1.Visually amplification-free rapid detection of 2019-nCoV nucleic acid based on CRISPR/Cas13a

Objective:Based on the specific cleavage and non-specific "trans-cleavage" activities of the clustered regularly interspaced short palindromic repeats (CRISPR) and CRISPR-associated protein(CRISPR/Cas13), we established a visually amplification-free rapid detection technique of 2019-nCoV nucleic acid. This technique is easily processed with a low detection limit and good specificity.Methods:According to the 2019-nCoV gene sequence, specific CRISPR RNAs were screened and designed by bioinformatics analysis, and then synthesized as universal signal-strained RNA transcription targets in vitro to establish and optimize the reaction system. Moreover, the 2019-nCoV pseudoviral nucleic acid was used as a standard substance to evaluate the detection limit. A total of 65 positive samples were collected from various 2019-nCoV variants, while 48 negative samples included other clinically common respiratory pathogens, such as influenza A virus, influenza B virus, human parainfluenza virus, Klebsiella pneumonia, etc. All samples were tested by quantitative PCR (qPCR), digital PCR, and the method established in this study. The sensitivity and specificity of the newly established method were analyzed and evaluated. Results:With the newly established technique, the detection time for 2019-nCoV nucleic acid could be minimized to 6 minutes. In addition, the detection limit was 14 copies/μl when assisted by the displaying instrument, whereas it increased to 28 copies/μl with the naked eye. This technique had a sensitivity and specificity of 98.5% (66/67) and 100% (46/46) respectively, showing no statistically significant difference compared to the gold standard qPCR( P=1). Conclusions:This study has successfully established a CRISPR/Cas13a-based visually rapid detection technique for 2019-nCoV nucleic acid. This technique offers the advantages of a simple process, convenient operation, low environmental operating requirements, a detection limit close to qPCR, and a strong potential for on-site testing applications.
2.Analysis of the characteristics of newly-diagnosed occupational diseases among female workers in Foshan City, 2013-2023

Objective To analyze the characteristics of newly-diagnosed occupational diseases among female workers in Foshan City from 2013 to 2023. MethodsClinical data of newly-diagnosed occupational diseases among female workers in Foshan City from 2013 to 2023 were collected from the "Occupational Disease and Hazardous Detection Information System" subsystem of the "China Disease Prevention and Control Information System", and their distribution characteristics were analyzed using descriptive epidemiology. Results A total of 218 cases of newly-diagnosed occupational diseases among female workers in Foshan City were reported from 2013 to 2023, categorized into seven categories and 17 types. The median age and median length of exposure to occupational hazards among female workers were 45.1 and 7.8 years, respectively. The top three categories of newly-diagnosed cases were occupational diseases of the ear, nose, throat, and oral cavity (90 cases, all related to occupational noise-induced hearing loss), occupational chemical poisoning (61 cases), and occupational pneumoconiosis and other respiratory diseases (51 cases, including 50 cases with occupational pneumoconiosis). The newly-diagnosed cases were concentrated in Nanhai District, Shunde District, and Gaoming District, accounting for 75.2%. The newly-diagnosed cases were found in non-metallic mineral products manufacturing industry, metal products industry, and electrical machinery and equipment manufacturing industry, accounting for 50.9%. Enterprises of newly-diagnosed cases were mainly distributed in private enterprises and foreign-invested enterprises, accounting for 80.3%. The scale of enterprises of newly-diagnosed cases was mainly small- and medium-sized, accounting for 79.4%. Among the 218 cases, there were 21 cluster cases, involving 16 cases of occupational silicosis and five cases of occupational acute n-hexane poisoning, involving six manufacture industries, with 81.3% cases worked in sanitary ceramic products industry. Conclusion Occupational noise-induced deafness should be listed as a key occupational disease for female workers in Foshan City. Attention should be paid to the prevention of occupational diseases among middle-aged female workers in the non-metallic mineral products industry and small and medium-sized enterprises, and the cluster outbreaks of occupational pneumoconiosis.
3.Targeting a cryptic allosteric site of SIRT6 with small-molecule inhibitors that inhibit the migration of pancreatic cancer cells.

SIRT6 belongs to the conserved NAD+-dependent deacetylase superfamily and mediates multiple biological and pathological processes. Targeting SIRT6 by allosteric modulators represents a novel direction for therapeutics, which can overcome the selectivity problem caused by the structural similarity of orthosteric sites among deacetylases. Here, developing a reversed allosteric strategy AlloReverse, we identified a cryptic allosteric site, Pocket Z, which was only induced by the bi-directional allosteric signal triggered upon orthosteric binding of NAD+. Based on Pocket Z, we discovered an SIRT6 allosteric inhibitor named JYQ-42. JYQ-42 selectively targets SIRT6 among other histone deacetylases and effectively inhibits SIRT6 deacetylation, with an IC50 of 2.33 μmol/L. JYQ-42 significantly suppresses SIRT6-mediated cancer cell migration and pro-inflammatory cytokine production. JYQ-42, to our knowledge, is the most potent and selective allosteric SIRT6 inhibitor. This study provides a novel strategy for allosteric drug design and will help in the challenging development of therapeutic agents that can selectively bind SIRT6.
4.Acute liver injury due to Gukang capsules (骨康胶囊) , Jieguqili pills (接骨七厘丸) , and ibuprofen and codeine phosphate tablets

