OBJECTIVES:
To evaluate the protective effect of Schistosoma japonicum cystatin (rSj-Cystatin) in a mouse mode of "two-hit" sepsis.
METHODS:
Sixty male C57BL/6 mice randomized equally into sham-operated group, protein group, "two-hit" modeling group, and protein intervention group. In the former two groups, the mice received an intraperitoneal injection of 100 μL PBS followed by exposure of the cecum and then by intraperitoneal injection of 100 μL PBS or 25 μg rSj-Cystatin 30 min later; In the latter two groups, 100 μL PBS containing LPS (5 mg/kg) was injected intraperitoneally 24 h before cecal ligation and puncture (CLP), and 100 μL PBS or 25 μg rSj-Cystatin were injected 30 min after CLP. At 12 h after rSj-Cystatin treatment, 6 mice from each group were sacrificed for detection of TNF-α, IL-6, IL-10, TGF-β, iNOS and Arg-1 in the serum, spleen, liver, lung and kidney tissues using ELISA, for examinations of liver, lung and kidney pathologies with HE staining, and for analysis of CD3+CD4+CD25+Foxp3+ T cell percentage in the spleen using flow cytometry. The remaining mice were observed for general condition and 72-h survival.
RESULTS:
The 72-h survival rates in the 4 groups were 100%, 100%, 0% and 20%, respectively, showing significant differences between the latter two groups. The mouse models of "two-hit" sepsis exhibited obvious tissue pathologies and significant elevations of TNF-α and IL-6 in both the serum and tissue homogenate, which were significantly ameliorated by rSj-Cystatin treatment. Treatment with rSj-Cystatin also increased IL-10 and TGF-β levels and spleen CD3+CD4+CD25+Foxp3+ T cell percentage. The septic mouse models also showed increased iNOS levels in all the detected tissues and a decreased Arg-1 level in the kidney, and these changes were obviously improved by rSj-Cystatin treatment.
CONCLUSIONS
rSj-Cystatin has a protective effect against "two-hit" sepsis in mice by regulating the inflammatory microenvironment.

3.Research on Building a Performance Evaluation Index System for the Specialty Cooperation Alliance in Hospital-Community

Objective To construct the performance evaluation index system of specialist cooperation alliance in hos-pital-community in order to scientifically evaluate the effect of the construction of the specialist cooperation alliance.Methods By searching literature related to the construction of specialized league,sort out national and local policy documents,and the"structure-process-result"model was adopted to initially construct the index system.The ex-pert letter consultation form of the indicator system was formed through the discussion of the special group,two rounds of Delphi expert consultation were carried out to determine the evaluation indicator system,and the weight coefficient of each indicator was determined by the simultaneous application of analytic hierarchy process.Results The effective recovery rates of the two rounds of questionnaire were 84.62%and 100.00%,respectively.The positive de-gree of experts was high,and the average authority degree of experts was 0.864.The importance,operability,sen-sitivity Kendall's W coefficient and x2 test value of all indexes were P<0.05.Finally,an evaluation index system covering 9 first-level indicators and 25 second-level indicators was established.Conclusion The constructed perfor-mance evaluation index is scientific and reliable.The practical application shows that it can be used as a tool to evalu-ate the effect of integrated,continuous and collaborative health service delivery in hospital-community.
4.Machine learning models based on ultrasonic texture features of coronary artery for predicting incomplete Kawasaki disease in children

Objective To explore the value of machine learning(ML)models based on ultrasonic texture features(TF)of coronary artery for predicting incomplete Kawasaki disease(IKD)in children.Methods Forty-eight children with IKD and 48 children without KD(non-KD)were enrolled with propensity score matching and divided into training set(n=67,34 cases of IKD and 33 cases of non-KD)and test set(n=29,14 of IKD and 15 of non-KD)at the ratio of 7∶3.Based on clinic-laboratory indicators(C-L)in training set and TF obtained with texture analysis of coronary artery ultrasound images,the optimal C-L-related features and TF were selected.Based on the optimal C-L correlated features,TF and their combinations,6 ML models,including random forest(RF),support vector machine(SVM),logistic regression(LR),gradient boosting decision tree(GBDT),decision tree(DT)and eXtreme gradient boosting(XGBoost)were respectively constructed for predicting IKD in children.The models were then trained in training set and validated in test set,and the best C-L ML,TF ML and C-L-TF ML models were selected.The area under the curve(AUC)of the best ML models were compared,and the clinical value of the best TF ML model was observed with decision curve analysis(DCA).Results Totally 3 optimal C-L related features and 8 optimal TF were selected.Among the constructed C-L ML,TF ML and C-L-TF ML models,C-L-LR model,TF-LR model and C-L-TF-SVM model were the optimal ones,with AUC in training set of 0.891,0.985 and 0.965,while in test set of 0.676,0.971 and 0.948,respectively.No significant difference of AUC was found between TF-LR model and C-L-TF-SVM model in both training set and test set(both P>0.05),which were both greater than those of C-L-LR model(all P<0.05).TF-LR model achieved higher clinical benefits in both training set and test set.Conclusion Ultrasound TF-LR model of coronary artery could be used to effectively predict IKD in children.

