1.Application of exosome-loaded hydrogel in nerve injury regeneration and wound healing

BACKGROUND:In recent studies,hydrogel loaded with exosomes has attracted wide attention as an emerging therapeutic strategy in the field of tissue engineering and regenerative medicine,and is considered as a promising means for the treatment of nerve regeneration and wound healing.OBJECTIVE:To review the application of hydrogel loaded with exosomes in nerve regeneration and wound healing and to provide reference and guidance for future research and clinical application.METHODS:The first author used a computer in May 2024 to retrieve the relevant literature published from January 2000 to May 2024 on PubMed and CNKI,searching for"exosome,hydrogel,nerve,nerve regeneration,wound,wound healing"in Chinese and English,eventually incorporating 66 papers for analysis.RESULTS AND CONCLUSION:(1)Hydrogel loaded with exosomes provides a promising path for nerve injury repair by exerting anti-inflammatory and anti-oxidation,stimulating axon growth and myelin regeneration.(2)Exosome-loaded hydrogel suppresses the level of inflammation and oxidative stress,accelerates the proliferation and migration of skin cells,collagen expression,and promotes blood vessel formation,significantly accelerates the wound healing process,and improves the healing quality.(3)The role of hydrogel loaded with exosomes in nerve regeneration and wound repair is still limited to cell and animal experiments,and does not involve clinical practice.In the future,more mechanistic studies,safety evaluation,and supplementary related clinical trials are still needed in the future.
2.Analyzing the influencing factors of hospitalization costs in patients with occupational noise-induced deafness

Objective To analyze the influencing factors of hospitalization costs in patients with occupational noise-induced deafness (ONID), and provide evidence for health policy development. Methods A total of 109 ONID patients in a hospital for occupational disease prevention and treatment in Guangdong Province were collected from 2018 to 2022 as the study subjects using a retrospective analysis method. Hospitalization information was collected, and influencing factors of hospitalization costs were analyzed. Results The average hospitalization cost per admission from 2018 to 2022 was 26 948.1, 27 805.0, 25 291.1,20 741.2 and 18 865.1 yuan, respectively, showing a downward trend. Grey relational analysis showed that the top three cost components most closely associated with the average hospitalization cost per admission were treatment expenses, medical service fees, and other expenses, with correlation coefficients of 0.786 1, 0.720 4, and 0.654 3, respectively. Univariate logistic regression analysis indicated that length of hospital stay was the primary influencing factor for average hospitalization cost per admission (P<0.01). Conclusion It is recommended to optimize clinical care pathways, improve diagnostic and treatment techniques, and reduce hospital stay duration, to lower the hospitalization costs of ONID patients.
3.Hepatitis E virus infection among blood donors in Ningbo

[Objective] To investigate the infection status and characteristics of HEV among voluntary blood donors in Ningbo, and to provide a basis for improving the blood screening strategy. [Methods] A total of 12 227 blood samples from voluntary blood donors in Ningbo from June 2022 to May 2023 were tested for HEV serology, enzymology, and nucleic acid testing. Furthermore, HEV gene sequencing was performed for genotyping analysis, and donors with reactive nucleic acid testing results were followed up to confirm their infection status. [Results] The reactivity rate of HEV Ag, anti-HEV IgM and anti-HEV IgG was 0.098%, 0.899% and 29.198%, respectively. There was no difference in the reactivity of anti-HEV IgM and anti-HEV IgG between genders, donation frequencies and donation types (P>0.05). The reactivity rate increased significantly with age (P<0.05). The rate of ALT disqualification (ALT>50U/L) was significantly higher than that in non-reactive samples (P<0.05). The HEV Ag reactivity rate (0.098%) was not correlated with gender, donation frequency, donation type or age. One HEV RNA positive case was found, with a positive rate of 0.008%(1/12 227). It was confirmed to be hepatitis E virus genotype 3 by sequencing analysis. Apart from HEV Ag reactivity, all other blood safety screening items were non-reactive, suggesting this case might be in the acute infection phase. The follow-up results showed that all indicators of the donor's previous blood donation were non-reactive. [Conclusion] Pre-donation ALT detection can reduce the risk of transfusion-transmitted HEV (TT-HEV) to a certain extent, and the effective way to prevent TT-HEV is to detect HEV RNA and serology of donor blood.
4.Research advances on necroptosis in viral infections

Programmed cell death plays an important role in the growth and development of organ-ism and homeostasis of tissues and organs.Necroptosis,a new mode of programmed cell death,characterized by necrosis in morphology,can cause cell breakdown and release of a significant num-ber of damage-associated molecular patterns,which play an important role in the occurrence and development of diseases.Viral infection is a serious health risk to both humans and domestic ani-mals.Research has confirmed that necroptosis functions as a cell death pathway in viral infections.In this paper,we review the current research advances on necroptosis related to viral infections,e-lucidate the molecular mechanism of mutual regulation between viral infection and necroptosis,and discuss the progress in the application of necroptosis inhibitors,to provide new ideas for the pre-vention and treatment of viral infections.
5.Application of active screening on carbapenem-resistant Enterobacterales monitoring in intensive care units:a multi-center study

