1.Comparative study on simultaneous bilateral and unilateral posterior vocal cord resection using CO 2 laser for bilateral vocal cord paralysis

Objective:To compare and analyze the efficacy of bilateral and unilateral posterior vocal cord resection with CO 2 laser under endoscopy in the treatment of bilateral vocal cord paralysis. Methods:This case series study retrospectively analyzed the data of 110 patients with bilateral vocal cord paralysis who underwent endoscopic CO 2 laser posterior cordotomy at the Department of Otolaryngology-Head and Neck Surgery, the First Affiliated Hospital of Naval Medical University, from October 2016 to January 2023. The cohort consisted of 36 males [mean age (45.5±9.1) years, range 24-72 years] and 74 females [mean age (47.2±10.1) years, range 22-67 years]. Among them, 47 patients underwent simultaneous bilateral posterior cordotomy (bilateral cordotomy group), while 63 patients underwent unilateral posterior cordotomy (unilateral cordotomy group). Pre-and postoperative indicators, including swallowing function, glottal size, and vocal function (subjective and objective assessments), were compared between the two surgical approaches. The Wilcoxon signed-rank test was used to analyze changes in swallowing function, glottal size, and vocal function (subjective and objective assessments) within each group before and after surgery, whereas the Mann-Whitney U test was utilized to assess differences between groups. Results:Postoperative follow-up was 1-5 years [median follow-up time was 1.6 (1.3, 2.0) years].The one-time extubation rate was 71.4%(45/63)in the unilateral posterior vocal cord resection group and 87.2%(41/47)in the bilateral posterior vocal cord resection group, significantly higher in the bilateral group ( χ2=3.94, P<0.05). One week after surgery, the swallowing function score of unilateral cordotomy group was 2 (1, 2.5) points, which was significantly better than that of bilateral cordotomy group [2 (1.5, 3) points, Z=-2.118, P<0.05], and the swallowing function score of both groups returned to normal 3 months after surgery. There were no significant differences in preoperative glottic closure during inhalation, auditory perceptual evaluation (GRBAS), objective voice analysis, and Voice Handicap Index-10 (VHI-10) between the two groups ( P>0.05). Postoperatively, the maximum transverse diameter of the posterior glottis during inspiration in the unilateral cordotomy group was 4.49 (4.24, 4.77) mm, significantly smaller than that in the bilateral cordotomy group, which was 5.05 (4.52, 5.62) mm ( Z=-4.103, P<0.05). Among the GRBAS parameters, G (grade of hoarseness), B (breathiness), and A (asthenia), as well as VHI-10 scores and objective voice analysis parameters [jitter, shimmer, harmonic-noise ratio (HNR), and maximum phonation time (MPT)], were significantly better in the unilateral cordotomy group compared to the bilateral cordotomy group, with statistically significant differences ( P<0.05). Conclusions:Unilateral posterior vocal cord resection using CO 2 laser is simple and feasible for the treatment of bilateral vocal cord paralysis, with shorter recovery time, maximal preservation of laryngeal phonatory function, and fewer complications compared to bilateral resection. However, the one-time extubation rate is higher with bilateral resection, reducing the need for a second surgery and associated patient discomfort. This study offers guidance for clinical decision-making in the surgical management of bilateral vocal cord paralysis.
2.Promotional effect of CHAaHGS on hair growth and its mechanism

