1.Preliminary study on the antibacterial material basis of Coptidis Rhizoma extracts by different methods based on spectrum-effect relationship

Objective To establish the HPLC fingerprints for the different extracts from Coptidis Rhizoma,and investigate the spectrum-effect relationship between HPLC fingerprints and anti-Staphylococcus aureus activity in vitro to analyze the pharmacodynamic material basis. Methods Nine kinds of Coptidis Rhizoma extracts were prepared, and establish the HPLC fingerprints for them. The antibacterial rate of each extract was determined by the broth microdilution method with Staphylococcus aureus as the test bacteria. The grey relational analysis (GRA) method was used to analyze the correlation between the fingerprint data and the in vitro antibacterial test data. Results The HPLC fingerprints of nine kinds of Coptidis Rhizoma extracts were established, nine common characteristic fingerprint peaks were calibrated, and 5 peaks were identified by the reference substance comparison method. GRA analysis experiments showed that the correlation between nine peaks and antibacterial effect was 0.559 1~0.803 3; and the peak 3, peak 8 (palmatine hydrochloride), peak 9 (berberine hydrochloride) were positively correlated with the inhibition of Staphylococcus aureus,while the peak 9 (berberine hydrochloride) had the strongest effect, and its correlation degree was 0.803 3. Conclusion The effective substance of antibacterial was preliminarily determined through the study of spectrum-effect relationship for Coptidis Rhizoma extracts,which may be the alkaloids, mostly containing hydrochloride, which provided a reference for further research on the pharmacodynamic material basis of Coptidis Rhizoma.
2.Effects and mechanisms of anti-enteritis properties of Yang Shuhua

Through molecular docking and experimental validation,24 SPF male KM mice were ran-domly divided into four groups:the control group(CON),model group(LPS),high-dose Yang Shuhua group(YSH-H),which was gavaged with drug solution containing 1 g/mL crude Yang Shuhua,and the low-dose Yang Shuhua group(YSH-L),which was gavaged with drug solution containing 0.5 g/mL crude Yang Shuhua for a 17-day experimental period.Mouse body weight was recorded during the experiment,and fecal scoring was done 1 h after intraperitoneal injection of LPS.At the end of the experiment,histopathological changes in the jejunum tissue were examined,and the expression of tight junction protein-related genes was detected.Compared to the CON group,mice in the LPS group showed significant decreases in villus height,villus height/crypt depth(V/C)ratio(P<0.01),and significant increase in crypt depth(P<0.01).The mRNA ex-pression levels of ZO-1 and claudin-1 were significantly decreased(P<0.01).Compared to the LPS group,mice in the YSH-H group showed significant decrease in crypt depth(P<0.05),significant increase in villus height(P<0.05),and significant increase in V/C ratio(P<0.01).The YSH-L group showed a significant increase in V/C ratio(P<0.05),a trend of decreased crypt depth,and an increasing trend of villus height.The mRNA expression of ZO-1 in the YSH-H group showed a significant increase(P<0.05)and claudin-1 showed an increasing trend.Luteolin,quercetin,kaempferol,eriodictyol,and the top 5 potential key targets TP53,IL-1β,TNF,AKT1,and IL-10 exhibited molecular docking scores less than-20.95 kJ/mol,indicating strong activity.GO and KEGG enrichment analysis suggested that Yang Shuhua compounds might act on enteritis through the TNF signaling pathway,IL-17 signaling pathway,and PI3K-Akt signaling pathway.The qPCR results showed an upward trend in the mRNA expression levels of TLR4 and AKT1,significant in-crease in Caspase-1 and ASC(P<0.05),and significantly increased expression of NF-κB and NL-RP3(P<0.01).The mRNA expression level of AKT1,TLR4,NLRP3,ASC,Caspase-1,and NF-κB decreased compared to the LPS group,with significant differences in Caspase-1 and NF-κB(P<0.05).This study suggests that Yang Shuhua exerts anti-enteritis effects through multiple compo-nents and pathways,with blockade of the TLR4-AKT-NF-κB pathway possibly being a key thera-peutic mechanism for treating enteritis.

4.Relationship between clopidogrel resistance and genetic variability in Kawasaki disease children with coronary artery lesions

