1.Shenxiao Tongluo Prescription Alleviates Kidney Injury in Diabetic Rats via PGC-1α/SIRT3/HIF-1α Pathway

ObjectiveTo investigate the mechanisms of mitochondrial dynamics and metabolic reprogramming in the treatment of diabetic nephropathy (DN) by Shenxiao Tongluo prescription via the peroxisome proliferator-activated receptor γ coactivator-1α (PGC-1α)/sirtuin-3 (SIRT3)/hypoxia-inducible factor-1α (HIF-1α) signaling pathway. MethodsSixty-five SD rats were randomized into a sham group (10 rats) and a modeling group (55 rats), and the modeling rats underwent left nephrectomy and intraperitoneal injection of streptozotocin (35 mg·kg-1) to prepare a DN model. After successful modeling, the rats were randomized into model, empagliflozin (10 mg·kg-1), and low-, medium-, and high-dose (7.656, 15.312, 30.624 g·kg-1, respectively) Shenxiao Tongluo prescription groups. The urine microalbumin (UmAlb), blood urea nitrogen (BUN), and serum creatinine (SCr) levels of rats in each group were assessed after continuous gavage for 8 weeks. The corresponding kits were used to measure the levels of lactate, superoxide dismutase (SOD), and malondialdehyde (MDA) in the kidney tissue. Hematoxylin-eosin staining, Masson staining, and periodic acid-Schiff staining were performed to observe the pathological changes in the kidney tissue. Transmission electron microscopy was employed to observe mitochondrial morphology. Immunohistochemistry was employed to determine the expression levels of dynamin-related protein 1 (DRP1) and pyruvate kinase M2 (PKM2) in the kidney tissue. Western blot was adopted to assess the protein levels of PGC-1α, SIRT3, HIF-1α, dynamin-related protein 1 (Drp1), optic atrophy 1 (OPA1), hexokinase 2 (HK2), and pyruvate kinase M2 (PKM2) in the kidney tissue. ResultsCompared with the sham group, the model group showed elevated levels of UmAlb, BUN, SCr, lactate, and MDA, decreased SOD level (P<0.05), glomerular hypertrophy, thickening of the mesangial basement membrane, vacuolar degeneration of renal tubular epithelial cells, and infiltration of renal interstitial inflammatory cells, oval mitochondria with disordered, blurred or disappearing cristae, down-regulated protein levels of PGC-1α, SIRT3, and OPA1, and up-regulated protein levels of HIF-1α, DRP1, HK2, and PKM2 (P<0.05). Compared with the model group, the treatment in all the groups increased the body weight, lowered the levels of GLU, UmAlb, BUN, and MDA, raised the level of SOD, alleviated the pathological damage in the kidney tissue and mitochondrial damage, up-regulated the expression of PGC-1α, SIRT3, and OPA1, and down-regulated the expression of HIF-1α, DRP1, and PKM2 (P<0.05). Empagliflozin and Shenxiao Tongluo prescription at medium and high doses lowered the levels of SCr and lactate and down-regulated the expression of HK2 (P<0.05), which had no statistical significance in the low-dose Shenxiao Tongluo prescription group. ConclusionShenxiao Tongluo prescription may regulate mitochondrial dynamics and metabolic reprogramming by activating the PGC-1α/SIRT3/HIF-1α pathway, thereby alleviating oxidative damage in the kidney tissue and delaying the progression of DN.

3.CT-Based Radiomics for Predicting the Therapeutic Effect of Hepatocellular Carcinoma After Transcatheter Arterial Chemoembolization