Objective:To investigate the effect of Chinese Herbal Anti-Alopecia and Hair Growth Solution(CHAaHGS)on the hair growth through in vitro experiments on the human dermal papilla cells(HDPCs),in vivo experiments in the C57BL/6 mice,and human efficacy tests,and to clarify its potential mechanism.Methods:The HDPCs were divided into control group,CHAaHGS group,and minoxidil group.MTT method was used to detect the proliferation activities of HDPCs in various groups;enzyme-linked immunosorbent assay(ELISA)method was used to detect the levels of vascular endothelial growth factor(VEGF),hepatocyte growth factor(HGF),insulin-like growth factor-1(IGF-1),and transforming growth factor β1(TGF-β1)in the supernatant of HDPCs in various groups;real-time fluorescence quantitative PCR(RT-qPCR)method was used to detect the expression levels of VEGF,HGF,IGF-1,TGF-β1,and alkaline phosphatase(ALP)mRNA in the HDPCs in various groups;Western blotting method was used to detect the expression levels of β-catenin,dishevelled segment polarity protein 1(DVL1),glycogen synthase kinase 3β(GSK-3β),phosphorylated GSK-3β(p-GSK-3β),and wingless-type MMTV integration site family member 3a(Wnt3a)proteins in the HDPCs in various groups.A total of 18 mice were randomly divided into control group,CHAaHGS group,and minoxidil group,with 6 mice in each group.The mouse hair loss model was established using hair removal cream,and corresponding drug treatments were administered immediately after hair removal.The lengths and weights of newly grown hair on day 21 of the mice in various groups were detected;HE staining was used to observe the morphology of hair follicles in the dorsal depilated skin areas of the mice in various groups on day 7;ELISA method was used to detect the levels of VEGF,HGF,IGF-1,and TGF-β1 in the skin tissue of dorsal depilated areas of the mice in various groups.Sixty subjects were randomly divided into control group and CHAaHGS group,with 30 subjects in each group.The numbers of hair loss and hair densities of the subjects in various groups were detected at weeks 0,4,8,and 12.Results:The MTT assay results showed that compared with control group,the proliferation activity of the cells in 50 mg·L-1CHAaHGS group was significantly increased(P<0.01).The ELISA assay results showed that compared with control group,the levels of VEGF,HGF,and IGF-1 in the cell supernatant of HDPCs in CHAaHGS group were significantly increased(P<0.05 or P<0.01),and the TGF-β1 level was significantly decreased(P<0.01).The RT-qPCR results showed that compared with control group,the expression levels of VEGF,HGF,IGF-1,and ALP mRNA in the cells in CHAaHGS group were significantly increased(P<0.05 or P<0.01),and the TGF-β1 mRNA expression level was significantly decreased(P<0.01).The Western blotting results showed that compared with control group,the expression levels of β-catenin,DVL1,p-GSK-3βand Wnt3a proteins in the cells in CHAaHGS group were significantly increased(P<0.05 or P<0.01),and the GSK-3β protein expression level was significantly decreased(P<0.05).In animal experiments,on day 21,compared with control group,the length of newly grown hair of the mice in CHAaHGS group was significantly increased(P<0.05),and the hair weight was significantly increased(P<0.01).On day 7,the HE staining results showed that compared with control group,the hair follicle spacing of the mice in CHAaHGS group was significantly decreased(P<0.05),and the number of hair follicles was significantly increased(P<0.01);the ELISA assay results showed that compared with control group,the levels of VEGF,HGF,and IGF-1 in skin tissue of dorsal depilated area of the mice in CHAaHGS group were significantly increased(P<0.05 or P<0.01),and the TGF-β1 level was significantly decreased(P<0.05).In human efficacy test,compared with control group,the number of hair loss of the subjects in CHAaHGS group was significantly decreased at week 12(P<0.01),and the local hair density was increased(P<0.05).Conclusion:CHAaHGS promotes hair growth,and the mechanism may be related to its ability to increase the proliferation activity of HDPCs,induce the secretion of VEGF,HGF,and IGF-1,and activate the Wnt/β-catenin signaling pathway.
3.Carotid ultrasound combined with TNF-α and receptor-interacting protein kinase 1 in evaluating prognosis of acute ischemic stroke

Objective To analyze the prognostic value of carotid ultrasound quantitative parameters combined with serum TNF-α and receptor-interacting protein kinase 1(RIPK1)levels for patients with acute ischemic stroke(AIS).Methods A total of 110 AIS patients admitted in our hospital from February 2020 to May 2024 were enrolled,and according to their modified Rankin scale(mRS)score after 3 months,they were divided into a good prognosis group(76 cases)and a poor prognosis group(34 cases).The carotid ultrasound quantitative parameters such as intima-media thickness(CIMT),end diastolic velocity(EDV)and peak systolic velocity(PSV)and serum TNF-α and RIPK1 levels were detected.The relationship of the parameters and the serum levels with prognosis was analyzed.Results The poor prognosis group had significantly higher carotid CIMT and EDV values and serum levels of TNF-α and RIPK1,and lower PSV value than the good prognosis group(P<0.01).The CIMT,PSV,EDV,TNF-α and RIPK1 were independent in-fluencing factors of prognosis of AIS patients(P<0.01).ROC curve analysis showed that the AUC value of CIMT,PSV,EDV,TNF-α and RIPK1 combined together in predicting the poor prognosis of AIS patients was 0.979,with a sensitivity of 94.10%and a specificity of 75.00%.Conclusion Carotid ultrasound quantitative parameters combined with serum TNF-α and RIPK1 levels can effectively assess the prognosis of AIS patients,with high predictive value.

