1.Epidemiological characteristics and influencing factors of severe fever with thrombocytopenia syndrome in Zhejiang Province

Objective:
To analyze the epidemiological characteristics and influencing factors of severe fever with thrombocytopenia syndrome (SFTS) in Zhejiang Province from 2019 to 2023, so as to provide the reference for strengthening SFTS prevention and control.
Methods:
Data on laboratory-confirmed SFTS cases in Zhejiang Province from 2019 to 2023 were collected through the Infectious Disease Reporting Information System of Chinese Disease Prevention and Control Information System. Meteorological data, geographic environment and socioeconomic factors during the same period were collected from the fifth-generation European Centre for Medium-Range Weather Forecasts, Geospatial Data Cloud, and Zhejiang Statistical Yearbook, respectively. Descriptive epidemiological methods were used to analyze the epidemiological characteristics of SFTS from 2019 to 2023, and a Bayesian spatio-temporal model was constructed to analyze the influencing factors of SFTS incidence.
Results:
A total of 578 SFTS cases were reported in Zhejiang Province from 2019 to 2023, with an annual average incidence of 0.23/105. The peak period was from May to July, accounting for 52.60%. There were 309 males and 269 females, with a male-to-female ratio of 1.15∶1. The cases were mainly aged 50-<80 years, farmers, and in rural areas, accounting for 82.53%, 77.34%, and 75.43%, respectively. Taizhou City and Shaoxing City reported more SFTS cases, while Shaoxing City and Zhoushan City had higher annual average incidences of SFTS. The Bayesian spatio-temporal interaction model showed good goodness of fit. The results showed that mean temperature (RR=1.626, 95%CI: 1.111-2.378) and mean wind speed (RR=1.814, 95%CI: 1.321-2.492) were positively correlated with SFTS risk, while altitude (RR=0.432, 95%CI: 0.230-0.829) and population density (RR=0.443, 95%CI: 0.207-0.964) were negatively correlated with SFTS risk.
Conclusions
SFTS in Zhejiang Province peaks from May to July. Middle-aged and elderly people and farmers are high-risk populations. Taizhou City, Shaoxing City, and Zhoushan City are high-incidence areas. Mean temperature, mean wind speed, altitude, and population density can all affect the risk of SFTS incidence.
2.Analysis of the management effect of community pharmacy outpatient service on patients with type 2 diabetes mellitus

Objective To explore the effect of community pharmacy outpatient service on patients with type 2 diabetes mellitus. Methods A non-randomized controlled study was conducted, and type 2 diabetes patients managed in the community were divided into an intervention group of 112 cases and a control group of 110 cases. The control group received routine medication guidance during general practice outpatient visits, while the intervention group received comprehensive pharmacy outpatient service intervention based on routine medication guidance in general practice. Follow-up visits were conducted every 3 months. Repeated measurement analysis of variance and multivariate linear regression analysis were used to evaluate the intervention effect of the pharmacy outpatient service. Results Fasting blood glucose and glycosylated hemoglobin levels in the intervention group showed a decreasing trend with the increase of intervention time compared to pre-intervention time (P<0.01), with increased duration of weekly exercise, decreased staple food intake, increased vegetable intake, and increased medication adherence score (P<0.01). After adjusting for confounding factors through multivariate linear regression model, pharmacy outpatient intervention was found to be an independent protective factor for fasting blood glucose level (β=−0.891, P<0.01) and glycosylated hemoglobin level (β=−0.760, P<0.01) in the study subjects. Conclusion The community pharmacy outpatient service could enhance the self-management ability of patients with type 2 diabetes mellitus, and effectively improve patients’ fasting blood glucose and glycosylated hemoglobin.
3.Relationship between school bullying and non-suicidal self-injury behaviors in adolescents with depressive disorders: the pathways of self-esteem and alexithymia

