1.Association between inflammation-related dietary patterns and cognitive impairment in older adults aged 65 years and above in longevity areas of China: a reduced rank regression analysis

Objective:To analyze the association between inflammation-related dietary patterns and the risk for cognitive impairment in older adults aged ≥65 years in longevity areas in China by using reduced rank regression (RRR) analysis.Methods:This study used cross-sectional data from the 2021 Healthy Aging and Biomarkers Cohort Study, including the information about study participants' demographic characteristics, lifestyles, daily life activities, and disease histories. Dietary intake was obtained by using a simplified food frequency questionnaire. Cognitive impairment was evaluated based on the Mini-Mental State Examination Scale combined with years of education. Fasting venous blood samples were collected to detect inflammatory markers, especially high-sensitivity C-reactive protein (hs-CRP) and the platelet-to-lymphocyte ratio (PLR). RRR analysis was used to obtain inflammation-related dietary patterns using hs-CRP and PLR as response variables. Multivariate logistic regression model was used to analyze the association between dietary pattern score and the risk for cognitive impairment. Restricted cubic spline was used to explore the dose response relationship, and mediation analysis was used to quantify the mediating effects of hs-CRP and PLR.Results:Two dietary patterns were identified with RRR. The primary pattern was characterized by higher intakes of flour, red meat, and dairy products, and lower intake of fresh vegetables, explaining 6.84% of the variance in food intake and 0.50% of the variance in inflammatory markers. Compared with the T1 group, the T3 group had significantly higher risk for cognitive impairment ( OR=1.242, 95% CI: 1.034-1.491). Each one standard deviation increase in the dietary pattern score was associated with an 8.7% increase in the risk for cognitive impairment ( OR=1.087, 95% CI: 1.008-1.172), with a significant linear trend (overall-model P<0.001, non-linear P=0.295). Mediation analysis indicated that hs-CRP mediated 6.2% of the association between the dietary pattern and the risk for cognitive impairment. Conclusion:The inflammation- related dietary pattern characterized by higher consumption of flour, red meat, and dairy products and lower consumption of fresh vegetables is associated with an increased risk for cognitive impairment in older adults, and hs-CRP partially mediates this association.
2.Prevalence of herpes zoster and influencing factors in population in China

Objective:To understand the prevalence of herpes zoster and its influencing factors in population in China, and provide reference for the development of herpes zoster prevention strategies.Methods:A survey was conducted in 27 868 study participants in 25 provinces in China from July to September 2024. The prevalence of herpes zoster was calculated, and multivariate logistic regression analysis was conducted to identify influencing factors.Results:The overall prevalence of herpes zoster in population in China was 6.15% (95% CI: 5.87%-6.43%) and the weighted overall prevalence was 5.89% (95% CI: 5.61%-6.17%). The prevalence in women (6.74%, 95% CI: 6.32%-7.15%) was higher than that in men (5.56%, 95% CI: 5.18%-5.94%). The prevalence increased with age, and the prevalence was 1.33% (95% CI: 1.05%-1.62%) in age group 0-17 years and 12.95% (95% CI: 11.77%-14.13%) in age group ≥70 years. The prevalence in urban population (6.95%, 95% CI: 6.54%-7.37%) was higher than that in rural population (5.30%, 95% CI: 4.92%-5.67%). The prevalence varied in people with different educational levels and increased with the increase of educational level. The prevalence had significant differences among people with different income levels. The prevalence of herpes zoster in adults with chronic diseases (11.09%, 95% CI: 10.45%-11.72%) was higher than that in those without chronic diseases (4.60%, 95% CI: 4.24%-4.97%). Multivariate logistic regression analysis showed that the risk for herpes zoster in women was 1.28 (95% CI: 1.15-1.42) times higher that in men. Older age, higher educational level, urban residence, and suffering from chronic diseases were all associated with increased risk for herpes zoster. Conclusions:The prevalence of herpes zoster was high in population in China. In the prevention and treatment of herpes zoster, it is necessary to take targeted protection measures for key populations, such as women, patients with chronic diseases, middle-aged and elderly people.

