures around the mandibular second, third, and fourth premolars of 6 beagles for 3 months. After ligature removal, periodontitis progressed spontaneously for 2 months. The animals’ hemimandibles were allocated among the following 3 groups: 1) no treatment (control), 2) scaling and root planing (SRP), and 3) SRP with LED irradiation at 470-nm and 630-nm wavelengths (SRP/LED). The probing pocket depth (PPD) and gingival recession (GR) were measured at baseline, 6 weeks, and 12 weeks. The clinical attachment level (CAL) was calculated. After 12 weeks, histological and histomorphometric assessments were performed. The distances from the gingival margin to the apical extent of the junctional epithelium (E) and to the connective tissue (CT) attachment were measured, as was the total length of soft tissue (ST).
Results:
PPD and CAL increased at 12 weeks compared with baseline in the control group (6.31±0.43 mm to 6.93±0.50 mm, and 6.46±0.60 mm to 7.61±0.78 mm, respectively). PPD and CAL decreased at 12 weeks compared with baseline in the SRP group (6.01±0.59 to 4.81±0.65 mm, and 6.51±0.98 to 5.39±0.93 mm, respectively). PPD and CAL decreased at 12 weeks compared with baseline in the SRP/LED group (6.03±0.39 to 4.46±0.47 mm, and 6.11±0.47 to 4.78±0.57 mm, respectively). The E/ST and CT/ST ratios significantly differed among the 3 groups (P<0.05). The clinical parameters and histologic findings demonstrated that 470-nm and 630-nm wavelength LED irradiation accompanying SRP could improve treatment results.
Conclusions
Within the study limitations, 470 nm and 630 nm wavelength LED irradiation might provide additional benefits for periodontitis treatment.
10.Basic Therapeutic Approach for Patients with Plaque Psoriasis:Korean Expert Consensus Using the Modified Delphi Method

Background:
Currently, there is no consensus on the treatment of psoriasis in Korean patients.
Objective:
This study aimed to establish a consensus on the basic therapeutic principles for Korean patients with plaque psoriasis.
Methods:
Using the modified Delphi method, a steering committee proposed 53 statements for the first Delphi round, which covered five subjects: (1) the goal of treatment and evaluation of disease severity, (2) topical therapy, (3) phototherapy, (4) conventional systemic therapy, and (5) biologic therapy. The panel of dermatologists scored the level of agreement for each statement on a ten-point scale with scores ranging from 1 (strongly disagree) to 10 (strongly agree). After discussing the results of the first round, the committee reformulated 41 statements. Finally, consensus was defined as more than 70% of the second round scores being ≥7.
Results:
The panel participants strongly agreed that the ideal treatment goals for Korean patients with plaque psoriasis should include complete skin clearance and high dermatological quality of life. A strong consensus was also reached on the use of topical agents for psoriasis of any severity, the consideration of phototherapy before biologics therapy, the conventional systemic agents for moderate-to-severe psoriasis, and the recommendation of biologic for retractable psoriasis to conventional systemic therapy and phototherapy.
Conclusion
This modified Delphi panel established an expert consensus on the therapeutic approach for Korean patients with plaque psoriasis. This consensus may improve the treatment outcomes for psoriasis in Korea.