BackgroundNon-suicidal self-injury (NSSI) behaviors are common among adolescents with depressive disorders, and school bullying is recognized as a major risk factor. Previous research has shown that self-esteem and alexithymia are closely associated with both school bullying and NSSI. However, the mediating roles of self-esteem and alexithymia in the link between school bullying and NSSI are unclear. ObjectiveTo explore the mediating roles of alexithymia and self-esteem in the relationship between school bullying and NSSI behaviors in adolescents with depressive disorders, in order to inform intervention strategies targeting NSSI in this population. MethodsA total of 335 adolescents diagnosed with depressive disorders and treated at the First Psychiatric Hospital of Harbin from July 2023 to October 2024 were enrolled. Assessments included a self-developed demographic questionnaire, Adolescent Non-suicidal Self-injury Assessment Questionnaire-Behavior (ANSAQ-B), Delaware Bullying Victimization Scale-Student (DBVS-S), Rosenberg Self-Esteem Scale (RSES), and 26-item Toronto Alexithymia Scale (TAS-26). Pearson correlation analysis was used to examine the relationship among variables. Controlling for gender and age at onset of depressive symptoms, mediation analysis was performed using the “mediation” package in R 4.4.2. ResultsScores on DBVS-S and TAS-26 were positively correlated with ANSAQ-B score (r=0.408, 0.417, P<0.01), while RSES scores were negatively correlated(r=-0.300, P<0.01). Regression analysis showed that school bullying and alexithymia significantly positively predicted NSSI behaviors (B=0.212, 0.333, P<0.01), while self-esteem negatively predicted NSSI behaviors (B=-0.368, P<0.01). Alexithymia was found to mediate the relationship between school bullying and NSSI behaviors, with an indirect effect of 0.040 (95% CI: 0.018~0.069) ,account for 17.17% of the total effect. The indirect effect through self-esteem was not statistically significant (95% CI: -0.004~0.069). ConclusionExposure to school bullying and high levels of alexithymia are important predictors of NSSI behavior in adolescents with depressive disorders, and school bullying may indirectly influence NSSI behavior through alexithymia. 4.Clinical Observation of Pirfenidone in Prevention and Treatment of Radiation-Induced Lung Injury in Esophageal Cancer

Objective To observe the effectiveness and safety of pirfenidone in the prevention and treatment of radiation-induced lung injury in esophageal cancer.Methods We retrospectively analyzed the data of 103 patients with esophageal cancer,of whom 53 in the combined group were treated with simultaneous chemoradiotherapy combined with pirfenidone and 50 in the control group were treated with simultaneous chemoradiotherapy only.The patients were followed up for three years to observe the treatment effects,adverse effects,and survival,as well as the incidence of radiation-induced lung injury,lung function,and changes in lung injury cytokine levels within one year after radiotherapy.Results Treatment efficiency in the combined group was higher than that in the control group(86.8%vs.70.0%,P<0.05).The two-and three-year survival rates in the combined group were 84.9%and 71.7%,respectively,which were higher than those(68.0%and 52.0%)in the control group(P<0.05).The one-,two-,and three-year disease-free survival rates in the combined group were 86.8%,67.9%,and 47.2%,respectively,which were higher than those(62.0%,46.0%,and 28.0%)in the control group(P<0.05).The incidence rates of radiation pneumonitis at three months,pulmonary fibrosis at six months,and one year after radiotherapy in the combined group were 22.6%,13.2%,and 14.0%,respectively,which were lower than those(42.0%,30.0%,and 31.8%)in the control group at the same time(P<0.05).At the end of radiotherapy and at three months,six months and one year after radiotherapy,the combined group showed higher levels of lung function indicators but lower levels of lung injury-related cytokines than the control group(P<0.05).The incidence of rash in the combined group was 18.9%,which was higher than that(2.0%)in the control group(P<0.05).However,no statistically significant difference in the incidence and severity of other adverse reactions was found between the two groups(P>0.05).Conclusion Pirfenidone not only effectively reduces radiation-induced lung injury and improves lung function in esophageal cancer patients undergoing simultaneous chemoradiotherapy,but also helps improve tumor control rates and patient survival with a good safety profile.

