1.Research on the application and ethical care of mental health services for the disabled elderly from the perspective of the holistic view of traditional Chinese medicine

With the deepening of population aging, the number of disabled and semi-disabled elderly individuals continues to increase, and the ethical care issues of mental health services for the disabled elderly are becoming increasingly prominent. When examining ethical issues, the concept of the “holistic view of traditional Chinese medicine” from ancient Chinese traditional culture was explored. While applying theories from the holistic view of traditional Chinese medicine, such as “the unity of form and spirit” and “the integration of form and spirit,” emphasis should be placed on the organic integration of psychology and physiology. The mental health status of the disabled elderly can be comprehensively improved by addressing both internal and external environmental factors. In response to the current situation of mental health services for the disabled elderly, ethical care should be provided for the mental health services of the disabled elderly, focusing on the value of subjectivity, difference, and continuity in ethical care, thereby improving the mental health status of the disabled elderly.
2.Research of DIP grouping of malignant tumor chemotherapy patients based on a decision tree model

Objective:To explore the grouping and standard cost of malignant tumors chemotherapy patients under the diagnosis-intervention packet (DIP) system based on the decision tree model, for references for optimizing the detailed grouping scheme of this disease.Methods:The data of the first page of medical records of malignant tumors chemotherapy patients in a tertiary hospital in 2022 were collected. Univariate analysis and multiple linear regression were used to analyze the influencing factors of patients′ hospitalization expenses. The Chi squared automatic interaction detection was used to construct the decision tree model to obtain the case grouping scheme and its standard expenses. The coefficient of variation and chi-square test were used to evaluate the grouping effect.Results:A total of 27 235 patients were included in this study. The number of surgical operations, length of hospital stay, gender, the number of other diagnoses, chemotherapy pathways and targeted therapy were taken as classification nodes and included in the decision tree model. A total of 13 groups were formed. The homogeneity within the groups was good(CV<0.70), and the heterogeneity between the groups was strong( χ2= 9 564.65, P<0.001). Conclusions:Based on the decision tree model, the grouping scheme for chemotherapy cases of malignant tumors was established by comprehensively considering factors such as surgical operations within the group, length of hospital stay, other diagnostic and chemotherapy pathways is relatively reasonable, which could provide references for relevant management departments to optimize the detailed grouping scheme of this disease and formulate relevant payment standards.

[Objective]To provide reference and beneficial assistance for the construction of contemporary medical ethics through studying the inherent requirements of Neo Confucianism in the Song and Ming dynasties,revealing its shaping role in the internal drive of medical ethics,the path of medical ethics cultivation,the goals of medical ethics and medical ethics ideas.[Methods]Systematically review the relevant discussions and moral practices of medical ethics by physicians under the influence of Neo Confucianism in the Song and Ming dynasties.Examine the shaping methods and processes of traditional Chinese medicine medical ethics in ancient China,study the core content of internal drive of medical ethics,key requirements of medical ethics cultivation path,practical approaches to medical ethics goals and fundamental principles of medical ethics thought.[Results]After the Song Dynasty,traditional Chinese medicine ethics was a value system centered on Neo Confucianism.Under the infiltration of Neo Confucianism's ideas such as"understanding benevolenc""filial piety is the foundation of benevolence""understanding the body and applying it""the concept of reason desire",ancient Chinese medicine gradually formed moral emotions such as"promoting oneself and others""knowing medicine for filial piety""practicing practical skills""benefiting the world and living people".[Conclusion]Ancient medical ethics still shine with the radiance of human nature in today's society.By setting a moral example,it guides medical personnel to form a moral standard of"medicine is benevolent"and"not seeking benefits,disregarding achievements".Neo Confucianism still has a positive guiding role today.Injecting the ideas of"benevolence""filial piety""practical application""rational desire"into the contemporary medical ethics construction system can help to build a new era of medical ethics system and promote the achievement of the strategic goal"Healthy China".
5.Pathological Networks Involving Dysmorphic Neurons in Type II Focal Cortical Dysplasia.

