Neuropathic pain (NP) is a chronic pain syndrome resulting from injury, inflammation, or dysfunction of the somatosensory nervous system. Its pathogenesis is complex and intimately associated with peripheral sensitization and maladaptive neuroplasticity within the central nervous system. The core clinical manifestations, including spontaneous pain, allodynia, and paresthesia, severely compromise patients’ quality of life and pose substantial challenges to clinical management. Endoplasmic reticulum stress (ERS), a critical cellular defense mechanism against disrupted intracellular homeostasis, is primarily initiated and modulated through three canonical branches of the unfolded protein response (UPR): the protein kinase RNA-like endoplasmic reticulum kinase (PERK) pathway, the inositol-requiring transmembrane kinase/endonuclease 1α (IRE1α) pathway, and the activating transcription factor 6 (ATF6) pathway. These signaling cascades play a pivotal role in the pathological progression of NP. Traditional Chinese medicine (TCM) exhibits unique advantages in the treatment of NP. Leveraging its hallmark characteristics of multi-target, multi-pathway, and holistic regulation, TCM can systematically modulate the ERS signaling network to inhibit neuronal apoptosis, attenuate neuroinflammatory infiltration, and rectify autophagic dysfunction, ultimately promoting the repair and regeneration of damaged neural tissue. This review systematically summarizes the core role of ERS in the occurrence and development of NP. It focuses on the latest research progress regarding ERS pathway modulation in NP amelioration by representative TCM interventions, including potential core bioactive components (e.g., tanshinoneⅡA, catalpol, and formononetin), classic and empirical compound formulas (e.g., Huangqi Guizhi Wuwu decoction, Compound Qiying granules, and Tangluoning), and TCM external therapies (e.g., electroacupuncture, moxibustion with seed-sized moxa cone, and acupotomy). Through an in-depth analysis of the functional characteristics and mechanisms of these TCM interventions, particularly their bidirectional regulation and multi-target synergy, this review aims to provide a solid theoretical foundation for further optimizing TCM-based strategies for NP prevention and treatment and to delineate future directions for clinical translation of related research achievements and targeted drug development.

Objective:To explore the deubiquitination modification of the deubiquitinase BRCC3 in rats with neuropathic pain (NPP) treated with meridian tuina of Zhuang medicine and its analgesic molecular mechanism. Methods:Rats were divided into normal group, sham-operation group, model group, sham-tuina group, and meridian tuina of Zhuang medicine group using a random number table method, with 9 rats in each group. Except for the normal and sham operation groups, spinal nerve ligation models were prepared in all other groups. On the first day after surgery, intervention was carried out on the meridian tuina of Zhuang medicine and sham-tuina groups for 14 days, while the other three groups were not intervened; the paw withdrawal mechanical threshold (PWMT) and paw withdrawal thermal latency (PWTL) of each group of rats were measured at 0 days before operation, 1, 7, and 14 days after operation. Western blot was used to detect the expressions of BRCC3 and NLRP3 proteins in spinal cord tissue, and ELISA was used to detect the level of IL-1β in serum. Results:On postoperative 7 and 14 d, compared with the model group, the meridian tuina of Zhuang medicine group showed an increase in PWMT and PWTL, a decrease in NLPR3 and BRCC3 expression in spinal cord tissue, and a decrease in serum IL-1β levels ( P<0.05). Compared with the sham-tuina group, the meridian tuina of Zhuang medicine group showed an increase in PWMT and PWTL, a decrease in NLRP3 protein expression in spinal cord tissue, and a decrease in IL-1β levels in serum ( P<0.05). Conclusion:Meridian tuina of Zhuang medicine can alleviate pain sensitivity in SNL model rats, and its mechanism is related to the inhibition of the expression of deubiquitinase BRCC3 by meridian massage of Zhuang medicine, which increases the ubiquitination level of NLRP3 and hinders its activation, thereby blocking the immune inflammatory response mediated by inflammatory factors.

Objective To evaluate the role of cardiopulmonary coupling (CPC) analysis in evaluating the sleep quality of patients with Parkinson's disease (PD),and explore its correlation with subjective complaints in patients with PD.Methods Sixty patients with PD,admitted to our hospital from October 2015 to December 2017,and 45 healthy controls accepted physical examinations were enrolled. The sleep qualities of all subjects were assessed using CPC and Pittsburgh sleep quality index (PSQI).Independent-sample t test was used to compare the CPC variables and PSQI results, and the correlations between CPC variables and PSQI results were analyzed. Results (1) High-frequency coupling (HFC) and sleep efficiency of PD patients were significantly lower than those of healthy controls (P<0.05);very low-frequency coupling and sleep latency of PD patients were significantly higher than those of healthy controls (P<0.05).(2) Results of PSQI in the PD patients were significantly increased as compared with those in the control group (P<0.05);there was a negative correlation between PSQI results and HFC ratio in the PD group (r=-0.391,P=0.002).Conclusion CPC analysis is effective in reflecting the subjective sleep quality of PD patients and can be used as a new technology in sleep monitoring.