6.Comparison of quality of sleep and rehabilitation after routine surgery versus day surgery in pediatric patients with snoring

Objective:To compare the quality of sleep and rehabilitation after routine surgery versus day surgery in pediatric patients with snoring.Methods:This was a prospective study.Seventy pediatric patients with snoring, aged 4-6 yr, of American Society of Anesthesiologists Physical Status classification Ⅰ or Ⅱ, undergoing adenoidectomy and/or tonsillectomy in the Guangzhou Women and Children′s Medical Center from March to June 2023, were divided into routine surgery group (group R) and day surgery group (group D), with 35 cases in each group. A sleep-detecting bracelet was used to monitor the sleep status. The proportion of deep sleep, light sleep and rapid eye movement, sleep continuity score and the maximum and minimum heart rate were recorded on the night before surgery, the first and second night after surgery. Rehabilitation was assessed using Quality of Recovery-15 at 1 day before surgery and 24 and 48 h and 28 days after surgery.Results:Compared with group R, the maximum heart rate at the night before surgery was significantly decreased, the proportion of light sleep on the second night after surgery was increased, the proportion of rapid eye movement sleep on the second night after surgery was decreased, the scores for items on " getting support from hospital doctors and nurses", " feeling comfortable and in control", and " having a feeling of general well-being" and the total score of Quality of Recovery-15 were significantly decreased at 24 and 48 h after operation, and the score for items on " feeling rested" and " have had a good sleep" at 48 h after surgery was decreased in group D ( P<0.05). Conclusions:Pediatric patients with snoring have poor quality of early sleep and rehabilitation after day surgery compared with routine surgery.
7.Effect of buccal acupuncture on analgesia after tonsilloadenoidectomy in pediatric patients

Objective:To evaluate the effect of buccal acupuncture on analgesia after tonsilloadenoidectomy in pediatric patients.Methods:This was a randomized controlled study. One hundred and twenty-six American Society of Anesthesiologists Physical Status classification Ⅰ or Ⅱ pediatric patients, aged 3-12 yr, weighing 12-34 kg, with body mass index <30 kg/m 2, undergoing elective tonsilloadenoidectomy with general anesthesia, were divided into 2 groups ( n=63 each) by the random number table method: buccal acupuncture group (group B) and control group (group C). All pediatric patients received the same anesthesia induction and intraoperative anesthesia maintenance. The concentration of sevoflurane was adjusted to keep the fluctuation amplitude of vital sign parameters within 20% of the baseline value. After surgery, the drug was immediately stopped and the children were transferred to the postanesthesia care unit for resuscitation under general anesthesia. In group B, the bilateral neck points, upper neck points, hologram points on the head and Zhongjiao points were selected before removal of the tracheal catheter, and disposable acupuncture needles were inserted directly into the acupoints and remained for 20-30 min. Group C received no buccal acupuncture. The pain Assessment Scale (FLACC) was used to assess the severity of postoperative pain. The postoperative agitation score was evaluated by Aono four-point rating method to evaluate the occurrence of agitation. The effective pressing times of patient-controlled analgesia, rescue analgesia and occurrence of nausea and vomiting within 48 h after operation were recorded. The occurrence of bleeding, infection and broken needle at acupuncture sites was recorded. Results:Compared with group C, the effective pressing times of patient-controlled analgesia and incidence of nausea and vomiting were significantly decreased in group B ( P<0.05). There was no significant difference in the rate of rescue analgesia and incidence of postoperative agitation between the two groups ( P>0.05). No infection or broken needle was found at acupuncture sites after buccal acupuncture, only 2 cases had slight bleeding at the puncture site, and there was no abnormality after pressing in group B. Conclusions:Buccal acupuncture can enhance the analgesic effect after tonsilloadenoidectomy in pediatric patients.

