1.Effect of Chai Yin Granules combined with oseltamivir on treatment of children with influenza virus infection and influence on miR-223/NLRP3 axis

OBJECTIVE To explore the effect of Chai Yin Granules combined with oseltamivir on treatment of the children with influenza virus infection and observe the impact on microribonucleic acid(miR)-223/nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)axis.METHODS A total of 104 children with influenza virus infection who were treated in Jinan Second Maternal and Child Health Hospital from Jan.2023 to Jan.2024 were enrolled in the study and were randomly divided into the study group with 52 cases and the control group with 52 cases.The study group was given Chai Yin Granules on basis of the treatment of the control group,the control group was given basic anti-infection therapy plus oseltamivir phosphate granules,and both groups were treated for 5 consecutive days.The clinical curative effect was compared between the two group;the time of im-provement of clinical symptoms,arterial blood gases(ABG)indexes,inflammatory indexes and miR-223/NLRP3 axis indexes before the treatment and after the treatment for 5 days as well as the incidence of adverse reactions during the treatment period were observed and compared between the two groups of children.RESULTS The total effective rate of the study group was 92.31%after the treatment for 5 days,higher than 76.92%of the control group(x2=4.727,P=0.030).The duration of virus turning to be negative,disappearance of sore throat,disap-pearance of cough and fever abatement was shorter in the study group than in the control group(P<0.05).The ar-terial partial pressure of oxygen(PaO2),pH,PaO2/oxygen concentration(FiO2)and miR-223 level of the two groups of children were higher after the treatment for 5 days than before the treatment;the above indexes of the study group were higher than those of the control group(P<0.05).The partial pressure of carbon dioxide(PaCO2),NLRP3 and inflammatory indexes levels were lower after the treatment for 5 days than before the treat-ment(P<0.05),and the levels of above indexes of the study group were lower than those of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.CONCLUSION Chai Yin Granules combined with oseltamivir can effectively improve the ABG indexes and clinical symptoms of the children with influenza virus infection and relieve the inflammatory reactions by regulating the ac-tivity of miR-223/NLRP3 axis so as to improve the illness condition of the children with influenza virus infection and boost the clinical curative effect,with the safety favorable.

3.Directional atherectomy combined with drug-coated balloon versus bare-mental stent for elderly femoropopliteal artery disease

Objective To compare the safety and efficacy of directional atherectomy with anti-restenosis therapy(DAART,drug-coated balloon)versus conventional balloon angioplasty(bare-metal stent,BMS)in elderly patients with femoropopliteal artery disease.Methods A retrospec-tive cohort study was conducted on 116 elderly patients undergoing endovascular intervention due to femoropopliteal artery disease in our hospital between May 2016 and September 2019,divided into DAART group(57 cases)and BMS group(59 cases).Results No statistical differences were observed between the DAART and BMS groups in terms of age,risk factors,distribution of Ruth-erford classification,lesion length,lesion type,lesion location,Global Limb Anatomic Staging Sys-tem grade,infra-popliteal runoff status,or preoperative ankle-brachial index(P>0.05).However,the BMS group had significantly larger proportion of chronic limb-threatening ischemia than the other group(P<0.05).Both groups achieved a 100%success rate of surgery.The DAART group obtained obviously higher primary patency rates at 1 and 2 years than the BMS group(90.4%vs 75.0%,76.3%vs 57.3%;P<0.045).There were no significant differences in the rate of freedom from target lesion revascularization at 1 and 2 years between the two group(96.1%vs 88.8%,91.6%vs 77.7%;P>0.05).In 2 years of follow-up,the incidence of major adverse events was 10.0%(5 cases)in the DAART group and 20.8%(12 cases)in the BMS group,but no obvious difference(P>0.05).Conclusion DAART demonstrates superior mid-term efficacy than BMS in treating femoropopliteal artery disease in elderly patients.However,comprehensive preoperative assessment is essential to optimize individualized treatment strategies for this population.
4.Impact of self-disclosure on fertility anxiety in women of childbearing age with thyroid cancer: the mediating role of dyadic coping

