4.Serological detection of anti-Mur and the distribution of the Mur antigen among voluntary blood donors

Objective: To analyze the serological characteristics of anti-Mur antibodies and investigate the distribution frequency of the Mur antigen among voluntary blood donors in Shiyan, thereby providing a basis for guiding clinical transfusion and establishing a Mur blood type database. Methods: ABO blood grouping of donors and patients was performed using an automated blood typing analyzer and the gel card method, respectively. Unexpected antibody screening and identification were performed using the saline, tube anti-human globulin, and polybrene methods. The specificity of anti-Mur antibodies was confirmed using Fisher's exact probability test. Plasma treated with 2-mercaptoethanol was used to distinguish IgM and IgG antibodies. IgM and IgG anti-Mur titers were determined by the saline tube method and the anti-human globulin tube method, respectively, at 4℃, room temperature, and 37℃. A total of 1 659 donor red blood cell samples were initially screened for the Mur antigen phenotype using three samples of human-derived anti-Mur plasma by the micro-tube method. Donors who tested positive for Mur antigen were further tested by the direct antiglobulin test (DAT); those with negative results were confirmed for Mur antigen by the gel card and polybrene methods. Results: Three blood samples were identified to contain mixed IgG and IgM anti-Mur antibodies. The titers of both IgM and IgG anti-Mur antibodies were highest at 4℃, intermediate at room temperature, and lowest at 37℃. The positive frequency of the Mur antigen among voluntary blood donors in Shiyan was 1.99% (33/1 659). Conclusion: anti-Mur antibodies were detected in both blood donors and patients in our region. The Mur antigen shows a certain distribution frequency among voluntary blood donors in Shiyan. Screening for the Mur blood type and establishing a corresponding database could enhance transfusion safety.
5.Joint effects of parent-child relationship and environmental sensitivity on adolescent mental health

Objective To examine the joint effects of parent-child relationship and environmental sensitivity(ES)on adolescent mental health from the perspective of sensitivity X family conflict interaction.Methods In 2023,we investigated 7,010 adolescents from six junior high schools in Shaanxi Province,China.Self-reported questionnaires were used to assess parent-child relationship,ES,and adolescent mental health.ES was assessed by Highly Sensitive Child Scale(HSC);Patient Health Questionnaire-9(PHQ-9),Strengths and Difficulties Questionnaire(SDQ),and Happiness index were used to measure depression,externalizing problems,and subjective well-being.Results ① Girls'ES scores were significantly higher than boys'(t=-7.504,P<0.01).② When predicting depression and externalizing problems,the main effects of parent-child relationship and ES in adolescents were significant(b=-0.263,0.237,0.332,P<0.001;b=0.201,0.220,P<0.001),and the interaction between family conflicts and ES was significant(b=-0.072,P<0.05;b=0.092,P<0.05).(3)Widaman's confirmatory and competitive approach results showed the significant interaction of sensitivity X family conflict with depression and externalizing problems,which were consistent with the differential susceptibility and diathesis-stress framework,respectively(BIC=2 170.98,2 162.6).Conclusion Adolescents with high environmental sensitivity suffer more from high family conflict and benefit more from low family conflicts.These sensitive adolescents can be described as having developmentally susceptibility rather than vulnerability.
6.Clinical Efficacy of Erdong Xiaoke Formula in Treating Type 2 Diabetes Dry Eyes with Yin Deficiency and Heat Excess Syn-drome and Its Influence on Serum IL-17 and IL-1β

