1.Long-Term Follow-Up Result of Benign Phyllodes Tumor of the Breast Diagnosed and Excised by Ultrasound-Guided Vacuum-Assisted Breast Biopsy.

PURPOSE: Percutaneous removal of benign breast tumors using ultrasound-guided vacuum-assisted breast biopsy (VABB) has been recently regarded as a feasible and safe method without serious complications. The aim of this study was to evaluate the efficacy and safety of the VABB in the treatment of benign phyllodes tumors, and to identify whether or not surgical re-excision is necessary for benign phyllodes tumors diagnosed and excised by VABB. METHODS: From January 2003 to December 2011, a total of 6,923 VABB were performed in 5,434 patients. Out of 6,923 lesions, 53 were benign phyllodes tumors. Among these, 31 lesions, with a follow-up period of longer than 24 months, were enrolled in this study. Ultrasonography follow-up was performed at 3 to 6 month intervals in order to assess recurrence. The mean follow-up period was 75.9+/-13.5 months (range, 24-94 months). RESULTS: The mean patient age at presentation was 31.6+/-9.4 years. The mean size of the lesion was 1.60+/-0.88 cm. The majority of lesions, 74.2% (23 cases), were palpable, and 25.8% (8 cases) were non-palpable. Twenty-two lesions (71.0%) were classified as Breast Imaging Reporting and Data System category 3, and nine lesions (29.0%) were classified as category 4a, by ultrasonography. During the follow-up period, local recurrence developed in one lesion, making the local recurrence rate 3.2%. CONCLUSION: If a benign phyllodes tumor is diagnosed, and sufficiently excised by VABB, observing the clinical course may be considered as an alternative to performing immediate wide local excision; this is the case despite the fact that it would need to be observed for a prolonged period of time.

2.Establishment of a Cavernous Fibrosis Model in a Rat Using Adenovirus Expressing Transforming Growth Factor-beta1.

PURPOSE: Transforming growth factor-beta1 (TGF-beta1) has been implicated in cavernous fibrosis due to a variety of causes of erectile dysfunction (ED), such as diabetes mellitus and post-radical prostatectomy. To examine the role of the TGF-beta signaling pathway in cavernous fibrosis, we established a rat model of cavernous fibrosis by using adenovirus expressing TGF-beta1 (ad-TGF-beta1). MATERIALS AND METHODS: Four-month-old male Sprague-Dawley rats received intracavernous injection of ad-TGF-beta1 (1x10(8), 1x10(9), or 1x10(10) virus particles [vp] in 100 microliter of PBS) and the penis was harvested for histologic examination at 10, 20, or 30 days after injection (n=4 per group and per time point). Based on the initial findings, the animals were divided into three groups (n=6 per group): Group 1, age-matched control; Group 2, intracavernous injection of ad-LacZ (1x10(10) vp/100 microliter); and Group 3, intracavernous injection of ad-TGF-beta1 (1x10(10) vp/100 microliter). At 30 days after injection, erectile function was evaluated during electrical stimulation of the cavernous nerve. The penis was then harvested and stained with Masson's trichrome and antibody to smooth muscle alpha-actin. RESULTS: Masson's trichrome staining revealed that intracavernous delivery of ad-TGF-beta1 sufficiently induced cavernous fibrosis in a dose-dependent manner. The fibrotic scars persisted up to 30 days after injection at the highest dosage (1x10(10) vp/100 microliter), whereas no histologic evidence of cavernous fibrosis was found in the control rats or the ad-LacZ-injected rats. The rats receiving ad-TGF-beta1 showed a higher cavernous collagen content and less smooth muscle content than the control rats or ad-LacZ-injected rats. Erectile function was significantly decreased in rats receiving ad-TGF-beta1 compared with that in controls or rats receiving ad-LacZ. CONCLUSIONS: This model induced by ad-TGF-beta1 may play an important role in understanding the pathophysiologic mechanisms of cavernous fibrosis-associated TGF-beta signaling and the development of new therapeutics targeting this pathway.

3.Ras association domain family 1A (RASSF1A) Ala133Ser polymorphism is associated with cervical cancer in Korean women.

OBJECTIVE: The ras association domain family 1A (RASSF1A) gene plays an important part in carcinogenesis by inactivation via promoter hypermethylation in several cancers. We aimed to examine the effect of the RASSF1A Ala133Ser polymorphism on cervical carcinogenesis in reference to environmental factors, such as smoking and age at first sexual intercourse. METHODS: Seven hundred five patients who were diagnosed with cervical neoplasia and who had a positive results for human papillomavirus were enrolled to experimental group, and 641 of normal healthy women were enrolled as controls. All of the subjects were genotyped using the SNaPshot single base primer extension assay. RESULTS: Compared with RASSF1A TT/GT, the odds ratio (95% confidence interval) for RASSF1A GG was 1.4 (0.9-2.1) for invasive cervical cancer and 1.1 (0.7-1.7) for cervical intraepithelial neoplasia (CIN) II or III. The risks for cervical cancer were higher in patients under 40 years old at diagnosis (1.6, 1.0-2.6), than in patients over 40 years old at diagnosis (1.0, 0.7-1.5). Women with RASSF1A GG who currently smoke had a 2.7 (1.3-5.9) times higher risk of cervical cancer. Women with RASSF1A GG who had an early age of first intercourse, as compared with RASSF1A TT/GT, were also at increased risk. CONCLUSION: The RASSF1A Ala133Ser polymorphism is associated with a higher risk of cervical cancer and particularly with an early onset of cervical carcinogenesis.