A 66-year-old female patient received Gukang capsules and Jieguqili pills orally for 11 days for fracture. On day 6 after the drugs withdrawal, she developed weakness, anorexia, and yellow urine. Drugs above-mentioned were re-given and ibuprofen and codeine phosphate tablets were added. Five days later, her symptoms were aggravated. Laboratory tests showed alanine aminotransferase (ALT) 778 U/L, aspartate aminotransferase (AST) 782 U/L, alkaline phosphatase (ALP) 151 U/L, total bilirubin (TBil) 34.3 μmol/L, direct bilirubin (DBil) 24.0 μmol/L, and gamma-glutamyltransferase (γ-GT) 111 U/L. It was considered to be liver injury caused by Gukang capsules, Jieguqili pills, and ibuprofen and codeine phosphate tablets. All drugs above were withdrawn and the liver-protective and anti-jaundice treatments were given. After 21 days, laboratory tests showed ALT 24 U/L, AST 17 U/L, ALP 104 U/L, TBil 13.6 μmol/L, DBil 9.0 μmol/L, and γ-GT 52 U/L.

6.Effects of systematic nursing intervention on calcium and phosphorus metabolism and pruritus in hemodialysis patients

Objective To explore the effects of systematic nursing intervention on calcium and phosphorus metabolism and pruritus in hemodialysis patients. Methods A total of 60 patients undergoing hemodialysis in the Department of Nephrology at Wuxi Second Hospital Affiliated to Nanjing Medical University from February to August 2017 were selected and divided into control group and observation group by random number method, with 30 cases in each group. The control group was given routine care. Dietary guidance, psychological care and comprehensive health education were implemented in the observation group on the basis of routine nursing. Diet, skin pruritus and calcium and phosphorus metabolism were compared between the two groups. Results The dietary compliance rate in the observation group (83.33%) was higher than that in the control group (60.00%) after the intervention, and the difference was statistically significant (P<0.05). There was no significant difference in serum calcium levels between the two groups (P>0.05). The differences in the total parathyroid hormone and serum phosphorus levels between the two groups were statistically significant (P< 0.05). The degree of pruritus, the distribution of the skin and the score of sleep disturbance in the observation group were significantly lower than those in the control group, with statistical difference (P<0.05). Conclusions Systematic nursing plays a positive role in calcium and phosphorus metabolism and pruritus in hemodialysis patients and is worthy of promotion and application.
7.Application of standardized patients in classroom teaching of psychiatry

Objective To evaluate the effect of using standardized patients (SP) in psychiatry class teaching.Methods A standardized patient was used when giving instructions on seven years clinic medicine students of Fudan university,the effect of this teaching model was assessed.Results Totally 96.19% of students believed that standardized patients teaching model was better than the traditional model.Students in classes of using standardized patients scored higher than students in classes without using standardized patients.Conclusion The standardized patients teaching mode,recognized by almost all students,is believed to help students to better master the knowledge of psychiatry.
8.A quantitative analysis of interactions among ethanol, chloral hydrate and naloxone

AIM: To analyse quantitatively the interactions among ethanol, chloral hydrate and naloxone with isobologram, Q-test and multiple logistic regression methods. METHODS: The hypnotic effects of the three drugs on Kunming mice were observed. In two drugs interaction study, chloral hydrate and ethanol were given at different ratios (25∶75, 50∶50, 78∶22 and 80∶20). In three drugs interactions study, 15 min after treatment of naloxone at fixed dose (0.5 mg*kg-1 and 0.2 mg*kg-1) the mixture of chloral hydrate and ethanol (at 1∶1 and 1∶3 ratio) was given to induce sleep. The ED50 for hypnotic action (righting reflex loss) of chloral hydrate, ethanol, naloxone and their mixtures were calculated by use of isobologram, interaction Q-index test and multivariate logistic regression analysis. RESULTS: The mixtures of ethanol and chloral hydrate in all ratios revealed partial but significant synergism. But in addition with naloxone the three agents showed different natures of interactions according to different naloxone levels. CONCLUSION: There are synergistic interactions in hypnotic ED50s between ethanol and chloral hydrate at different ratios and antagonistic interaction in adding a fixed dose of naloxone. The results coincide with the pharmacologistic mechanism discussed in this paper.
9.Emphasis on the Humanities Education During Diagnostic Practice Teaching
