As a key receptor for immune regulation, triggering receptor expressed on myeloid cells 2 (TREM2) plays an important role in the development and progression of liver diseases, but its specific mechanisms and regulatory strategies in different types and stages of liver diseases remain to be systematically elucidated. This article systematically reviews the molecular characteristics and signaling pathways of TREM2, elaborates on the multiple functions of TREM2+ macrophages in lipid metabolism, phagocytic function, inflammatory response, and fibrosis, and analyzes the dynamic and bidirectional roles of TREM2 in different liver diseases. Targeting the TREM2 signaling axis is expected to become a new strategy for regulating the hepatic immune microenvironment and intervening in disease progression.

Purpose To investigate the clinicopathological characteristics,immunophenotype,high-risk human papillomavirus(hrHPV)infection status, treatment, and prognosis of cervical carcinoma exhibiting adenoid cystic car-cinoma(ACC)features.Methods Clinical data from 10 cases of cervical cancer with ACC features were collected.A retrospective analysis was proformed on the patients'clinicopathological data, histological features, and immunopheno-type(EnVision method),along with in situ hybridization detection of hrHPV E6/E7 mRNA covering 18 hrHPV types. Results The median age of the patients was 67.5 years, comprising 4 biopsy specimens and 6 surgical specimens.Except for 2 cases detected incidentally during physical examination, the remaining 8 cases presented with unexplained vaginal bleeding or contact bleeding. The mean tumor diameter was 4.5 cm(range:2.5 to 9.0 cm),and tumor stages were available for 8 patients(5 cases in stage Ⅰ and 1 case each in stages Ⅱ,Ⅲ,and Ⅳ).The follow-up period was 8 to 42 months, excluding 1 case lost to follow-up,2 cases(Ⅲ C1 and ⅣB)died within 1 year after surgery.Histologi-cally,4 cases exhibited pure ACC morphology, while 5 cases were mixed carcinomas(combined with squamous cell carcinoma, adenoid basal cell carcinoma,high-grade neuroendocrine carcinoma,or carcinosarcoma, respectively). The characteristic ACC morphology manifested as cribriform,pseudo-glandular,tubular,trabecular,and/or solid pat-terns with one case mixed carcinoma's lymph node metastasis showing only ACC morphology.Immunohistochemically, all 10 tumors exhibited diffuse strong positivity for p16 and p63,a high Ki67 proliferation index(40%to 90%),and wild-type p53 expression, and in 50%of cases, only a minority of cells were positive for c-MYB.Additionally,SOX10 was positive in 8 of 9 cases and CD117 was positive in 7 of 10 cases, respectively. In situ hybridization for hrHPV E6/E7 mRNA confirmed the presence of transcriptionally active HPV infection in all cases. Conclusion Cervical cancer with ACC features is rare, and predominantly occurs in postmenopausal elderly women. It represents an HPV-associat-ed high-grade carcinoma in which clinical stage is a critical prognostic factor.Immunohistochemical staining for CD117 and SOX10 aids in the pathological diagnosis of this tumor type.

Lipoprotein X(LpX)is an atypical lipoprotein that abnormally accumulates in the plasma of patients with cholestatic liver disease, and it is also a major pathogenic factor for hypercholesterolemia. The formation of LpX is closely associated with the abnormal metabolism of free cholesterol, phospholipids, and other lipid components in bile.LpX cannot be cleared via the low-density lipoprotein receptor pathway and is mainly metabolized by the reticuloendothelial system. The abnormal accumulation of LpX is closely associated with various complications of cholestatic liver disease, including xanthoma and neuropathy.Although there is no significant correlation between the high level of LpX and the risk of atherosclerosis, the role of LpX in cholesterol metabolism disorders cannot be neglected.Due to the similarities in density and characteristics between LpX and low-density lipoprotein cholesterol, clinical testing may result in misdiagnosis and related treatment risks.Current studies mainly focus on the mechanisms of LpX formation, the clinical significance of LpX,related detection methods, and potential therapeutic strategies.Plasma exchange is considered the preferred treatment in the state of high LpX,while traditional lipid-lowering drugs have a limited effect on LpX.This article explores the formation and clearance mechanisms of LpX in cholestatic liver disease, along with its impact of cholestatic liver disease and related detection methods,in order to improve the understanding of the pathophysiology and clinical significance of LpX,provide new strategies for the management of cholestatic liver disease and its complications, and finally improve the prognosis of patients.

Objective To use multiple model integration to predict the potential distribution of suitable areas for Hedysari Radix in China and the main environmental factors affecting its distribution. Methods Based on 119 geographical distribution points obtained from species distribution databases, and incorporating 19 bioclimatic and topographic factors,a species distribution model was constructed using the Biomod2 software package 3.5-1 version ensemble modeling platform, integrating six algorithms including generalized linear model, gradient boosting machine, random forest, and others.Geographic information system spatial analysis methods were used to quantitatively assess the distribution characteristics of suitable habitats of Hedysari Radix under current climate conditions and under future climate scenarios, while identifying the primary environmental drivers of its distribution. Results The ensemble model showed significantly superior predictive performance.TSS value and AUC value were 0.924 and 0.992,respectively.Key environmental factors significantly influencing the distribution of Hedysari Radix included slope, aspect, daily average temperature difference, isothermity, seasonal variation coefficient of temperature, lowest temperature in the coldest month, annual precipitation, driest month precipitation, and seasonal variation coefficient of precipitation.Under current climate conditions, suitable habitats for Hedysari Radix were primarily concentrated in the regions of Longnan and Dingxi in Gansu Province, covering an area of approximately 26.17×104 km2.Under future climate scenarios, suitable habitats will gradually shift toward the northwest into lower temperature zones, with a significant reduction in area. Conclusion The habitat suitability model developed in this study provides a basis for the conservation and sustainable utilization of Hedysari Radix genetic resources, while also offering a methodological reference for ecological adaptability studies of medicinal plants.

Primary biliary cholangitis (PBC) is an autoimmune liver disease characterized by progressive and non-purulent inflammation of small- and medium-sized bile ducts in the liver. Recent studies have shown that abnormal lipid metabolism is relatively common in patients with PBC, and 76% of PBC patients have dyslipidemia. The effects and harms of dyslipidemia have attracted much attention. Lipid metabolism disorders play an important role in the progression of PBC. This article mainly reviews the research advances in the manifestation, role, diagnosis, and treatment of lipid metabolism disorders in PBC, so as to provide new ideas for the treatment of PBC.

Journal of Clinical Hepatology 2024;40(4):839-843
Efferocytosis refers to the process by which apoptotic cells are engulfed and cleared by phagocytes, including professional phagocytes, such as macrophages and dendritic cells, and non-professional phagocytes, such as epithelial cells. Liver macrophages are the main cells with the function of efferocytosis in the liver. In recent years, an increasing number of studies have shown that various acute and chronic liver diseases are associated with the efferocytosis function of liver macrophages, including acute liver injury, alcoholic liver disease, nonalcoholic fatty liver disease, autoimmune liver disease, liver fibrosis, and liver cancer. This article elaborates on the expression of molecules associated with the efferocytosis function of macrophages, the process of efferocytosis, and the role of efferocytosis function in different liver diseases, so as to provide new ideas for the treatment of liver diseases.