4.Assessment of the Anterior Segment Using Slit-Lamp Biomicroscope and Gonioscope.

PURPOSE: To evaluate the accuracy of the Smith method, van Herick technique, and Shaffer grading system, using a slit-lamp biomicroscope and gonioscope that measure anterior chamber depth (ACD), peripheral chamber depth (PCD), and angle width. METHODS: The anterior chamber of 94 eyes of 53 subjects was evaluated by slit-lamp examination with a gonioscope. An A-scan was carried out on all of the subjects, but ultrasound biomicroscope (UBM) was performed on only 42 of 94 eyes. Slit-lamp biomicroscope-assisted measurements, such as ACD, PCD, and angle width, were compared with those by A-scan and UBM. RESULTS: The correlations among ACD measurements, obtained by the Smith method, and those obtained using A-scan (r=0.673, p<0.001) and UBM (r=0.824, p<0.001) were statistically significant. The mean PCD, measured by van Herick technique, was 14.01+/-15.04 %, and its correlation with that of UBM was statistically significant (r=0.706, p<0.001). In addition, the correlation between the mean angle width, measured by Shaffer grading system and by UBM, was also statistically significant (r=0.853, p<0.001). CONCLUSIONS: Assessments of the anterior segment, using a slit-lamp biomicrosope and a gonioscope, are comparable to those of A-scan and UBM measurements and are, therefore, useful for evaluation of the anterior chamber.

5.Distribution and Characteristics of Peripheral Anterior Synechiae in Primary Angle-Closure Glaucoma.

PURPOSE: To evaluate the characteristics of peripheral anterior synechiae (PAS) in primary angle-closure glaucoma (PACG). METHODS: We reviewed the charts of 155 patients (244 eyes) with PACG. We divided these patients into one of four clinical subtypes: acute angle-closure glaucoma (ACG), chronic ACG, angle-closure hypertension, and ACG suspect. The prevalence, extent, and location of PAS were evaluated according to PACG subtypes. Correlation analysis was used to evaluate relationships between the highest IOP level without treatment and the extent of PAS. RESULT: The average degree of angle-closure with PAS was 14.6+/-9.1 in eyes that were classified as ACG suspect, 83.8+/-48.3 in angle-closure hypertension, 140.5+/-31.3 in acute ACG, and 180.3+/-31.9 in chronic ACG (ANOVA test, P<0.05). PAS was most frequently found in the superior part of the eye, especially from 12 to 1 o'clock. The incidence of broad PAS (PAS over 30 degrees in width) was highest in superior part, but the medium and narrow PAS (PAS limited to within 30 degrees in width) was distributed throughout all 12 sectors relatively equally. Prior to a laser iridotomy (LI) and other medical treatments, a positive correlation was found between the highest IOP (intraocular pressure) levels and the extent of PAS in chronic ACG (r=0.423, P<0.0001). However, statistically significant relationships were not found between the highest IOP levels before treatment and the extent of PAS in any of the other clinical subtypes. CONCLUSIONS: Our results suggest that acute and chronic ACG patients are most likely to have a greater extent of PAS than patients in the angle-closure hypertension or ACG suspect subtypes. PAS may be narrower in earlier stages and broader in later stages. PAS was also found most frequently in the superior part of the eye. The extent of synechial closure of the angle may play a role in raising IOP levels in later stages of the disease rather than early on.

PURPOSE: The first Korean national population- based cancer registry using nationwide hospital-based recording system and the regional cancer registries provided the source to obtain national cancer incidences for the period 1999~2001. MATERIALS AND METHODS: The incidence of cancer in Korea was calculated based on the Korea Central Cancer Registry database, data from additional medical record review survey, the Regional Cancer Registry databases, site-specific cancer registry databases, and cancer mortality data from the Korea National Statistical Office. Crude and age-standardized rates were calculated by sex for 18 age groups. RESULTS: The overall crude incidence rates (CR) were 247.3 and 188.3 per 100, 000 for men and women and the overall age-standardized incidence rates (ASR) were 281.2 and 160.3 per 100, 000, respectively. Among men, five leading primary cancer sites were stomach (CR 58.6, ASR 65.6), lung (CR 42.1, ASR 50.9), liver (CR 41.9, ASR 44.9), colon and rectum (CR 24.2, ASR 27.3) and bladder (CR 7.7, ASR 9.2). Among women, the most common cancers were stomach (CR 30.8, ASR 25.8), breast (CR 25.7, ASR 21.7), colon and rectum (CR 19.6, ASR 16.7), uterine cervix (CR 18.4, ASR 15.5), and lung cancer (CR 15.1, ASR 12.4). In 0~14 age group, leukemia was most common for both sexes. For men, stomach cancer was most common in 15~64 age group, but lung cancer was more frequent for over 65 age group. For women, thyroid cancer in 15~34 age group, breast cancer in 35~64 age group, and stomach cancer in over 65 age group were most common for each age group. The proportions of death certificate only were 7.5% for men and 7.4% for women. CONCLUSION: This is the first attempt to determine the national cancer incidence and this data will be useful to plan for research and national cancer control in Korea.

