Objective:To establish a general method for the simultaneous determination of hydroxyphenyl esters and quaternary ammonium bacteriostatic agents in eye drops.Methods:The chromatographic analysis was per-formed on an Agilent C18 column(4.6 mm ×250 mm,5 μm)with 1%triethylamine solution(pH adjusted to 5.0 with phosphoric acid)as mobile phase A and methanol as mobile phase B.Gradient elution was performed at col-umn temperature of 40 ℃.The detection wavelength was 214 nm,the flow rate was 1 mL·min-1,and the injec-tion volume was 20 μL.Results:Methylparaben,ethylparaben,propylparaben,butylparaben,benzalkonium chlo-ride and benzalkonium bromide were 0.11-559.0,0.10-513.0,0.10-258.8,0.11-270.5,1.07-537.0 and 1.03-512.8 μg·mL-1,respectively.The linear range was good(r>0.999).The average recoveries of meth-ylparaben,benzalkonium bromide and benzalkonium chloride were 104.7%(RSD=1.3%),102.6%(RSD=1.1%)and 100.9%(RSD=1.1%),respectively.The contents of bacteriostatic agent in 100 batches of eye drops from 36 varieties of 12 enterprises were determined,and the accurate results were obtained.Conclusion:This meth-od provides a reference for the content quality control and safety evaluation of bacteriostatic agents in eye drops.
2.Precise assessment and analysis of the effects of the modified Wuqinxi on balance function in stroke pa-tients

Objective:To observe the effects of the modified Wuqinxi on balance function in stroke patients,and to ex-plore the correlations among three balance assessment methods which are the sacral marker method,the Pro-Kin system,and the Berg balance scale(BBS). Method:Fifty-six patients with stroke were randomly divided into a control group and an experimental group,with 28 patients in each.The control group received conventional physiotherapy,while the experimental group received both conventional physiotherapy and the modified Wuqinxi for four weeks.Balance was assessed us-ing the BBS,three-dimensional motion analysis system via sacral marker method,and the Pro-Kin balance de-vice before and after treatment.Trunk control was evaluated using the Trunk Impairment Scale(TIS). Result:①Pre-and post-intervention comparisons showed significant improvements in BBS scores,the sway path of the sacral markers,which is the center of gravity during standing on two feet and one foot,the move-ment track length of the pressure center,and the area of motion ellipse in the seated position,the stability lim-it,and the TIS scores were significantly improved in both groups(P<0.05).②Inter-group comparisons post-treatment revealed that the experimental group showed significant improvements in BBS scores,the sway path of the center of gravity standing on two feet and one foot,the movement track length of pressure center,and the area of motion ellipse in the seated position,and the TIS scores compared to the control group(P<0.05).Compared with the control group,the difference of BBS,the difference of the sway path of the center of gravity standing on two feet and one foot,the difference of the area of motion ellipse in the seated posi-tion,and the difference of the stability limit were significantly improved in the two groups at post-intervention(P<0.05).③Before the intervention,the sway path of the center of gravity,the movement track length of the pressure center,and the area of motion ellipse were negatively correlated with the BBS scores(r=-0.324--0.562,P<0.05).The sway path of the center of gravity was positively correlated with the movement track length of pressure center and the area of motion ellipse(r=0.547-0.809,P<0.01). Conclusion:Trunk control,postural stability,and balance function can be improved effectively by the modi-fied Wuqinxi in stroke patients.There are certain correlations among the sacral marker method,the Pro-Kin system,and the BBS in terms of balance function assessment,all of which can objectively reflect the balance function of stroke patients.The sacral marker method based on three-dimensional motion analysis can detect the balance of stroke patients in different states more objectively therefore is worthy to be recommended and applicated clinically.
3.Systematic review of risk prediction models for ventilator-associated pneumonia in mechanically ventilated patients in Intensive Care Unit

