1.Evaluation the clinical efficacy of vitrectomy for diabetic retinopathy by optical coherence tomography angiography(OCTA)

Objective OCTA analysis was employed to assess the alterations in retinal microcirculation following vitrectomy in patients with diabetic retinopathy.Methods The clinical data of 60 eyes from 60 diabetic retinopathy patients who accepted vitrectomy from Mar.2022 to May.2024 in Daqing Oilfield General Hospital were analyzed prospectively.The preoperative and postoperative outcomes of best corrected visual acuity(BCVA),intraocular pres-sure(IOP),and optical coherence tomography angiography(OCTA)were compared at baseline,as well as at 1 day,1 week,1 month,and 3 months following surgery.Results There was statistically significant differences in IOP between preoperative and postoperative 1 day and 1 week(P<0.01).There were significant differences in BCVA and CMT observed prior to surgery,as well as at 1 day,1 week,1 month,and 3 months post-surgery(P<0.01).FAZ and RPC exhibited significant differences prior to surgery,as well as at 1 week,1 month,and 3 months postoperatively(P<0.01).The SVD,DVD,and RNFL exhibited significant differences prior to surgery and at 1 month and 3 months postoperatively(P<0.01).Conclusions Vitrectomy can substantially enhance the visual acuity of patients with DR,and this improvement tends to stabilize approximately one month post-surgery,potential-ly correlating with the stability of central macular thickness(CMT)observed in patients at that time.The procedure can significantly decrease intraocular pressure in patients,and FAZ along with the radial peripapillary capillaries(RPC)in the macular region exhibited earlier improvement postoperatively.

OBJECTIVE To explore the effect mechanism of Eucommia ulmoides on improving postpartum depression in rats. METHODS Pregnant rats were randomly divided into normal group, postpartum depression group, and low-dose and high-dose groups of E. ulmoides (1.34, 2.68 g/kg, calculated by crude drug), with 10 rats in each group. Except for the normal group, the rats in other groups suffered from fear stress to induce postpartum depression model during pregnancy; at the same time of modeling, the administration groups were given relevant medicine intragastrically, while the normal group and postpartum depression group were given physiological saline intragastrically for 21 days. Postpartum behaviors of rats during the experiment were assessed using the open field test, Morris water maze test and sucrose preference test. Additionally, the levels of corticosterone (CORT) in serum, corticotropin releasing factor (CRF) and urocortin (UCN) in hypothalamus, and adrenocorticotropic hormone (ACTH) in hypophysis were detected; meanwhile, the protein expressions of CRF receptor 1 (CRFR1), CRFR2, and voltage-dependent anion channel 1 (VDAC1) in hippocampal tissue were measured; the proportions of apoptotic cells and JC-1 high potential cells in hippocampal tissue were determined, and the morphology of hippocampal tissue was observed. RESULTS Compared with postpartum depression group, the high-dose group of E. ulmoides showed improvements in appetite, mental state, and hair color in rats; their body weight had increased; the scores of vertical movement, horizontal movement and self-sorting significantly increased; from the 2ed to 4th day avoidance latency significantly shortened, and the times of crossing the platform and the time of crossing the platform Δ 基金项目国家自然科学基金青年基金项目(No.82204789) significantly increased/prolonged (P<0.05); the ratio of glucose and water consumption significantly increased at 20 days of pregnancy and 30 days postpartum (P<0.05); the levels of CRF, UCN, ACTH and CORT, phagocytic rate, protein expressions of CRFR2 and VDAC1, and the proportion of apoptosis cells in hippocampal tissue were decreased significantly (P<0.05); the proportion of JC-1 high potential cells significantly increased (P<0.05), and the phenomenon of edema around neuronal cells was significantly improved. CONCLUSIONS E. ulmoides can improve postpartum depression by inhibiting excessive activation of hypothalamic-pituitary-adrenal axis, decreasing the expression of CRFR2, thereby inhibiting the expression of VDAC1, and decreasing the apoptosis of neuronal cells.

