Objective:To evaluate the associations of sociodemographic characteristics and lifestyle factors with longevity status in older adults in China.Methods:After excluding those born after 31 st December 1938, a total of 51 870 older adults from the China Kadoorie Biobank (CKB) were included. The attained age was defined according to the survival age or age on 31 st December 2018. According to the attained age, the old persons were categorized into non-longevity (died before age 80 years) and longevity (attained age ≥80 years). The longevity group was further divided into two groups: longevity with death occurring before 2019, and longevity and survival to 2019. The information about socio-demographic characteristics and lifestyles was collected at the 2004-2008 baseline survey. Multinomial logistic regression models were used to analyze the associations between exposure factors and outcomes by taking the non-longevity group as the reference group. Results:A total of 51 870 older adults aged 65-79 years in the baseline survey were included for analysis. During a follow-up for (10.2±3.5) years, 38 841 participants were longevity, and 30 354 participants still survived at the end of 2018. Compared to men, rural populations, non-married individuals, those with an annual household income of less than 10 000 yuan, and those with education levels of primary school or below, the adjusted ORs(95% CI) for longevity and survival to 2019 in women, urban residents, married individuals, those with annual household incomes ≥20 000 yuan, and those with education levels of college or university were 1.68 (1.58-1.78), 1.69 (1.61-1.78), 1.15 (1.10-1.21), 1.44 (1.36-1.53), and 1.32 (1.19-1.48), respectively. The OR (95% CI) for longevity and survival to 2019 was 1.09 (1.08-1.10) for those with an increase of 4 MET-hour/day in total physical activity level. With those who never or almost never smoked, had no alcohol drinking every week, had normal weight (BMI: 18.5-23.9 kg/m 2), and WC <85 cm (man)/<80 cm (woman) as the reference groups, the ORs(95% CI) of longevity and survival to 2019 were 0.64 (0.60-0.69) for those smoking ≥20 cigarettes per day, 1.29 (1.14-1.46) for those with alcohol drinking every week, 1.13 (1.01-1.26) for those with pure alcohol drinking <30 g per day, 0.56 (0.52-0.61) for those being underweight, 1.27 (1.19-1.36) for those being overweight, 1.23 (1.11-1.36) for those with obesity, and 0.86 (0.79-0.93) for those with central obesity. Further stratified analysis by WC was performed. In the older adults with WC <85 cm (man)/<80 cm (woman), the ORs (95% CI) of longevity and survival was 1.80 (1.69-1.92) for those with each 5 kg/m 2 increase in BMI and 1.02 (0.96-1.08) for those with WC ≥85 cm (man)/≥80 cm (woman). There was a statistically significant difference in the association between BMI and longevity between the two WC groups (interaction test P<0.001). Conclusion:This study showed that women, the married, those with higher socioeconomic status and education level, and those with healthy lifestyles were more likely to achieve longevity.

Objective To investigate the job burnout of medical staff during long voyage in a navy warship troop and its influencing factors.Methods A total of 141 medical staff,who participated in a long voyage between January 2021 and December 2021,were consecutively included for surveys about basic information,job burnout,sleep quality,and regulation mode.Univariate and multivariate analyses were used to explore influencing factors for job burnout.Results The proportion of mild and below job burnout and moderate to severe job burnout in the cohort was 45.39%and 54.61%,respectively.Univariate analysis showed that married status,Master's or Doctor's degree,moderate to heavy workload,no psychological training experience,sleep disorder and low-exercise regulation mode were significant influencing factors for job burnout(P<0.05).Multivariate analysis showed that married status,Master's or Doctor's degree,sleep disorder and low-exercise and high-evaluation regulation mode were independent risk factors for job burnout(P<0.05).However,psychological training experience was considered to be a significant protective factor(P= 0.021).Conclusion Medical staff in a navy warship troop experiences severe job burnout during long voyage.The people who are married,have a Master's or Doctor's degree,have sleep disorder and low-exercise and high-evaluation regulation mode are more likely to experience job burnout,which should be paid more attention to.

