1.Impact of self-disclosure on fertility anxiety in women of childbearing age with thyroid cancer: the mediating role of dyadic coping

Objective:To investigate the mediating effect of dyadic coping on the relationship between self-disclosure and fertility anxiety in women of childbearing age with thyroid cancer.Methods:A total of 258 women of childbearing age diagnosed with thyroid cancer who received treatment at the Department of Thyroid Surgery, First Hospital of Shanxi Medical University between January and October 2024 were selected using convenience sampling. Participants completed a general information questionnaire, the Chinese version of the Reproductive Concerns After Cancer scale (RCAC), the Dyadic Coping Inventory (DCI), and the Distress Disclosure Index (DDI). Structural equation modeling was constructed using SPSS PROCESS v4.1 to examine the mediating role of dyadic coping between self-disclosure and fertility anxiety. Pearson correlation analysis was used to assess the relationships among self-disclosure, dyadic coping, and fertility anxiety.Results:A total of 258 questionnaires were distributed, and 245 valid responses were collected, with an effective response rate of 94.96% (245/258). Among the 245 participants, the mean score of DDI was (37.38±8.42), RCAC was (53.54±12.53), and DCI was (117.10±29.07). Self-disclosure was positively correlated with dyadic coping ( P<0.01), while both self-disclosure and dyadic coping were negatively correlated with fertility anxiety ( P<0.01). Dyadic coping played a partial mediating role between self-disclosure and fertility anxiety, accounting for 43.38% of the total effect. Conclusions:Fertility anxiety in women of childbearing age with thyroid cancer deserves close clinical attention. Interventions should focus on enhancing patients' self-disclosure and dyadic coping abilities to effectively reduce fertility-related anxiety.

3.Two cases of pyridaben poisoning were successfully treated by blood purification

Pyridaben is a broad-spectrum acaricide widely used in agriculture, accidental or self-administration of large doses of pyridaben can cause multiple organ failure in patients. Due to its damage to multiple organs and no specific antidote, the mortality rate is high. This paper reports two patients who took a large amount of pyridaben, developed severe metabolic acidosis, hyperlactatemia, toxic encephalopathy, and liver, kidney, heart and digestive tract damage. After timely gastric lavage, catharsis, organ support andblood purification treatment, the condition improved and discharged. It is expected to provide clinical ideas for the treatment of pyridaben poisoning.

Objective To explore the effects of cognitive behavioral therapy based on 5G internet of things(IOT)on rehabilitation and kinesiophobia in elderly patients after percutaneous coronary intervention(PCI)for coronary artery disease.Methods Sixty elderly postoperative PCI patients with coronary artery disease admitted from June to December 2023 were selected as study subjects and randomly divided into the control group and the 5G group,with 30 cases in each group.The control groupreceived conventional nursing intervention,while the 5G group receivedcognitive behavioral therapy based on 5G IOT in addition to traditional nursing.Left ventricular ejection fraction(LVEF),6-minute walking distance(6MWD),the Tampa Scale for kinesiophobia Heart(TSK-SV Heart)scores,the Seattle Angina Questionnaire(SAQ)scores,and rehabilitation compliance were compared between the two groups.Results 12 weeks afterintervention,LVEF and 6MWD in the 5G groupwere significantly improved as compared with those in the control group(P<0.05).The TSK-SV Heart scores in the 5G group werelower than those in the control group(P<0.05),but the SAQ scores were higher(P<0.05).And the rate of better rehabilitation compliance in the 5G group was significantly higher than thatin the control group(P<0.05).Conclusions Cognitive behavioral therapy based on 5G IOT helps to improve the rehabilitation effect in elderly patients with coronary artery disease after PCI,reduce kinesiophobia,improve quality of life,and enhance reha-bilitationcompliance,thus promoting the rehabilitation process of the patients.