Focal cortical dysplasia (FCD) is one of the most common causes of drug-resistant epilepsy. Dysmorphic neurons are the major histopathological feature of type II FCD, but their role in seizure genesis in FCD is unclear. Here we performed whole-cell patch-clamp recording and morphological reconstruction of cortical principal neurons in postsurgical brain tissue from drug-resistant epilepsy patients. Quantitative analyses revealed distinct morphological and electrophysiological characteristics of the upper layer dysmorphic neurons in type II FCD, including an enlarged soma, aberrant dendritic arbors, increased current injection for rheobase action potential firing, and reduced action potential firing frequency. Intriguingly, the upper layer dysmorphic neurons received decreased glutamatergic and increased GABAergic synaptic inputs that were coupled with upregulation of the Na+-K+-Cl- cotransporter. In addition, we found a depolarizing shift of the GABA reversal potential in the CamKII-cre::PTENflox/flox mouse model of drug-resistant epilepsy, suggesting that enhanced GABAergic inputs might depolarize dysmorphic neurons. Thus, imbalance of synaptic excitation and inhibition of dysmorphic neurons may contribute to seizure genesis in type II FCD.

6.Oral lichen planus induced by long-term use of antimicrobials for recurrent aphthous ulcer:A case report and literature review

The precise etiology of oral lichen planus(OLP)is still unclear,but the existing evidence suggests that drug intake,virus infection,fungal infection,psychological disorders,and immunodeficiency are closely associated with the pathogenesis of OLP.We report a case of OLP accompanied with candidiasis induced by long-term use of antimicrobials for recurrent aphthous ulcer(RAU)and update the literature,to discuss the possible association between OLP and misuse of antimicrobials,and to inform general dentists and pharmacists the importance for practice with optimal antimicrobial stewardship.In this case,a 42-year-old man presented to Xiangya Stomatological Hospital with white reticular patterns spreading in the oral cavity for almost 1 year.He was diagnosed with OLP via histopathological examination.He had a 5-year history of RAU which occurred every 1-2 months,and he was given antimicrobials ingested or injected whenever the ulcers came up.Satisfactory treatment results were obtained by stopping the abuse of antimicrobials and local antifungal therapy.Meanwhile,the exacerbation and alleviation of OLP was closely related to the administration of antimicrobials.Combined with literature review,antimicrobial might contribute to the development of OLP by inducing candidiasis,a common side-effect of misuse of antimicrobials.Considering the seriousness of antimicrobial resistance and opportunistic infection,dentists should prescribe antimicrobials judiciously according to guidelines and evidence-based indications.Appropriate prescribing of antimicrobials is a professional responsibility to all dentists.
7.Nomenclature standardization of radiotherapy in cervical cancer

Objective:To standardize the naming of organ at risk (OAR) and target area during cervical cancer radiotherapy based on AAPM TG-263.Methods:After self-programming of Matlab software to implement the reading and resolution of radiotherapy structure files, the naming of each substructure was automatically output, recorded and restored. After naming all substructures, the structure names were classified by keywords. According to TG-263, a standard naming conversion table of OAR and target area was developed, and the classified structure names were standardized through procedures. Finally, the standardized named radiotherapy structure files were output and imported into the treatment planning system (TPS).Results:The radiation structure of 144 patients with cervical cancer was successfully transformed and displayed correctly in TPS. Before the transformation, the naming of OAR and target area lacked of uniform norms and standards, and the naming of the same structure significantly differed. After the transformation, 43 naming methods of OAR and 74 naming methods of the target area were unified into 20 and 8 naming methods, which were more convenient for staff understanding and communication.Conclusion:The standardization of cervical cancer radiotherapy structure naming can reduce the inconsistency of naming and provide reference for the standardized naming of pelvic tumors.
8.Safety and efficacy of therapeutic ERCP for patients of over 90 years of age

Objective To evaluate the safety and efficacy of therapeutic ERCP for patients above 90 years of age.Methods The data of 37 patients of above 90 years who underwent 42 ERCP procedures from January 2001 to December 2009 were studied retrospectively and compared with those of 152 matched patients ( 168 procedures) below 65 years old at a 1∶4 ratio for success rate and complications.Results The rate of complete success,partial success,and failure in observation group was 73.81% (31/42),19.05%(8/42) and 2.38% (1/42),respectively,which were similar (P >0.05) with those in control group,with complete success rate at 85.12% ( 143/168),partial success rate at 12.50% (21/168) and failure rate at 2.38% (4/168).The rate of terminated operation in observation group (4.76%,2/42) was significantly higher than that of the control group (0.00%,0,P =0.039).The overall rate of complication in observation group was 7.14% ( 3/42 ),slightly higher than that of the control group ( 6.55%,11/168,P >0.05 ).There was no significant difference between the two groups regarding the rates and severity of such complications as pancreatitis,hemorrhage and infection ( P > 0.05 ).No perforation or death was observed.Conclusion Therapeutic ERCP for patients of 90 years or older is safe and effective.Adverse events related to chronic concomitant diseases need early detection and proper management.