6.Study on the Mechanism of Gegen Qinlian Decoction for Lowering Blood Lipids and Preventing Blood Glucose Increase Based on Intestinal Flora

OBJECTIVE:To study the ef fects of Gegen qinlian decoction (GGQLD)on blood lipid and blood glucose of hyperlipidemia(HLP)model rats ,and to explore its mechanism from the perspective of intestinal flora. METHODS :Totally 48 rats were randomly divided into blank control group (n=8)and modeling group (n=40). For consecutive 5 weeks,model group was given high-lipid diet to induce HLP model ;blank control group was given routine diet. After modeling ,30 modeling rats were randomly divided into model group ,simvastatin group (positive control ,10 mg/kg),GGQLD high-dose ,medium-dose and low-dose groups (14.85,4.95,1.65 g/kg,by crude drug ),with 6 rats in each group. Blank control group and model group were given constant volume of normal saline intragastrically ;administration groups were given relevant medicine intragastrically ,once a day,for consecutive 11 weeks. At the same time ,each group was continuously given corresponding diet. After the last medication , body mass and body length of rats were determined ,and Lee ’s index was calculated. Serum levels of TG ,TC,HDL-C,LDL-C and fasting blood glucose (FBG)were determined in rats. DNA of rat caecum content was extracted for 16S rRNA V 3-V4 region sequencing. The Two-part model was used to analyze the correlation between intestinal flora with lipids and blood glucose. RESULTS:After 11 weeks of administration ,compared with blank control group ,the body mass ,body length ,Lee’s index , serum levels of TC ,TG,HDL-C and FBG of model group were increased significantly (P<0.05 or P<0.01),while the level of HDL-C was decreased significantly (P<0.05). Compared with model group ,body mass and Lee ’s index and serum levels of TG , FBG of rats in GGQLD high-dose group ,and serum levels of TC ,TG in GGQLD medium-dose group ,as well as serum level of TG of rats in GGQLD low-dose group was decreased significantly (P<0.05 or P<0.01). Correlation analysis with intestinal flora showed that TC and TG shared 3 operational taxonomic units (OTU),including OTU 559,OTU701 and OTU 135(OTU135 was also shared with FBG ),which were all positively correlated with the level of TC ,TG and FBG (P<0.01). The three OTU were annotated as Tyzzerella of Spirillaceae ,Anaerotruncus of Verrucaceae and Peptoclostridium of Streptococcidae ,respectively. High-dose and low-dose GGQLD had a down-regulating effect on Tyzzerella and Anaerotruncus(P<0.05 or P<0.01),while had up-regulating effect on Peptoclostridium(P<0.01). CONCLUSIONS :High-dose GGQLD (14.85 g/kg)can effectively reduce the body mass and blood lipid of HLP model rats ,and can prevent the abnormal increase of blood glucose of model rats. The mechanism may be associated with that the reduction of intestinal flora (Tyzzerella,Anaerotruncus)content.
7.Clinical analysis of selective laryngeal reinnervation using upper root of phrenic nerve and hypoglossal nerve branch in the treatment of bilateral vocal fold paralysis

Objective:To evaluate the airway and voice quality improvement in patients with bilateral vocal fold paralysis (BVFP) who underwent selective laryngeal reinnervation surgery.Methods:From January 2012 to December 2016, a retrospective study was conducted in 39 patients with BVFP who underwent selective laryngeal reinnervation surgery in Department of Otorhinolaryngology Head and Neck Surgery, the First Affiliated Hospital of Navy Medical University. All patients were examined by videostroboscopy, vocal function assessment, laryngeal electromyography and pulmonary function test before and after the surgery, and followed up for at least 2 years to evaluate the efficacy and safety of the surgery.Wilcoxon signed rank test was used to analyze the G score and VHI-10 score data. Paired t-test was used to analyze acoustic parameters, MPT values and pulmonary function parameters. Results:Postoperative infection and hemorrhage occurred in one patient separately.Videostroboscopic videos showed that at 4-8 months postoperatively, vocal folds in 35 patients achieved moderate or severe abduction during inspiration, 2 patients only achieved mild abduction, 2 patients showed no abduction,while all patients achieved adduction in bilateral vocal cords during phonation. The recovery rate of moderate-to-severe abduction was 89.7% (35/39), and these patients were decannulated successfully. At 12 months after operation, G score and VHI-10 score were significantly lower than those before operation ( P<0.05), and the acoustic parameters jitter, shimmer, HNR and MPT were significantly improved ( P<0.05). Most of the parameters of the pulmonary function test at 3 months postoperatively returned to the normal reference level, while the maximum inspiratory pressure (PImax) at 12 months after operation was still slightly lower than the normal level, but it was significantly improved compared with preoperative value ( P<0.05). The EMG data at 12 months postoperatively showed full interference potentials in 37 patients in bilateral posterior cricoarytenoid muscles during inspiration, and full interference potentials in bilateralthyroarytenoid muscles during phonation. Obvious misdirected regeneration electric activitieswere found in two of them. Potentials in posterior cricoarytenoid muscle were weak in 2 cases with poor abduction. During long-term follow-up, only one case showed decreased abduction, but did not affect respiratory function. Conclusions:The selective laryngeal reinnervation procedure applied in the present study can restore physiological motion of vocal cords. The success rate was high, the curative effect was stable, and the complications were rare. It is worth of promotion.
8.Guidance value of TOI classification for treatment of traumatic T-type atlantoaxial dislocation