5.Exploration of radiotherapy as a combined treatment modality with in situ vaccines in the treatment of advanced soft tissue sarcomas

Objective:To evaluate the efficacy and safety of radiotherapy as a combined mode with in-situ vaccine for patients with advanced soft tissue sarcoma(STS).Methods:The clinical data of 12 patients with advanced STS who received combination therapy mode at the Cancer Center of Gulou Hospital Affiliated to the School of Medicine of Nanjing University between December 2020 and September 2024 were retrospectively analyzed.All 12 patients received combined therapy.The main radiotherapeutic approach was hypofractionated radiotherapy.The targeted therapy mainly involved Anlotinib(in 10 cases)or Apatinib(in 2 cases).Immunotherapy mainly involved PD-1 antibodies.The primary endpoint was disease control rate(DCR),and the secondary endpoints were objective response rate(ORR)and safety.Results:Among the 12 STS patients who received combined treatment,0 cases achieved CR,4 cases achieved PR;7 cases had SD,and 1 case had PD.The ORR was 33%,and the DCR was 91.7%,among which the DCR of the target lesions was 100%.Among the 12 patients,9 patients experienced grade Ⅰ to grade Ⅱ adverse reactions.The most frequently occurring hematological adverse reactions were anemia(6 cases)and abnormal results of liver function tests(3 cases).The most frequently occurring non-hematological adverse reactions were proteinuria(5 cases),hypertension(4 cases),abnormal thyroid function(3 cases),anorexia(3 cases),and nausea and vomiting(2 cases).Only 2 cases had grade Ⅲ hematological toxicity,and 1 case had grade Ⅲpneumothorax.Conclusion:Radiotherapy as a combined therapy mode with in situ vaccine can achieve a higher DCR in advanced soft tissue sarcomas without serious adverse reactions.This combined treatment modality demonstrates good efficacy and safety.
6.Status quo and development recommendations for infectious disease early warning system in public general hospitals in China

Early warning of infectious disease outbreak is key to controlling epidemics. Public general hospitals can effectively play their " outpost" role in the early warning and response of infectious diseases, which is directly related to the success of epidemic prevention and control. The authors summarized the current construction and main functions of infectious disease early warning systems at national level, regional level and public general hospitals, analyzed the problems existing in such construction, and put forward targeted suggestions. As found by the authors, some public general hospitals in China have initially built an early warning system for infectious diseases, achieving early recognition, early warning and reporting of infectious diseases to some extent. However, these systems were challenged by such shortcomings as insufficient intelligence, lack of data information exchange, immature practice and application, and lack of attention to the monitoring and early warning of endemic infectious diseases. It is suggested to improve the policy and system support in the future, to expand the types of infectious diseases for monitoring and early warning, to enlarge the sources of monitoring data and to strength hospital informationization construction.
7.Intraductal papillary mucinous neoplasm of the pancreas: clinical characteristics and treatment outcomes of 361 patients

Objective To analyse the clinical characterization of intraductal papillary mucinous neoplasm of pancreas (IPMNs) and to improve the understanding of IPMNs.Methods Three hundred and sixty-one patients with IPMN at Shanghai Changhai Hospital between 1993 and 2012 were retrospectively analyzed.Results Among 361 cases of IPMNs,241 were men and 120 were female,with a female to male ratio of 2.6∶1.Mean age of the patients was 62 years old (16 ~ 87 years old).The duct type included the main duct in 102 patients (28.3%),a branch duct in 109 (30.2%),and mixed ducts in 150 (41.6%).The most common symptom at presentation is pancreatitis,which occur in 167 patients (46.3%).The sensitivity for the detection of IPMN was 80.2% by ERCP,76.9% by MRCP,63.9% by CT,and 50.5% by EUS.One hundred and twenty-nine patients (35.7%) were operated and diagnosed with 87 (67.2%) IPMA,21 (16.4%) IPMB,10(8.2%) IPMC(CIS) and 10 (8.2%) had invasive carcinomas.The 5-year survival rates of IPMA,IPMB,IPMC,and invasive carcinomas were 100%,100%,66.5%,and 44.7%,respectively.Conclusions IPMNs were fregrently occured is 60 years old,half of patients had acute pancreatitis history and the frequtly attack,mixed type is most anatomy type,and the outcome after surgery is good.
