Objective This review aims to investigate the application of eye tracking(ET)in patients with speech impairment in the ICU.The review summarizes the current status and prospects of ET technology applications. Methods Following the scoping reviews framework,a systematic search was conducted across Web of Science,PubMed,Cochrane Library,Embase,CINAHL,CNKI,Wanfang Data,Chinese Medical Database, and Chinese biomedical database. The search covered publications from the inception of each database to October 29th,2024.The included studies were comprehensively analyzed. Results A total of 12 pieces of the literature were included, including 4 prospective cohort studies,4 experimental studies,3 prospective observational studies, and 1 randomized controlled trial. The application population of ET in the ICU mainly includes patients on mechanical ventilation, those at high risk of delirium, patients with spinal cord injury, etc. The types of integrated ET system equipment are mainly head-mounted and fixed;the types of ET involve gaze, blinking, etc.;the functions include standardized scale assessment, free interaction, and eye-movement model recognition. The main outcome indicators of the research are feasibility, physical symptoms and social-psychological status. Conclusion ET is applicable to a specific group of ICU patients with aphasia, and has shown good feasibility and effectiveness in the expression of patients'basic needs,self-assessment of symptoms and improvement of psychosocial status.

Objective To independently design a protective restraint band used for the interventional catheter bed and to evaluate its application effect. Methods The self-designed restraint belt consisted of a restraint belt body, first movable band and second movable band.A total of 200 patients, who received interventional operation at the Affiliated Run Run Shaw Hospital,School of Medicine of Zhejiang University of China from October 2022 to March 2023,were collected for this study. The patients were divided into the study group(n=100,using the self-designed restraint belt)and the control group(n=100,not using the self-designed restraint belt). The duration of surgical interruption due to patient's factors and the incidence of restraint belt-related adverse events were compared between the two groups. Results The duration of surgical interruption of the study group was(0.26±0.12)min,which was significantly shorter than(1.46±0.50)min in the control group, the difference was statistically significant(t=-23.570,P<0.01).The incidence of restraint belt-related adverse events in the study group and the control group was 0%and 7%respectively,the difference was statistically significant(x2=6.380,P=0.012).Conclusion The protective restraint band used for the interventional catheter bed can reduce the incidence of restraint belt-related adverse events and improve surgical efficiency.

Objective:To explore the psychological experience of ICU nurses caring for young patients with suicide attempts in a general hospital, and to lay a foundation for the study of psychological adjustment and intervention for ICU nurses. Methods:Twelve ICU nurses who had experience of caring for suicide attempted- young patients from Sir Run Run Shaw Hospital Affiliated to Medical College of Zhejiang University were selected by using descriptive qualitative research method and the maximum difference method of the purposive sampling during October to December in 2023, and semi-structured in-depth personal interviews were conducted. Traditional content analysis method was used to sort out and analyze the data and extract the themes. Results:A total of 12 nurses completed the interview, 5 males, 7 females, aged 23-38(29.33 ± 4.14) years, three themes and seven sub-themes were extracted: psychological characteristics of care (avoidance of active communication, fear of care, empathy); the support required for nursing care (support from colleagues; support from nursing managers); self-growth after caregiving (self-growth in terms of life and death, and self-growth in terms of children′s education). Conclusions:ICU nurses have a certain degree of psychological pressure when taking care of young patients with suicide attempts, which will produce empathy for patients and affect their self-values. At the same time, they need support from colleagues and nursing managers. Therefore, general hospitals should pay attention to the psychological state of ICU nurses, carry out the targeted communication training and mental health support for ICU nurses, and help them gain positive nursing career growth and values.

8.Sinicization of the Ethical Decision-Making Confidence Scale for nurse leaders and the test of its reliability and validity
Chinese Journal of Nursing 2024;59(11):1376-1381
Objective The Ethical Decision-Making Confidence Scale(EDMC)was translated into Chinese, and its reliability and validity were tested. Methods The English version of EDMC was translated into Chinese and back-translated according to the Brislin model. After being reviewed by the original authors, cultural adjustments were made, and a pretest was conducted to finalize the Chinese version of the scale. To test the reliability and validity of the scale,228 nurse leaders were investigated using a convenience sampling method in a tertiary hospital in Zhejiang from April to May 2023.Additionally,20 nurse leaders were selected to provide test-retest data 2 weeks after the survey. Results The Chinese version of EDMC contained 2 dimensions, namely skill-related confidence(9 items)and behavior-related confidence(6 items). The I-CVI ranged from 0.833 to 1.000,and the S-CVI/Ave was 0.967.Exploratory factor analysis extracted 2 common factors with factor loadings ranging from 0.546 to 0.828 for each item, contributing to a cumulative variance rate of 65.449%.The Cronbach's α coefficient of the overall scale was 0.947,and the Cronbach's α coefficient for each dimension ranged from 0.911 to 0.919.The split-half reliability of the total scale was 0.882,and the test-retest reliability was 0.752.Conclusion The Chinese version of EDMC demonstrated good reliability and validity, making it suitable for a comprehensive evaluation of the ethical decision-making confidence of nurse leaders. It can provide corresponding bases for medical administrators to make educational plans and targeted intervention strategies.

Objective To develop a geriatric-friendly care plan for elderly patients with chronic heart failure based on the 4Ms theoretical model and to validate its clinical effectiveness. Methods Based on literature review and patient interviews,a 4Ms geriatric-friendly care plan for elderly patients with heart failure was developed. The convenience sampling method was used to select A total of 160 patients admitted to a tertiary hospital in Zhejiang Province from August 2022 to June 2023 were selected as the study subjects.Among them,80 patients from January to June 2023 were assigned to an experimental group, and 80 patients from August to December 2022 were assigned to a control group. The experimental group received the 4Ms geriatric-friendly care plan developed in this study, while the control group received routine care. The differences in elderly-related complications during hospitalization, length of hospital stay, total hospitalization costs, satisfaction, and post-discharge outcomes(including readmission rates, improper medication and exercise capacity)were compared between the 2 groups. Results A total of 3 cases were lost, resulting in 78 cases in the experimental group and 79 cases in the control group. The experimental group had higher hospital satisfaction, lower hospitalization days, and lower total hospitalization costs compared to the control group(all P<0.01).Within 3 months post-discharge,the experimental group had lower readmission and inappropriate medication score, with better exercise capacity compared to the control group(all P<0.05).Conclusion The 4Ms geriatric-friendly care plan developed in this study demonstrates scientific and practical value. It effectively improves patient satisfaction, medication compliance, and exercise capacity, while reducing hospitalization days and readmission rates. This plan has significant references.
