Hepatitis B virus (HBV) exclusively infects liver parenchymal cells and forms covalently closed circular DNA (cccDNA) within their nuclei. HBV cccDNA serves as the essential template for viral gene transcription, the sole source of progeny virus production, and the key driver of viral antigen expression, and it is the molecular basis for the persistence of HBV infection. Therefore, elimination and/or functional silencing of cccDNA is the key to eradicate chronic HBV infection. This article discusses the critical scientific issues that need to be solved during elimination of the harm of HBV infection from the perspectives of the synthesis, transcription, and clearance of cccDNA, as well as the impact of nonparenchymal cells on cccDNA, in order to provide a reference for eradicating HBV infection in the future.

Objective:To observe the therapeutic effect of combining low-frequency pulsed electrical stimulation with Beckman oral muscle exercise training in relieving drooling among persons with Parkinson′s disease (PD).Methods:A random number table was used to divide 120 PD patients with drooling into a mouth muscle training group, an electrical stimulation group, and an observation group, with 40 patients in each group. In addition to routine medication, the oral muscle training group was given Beckman oral muscle exercise training, the electrical stimulation group underwent low-frequency pulsed electrical stimulation treatment, while the observation group was provided with both. Before and after 4 weeks of treatment, the severity of salivation, the frequency of repeated empty swallowing, oral motor function, saliva secretion, and life quality of the three groups were evaluated using the Saliva Rating Scale (DRS), the Repeated Saliva Swallowing Test (RSST), oral motor function grading, the Parkinson′s Disease Saliva Clinical Scale (SCS-PD), saliva weighing, and the PD Quality of Life Scale (PDQ-39).Results:After the treatment the average DRS, SCS-PD, saliva weighing and PDQ-39 results of the observation group were significantly better than those before treatment and better than the other 2 groups′ averages. That group′s average RSST and oral motor function scores had increased significantly compared to before treatment, and compared with the other 2 groups′ averages at the same time point. Conclusions:Combining low-frequency pulsed electrical stimulation with Beckman oral muscle exercise can improve oral motor function, swallowing, and the life quality of PD patients who drool. It is more effective than electrical stimulation or oral muscle exercise training alone. Such combination therapy is worthy of clinical promotion and application.

Objective:To investigate the anatomical feasibility of the endoscopic transorbital approach (ETOA) to the orbital apex and lateral middle cranial fossa, to identify stable and recognizable surgical landmarks under endoscopic visualization, and to provide morphometric data for preoperative planning and intraoperative navigation. Methods:Stepwise anatomical dissection was performed on five formalin-fixed cadaveric heads and one fresh arterially injected cadaveric specimen to simulate the ETOA using a 0° endoscope. Key structures and their anatomical relationships were observed and recorded. Additionally, high-resolution CT scans of 50 adults were retrospectively analyzed. Three-dimensional reconstructions and measurements were performed using Mimics 17.0 software. Spatial validation was performed using 17 dry skulls to verify the consistency and reliability of osseous anatomical landmarks. Results:Cadaveric dissection identified the meningo-orbital band, superior orbital fissure, optic canal, foramen rotundum, and foramen ovale as reliable surgical landmarks for the ETOA. A topographic map of the surgical region was established based on the endoscopic view. CT measurements revealed the following distances (Mean±SD): the midpoint of the supraorbital rim to the foramen rotundum (57.31±3.59) mm and foramen ovale (71.46±3.42) mm; the lateral orbital rim to the lateral edge of the superior orbital fissure (37.38±2.52) mm; the distance from the superior orbital fissure to the optic canal (9.98±1.49) mm; and the distance from the anterior ethmoidal artery to the optic canal (19.98±2.05) mm. These measurements were consistent with dry skull data, indicating that these osseous landmarks had stable spatial relationships and were suitable for intraoperative localization. Conclusions:The ETOA provides favorable anatomical accessibility and clinical feasibility for lesions involving the orbital apex and lateral skull base. Key osseous structures demonstrate high identifiability and stable spatial relationships, serving as critical references for intraoperative navigation and preoperative pathway planning. The quantitative anatomical framework established in this study provides critical morphometric support for minimally invasive surgery targeting lesions in this region.

Objective:To analyze the epidemiological characteristics and genetic evolution of human coronavirus OC43(HCoV-OC43)in Hangzhou from 2018 to 2023,and provide molecular epidemiological basis for the prevention and control of HCoV-OC43 infection. Methods:A total of 11 904 samples of cases with acute respiratory tract infection were collected from two sentinel hospitals in Hangzhou. The collection period was from January 2018 to December 2023. HCoV-OC43 was detected by real-time fluorescent quantitative RT-PCR. The positive samples were amplified by PCR using specific primers, and the amplified products were sequenced using third-generation nanopore technology for the whole genome. The genetic evolution features were analyzed. Results:A total of 265 HCoV-OC43 positive samples were detected, with a positive detection rate of 2.23%(265/11 904). There was no significant difference in detection rate between males and females(χ 2=0.042, P>0.05);the detection rate varied significantly among different age groups(χ 2=60.104, P<0.01),with higher detection rates observed in children under five years old and individuals over 65 years old. The infection peaks in 2018-2019 and 2023 occurred in spring and summer, while the infection peak in 2020-2022 was in summer and autumn. A total of 85 complete HCoV-OC43 whole genome sequences were obtained through high-throughput sequencing. The evolutionary analysis indicated that the K genotype was predominant in Hangzhou from 2018 to 2023. In 2019,both J and K genotypes co-existed. In 2023,a new genotype, temporarily named genotype L,was discovered, which originated from genotypes J and K. The K genotype had 22 specific amino acid variation sites, most of which were located in the important domain of S protein. The J genotype had seven specific amino acid variation sites, most of which were located in non-structural proteins. The new genotype L had 10 specific amino acid mutation sites, which were distributed across six proteins, and these mutation sites had the most extensive coverage. Conclusions:HCoV-OC43 is an important pathogen causing respiratory diseases in children and the elderly in the Hangzhou area, and there are multiple genotypes in the Hangzhou area, with a new recombinant genotype L discovered in 2023.

Journal of Preventive Medicine 2025;37(6):577-582
With the introduction and implementation of relevant policies such as the outline of "Healthy China 2030" plan and Healthy China Action (2019-2030), the health service function of traditional Chinese medicine (TCM) has been valued and developed. TCM health literacy has become one of the important ways to promote the health of the whole population. TCM health preservation literacy refers to the ability of individuals to obtain TCM health preservation concepts, knowledge and technical services, and to make correct health care decisions to promote their own health by using knowledge and information as well as medical and technical services. According to survey data from the National Administration of Traditional Chinese Medicine, the compliance rate of TCM health literacy among Chinese citizens has increased from 8.55% in 2014 to 24.62% in 2023, and is still on the rise. However, there are differences in the level of TCM health literacy among different regions, genders, and occupational groups. This article retrieves relevant literature published on CNKI and Wanfang Data from the establishment of the databases until December 2024. The content is sorted out using thematic analysis, and a review is conducted on the concept, evaluation tools, current status, and influencing factors of TCM health literacy. This article provides a reference for enhancing public health awareness and improving the level of TCM health literacy.