6.Effect of hyperthermic intraperitoneal chemotherapy (HIPEC) on postoperative immune function and tumor marker levels in patients with advanced gastric adenocarcinoma

Objective:To explore the effect of hyperthermic intraperitoneal chemotherapy (HIPEC) on postoperative immune function and tumor marker levels in patients with advanced gastric adenocarcinoma.Methods:A prospective cohort study was conducted, enrolling 72 patients with stage ⅢA-ⅢC gastric adenocarcinoma admitted to Jinjiang Hospital from August 2022 to December 2023. Patients were divided into the HIPEC group ( n=36, radical resection+ HIPEC within 3 days after surgery) and the control group ( n=36, radical resection alone) using the random number table method. The HIPEC protocol was perfusion with raltitrexed (4 mg in 4, 000 ml normal saline, at 43 ℃) for 60 minutes. Peripheral blood immune cells (CD3 +, CD4 +, CD8 +, CD4 + /CD8 + ratio) before treatment and 7 days after treatment, and serum levels of carcinoembryonic antigen (CEA), carbohydrate antigen 19-9 (CA19-9), and carbohydrate antigen 72-4 (CA72-4) 30 days after treatment were compared between the two groups. Results:Before treatment, there were no statistically significant differences in CD3 +, CD4 +, CD8 +, or CD4 + /CD8 + levels between the two groups (all P>0.05). Seven days after treatment, the HIPEC group had higher levels of CD3 +, CD4 +, and CD4 + /CD8 + ratio, and a lower CD8 + level than the control group, with statistically significant differences (all P<0.05). Before treatment, there were no significant differences in CEA, CA19-9, or CA72-4 levels between the two groups (all P>0.05). Thirty days after treatment, the HIPEC group had lower levels of CEA, CA19-9, and CA72-4 than the control group, with statistically significant differences (all P<0.05). Conclusions:HIPEC can significantly improve the postoperative immune function of patients with advanced gastric adenocarcinoma (by increasing the CD4 + /CD8 + ratio and immune cell activity) and effectively reduce tumor marker levels, which may provide a new strategy for preventing postoperative recurrence.

Objective:To evaluate the long-term effectiveness of lingual mucosal graft ureteroplasty (LMGU) for managing long-segment (≥5 cm) ureteral strictures in a multi-institutional cohort of patients.Methods:A multi-center retrospective case series study was conducted on clinical data from 42 patients undergoing LMGU for long-segment ureteral strictures (≥5 cm) across five institutions between February 2017 and June 2024. The cohort comprised 31 males and 11 females, with an age of (43.4±12.0) years (range: 15 to 64 years) and a body mass index of (24.6±2.6) kg/m2 (range: 16.0 to 30.0 kg/m2). Strictures involved the left ureter in 24 cases and right ureter in 18 cases, demonstrating a stricture length of (6.4±1.5) cm (range: 5.0 to 11.5 cm). Surgical interventions included either onlay ureteroplasty or augmented anastomotic ureteroplasty, selected according to intraoperative findings. Intraoperative parameters, postoperative complications, and follow-up outcomes were analyzed.Results:Laparoscopic surgery was performed in 22 cases and robot-assisted surgery in 20 cases. Among the 42 patients, 22 underwent onlay ureteroplasty while 20 received augmented anastomotic ureteroplasty. The graft length was (5.9±1.8) cm (range: 3.0 to 12.0 cm), operative time (191.5±55.6) minutes (range: 105.0 to 350.0 minutes), and intraoperative estimated blood loss (86.7±73.6) ml (range: 10.0 to 400.0 ml). All procedures were successfully completed without conversion to open surgery. The postoperative hospital stay was (7.6±2.0) days (range: 4.0 to 15.0 days), with double-J stent removal at 6 to 8 weeks postoperatively. During a follow-up of (49.1±25.0) months (range: 12.0 to 99.0 months), no stricture recurrence was observed in any patient.Conclusion:LMGU is a safe, feasible, and effective long-term technique for managing long-segment (≥5 cm) ureteral strictures.