OBJECTIVE To observe the clinical efficacy of Erdong Xiaoke Formula in the treatment of type 2 diabetes dry eyes with yin deficiency and heat excess syndrome and its effect on serum interleukin 17(IL-17)and interleukin 1β(IL-1β),and to ex-plore the therapeutic mechanism of Erdong Xiaoke Formula.METHODS 110 cases of type 2 diabetes patients with dry eyes of yin deficiency and heat excess syndrome from Jiangsu Province Hospital of Chinese Medicine were enrolled and randomly divided into a treatment group and a control group,55 cases each.5 cases dropped out of the treatment group and 4 cases dropped out of the control group.The control group was given a basic hypoglycemic regimen combined with topical sodium hyaluronate eye drops.The treatment group was given Erdong Xiaoke Formula in addition to the treatment in the control group.The treatment course for both groups was 4 weeks.Changes in TCM syndrome scores of the two groups of patients were observed before and after treatment,and the clinical effica-cy of TCM was evaluated.Blood glucose and pancreatic islet function-related indicators[fasting blood glucose(FBG),fasting insulin(FINS),fasting C-peptide(FCP),postprandial 2 h blood glucose(PBG),insulin secretion function index(HOMA-β),insulin re-sistance index(HOMA)-IR)],ocular surface indicators[tear break up time(BUT),corneal sodium fluorescein staining(FL),Schirmer Ⅰ test(SⅠT)],and inflammation-related indicators(IL-17,IL-1β)were detected.The occurrence of adverse reactions in the two groups of patients was observed during the treatment period.RESULTS After treatment,the total scores of TCM syn-dromes in both groups were significantly reduced(P<0.01),the treatment group was better than the control group(P<0.01),and the total effective rate of TCM syndromes in the treatment group was higher than that of the control group(P<0.01);SⅠT and BUT of pa-tients in both groups increased significantly(P<0.01),and the treatment group was better than the control group(P<0.05,P<0.01);the FL of patients in both groups significantly reduced(P<0.01),and there was no significant difference between the groups;the ser-um IL-17 and IL-1β of the patients in the treatment group decreased significantly(P<0.01),which was significantly better than that of the control group(P<0.05).There were no significant changes in blood glucose and pancreatic islet function-related indicators in the two groups before and after treatment(P>0.05).During the treatment,no obvious adverse reactions were observed in the two groups.CONCLUSION Erdong Xiaoke Formula can improve SⅠT,BUT,FL and eye symptoms in patients with diabetic dry eye,effectively treat diabetic dry eye,and reduce ocular surface inflammation.Its mechanism may be related to reducing serum inflammato-ry factors IL-17 and IL-1β.
7.Observational Study on the Impact of Comorbidities and Frailty on Anticoagulation Therapy Compliance in Elderly Patients With Atrial Fibrillation

Objectives:This study was designed to observe the impact of frailty status and multiple comorbidities on anticoagulation therapy adherence in elderly patients(≥75 years old)with atrial fibrillation. Methods:This retrospective study enrolled 528 patients(≥75 years old)with atrial fibrillation who were admitted to the geriatric department of Peking University Third Hospital from October 2016 to October 2019.A total of 443(83.9%)elderly patients receiving anticoagulation therapy were included,with an average age of(84.72±5.39)years.Thromboembolic and bleeding risk were evaluated by CHA2DS2-VASc and HAS-BLED,ATRIA bleeding score,while the frailty index(FI)and the age-adjusted Charlson comorbidity index(ACCI)were evaluated.Those with FI≥3 and FI between 1~2 were categorized into the frailty group and pre-frailty group.Patients with ACCI≥5 were defined as elderly multiple comorbidity group(high comorbidity burden).Patients who received anticoagulation therapy after discharge were followed up for one year in the outpatient clinic to observe their medication adherence and to find the related reasons. Results:Thromboembolic risk and bleeding scoring were performed in 443 elderly patients,with CHA2DS2-VASc and HAS-BLED scores of 5.61±1.57 and 3.08±0.89,respectively,and ATRIA score was 4.13±1.90.The FI assessment showed that all enrolled patients were in a state of frailty or pre-frailty.The average ACCI was 5.73±1.51.At one year of follow-up,96(21.67%)patients discontinued anticoagulation therapy.Frailty,high comorbidity burden,and high bleeding risk according to ATRIA(ATRIA≥5)were factors affecting medication adherence,with OR values of 1.114(95%CI:1.041-1.205,P=0.003),3.201(95%CI:2.126-4.509,P=0.001),and 1.457(95%CI:1.212-2.075,P=0.024),respectively.Considering non-death endpoint,both the HAS-BLED score(HAS-BLED≥3 分,OR=1.098,95%CI:1.035-1.116,P=0.014)and warfarin(OR=1.015,95%CI:1.001-1.030,P=0.040)were influential factors of medication compliance. Conclusions:The adherence to anticoagulation therapy post-discharge in elderly patients with atrial fibrillation is significantly influenced by frailty status and high comorbidity burden.If terms of non-death endpoint,high bleeding score and use of warfarin are influential factors of medication compliance.
8.Evidence summary for prevention and management of extravasation in peripheral intravenous infusion in NICU neonates