Objective To evaluate the effectiveness of active screening in improving the detection rate of carbape-nem-resistant Enterobacterales(CRE)in the intensive care units(ICUs).Methods From July 2023 to June 2024,active screening of rectal swab CRE was conducted on ICU patients in 10 hospitals.ICU patients who underwent ac-tive screening from July 2023 to June 2024 were selected as the study group,while those who did not undergo active screening from July 2022 to June 2023 were selected as the control group.Difference in CRE detection rates between the two groups of patients was compared.Results A total of 7 803 ICU patients were included in the study group,744 CRE strains were detected,with a detection rate of 9.53%,out of which 304 CRE strains were detected through routine detection(detection rate 3.90%),3 707 patients underwent active screen,440 CRE strains were detected(detection rate 11.87%).7 561 ICU patients were included in the control group,out of which 250 CRE strains were detected through routine detection,with a detection rate of 3.31%.There was a statistically significant difference in the overall detection rate of CRE between two groups of patients(x2=246.18,P<0.001).In the study group,CRE detection rate of active screening(11.87%)was higher than that of routine detection(3.90%),with statistically significant difference(x2=264.26,P<0.001).A total of 17 CRE strains were detected from the study group.The proportions of Klebsiella pneumoniae(80.92%vs 73.41%)and Serratia marcescens(2.30%vs0.23%)in the routine detection group were both higher than in the active screening group,while the proportion of Escherichia coli in the routine detection group was lower(8.22%vs 19.55%),all with statistically significant differences(all P<0.05).Conclusion The prevalence of CRE in ICUs is relatively high,with a wide range of bac-terial species.Active screening can improve the detection rate of CRE.
6.Comprehensive Evaluation of the Quality of Huaihua Powder by Different Hospital Decoction Methods

Objective To comprehensively evaluate the quality difference of Huaihua powder obtained by the hospital's electromagnetic furnace decoction method and automatic decoction machine decoction method.Methods Five methods(induction cooker decoction(once decoction,twice decoction),normal pressure decoction(once decoction,twice decoction),and pressure decoction)were used to prepare the decoction.The HPLC method used a Shiseido CAPCELL PAK-C18(250 mm×4.6 mm,5 μm)column,with a UV detection wavelength of 254 nm,mobile phase of acetonitrile 0.1%phosphoric acid water,column temperature of 30℃,flow rate of 1.0 mL/min,and detection of index components including rutin,naringin,quercitrin,neohesperidin,quercetin,and menthone.The extraction rate of the index components was calculated and the cream yield of each decoction was detected.The evaluation of quality differences is based on the comprehensive score of cream yield(40%)and indicator components(60%).Results The HPLC fingerprint similarity of Huaihua powder prepared by 5 decoction methods was greater than 99%.There was no significant difference(P>0.05)in the yield of ointment and the content of indicator components between the induction cooker one decoction and the induction cooker twice decoction methods.The yield of ointment and the extraction rate of indicator components were significantly higher in twice normal pressure decoction using a decoction machine than once normal pressure decoction using a decoction machine(P<0.05).The index component score of machine under pressure decoction once was the highest,but the comprehensive score of twice normal pressure decoction was the highest,with a combined score of 92.13 and 89.58,respectively,far higher than the 36.01,18.79,and 15.60 of the other three decoction methods.Classify the decoction into three categories through cluster analysis.Orthogonal partial least squares-discriminant analysis was used to screen for rutin,naringin and neohesperidin as differential biomarkers.Conclusion The quality of Huaihua powder decocted by a decoction machine is significantly better than that by an induction cooker,and the twice decocted at normal pressure by the decoction machine are significantly better than those decocted at normal pressure once.Decoction under pressure once may be a better choice.
7.The psychological experience of ICU nurses caring for young patients with suicide attempts:a qualitative study

Objective:To explore the psychological experience of ICU nurses caring for young patients with suicide attempts in a general hospital, and to lay a foundation for the study of psychological adjustment and intervention for ICU nurses.Methods:Twelve ICU nurses who had experience of caring for suicide attempted- young patients from Sir Run Run Shaw Hospital Affiliated to Medical College of Zhejiang University were selected by using descriptive qualitative research method and the maximum difference method of the purposive sampling during October to December in 2023, and semi-structured in-depth personal interviews were conducted. Traditional content analysis method was used to sort out and analyze the data and extract the themes.Results:A total of 12 nurses completed the interview, 5 males, 7 females, aged 23-38(29.33 ± 4.14) years, three themes and seven sub-themes were extracted: psychological characteristics of care (avoidance of active communication, fear of care, empathy); the support required for nursing care (support from colleagues; support from nursing managers); self-growth after caregiving (self-growth in terms of life and death, and self-growth in terms of children′s education).Conclusions:ICU nurses have a certain degree of psychological pressure when taking care of young patients with suicide attempts, which will produce empathy for patients and affect their self-values. At the same time, they need support from colleagues and nursing managers. Therefore, general hospitals should pay attention to the psychological state of ICU nurses, carry out the targeted communication training and mental health support for ICU nurses, and help them gain positive nursing career growth and values.