Objective:To systematically search and evaluate risk prediction models for ventilator-associated pneumonia (VAP) of ICU in order to provide references for developing higher-quality VAP risk prediction models.Methods:Relevant literature was retrieved from databases including China Biology Medicine disc, WanFang data, China National Knowledge Infrastructure, Embase, PubMed, CINAHL, Web of Science, and Cochrane Library. The search timeframe was from the establishment of the databases to September 30, 2023, limited to English and Chinese languages. Two researchers independently screened the literature and extracted data, and the PROBAST tool was used to evaluate the risk of bias and applicability of the included studies.Results:A total of 15 studies on VAP risk prediction models were included. The area under the receiver operating characteristic curve for the 15 models ranged from 0.722 to 0.982. The most frequently involved predictors were age, duration of mechanical ventilation, ICU length of stay, and comorbid chronic obstructive pulmonary disease. The overall adaptability was good, but the risk of bias was high. The main sources of bias included insufficient sample size, inappropriate data sources, lack of model performance evaluation, and inadequate attention to missing data.Conclusions:The risk of bias in studies on VAP risk prediction models is high, indicating that the field is still developing. Future research should focus on the effectiveness of different risk assessment methods to construct models with low bias, excellent predictive performance, and suitability for clinical practice in China.
4.The application of cardiac magnetic resonance imaging in immune checkpoint inhibitor associated myocarditis

Immunocheckpoint inhibitors have been widely used in the treatment of various tumors. Although the incidence rate of immunocheckpoint inhibitor related myocarditis caused by them is low, the mortality is high. Cardiac magnetic resonance imaging is the gold standard for evaluating cardiac morphology and function, and it has important clinical application value in the grading diagnosis, risk stratification, adverse prognosis prediction, and treatment monitoring of immune checkpoint inhibitor associated myocarditis.

6.Analysis of positive umbilical cord blood culture results in 111 neonates with early onset sepsis

Objective:To investigate and evaluate the pathogenic bacterial composition,drug susceptibility of pathogens,and the time to positive in umbilical cord blood culture(UCBC)of newborns with early onset sepsis(EOS).Methods:The clinical data of 111 newborns with EOS with positive UCBC admitted to the neonatology department in Women's Hospital School Medicine of Zhejiang University from 1st January 2021 to 31st December 2022 were retrospectively analyzed.The patients were divided into Gram-positive bacteria group,Gram-negative bacteria group and fungal group.The pathogenic bacteria composition,drug susceptibility of pathogens and time to positive in UCBC of the three groups were compared.Results:A total of 114 strains of bacteria were isolated from 111 neonates and 2 types of pathogenic bacteria were cultured from 3 newborns of UCBC.Among the 114 pathogenic bacteria,69 strains were Gram-positive,accounting for 60.5%,and the Gram-positive bacteria were mainly Enterococcus faecalis and Group B streptococcus(GBS).There were also 43 Gram-negative strains accounting for 37.7%,primarily Escherichia coli.Two strains of fungi,both of which were Candida albicans,accounted for 1.8%.Additionally,the time to positive in UCBC varied among different pathogens.Gram-positive bacteria had a significantly longer detection time than Gram-negative bacteria[(36.3(21.6,48.2)hours vs.20.4(16.5,33.8)hours), Z=3.539, P<0.01].The rate of drug resistance was found to be high in Enterococcus faecalis against tetracycline(52.9%)and erythromycin(47.1%).GBS showed resistance against clindamycin(87.5%)and erythromycin(75.0%).Both Enterococcus faecalis and GBS exhibited sensitivity towards penicillin,linezolid,and vancomycin.Among Gram-negative bacteria,Escherichia coli was sensitive to imipenem,ertapenem,cefoxitin,cefoperazone-sulbactam,amikacin and tigecycline,and had a high resistance rate to cotrimoxazole(61.3%). Conclusion:Enterococcus faecalis,GBS,and Escherichia coli were predominant microorganisms identified in UCBC of newborns with EOS.The positive time for Gram-negative bacteria in UCBC was significantly shorter compared to Gram-positive bacteria,while the bacteria isolates from UCBC exhibited a low resistance rate to commonly used antibiotics.
7.Prevention and treatment of chronic graft-versus-host disease by mesenchymal stem cells: advances and challenges

Chronic graft-versus-host disease (cGVHD) is the main complication after allogeneic hematopoietic stem cell transplantation, which is also the major cause of non-relapse -related death. Due to its complex pathophysiological process, the response rate of conventional glucocorticoids combined with immunosuppressants is less than 50%. Second-line therapy should be given for patients with glucocorticoid-resistant cGVHD. Nevertheless, no consensus has been reached on current second-line therapy and the therapeutic effect is relatively poor. Mesenchymal stem cell (MSC) is one of the most common adult stem cells. Due to multi-dimensional and multi-target immune regulating function, MSC has been widely applied in the prevention and treatment of cGVHD. In addition, accumulated studies have confirmed the safety and efficacy of MSC in the treatment of cGVHD, which is expected to become a novel strategy for the prevention and management of cGVHD. In this article, research progress, mechanism and existing problems of prevention and treatment of cGVHD by MSC were reviewed, aiming to provide novel ideas for optimizing therapeutic regimens of MSC and enhancing the prevention and treatment effect of cGVHD in subsequent research.