Background::Reperfusion therapy is fundamental for ST-segment elevation myocardial infarction (STEMI). However, the details of contemporary practice and factors associated with reperfusion therapy in China are largely unknown. Therefore, this study aimed to explore reperfusion practice and its associated factors among hospitalized patients with STEMI in China.Methods::Patients with STEMI who were admitted to 159 tertiary hospitals from 30 provinces in China were included in the Improving Care for Cardiovascular Disease in China-Acute Coronary Syndrome project from November 2014 to December 2019. The associations of the characteristics of patients and hospitals with reperfusion were examined using hierarchical logistic regression. The associations between therapies and in-hospital major adverse cardiovascular events were examined with a mixed effects Cox regression model.Results::Among the 59,447 patients, 37,485 (63.1%) underwent reperfusion, including 4556 (7.7%) receiving fibrinolysis and 32,929 (55.4%) receiving primary percutaneous coronary intervention (PCI). The reperfusion rate varied across geographical regions (48.0%-73.5%). The overall rate increased from 60.0% to 69.7% from 2014 to 2019, mainly due to an increase in primary PCI within 12 h of symptom onset. Timely PCI, but not fibrinolysis alone, was associated with a decreased risk of in-hospital major adverse cardiovascular events compared with no reperfusion, with an adjusted hazard ratio (95% confidence interval) of 0.64 (0.54,0.76) for primary PCI at <12 h, 0.53 (0.37,0.74) for primary PCI at 12 to 24 h, 0.46 (0.25,0.82) for the pharmaco-invasive strategy, and 0.79 (0.54,1.15) for fibrinolysis alone.Conclusions::Nationwide quality improvement initiatives should be strengthened to increase the reperfusion rate and reduce inequality in China.Trial registration::www.ClinicalTrials.gov, NCT02306616

Objective This study intends to analyze the radioactivity level of slag and its building materials, estimate the effective dose of the residents caused by the comprehensive utilization of slag in building materials, and provide basis for rational utilization of slag. Methods Data of slag production and its comprehensive utilization in building materials, radioactive level investigation and detection of slag and building materials in different periods and regions were collected from 2005 to 2016. Using the room model of concrete structure and the model of calculating equilibrium equivalent radon concentration with the specific activity of ²²⁶Ra in indoor building materials, the external and internal radiation doses of residents caused by comprehensive utilization of slag for building materials were estimated respectively, and then the annual additional effective dose and collective dose of residents were estimated. Results The contents of ²²⁶Ra and ²³²Th in the slag were relatively high, while the contents of ⁴⁰K were relatively low. The radionuclide content in slag cement decreased significantly. The annual additional effective doses of slag used in concrete and cement mixture relative to ordinary cement were 0.40 mSv·a⁻¹ and 0.20 mSv·a⁻¹ respectively. The annual additional collective dose and 50-year collective dose caused by slag used in building materials were 3.87 × 10³～1.84 × 10⁴ man·Sv and 1.94 × 10⁵～9.20 × 10⁵ man·Sv respectively. Conclusion The comprehensive utilization of slag in building materials increases the additional effective dose of residents, so the use of some slag in building materials needs to be paid more attention to. In addition, the influence of comprehensive utilization of slag in building materials, such as the way, the amount of slag and the amount of building materials, on the annual additional effective dose of residents caused should also be paid attention to.