7. Effect of GSK-3β inhibitor on the expression of RANK-RANKL in rats kidney tissue with diabetic nephropathy

Objective:
To investigate the effect and significance of GSK-3β inhibitor(LiCl)and RANK-RANKL on the renal tissue of diabetic nephropathy(DN) rats.
Methods:
SD rats were divided into normal control group (NC), DN model group (DN) and GSK-3β inhibitor intervention group (LiCl). Twenty-four hour urine protein of rats were determined by Coomassie brilliant blue. Kidney tissue sections were stained by HE. The expression of GSK-3β, RANK and RANKL protein were determined by immunohistochemistry staining. The mRNA of GSK-3β, RANK, RANKL was detected by RT-qPCR.
Results:
Compared with NC group[(14.72±3.37)g], the level of 24-hour urinary protein[(154.17±20.65)g] increased significantly in DN group; compared with DN Group, the level of 24-hour urinary protein [(107.22±31.15)g]decreased in LiCl group(
8.Artery bypass surgery and endovascular treatment for subclavian arterial sclerosis occlusion disease

Objective To investigate the safety and medium-and long-term effects of endovascular stenting,axilloaxillary bypass (AAB),carotid-subclavian bypass (CSB) in patients of subclavian arterial occlusion.Method From 2001 to 2013,311 consecutive patients with subclavian arteriosclerosis obliterans were treated with endovascular stenting (n =191),axilloaxillary bypass (n =96) or carotidsubclavian bypass(n =32).We collected patients' medical data,calculated patency of the graft or stent with life-table method and compared patency between three approaches with Log-rank.Results The incidence of perioperative complications was 4.1% in the stenting group vs.11.5% in AAB group vs.18.7% in CSB group.There was significant statistical differences between the stenting group and bypass group about the incidence of perioperative complications.The primary patency rates at 1,3 and 5 years were 90.3%,84%,81.6% in stenting group vs.95.3%,92.6%,88.9% for AAB group vs.100%,96.4%,96.4% for CSB group.There was significant statistical differences between the stenting group and bypass group about the primary patency rates.Conclusions Both endovascular stenting and extrathoracic surgical bypass are safe and effective treatments for subclavian arteriosclerosis obliterans.However,effect of extrathoracic surgical bypass is more durable in the medium-and long-term.

10.Impact of glycogen synthase kinase-3β (GSK-3β) inhibitor on Wnt and NF-κB signal pathways in a rat model of diabetic nephropathy.

OBJECTIVETo explore the impact of glycogen synthase kinase-3β (GSK-3β) on Wnt and NF-κB pathways in a rat model of diabetic nephropathy (DN).
METHODSSD rats were randomly divided into normal control group (NC), DN model group (DM) and GSK-3β inhibitor group (DI). Blood glucose and 24-hour urine protein were monitored in three groups. Renal tissue samples were stained by HE. The expression of GSK-3β and NF-κB proteins was studied by immunohistochemistry. GSK-3β and NF-κB mRNAs were detected by RT-qPCR.
RESULTSTen weeks after STZ injection, the level of blood glucose increased significantly in DM group [(23.2±5.4) mmol/L] and DI group [(25.0±4.0) mmol/L], compared with NC group, and the level of 24-hour urinary protein increased significantly in DM group [(185.2±35.6) g/24 h] and DI group [(179.6±44.7) g/24 h], compared with NC group. Two weeks after LiCl injection, the level of blood glucose and 24-hour urinary protein decreased in DI group (17.6±2.1) mmol/L, (106.9±30.0) g/24 h], compared with DM Group. Compared with NC group, pathological changes of the kidney of DM group aggravated along with increased mRNA and protein expression of GSK-3β and NF-κB. But the pathological changes of the kidney in DI group alleviated along with declined mRNA and protein expression of GSK-3β and NF-κB as compared with DM group (all P<0.05).
CONCLUSIONSNF-κB protein expression positively correlates with the GSK3β expression. Wnt and NF-κB signal pathways play an important role in the development of diabetic nephropathy.