Objective To investigate the guidance value of TOI classification in treating traumatic T-type atlantoaxial dislocation (ADD).Methods A retrospective case series study was made on 32 cases of traumatic TOI T-type ADD treated between January 2012 and December 2015.There were 19 males and 13 females,aged (38.4 ± 14.7) years.Fifteen cases of T1-type underwent external fixation or internal fixation without fusion,while 17 cases of T2-type underwent internal fixation with fusion.Symon-Lavender clinical standard,Japanese orthopedic association score (JOA),visual analogue scale (VAS),atlas-dens interval (ADI) and space available for the cord (SAC) were used to evaluate the therapeutic effect.Results Patients were followed up for 6-54 months (mean,32.4 months).At final follow-up,ADI was decreased to (2.3 ± 1.4) mm from preoperative (5.6 ± 1.6) mm,but SAC was increased to (15.4 ± 1.9) mm from preoperative (12.0 ± 2.9) mm(P < 0.01).At final follow-up,cervical axial rotation range of motion was 102°-154° in T1-type cases and 57°-93° in T2-type cases.Range of motion for atlantoaxial joint was preserved in T1-type cases,but lost in T2-type cases.According to the Symon-Lavender clinical standard,there were 14 cases of mild disability,nine moderate disability,eight severe disability and one extremely severe disability before operation;there were 21 cases of mild disability,nine moderate disability and two severe disability at last follow-up.Significant difference was observed in the grades according to the Symon-Lavender clinical standard before operation and at last follow-up (P <0.05).At last follow-up,JOA score was increased to (14.6 ± 2.9) points from preoperative (9.9± 3.2) points,and VAS was decreased to (2.7 ± 1.3)points from preoperative (6.0 ± 1.6)points (P < 0.01).Conclusions By using TOI classification,reconstruction of stability and improved neurological function can be achieved in treatment of traumatic T-type atlantoaxial dislocation.Non-fusion treatment of T1-type atlantoaxial dislocation can preserve range of motion for atlantoaxial joint.
9.Effect of Ginseng, Ginseng polysaccharides and Ginsenoside on the human lung cancer cells A549 in co-culture system of TAMs with A549 cells

This study was aimed to explore the anti-tumor functions of Ginseng on the human lung cancer cells A549, through investigating the effects of water extract of Ginseng(WEG), Ginseng polysaccharides(GPS)and Ginseng total saponins(GTS)on cell proliferation, cell migration and cytoskeleton of A549 cells. Using A549 cells as target cells, the TAMs model in vitro was established from THP-1. MTT method was used to observe the effects of different concentrations of WEG, GPS, GTS on A549 cells in co-culture system of TAMs with A549 cells. The cell migration of A549 cells was assayed by real-time cell analysis(RTCA). The cytoskeleton of A549 cells was detected by high content screening(HCS). It showed that in the co-culture system, WEG inhibited the proliferation, migration, and the area of cytoskeleton on A549 cells significantly(P< 0. 01). GPS inhibited A549 cell migration and the area of cytoskeleton(P < 0. 01)in co-culture system, but it showed no significant effect on the proliferation of A549 cells. Moreover, GTS can significant inhibit the proliferation of A549 cells(P< 0. 01), and the effect on the cell migration and the area of cytoskeleton was in conspicuous in the co-culture system. It was found that the two main components of ginseng showed different functions by the comparison of WEG, GPS, GTS in TAMs co-culture system. Results indicate that the antitumor effects of ginseng may be due to its multi-component regulation in the tumor microenvironment.