Objective To understand the changing distribution and antimicrobial resistance profiles of Klebsiella strains in 52 hospitals across China in the CHINET Antimicrobial Resistance Surveillance Program from 2015 to 2021.Methods Antimicrobial susceptibility testing was carried out according to the unified CHINET protocol.The susceptibility results were interpreted according to the breakpoints in the Clinical & Laboratory Standards Institute(CLSI)M100 document.Results A total of 241,549 nonduplicate Klebsiella strains were isolated from 2015 to 2021,including Klebsiella pneumoniae(88.0%),Klebsiella aerogenes(5.8%),Klebsiella oxytoca(5.7%),and other Klebsiella species(0.6%).Klebsiella strains were mainly isolated from respiratory tract(48.49±5.32)%.Internal medicine(22.79±3.28)%,surgery(17.98±3.10)%,and ICU(14.03±1.39)%were the top 3 departments where Klebsiella strains were most frequently isolated.K.pneumoniae isolates showed higher resistance rate to most antimicrobial agents compared to other Klebsiella species.Klebsiella isolates maintained low resistance rates to tigecycline and polymyxin B.ESBLs-producing K.pneumoniae and K.oxytoca strains showed higher resistance rates to all the antimicrobial agents tested compared to the corresponding ESBLs-nonproducing strains.The K.pneumoniae and carbapenem-resistant K.pneumoniae(CRKP)strains isolated from ICU patients demonstrated higher resistance rates to majority of the antimicrobial agents tested than the strains isolated from non-ICU patients.The CRKP strains isolated from adult patients had higher resistance rates to most of the antimicrobial agents tested than the corresponding CRKP strains isolated from paediatric patients.Conclusions The prevalence of carbapenem-resistant strains in Klebsiella isolates increased greatly from 2015 to 2021.However,the Klebsiella isolates remained highly susceptible to tigecycline and polymyxin B.Antimicrobial resistance surveillance should still be strengthened for Klebsiella strains.
9.Research of Generalized Boosting Model Combined with Propensity Score Overlap Weighting

Objective This study constructed the generalized boosting model combined with propensity score overlap weighting(GBM-OW).Methods Within the situations that there are complex relationships between confounders and treatment factors,and different sample size and different propensity score overlap,we explored the performance of GBM-OW model in balance confounders and estimate effect.And compared with multivariate adjusted model and other three propensity score weighting models.Results and Conclusion From the simulation results we concluded that when the relationship between variables is complex,the sample size is large,and the propensity score value overlap is small,the GBM-OW model has a good performance in all aspects and can be used in observational studies.
10.Research progress on dedifferentiated fat cells and their application in oral and maxillofacial bone tissue engineering

The identification of suitable seed cells represents a critical scientific problem to be solved in the field of oral and maxillofacial bone tissue regeneration. The application of adipose-derived stem cells (ASCs) in tissue and organ repair and regeneration has been studied extensively. In recent years, dedifferentiated fat (DFAT) cells have also shown broad application prospects in the field of bone tissue engineering. DFAT cells express stem cell-related markers and have the potential to differentiate into adipocytes, osteoblasts, chondrocytes, nerve cells, cardiomyocytes and endothelial cells. In addition, DFAT cells also have the advantages of minimally invasive acquisition, strong proliferation and high homogeneity. Currently, all studies involving the application of DFAT cells in scaffold-based and scaffold-free bone tissue engineering can confirm their effectiveness in promoting bone regeneration. However, cytological research still faces some challenges, including relatively low cell culture purity, unclear phenotypic characteristics and undefined dedifferentiation mechanisms. It is believed that with the continuous development and improvement of isolation, culture, identification and directional induction of osteogenic differentiation methods, DFAT cells are expected to become excellent seed cells in the field of oral and maxillofacial bone tissue engineering in the future.