“Cold-dampness entering ying (营)” is the key to the worsening of cold-dampness epidemic, and is more common in the elderly or critically ill cases of cold-dampness epidemic with pathogen exuberance and healthy qi deficiency. This paper reported a case of critically ill COVID-19 combined with multiple organ dysfunction treated by integrative traditional Chinese and western medicine based on “cold-dampness entering ying” theory. The patient did not have high fever after being infected with SARS-Cov-2, but D-dimer continued to increase, and she developed multiple thrombosis throughout the body and multiple organ dysfunctions such as pulmonary embolism, edema, oliguria, and shock. The patient were with enlarged and dusky tongue, with yellow, thick and greasy coating, and sublingual blood stasis, and thready, rapid and rough pulse. All these were characteristic manifestations of “cold-dampness entering ying”, and was differentiated as cold-dampness stasis. For the treatment, symptomatic and supportive western medicine of improving heart function, anti-infection, relieving asthma, stopping cough and reducing phlegm was given as the basic therapy, and additionally, traditional Chinese medicine to open the constraint and the blocked, save from collapse and restore yang, boost qi and relieve collapse, invigorate blood and drain water was used, usually with Modified Poge Zilong Xuanbai Chengqi Decoction (破格子龙宣白承气汤加减), which was in accordance with the pathogenesis and thus achieving good effect.
9.Clinical Application Advances of Antipsychotic Drugs in Female Schizophrenic Patients

Female patients with schizophrenia exhibit a slower rate of absorption and excretion,leading to higher blood levels of antipsychotic drugs in their bodies and consequently causing more drug side effects.Women also have different levels of estrogen at different stages of life,and estrogen improves the efficacy of antipsychotic drugs by increasing dopamine D2receptor sensitivity.It is necessary to adjust the dosage of antipsychotic medication based on the level of estrogen,while also monitoring indicators such as blood glucose,triglycerides,cholesterol,and prolactin to minimize medication side effects.Nevertheless,the current literatures do not provide specific treatment plans for schizophrenia tailored to different genders,which is not the optimal treatment strategy for female patients.This review aims to summarize and categorize the differences in prescription,pharmacokinetics,pharmacodynamics,efficacy,and side effects in female patients with schizophrenia,and to provide scientific recommendations for drug treatment strategies in different stages of a woman's life(premenopausal,pregnancy,lactation,postmenopausal).
10.Quality of life and associated factors among HIV-infected individuals in Jing’an District, Shanghai

ObjectiveTo determine the quality of life among HIV-infected individuals and risk factors in Jing’an District of Shanghai. MethodsThis is a cross-sectional study. Cluster sampling strategy was used to select HIV-infected individuals who were registered with the national HIV/AIDS case reporting system in eight community health service centers in Jing’an District of Shanghai. The short form 36 (SF-36) scale, self-rating anxiety scale (SAS), social support scale (SSS) and specifically designed questionnaires were used to collect information. Logistic regression was used to determine the factors associated with quality of life. ResultsIn total, 365 HIV-infected individuals were included in the study. The total average score of quality of life was 79.5±10.4. The scores in general health, vitality and emotional well-being were lower than the norm, whereas the scores in other dimensions were higher than the norm. Multivariable logistic regression indicated that age of 45‒ years (vs age of 18‒ years: OR=0.23, 95%CI: 0.08‒0.68) was significantly associated with the quality of life. Furthermore, smoking (OR=2.18, 95%CI: 1.22‒3.90), lower CD4+T cell counts (<350 cell·μL-1, OR=2.89, 95%CI: 1.54‒5.40), comorbidity (≥2 kinds, OR=3.40, 95%CI: 1.15‒10.04) and anxiety (OR=1.09, 95%CI: 1.05‒1.14) were risk factors associated with declining quality of life. ConclusionIt warrants more attention to those HIV-infected individuals with poor psychological status, smoking, low CD4+T cell counts and comorbidities. Targeted health care and social support could effectively improve the quality of life among HIV-infected individuals.