Objective:To investigate the mediating effect of dyadic coping on the relationship between self-disclosure and fertility anxiety in women of childbearing age with thyroid cancer.Methods:A total of 258 women of childbearing age diagnosed with thyroid cancer who received treatment at the Department of Thyroid Surgery, First Hospital of Shanxi Medical University between January and October 2024 were selected using convenience sampling. Participants completed a general information questionnaire, the Chinese version of the Reproductive Concerns After Cancer scale (RCAC), the Dyadic Coping Inventory (DCI), and the Distress Disclosure Index (DDI). Structural equation modeling was constructed using SPSS PROCESS v4.1 to examine the mediating role of dyadic coping between self-disclosure and fertility anxiety. Pearson correlation analysis was used to assess the relationships among self-disclosure, dyadic coping, and fertility anxiety.Results:A total of 258 questionnaires were distributed, and 245 valid responses were collected, with an effective response rate of 94.96% (245/258). Among the 245 participants, the mean score of DDI was (37.38±8.42), RCAC was (53.54±12.53), and DCI was (117.10±29.07). Self-disclosure was positively correlated with dyadic coping ( P<0.01), while both self-disclosure and dyadic coping were negatively correlated with fertility anxiety ( P<0.01). Dyadic coping played a partial mediating role between self-disclosure and fertility anxiety, accounting for 43.38% of the total effect. Conclusions:Fertility anxiety in women of childbearing age with thyroid cancer deserves close clinical attention. Interventions should focus on enhancing patients' self-disclosure and dyadic coping abilities to effectively reduce fertility-related anxiety.
5.Application Progress of Right Ventricular-Pulmonary Artery Coupling in Cardiovascular Disease

Right ventricular-pulmonary artery coupling(RVPAC)refers to the relationship between right ventricular contractility and afterload.Normal RVPAC is maintained only when right ventricular function and pulmonary vascular resistance are appropriately matched.When right ventricular contractility cannot increase to match afterload,thus resulting in right ventricular dysfunction.RVPAC plays an important role in pathophysiology and progression of many cardiovascular diseases,which is crucial for the prognosis of patients.Therefore,early and accurate evaluation of RVPAC has great significance for patient's condition assessment,clinical decision-making,risk stratification and prognosis judgment.There are many evaluation methods currently,which can be divided into invasive and non-invasive methods,among which the non-invasive methods are mainly correlated with ultrasound.This review summarizes pathological mechanism and evaluation methods of RVPAC,advantages and disadvantages of each method and application value of RVPAC in various cardiovascular diseases.
6.Research on the construction and practice of a standardized supply model for ophthalmic surgical in-struments based on lean theory

Objective To establish a standardized supply model for ophthalmic surgical instruments using lean manage-ment methods,develop a formula for determining optimal instrument inventory levels,and improve the management efficiency of ophthalmic surgical instruments.Methods A total of 2,281 cataract instrument sets from January 2023 to December 2024 were analyzed.From January to December 2023(1 141 sets),conventional process management was implemented,while from January to December 2024(1 140 sets),a lean theory-based standardized model was applied.Key parameters including surgical time,sterilization workflow,and instrument management were recorded to establish the model.A quantitative analysis incorporating sur-gical time,turnover interval,transportation time,relaxation rate,cycle interval,and target instruments was conducted to derive the inventory formula.A two-month pilot study on cataract surgeries was performed to validate the model's feasibility.The inci-dence of special events before and after implementing the standardized model was compared.Results The formula-derived data aligned with clinical demands,demonstrating precise quantification of ophthalmic instrument sets and high process stability.The incidence of special events post-implementation was significantly lower than pre-implementation(P<0.05).Conclusion The lean theory-based standardized supply model effectively enhances the management of ophthalmic surgical instruments.

8.Prevention and control effect of strengthening quality control management on potential risks in disin-fection supply room

Objective To clarify the potential risk factors of disinfection and supply room,and explore the application effect of strengthening quality control management,so as to improve the service quality of disinfection and supply room and reduce the risk of medical accidents.Methods From January 2022 to December 2023,800 medical devices processed in a disinfection supply room were selected as the study subjects.They were randomly divided into two groups of 400 each:a control group receiv-ing conventional management and an observation group receiving enhanced quality control management.Statistical analysis was conducted on potential risk factors,and the work quality of the two groups was compared.Results Univariate and multivariate logistic regression analyses revealed that potential risk factors included insufficient professional competence of staff,deficiencies in the management system,infrequent departmental training,and a noisy work environment(P<0.05).The observation group showed significantly higher qualified rates in various indicators,such as biological monitoring,compared to the control group(P<0.05).Furthermore,the observation group experienced significantly fewer risk events than the control group(P<0.05),indicating a statistically significant difference.Conclusion The primary potential risk factors in disinfection supply rooms in-clude systemic issues,staff quality,work environment,and training frequency.Addressing these factors by strengthening quality control management can improve staff professional competence,basic quality,and work attitude,thereby enhancing management quality,reducing the likelihood of risk events,and ultimately improving management safety.
