1.Research progress on the pathogenesis of chronic obstructive pulmonary disease complicated with lung cancer

Chronic obstructive pulmonary disease (COPD), which predominantly affects middle-aged and elderly individuals, is associated with a significantly reduced quality of life and often triggers various other pulmonary conditions. Lung cancer, as one of the most prevalent and deadly pulmonary malignancies worldwide, poses a severe threat to global public health. The risk of developing lung cancer is markedly higher in COPD patients compared to the general population, indicating numerous associations between the two conditions that warrant in-depth investigation. Although a substantial body of research has explored the relationship between COPD and lung cancer, studies focusing on the molecular mechanisms underlying their connection remain limited. This article reviews the latest research progress on the mechanisms of COPD complicated by lung cancer from four perspectives: the role of chronic pulmonary inflammation, programmed cell death, genetic and molecular interactions, and dysbiosis of the pulmonary microbiome. The aim of this article is to provide new insights and references for the prevention and therapeutic strategies of COPD complicated with lung cancer.
2.Type Ⅱ endoleak originating from inferior mesenteric artery after endovascular abdominal aortic aneurysm repair: A retrospective analysis in a single center

Objective To investigate the management experience of type Ⅱ endoleak originating from inferior mesenteric artery (IMA) after endovascular abdominal aortic aneurysm repair (EVAR). Methods The clinical data of patients with type Ⅱ endoleak originating from IMA after EVAR treated in the Department of Vascular Surgery, Shandong Provincial Hospital Affiliated to Shandong First Medical University from October 2016 to November 2021 were collected and analyzed. Results There were 12 males and 3 females at age of 57-89 (68.00±7.84) years. Eleven patients received embolization of the abdominal aortic aneurysm lumen and initial segment of the IMA via the superior mesenteric artery-middle colic artery-Riolan arch-left colic artery-IMA route. Three patients received embolization of the initial segment of the IMA by the above route. One patient underwent open dissection of the abdominal aortic aneurysm, and orifice of IMA was sutured in the aneurysm cavity while stents were retained. All 15 patients were successfully treated by surgery. The symptoms of back pain, abdominal pain and abdominal distension disappeared in 6 patients after surgery. Neither perioperative deaths nor complications happened during the treatment and follow-up period. The median follow-up time was 11.00 (9.00, 18.00) months. Two patients with typeⅡendoleak recurred during the follow-up period and were admitted to hospital for secondary embolization. No recurrence was observed at 12 months postoperative follow-up. Conclusion Type Ⅱ endoleak is one of the most common complications after EVAR. IMA is the most common criminal origin of typeⅡendoleak. TypeⅡendoleak that lead to persistent expansion of the aneurysm cavity requires aggressive intervention.
3.18F-FDG PET/CT combined with CT three-dimensional reconstruction in the differentiation of benign and malignant pulmonary nodules: A retrospective cohort study

Objective To investigate the accuracy of 18F-FDG positron emission tomography/computed tomography (PET/CT) combined with CT three-dimensional reconstruction (CT-3D) in the differential diagnosis of benign and malignant pulmonary nodules. Methods The clinical data of patients who underwent pulmonary nodule surgery in the Department of Thoracic Surgery, Northern Jiangsu People39;s Hospital from July 2020 to August 2021 were retrospectively analyzed. The preoperative 18F-FDG PET/CT and chest enhanced CT-3D and other imaging data were extracted. The parameters with diagnostic significance were screened by the area under the receiver operating characteristic (ROC) curve (AUC). Three prediction models, including PET/CT prediction model (MOD PET), CT-3D prediction model (MOD CT-3D), and PET/CT combined CT-3D prediction model (MOD combination), were established through binary logistic regression, and the diagnostic performance of the models were validated by ROC curve. Results A total of 125 patients were enrolled, including 57 males and 68 females, with an average age of 61.16±8.57 years. There were 46 patients with benign nodules, and 79 patients with malignant nodules. A total of 2 PET/CT parameters and 5 CT-3D parameters were extracted. Two PET/CT parameters, SUVmax≥1.5 (AUC=0.688) and abnormal uptake of hilar/mediastinal lymph node metabolism (AUC=0.671), were included in the regression model. Among the CT-3D parameters, CT value histogram peaks (AUC=0.694) and CT-3D morphology (AUC=0.652) were included in the regression model. Finally, the AUC of the MOD PET was verified to be 0.738 [95%CI (0.651, 0.824)], the sensitivity was 74.7%, and the specificity was 60.9%; the AUC of the MOD CT-3D was 0.762 [95%CI (0.677, 0.848)], the sensitivity was 51.9%, and the specificity was 87.0%; the AUC of the MOD combination was 0.857 [95%CI (0.789, 0.925)], the sensitivity was 77.2%, the specificity was 82.6%, and the differences were statistically significant (P<0.001). Conclusion 18F-FDG PET/CT combined with CT-3D can improve the diagnostic performance of pulmonary nodules, and its specificity and sensitivity are better than those of single imaging diagnosis method. The combined prediction model is of great significance for the selection of surgical timing and surgical methods for pulmonary nodules, and provides a theoretical basis for the application of artificial intelligence in the pulmonary nodule diagnosis.
4.Sleep disturbance and hehavioral subsyndrome in patients with Alzheimer39;s Disease

Objective To investigate the characteristics of behavioral and psychological symptoms of dementia (BPSD) subsyndrome in patients with Alzheimer39;s disease (AD) and analyze the causes. Methods The demographic data, medical history and dietary habits of 137 patients with Alzheimer39;s disease who visited our hospital from January 2019 to October 2021 were collected by questionnaire. Excel 2007 and SPSS 22.0 software were used for statistical analysis of the occurrence of sleep disorders and BPSD subsyndrome. Multivariate logistic regression was used to analyze the influencing factors of its occurrence. Results There were 82 cases of sleep disorder (59.85%) and 118 cases of psycho-behavioral subsyndrome (86.13%) among those137 patients, of which 82 cases of sleep disorder mainly had low sleep quality (87.80%, 72/82), insufficient sleep time (73.17%, 60/82) and difficulty falling asleep (69.51%, 57/82); of which 118 cases of BPSD subsyndrome mainly had affective disorder (88.14%, 104/118), psychotic symptoms (76.27% , 90/118) and high activity (43.22% , 51/118). Multivariate logistic regression analysis showed that physical work , combined cerebral and cardiovascular diseases, dietary preference and severity of illness were the factors affecting sleep disorder (P<0.05) , while gender, residence, sleep disorder, whether the caregivers were full-time and severity of illness were the factors affecting the occurrence of behavioral subsyndrome (P<0.05). Conclusion Different types of sleep disorders and BPSD subsyndromes are common in patients with Alzheimer39;s disease. Attention should be paid to patients and intervention should be performed to improve their sleep and mental status.
5.The immediate implant and immediate restoration in the anterior maxilla with the digital technique: case report and literature review

Objective :
To describe a novel workflow for the anterior dental esthetics in immediate implant and immediate aesthetic restoration with digital technique.
Methods :
Before the implant placement, a intra-oral optical scan(IOS) combined with CBCT was performed to virtually design the 3D implant position and restoration. A surgical guide and an individualized Ti abutment restoration was CAD/CAM-fabricated out of the PMMA-based in a fully digital workflow and seated at the stage of minimal invasive surgery.
Results :
The technique achieved immediate implant and immediate restoration after tooth extraction. Immediate restoration of temporary crown could be completed immediately in second patient visit after the operation, finally realized the aesthetic implantation with preserved soft tissue contour.
Conclusin
The fully digital technique changed the conventional workflow which achieves more efficiency and better aesthetic effect.