Objective To systematically search,evaluate and summarize the best evidence for prevention and management of extravasation in peripheral intravenous infusion in NICU neonates,and provide a reference for clinical practice and standard formulation.Methods A comprehensive systematic search of websites and databases was conducted to explore literature on prevention and management of extravasation in NICU neonates,including clinical decisions,guidelines,expert consensuses,evidence summaries and systematic reviews.The search encompassed the entire period from database inception to July 2023.2 researchers independently evaluated the quality of the literature,extracted and integrated the evidence.Results The study included a total of 9 articles,comprising 1 clinical decision,3 guidelines,3 expert consensus documents,and 2 evidence summaries.Ultimately,25 pieces of evidence were synthesized,covering risk factors,catheter indwelling and maintenance,extravasation assessment and treatment,and team building,education and training.Conclusion The evidence provided practical and specific recommendations that can guide healthcare institutions in formulating strategies to prevent and treat extravasation during peripheral intravenous infusion in NICU neonates,while also offering evidence-based guidance for applying the evidence in clinical practice.
9.Design and thinking of tumor comprehensive experimental course based on molecular biology and immunology technology

In order to meet the needs of the comprehensive development of life sciences and medicine,and fulfill the coher-ence and intersectionality of undergraduate experimental courses,it is necessary to break the teaching contents of conventional single experiment courses,and carry out the teaching reform of tumor comprehensive experimental course.Molecular biology and immuno-logy are two basic subjects which are closely related to the scientific research of life science,basic medicine and clinical medicine.Meantime,they are important sources for students to acquire skill training and scientific thinking as well.In this study,we try to inte-grate the experimental courses about the analysis and intervention of tumor-related genes based on the molecular biology and immunology technologies,facilitating to establish a new tumor comprehensive experimental course system.The course focuses on the development of students'logical thinking,and simulates the scientific research process through database screening,experimental operation,experi-mental report and paper writing,so as to significantly improve undergraduates'interest in scientific research and their ability to com-bine theory with practice,and cultivate students'rigorous scientific and innovative thinking ability as well as their ability to compre-hensively analyze and solve problems.It lays a solid foundation for future postgraduate related scientific research.At the same time,it is helpful to train teachers with multi-disciplinary knowledge reserves,and promote the coordinated development of scientific research through teaching.
10.Analysis of positive umbilical cord blood culture results in 111 neonates with early onset sepsis

Objective:To investigate and evaluate the pathogenic bacterial composition,drug susceptibility of pathogens,and the time to positive in umbilical cord blood culture(UCBC)of newborns with early onset sepsis(EOS).Methods:The clinical data of 111 newborns with EOS with positive UCBC admitted to the neonatology department in Women's Hospital School Medicine of Zhejiang University from 1st January 2021 to 31st December 2022 were retrospectively analyzed.The patients were divided into Gram-positive bacteria group,Gram-negative bacteria group and fungal group.The pathogenic bacteria composition,drug susceptibility of pathogens and time to positive in UCBC of the three groups were compared.Results:A total of 114 strains of bacteria were isolated from 111 neonates and 2 types of pathogenic bacteria were cultured from 3 newborns of UCBC.Among the 114 pathogenic bacteria,69 strains were Gram-positive,accounting for 60.5%,and the Gram-positive bacteria were mainly Enterococcus faecalis and Group B streptococcus(GBS).There were also 43 Gram-negative strains accounting for 37.7%,primarily Escherichia coli.Two strains of fungi,both of which were Candida albicans,accounted for 1.8%.Additionally,the time to positive in UCBC varied among different pathogens.Gram-positive bacteria had a significantly longer detection time than Gram-negative bacteria[(36.3(21.6,48.2)hours vs.20.4(16.5,33.8)hours), Z=3.539, P<0.01].The rate of drug resistance was found to be high in Enterococcus faecalis against tetracycline(52.9%)and erythromycin(47.1%).GBS showed resistance against clindamycin(87.5%)and erythromycin(75.0%).Both Enterococcus faecalis and GBS exhibited sensitivity towards penicillin,linezolid,and vancomycin.Among Gram-negative bacteria,Escherichia coli was sensitive to imipenem,ertapenem,cefoxitin,cefoperazone-sulbactam,amikacin and tigecycline,and had a high resistance rate to cotrimoxazole(61.3%). Conclusion:Enterococcus faecalis,GBS,and Escherichia coli were predominant microorganisms identified in UCBC of newborns with EOS.The positive time for Gram-negative bacteria in UCBC was significantly shorter compared to Gram-positive bacteria,while the bacteria isolates from UCBC exhibited a low resistance rate to commonly used antibiotics.