5.Combined liver-kidney transplantation for giant polycystic liver and polycystic kidney involving iliac fossa: one case report

This report described one patient of giant polycystic liver and polycystic kidney involving iliac fossa. Preoperative computed tomography (CT) revealed a large polycystic kidney occupying partially iliac fossa space. A decompression of lower pole of original kidney was planned for placing transplanted kidney. During total liver resection plus orthotopic liver transplantation, right polycystic kidney could move up on its own and iliac fossa space was released for placing transplanted kidney smoothly. Polycystic kidney shrunk markedly post-operation. It provided references for surgical planning of combined liver-kidney transplantation for this type of disease.

However, the concept of "coronary plaque burden" does not accurately reflect the complex pathological progression of coronary artery disease. In this review, various aspects of the total coronary atherosclerosis burden are present, including its mechanics, geometrical characteristics, plaque morphology, coronary artery calcium deposition, and coronary inflammation, to provide a complete view. Different tools used to evaluate the coronary atherosclerosis burden are also assessed according to the most recent studies. Compelling evidence is provided by our findings to advocate for a comprehensive use of the term "coronary atherosclerosis burden" .

BACKGROUND:
Perturbations in bone marrow mesenchymal stem cell (BMSC) differentiation play an important role in steroid-induced osteonecrosis of the femoral head (SONFH). At present, studies on SONFH concentrate upon the balance within BMSC osteogenic and adipogenic differentiation. However, BMSC apoptosis as well as proliferation are important prerequisites in their differentiation. The hedgehog (HH) signaling pathway regulates bone cell apoptosis. Baicalin (BA), a well-known compound in traditional Chinese medicine, can affect the proliferation and apoptosis of numerous cell types via HH signaling. However, the potential role and mechanisms of BA on BMSCs are unclear. Thus, we aimed to explore the role of BA in dexamethasone (Dex)-induced BMSC apoptosis in this study.
METHODS:
Primary BMSCs were treated with 10 -6 mol/L Dex alone or with 5.0 μmol/L, 10.0 μmol/L, or 50.0 μmol/L BA for 24 hours followed by co-treatment with 5.0 μmol/L, 10.0 μmol/L, or 50.0 μmol/L BA and 10 -6 mol/L Dex. Cell viability was assayed through the Cell Counting Kit-8 (CCK-8). Cell apoptosis was evaluated using Annexin V-fluorescein isothiocyanate/propidium iodide (PI) staining followed by flow cytometry. The imaging and counting, respectively, of Hochest 33342/PI-stained cells were used to assess the morphological characteristics and proportion of apoptotic cells. To quantify the apoptosis-related proteins (e.g., apoptosis regulator BAX [Bax], B-cell lymphoma 2 [Bcl-2], caspase-3, and cleaved caspase-3) and HH signaling pathway proteins, western blotting was used. A HH-signaling pathway inhibitor was used to demonstrate that BA exerts its anti-apoptotic effects via the HH signaling pathway.
RESULTS:
The results of CCK-8, Hoechst 33342/PI-staining, and flow cytometry showed that BA did not significantly promote cell proliferation (CCK-8: 0 μmol/L, 100%; 2.5 μmol/L, 98.58%; 5.0 μmol/L, 95.18%; 10.0 μmol/L, 98.11%; 50.0 μmol/L, 99.38%, F = 2.33, P > 0.05), but it did attenuate the effect of Dex on apoptosis (Hoechst 33342/PI-staining: Dex+ 50.0 μmol/L BA, 12.27% vs. Dex, 39.27%, t = 20.62; flow cytometry: Dex + 50.0 μmol/L BA, 12.68% vs. Dex, 37.43%, t = 11.56; Both P < 0.05). The results of western blotting analysis showed that BA reversed Dex-induced apoptosis by activating the HH signaling pathway, which down-regulated the expression of Bax, cleaved-caspase 3, and suppressor of fused (SUFU) while up-regulating Bcl-2, sonic hedgehog (SHH), and zinc finger protein GLI-1 (GLI-1) expression (Bax/Bcl-2: Dex+ 50.0 μmol/L BA, 1.09 vs. Dex, 2.76, t = 35.12; cleaved caspase-3/caspase-3: Dex + 50.0 μmol/L BA, 0.38 vs . Dex, 0.73, t = 10.62; SHH: Dex + 50.0 μmol/L BA, 0.50 vs . Dex, 0.12, t = 34.01; SUFU: Dex+ 50.0 μmol/L BA, 0.75 vs . Dex, 1.19, t = 10.78; GLI-1: Dex+ 50.0 μmol/L BA, 0.40 vs . Dex, 0.11, t = 30.68. All P < 0.05).
CONCLUSIONS
BA antagonizes Dex-induced apoptosis of human BMSCs by activating the HH signaling pathway. It is a potential candidate for preventing SONFH.