Methods: A total of 509 diabetic patients admitted to Department of Endocrinology and Metabolism at the Affiliated Hospital of Nantong University were recruited. Anthropometric parameters, serum biochemical index, glycosylated hemoglobin, urinary microalbumin/creatinine ratio, ISAs, fat mass, and islet β-cell function were measured. Multiple linear regression analysis was performed to identify relationships between ISA titers and clinical characteristics. Results: Compared with autoantibody negative group, blood pressure, weight, total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), visceral fat mass, fasting C-peptide (FCP), 120 minutes C-peptide (120minCP) and area under C-peptide curve (AUCCP) of patients in either autoantibody positive or glutamate decarboxylase antibody (GADA) positive group were lower.Body mass index (BMI), waist circumference, triglycerides (TGs), body fat mass of patients in either autoantibody positive group were lower than autoantibody negative group. GADA titer negatively correlated with TC, LDL-C, FCP, 120minCP, and AUCCP.The islet cell antibody and insulin autoantibody titers both negatively correlated with body weight, BMI, TC, TG, and LDL-C. After adjusting confounders, multiple linear regression analysis showed that LDL-C and FCP negatively correlated with GADA titer. Conclusion Diabetic patients with a high ISA titer, especially GADA titer, have worse islet β-cell function, but less abdominal obesity and fewer features of the metabolic syndrome.

OBJECTIVETo explore the status of articles published in Chinese Journal of Pediatrics from 2005 to 2014.

METHODAll the articles published in Chinese Journal of Pediatrics from 2005 to 2014 were searched at Wanfang Medical Online database. The total number and citations of articles, authors, agency, single article citation, internet downloads, columns, fund and Mesh were analyzed. The end of searching period was January 2015.

RESULTFrom 2005 to 2014, 2814 articles were published in Chinese Journal of Pediatrics, 235 to 380 articles per year. A total of 1 596 articles were cited, the citation rate was 56.16%, total number of citation was 15 428. Among single article citations, of the top 20 articles, 55% (11/20) were those published in the Standard/Protocol/Guide column. Of the top 20 papers most frequently downloaded on internet, 100% were articles published in the Standard/Protocol/Guide column. During the recent 10 years, the source of the papers published in the journal covered 31 provinces, municipalities and autonomous regions. The column that published the largest number of articles was Original Article (911, 32.05%), followed by Case Report (336,11.82%) and Review (245, 8.62%). Of the total number of articles published in the journal, 747 were supported by fund, which accounted for 26%. The articles supported by national fund accounted for 8%.

CONCLUSIONArticles published in Chinese Journal of Pediatrics had high-quality and can reflect the development and research progress in pediatric medicine. It is one of the most important information resources in pediatric academic fields in China.

BACKGROUNDThough there have been various methods for harvesting and preserving descemet membrane (DM) and intact endothelium, there is no literature about the morphological evaluation of endothelium after graft preparation for descemet membrane endothelial keratoplasty (DMEK). The aim of this study was to establish and improve a simple method for preparing, preserving, and morphologically evaluating the donor graft for DMEK.

METHODSTo obtain a donor graft, an air bubble was formed by injecting a 29 G needle with 1 ml sterile air into a small edge created outside the Schwalbe line. Another needle was inserted into the bubble through the stroma to aspirate the air or replace half the air with organ culture medium. Trypan blue was used to mark the location for small incision to improve the success rate. Frozen sections were stained with hematoxylin and eosin (HE). Based on the air bubble, DM grafts were divided into four groups: group A (normal control), graft without any operative technique; group B, graft with zero-pressure air bubble; group C, graft with full-pressure air bubble; group D, graft with half-pressure air bubble. The four groups of grafts were preserved for 24 hours to observe the effect of bubbles on cells. The gross and ultrastructure morphologies were evaluated using alizarin red and scanning electron microscopy (SEM), respectively.

RESULTSDonor grafts were harvested via the air bubble technique, facilitated by prior trypan blue staining. HE-stained sections revealed a pure graft without stroma. There were no significant changes under light microscope. In group A, SEM revealed a confluent layer of polygonal endothelium with distributed microvilli exhibiting characteristics of interdigitating junctions. In group B, intercellular borders became thinner. In group C, interdigitations were almost flat and microvilli were observed less frequently. In group D, other than less microvilli, there were minimal changes.

CONCLUSIONSThe donor graft preparation method appears to be effective and convenient. Properly decreasing the air pressure could protect and preserve the endothelium.