BACKGROUND/AIMS: Urea breath test (UBT), the noninvasive test for diagnosing Helicobacter pylori infection, was developed in 1987 and had advanced in accuracy and convenience by improvement of analytic device, 13C or 14C urea regimen, expiration sampling protocol and test meal. However, conventional UBT using 75 mg or 100 mg of 13C-urea is expensive and time consuming. The objective of this study was to evaluate the diagnostic performance of UBT using capsulated 38 mg low dose 13C-urea (HeliFinder(R)) developed by Medichems Co., Ltd. METHODS: A total of one hundred forty seven volunteers were enrolled and examined at Catholic University, Korea University, and Soon Chun Hyang University hospital. UBT was performed using 38 mg 13C urea capsule and compared with the gold standard methods (rapid urease test and histology). Baseline and 20 min breath samples were collected. We used delta13C 2.0permile as the cut-off value suggested by the manufacturer. RESULTS: Of the 147 subjects, 142 cases were available for analysis. The sensitivity and specificity of UBT using the 38 mg 13C urea capsule at 20 min were 98.7% and 100% respectively. CONCLUSIONS: A 20 min, 38 mg capsule based 13C urea breath test protocol is more efficient, cost effective, and convenient than conventional protocol.

BACKGROUND/AIMS: Polyethyleneglycol (PEG) is safe but its large volume and bad taste reduces patients' compliance. Sodium phosphate (NaP) is a hyperosmotic agent and its small volume can increase patients' compliance but electrolyte imbalance is a problem. Therefore, we conducted a study to determine whether patients' compliance can be enhanced and electrolyte imbalance reduced by combining these two agents. METHODS: Forty-one admitted patients receiving colonoscopy at Korea University Hospital from June 28, 2004 to August 14, 2004 were randomly divided into two groups for colon cleansing with either PEG 4 L (n=21) or PEG 2 L plus NaP 45 mL (n=20). Patients were assessed for patient tolerance, quality of preparation, and changes of biochemical parameters. RESULTS: Overall discomfort was statistically lower in the combination group, PEG 2 L plus NaP 45 mL (p=0.035). Although patients in the combination group reported less fullness (p=0.076) and nausea (p=0.087), the findings were not statistically significant. The quality of the preparation was comparable between the two groups (p=0.872). The phosphorus level in the combination group showed a statistically significant increase (0.58+/-0.46) after colon cleansing (p=0.020) but was not clinically significant. CONCLUSIONS: The combination of NaP 45 mL and PEG 2 L showed less overall discomfort with comparable quality of preparation and without serious electrolyte abnormality compared to 4 L of PEG. Therefore, the combination of NaP 45 mL and PEG 2 L could be used as an alternative colonoscopic colon cleansing agent when patients have trouble taking 4 L of PEG alone.

BACKGROUND/AIMS: Recently, treatment using an endoscopic device is actively being developed. Techniques for incision and bleeding control have been developed, but endoscopic suturing remains to be perfected. Using an endoscopic suturing technique, a variety of diseases could be managed noninvasively. We have attempted to use a new endoscopic suturing device which enables a continuous suture using a flexible upper endoscope. METHODS: A suturing device was designed where a semicircular needle could sew through tissue by rotation making a continuous suture possible. RESULTS: We successfully sutured the stomach tissue of a dog using an optimized suturing device model. CONCLUSIONS: The initial model was found to have some problems. However, we hope to develop a continuous suturing device for flexible upper endoscope procedures in Korea.

Meckel's diverticulum is an embryonic derivative of the omphalomesenteric duct and the most commonly encountered congenital anomaly of the gastrointestinal tract. Its incidence records about 2%. Among them, only 5% are symptomatic with complications-bleeding, intestinal obstruction, inflammation, and perforation. In particular, bleeding is a common complication and has always been caused by an ulceration of the ileal mucosa adjacent to the acid-producing ectopic mucosa in a Meckel's diverticulum. Wireless capsule endoscopy is a new method enabling non-invasive diagnostic endoscopy of the entire small intestine. We experienced a case of Meckel's diverticulum detected by wireless capsule endoscopy in a 34 year-old man who presented with chronic obscure gastrointestinal bleeding.