9.Advances in the diagnosis and treatment of acute respiratory distress syndrome induced by pancreatitis

Acute pancreatitis (AP) is one of the common diseases of the digestive system mainly characterized by acute inflammation of the pancreas and acinar cell destruction, and it can often develop into severe acute pancreatitis (SAP). Acute respiratory distress syndrome (ARDS) is one of the most common and serious complications of SAP and a common cause of death in patients with SAP. In recent years, there have gained new understanding of the pathogenesis, diagnosis, and treatment of SAP-related ARDS, however, a variety of drugs that have been shown to prevent ARDS in SAP animal models have not improved the prognosis of SAP-related ARDS patients in clinical researches. At present, the treatment of SAP-related ARDS still follows the general treatment principles of ARDS, such as treatment of primary disease, lung protective ventilation strategy, prone position ventilation, early short-term use of neuromuscular blockers and extracorporeal membrane oxygenation (ECMO), and so on. However, the influence of high intra-abdominal pressure, intestinal function failure, abdominal incision, and drainage tube on the treatment measures should also be considered for patients with SAP-related ARDS. This article reviews the recent progress in diagnosis and treatment of SAP-related ARDS.
10.The impact of joint line alteration on patients underwent primary medial pivot total knee arthroplast

Objective:To investigate the influence of joint line alteration on the mid-term clinical effects of primary medial pivot total knee arthroplasty (MP-TKA).Methods:Data of a total 189 patients (233 knees) with 30 males (30 knees) and 159 females (203 knees) with age 66.42±6.12, diagnosed as knee osteoarthritis and underwent primary MP-TKA from January to December 2015, were retrospectively analyzed. All cases were followed up for at least 4 years. The patients were divided into four groups according to the extent of alteration, namely <-4 mm, -4- mm, 0- mm and >4 mm. Range of motion (ROM), Knee Society's Knee Scoring System (KSS), the Western Ontario and McMaster University (WOMAC) osteoarthritis index score were compared preoperatively and at 4 years postoperatively.Results:The postoperative follow-up duration was 52.20± 2.94 months (48-59 months). A total of 133 knee (57%) had joint line raised and 100 cases (43%) joint line distal shifted. The overall height of joint line before surgery was 16.56±3.54 mm (7.33-25.32 mm) and 17.34±2.92 mm (7.25-25.43 mm) postoperatively. The average alteration was 0.78±3.17 mm (-6.00-9.91 mm) with statistically significant difference ( t=-2.596, P=0.010). There was no statistical difference among the four groups in terms of gender, body mass index, pre-operative tibia slope and post condylar offset. The overall preoperative ROM of the all groups was 90.45°±20.44°, which increased from baseline to 108.95°±12.55° at 4 years postoperatively ( t=-22.381, P<0.001). There was statistical difference in term of ROM among each group ( F=7.101, P<0.001). The ROM in group 0- mm and >4 mm was significantly higher than that in group <-4 mm and -4- mm ( P<0.05). The KSS function score and clinic score at 4 years postoperatively were significantly higher that those before surgery, the WOMAC score was statistically lower at 4 years postoperatively ( F=0.723, P<0.001; F=1.138, P<0.001; F=2.502, P<0.001). There was no significant difference among the four groups in terms of KSS score and WOMAC ( P>0.05). One case in >4 mm group had gasket prolapse at 6 month after surgery and 3 cases with immediate anterior knee pain occurred in <-4 mm group. Conclusion:MP-TKA can lead to significant joint line alteration. The overall ROM after surgery is positively correlated with the quantity of joint line alteration. However, there was no correlation between quantity of joint line alteration and the joint function scores. Thus, MP-TKA can achieve good mid-term clinical effects.