Objective:To analyze the distribution of clopidogrel metabolism-related gene variability in Kawasaki disease (KD) children with coronary artery lesions (CAL) across different age groups and the impact of genetic variability on the efficacy of clopidogrel antiplatelet therapy.Methods:A retrospective cohort study was conducted. Clinical data were collected from 46 KD children with CAL who were hospitalized in the Cardiovascular Center of Children′s Hospital of Fudan University between January 2021 and August 2022 and were treated with clopidogrel, including gender, age, body mass index, course of KD, CAL severity grade, and baseline platelet count. According to their age, the children were divided into ≥2-year-old group and <2-year-old group. Their platelet responsiveness was assessed by adenosine diphosphate-induced platelet inhibition rate (ADPi) calculated via thromboelastography, and children were categorized into high on-treatment platelet reactivity (HTPR) and normal on-treatment platelet reactivity (NTPR) groups. Genotypes of CYP2C19, PON1 and ABCB1 were detected. The t test, one-way analysis of variance and Chi-square test were used for intergroup comparison. Results:Among the 46 KD children with CAL, 34 were male and 12 were female; 37 were ≥2-year-old and 9 were <2-year-old; 25 cases were in the HTPR group and 21 cases were in the NTPR group, with 19 HTPR and 18 NTPR in the ≥2-year-old group, and 6 HTPR and 3 NTPR in the <2-year-old group. Genetic analysis showed that 92 alleles among the 46 children, with frequencies of CYP2C19*1, CYP2C19*2, CYP2C19*3, CYP2C19*17, PON1 192Q, PON1 192R, ABCB1 3435C, ABCB1 3435T at 59% (54/92), 32% (29/92), 9% (8/92), 1% (1/92), 36% (36/92), 64% (59/92), 63% (58/92) and 37% (34/92), respectively. Analysis of the impact of genotype on ADPi revealed that in children aged ≥2 years, those with CYP2C19*1/*3 genotype had significantly lower ADPi than those with CYP2C19*1/*1 genotype ((34±15)% vs. (61±29)%, t=2.18, P=0.036). There were also no significant difference in ADPi among children with PON1 192Q homozygous, PON1 192R heterozygote and PON1 192R homozygous genotypes ((40±22)% vs. (52±33)% vs. (65±27)%, F=2.17, P=0.130), or among those with ABCB1 3435C homozygous, ABCB1 3435T heterozygote and ABCB1 3435T homozygous genotypes ((55±34)% vs. (60±27)% vs. (49±24)%, F=0.33, P=0.719). In <2-year-old group, there were no significant differences in ADPi across CYP2C19*1/*1, CYP2C19*1/*2 and CYP2C19*2*2 genotypes ((40±20)% vs. (53±37)% vs. (34±16)%, F=0.37, P>0.05). There were no significant differences in ADPi across CYP2C19*1/*1 and CYP2C19*1/*3 genotypes ((44±27)% vs. (42±20)%, t=0.08, P>0.05). There were no significant differences in ADPi across PON1 192Q homozygous, PON1 192R heterozygote and PON1 192R homozygous genotypes (45% vs. (55±27)% vs. (24±5)%, F=1.83, P>0.05). There were no significant differences in ADPi across ABCB1 3435C homozygous, ABCB1 3435T heterozygote and ABCB1 3435T homozygous genotypes ((36±16)% vs. (50±35)% vs. 45%, F=0.29, P>0.05). The risk analysis of HTPR in different genotypes revealed that in children aged ≥2 years, carrying at least 1 or 2 loss-of-function alleles of CYP2C19 was a risk factor for HTPR ( OR=4.69, 10.00, 95% CI 1.11-19.83, 0.84-119.32, P=0.033, 0.046, respectively), and PON1 192R homozygosity and carrying at least one PON1 192R allele were protective factors against HTPR ( OR=0.08, 0.13, 95% CI 0.01-0.86, 0.01-1.19, P=0.019, 0.043, respectively). Conclusion:KD children aged ≥2 years carrying CYP2C19 loss-of-function alleles and PON1 192Q are more likely to develop HTPR.
5.Feasibility of Two-Screw Anterior Fixation for Odontoid Fractures in a Chinese Population:A Morphometric Study Based on Computed Tomography

Background:
To evaluate the feasibility of treating odontoid fractures in the Chinese population with two cortical screws based on computed tomography (CT) scans and describe a new measurement strategy to guide screw insertion in treating these fractures.
Methods:
A retrospective review of cervical computed tomographic scans of 128 patients (aged 18–76 years; men, 55 [43.0%]) was performed. The minimum external transverse diameter (METD), minimum external anteroposterior diameter (MEAD), maximum screw length (MSL), and screw projection back angle (SPBA) of the odontoid process were measured on coronal and sagittal CT images.
Results:
The mean values of METD and MEAD were 10.0 ± 1.1 mm and 12.0 ± 1.0 mm, respectively, in men and 9.2 ± 1.0 mm and 11.0 ± 1.0 mm, respectively, in women. Both measurements were significantly higher in men (p < 0.001). In total, 87 individuals (68%) had METD > 9.0 mm that could accommodate two 3.5-mm cortical screws. The mean MSL value and SPBA range were 34.4 ± 2.9 mm and 13.5°–24.2°, respectively, with no statistically significant difference between men and women.
Conclusions
The insertion of two 3.5-mm cortical screws was possible for anterior fixation of odontoid fractures in 87 patients (68%) in our study, and there was a statistically significant difference between men and women.
6.Effects of hyperbaric oxygen combined with the intervention of functional position keeping pillow for the hemiplegic hand on the efficacy and motor function of hemiplegic patients after brain injury

Objective:To study the effects of hyperbaric oxygen (HBO) combined with the intervention of functional position keeping pillow for the hemiplegic hand on the efficacy and motor function of hemiplegic patients after brain injury.Methods:A total of 90 patients with hemiplegia after brain injury treated in Hai’an People’s Hospital from January 2017 to January 2020 were selected as the research subjects and divided into observation group and control group according to the random number table method, with 45 cases in each group. Both groups were given the intervention of functional position keeping pillow for the hemiplegic hand. The observation group was additionally treated with HBO. All patients received two-month treatment. The hand function was evaluated by the Brunnstrom Hand Function Scale and Barthel Index. The severity of pain was evaluated by the visual analogue scale (VAS). The conditions of hand care were evaluated by the hand care scale (including pressure sores, cleanliness, moisture, odor) designed by Hai’an People’s Hospital. The rehabilitation compliance was evaluated by the Posttraumatic Growth Inventory (PTGI) score and the Chinese version of the Connor-Davidson Resilience Scale (CD-RISC).Results:After treatment, the scores of Brunnstrom Hand Function Scale and Barthel Index of the two groups were significantly increased, and the scores of the observation group were significantly higher than those of the control group ( P<0.05); the VAS scores of the two groups were significantly decreased, and the scores of the observation group were significantly lower than those of the control group ( P<0.05); in terms of pressure sores, cleanliness, moisture, odor, and swelling degree, the scores of the observation group were significantly lower than those in the control group ( P<0.05); the scores of PTGI and CD-RISC of the two groups were significantly increased, and the scores of the observation group were significantly higher than those of the control group ( P<0.05). Conclusion:After brain injury, hyperbaric oxygen combined with the intervention of functional position keeping pillow for the hemiplegic hand can significantly improve the motor function of patients.

8.Determination of total polysaccharide content and analysis of monosaccharide components in the caulis polygoni multiflori mixture

Objective To establish a method for analyzing total polysaccharide content and its monosaccharide composition in the caulis polygoni multiflori mixture. Methods The polysaccharides of the caulis polygoni multiflori mixture were extracted by water-extraction and alcohol-precipitation. After the treatment with phenol-sulfuric acid, the content of total polysaccharides in the preparation was determined by ultraviolet spectrophotometry. In addition, after the polysaccharide was hydrolyzed into monosaccharides wtih trifluoroacetic acid, the hyrolysate was derivatized with PMP, and then the PMP derivates of monosaccharides were analyzed by HPLC method. Yilite krosmasil C18(4.6 mm×250 mm, 5 μm) was used at 30 ℃. Acetonitrile-0.1% NaH2PO4 (pH=6.8) (16:84) was the moblie phase with a flow rate of 1.0 ml/min. The detecting wavelength was at 250 nm. The injection volume was 20 μl. Results concentration of D-anhydrous glucose in the range of 21 ~ 105 μg/ml had a good linear relationship with the absorbance. The linear regression equation was A= 0.007x+0.0105, r=0.9982. The average recovery rate was (100.45±1.57)% (n=6). The average contents of total polysaccharides in four batches of samples were 14.24, 21.09, 17.85 and 18.17 mg/ml. The polysaccharide of the caulis polygoni multiflori mixture mainly consisted of D-mannose, D-glucosamine D-hydrochloride, D-Galacturonic acid, D-glucose, galactose, L-arabinose. The monosaccharides peak area ratios were about 9.10:0.26:1.00:3.02:4.14:2.12. Conclusion The method is accurate and reliable, and can be used for the determination of total content of polysaccharides and the analysis of monosaccharide composition in the preparation.
9.Optimization of Ultrasonic Extraction Process of Total Alkaloids from Oxytropis Falcata Bunge by Central Composite Design and Response Surface Method

Objective:To optimize the ultrasonic extraction process of total alkaloids from Oxytropis falcata bunge. Methods:The independent variables were solvents ratio, extracting time and ethanol concentration, and the dependent variable was content of total al-kaloids. Based on single factor tests, central composite design and response surface methodology was adopted to optimize the extraction technology. Results:The optimal extraction conditions were as follows: extracted twice with 36-fold amount of 72% ethanol ( contai-ning 1% acetic acid) at 60 ℃, and extracted 77 minutes each time. Under the above conditions, the content of total alkaloids was 2. 793 mg·g-1 with the bias ratio less than 2% when compared with the model predictions. Conclusion:Ultrasonic extraction process of total alkaloids from Oxytropis falcata Bunge optimized by central composite design and response surface method is simple, highly pre-cise, reliable and predictable.
10.Quality Standard for Compound Yinchen Mixture

Objective:To establish the quality standard for compound Yinchen mixture. Methods:A TLC method was used to i-dentify Artemisiae scopariae Herba,Gardeniae Fructus, and Rhei Radix et Rhizoma in the mixture. An HPLC method was used to quan-titatively analyze the concentration of chlorogenic acid and geniposide. Results:The TLC spots were clear without any interference the negative control. The linear range of chlorogenic acid was 0. 04-0. 20 mg·ml-1 ,and that of geniposide was 0. 05-0. 25 mg·ml-1 . The average recovery of chlorogenic acid was 94. 4% with RSD of 1. 85%, and that of geniposide was 102. 2% with RSD of 1. 15%( n=6). Conclusion:The method is accurate,reliable,specific and reproducible,which can be used for the quality control of compound Yinchen mixtures.