Purpose To identify hepatocellular carcinoma that responds to repetitive transcatheter arterial chemoembolization(TACE)based on CT radiomics.Materials and Methods A total of 96 patients diagnosed as hepatocellular carcinoma in the First Affiliated Hospital of Xinjiang Medical University from February 2018 to May 2024 were randomly divided into a training group(n=67)and internal validation group(n=29)at a ratio of 7∶3.All patients received three or more TACE treatments.Radiomics features were extracted from lipiodol of the target-lesions by semi-automatic segmentation on the axial CT image after TACE within 24 hours.The radiomics model were constructed by five features for differentiating non-response group from response group.Receiver operating characteristic curve analysis and decision curve were performed to evaluate the performance of the model.Results Child-Pugh classification(OR=2.737,P<0.05),BCLC stage(OR=2.907,P<0.05),multifocal tumors(OR=4.505,P<0.01)and alpha-fetoprotein(OR=1.002,P<0.01)were independent risk factors for predicting tumor response after TACE.The area under the curve of the non-contrast CT based model and the arterial-enhanced CT based model were 0.813 and 0.831 in the experimental group;0.748 and 0.788 in the validation group,respectively.Both of the two models showed good prediction performance.Conclusion The radiomics model based on CT imaging features after first TACE is effectively for differentiating non-response group from response group,lipiodol retention patterns from the target lesion can be the imaging biomarkers for TACE response prediction.

Deutetrabenazine(DTBZ)is a selective oral small molecule inhibitor of vesicular monoamine transporter 2(VMAT2).Its pharmacological action works by inhibiting VMAT2,thereby reducing the release of presynaptic dopamine and alleviating tardive dyskinesia symptoms caused by long-term use of dopamine receptor antagonists.Compared with tetrabenazine,DTBZ has longer half-life,lower peak plasma concentration,and smaller plasma concentration fluctuations.Clinical studies demonstrate that DTBZ significantly improves abnormal involuntary movement in patients with tardive dyskinesia and has a favourable safety profile.Based on available clinical evidence and practical experience,this paper discuss the common questions about DTBZ including the suitable population,dose,duration of treatment,combination administration with antipsychotics,efficacy assessment and application in special populations.This article aimed to provide guidance and recommendations on clinical application of DTBZ for clinicians.

Intervertebral disc herniation is a highly prevalent orthopedic disorder,and intervertebral disc degeneration(IDD),the key pathological basis,is a complex pathological process characterized by progressive degradation of extracellular matrix,structural failure,and loss of biomechanical function,which not only shows higher prevalence in the population,but is also the primary cause of chronic low back pain and dysfunction worldwide,causing a huge socioeconomic burden.Although constructing IDD animal models is important for exploring the pathological mechanisms and promoting translational research of this disease,the etiology and pathophysiological mechanisms of IDD have not been fully elucidated.There are significant differences between humans and common laboratory animals in spinal anatomy,biomechanics,and degenerative course,coupled with the diversity and lack of unified standards of existing IDD animal models.This guide systematically reviews IDD animal models of rodents,non-human primates,as well as different species such as rabbits,goats/sheep,pigs,and dogs,focusing on the modeling principles of three main types of models:inducible models(such as annulus fibrosus/nucleus pulposus/endplate injury and mechanical injury)are suitable for simulating acute injury and rapid screening of therapies due to their high controllability and short cycle;spontaneous models can better simulate the age-related natural degeneration process in humans;genetically modified models provide powerful tools for analyzing specific molecular pathways.The guideline deeply analyzes the key technical points,reproducibility,and clinical relevance of these models.It also compares their advantages,limitations,and applicable research scenarios to guide researchers to conduct"scientific question-driven"precise model selection.Meanwhile,to improve the depth and comparability of research results,this guideline proposes a multidimensional endpoint evaluation system for IDD animal model experiments covering imaging,histology,biochemistry/molecular biology,biomechanics,and pain-related behavior,with recommended observation time windows.It also clarifies the"3Rs(replacement,reduction,and refinement)"ethical principles and animal welfare requirements throughout the experiment.In addition,the guideline outlines future research directions such as integrating single-cell omics,multiscale mechanical analysis,and strengthening pain-related phenotype assessment.This guideline aims to provide researchers with a systematic and standardized methodological framework for the rational selection and application of IDD animal models under specific scientific questions and resource constraints,in order to reduce inter-study heterogeneity,enhance the translation efficiency of preclinical findings,promote high-quality development in the field,and ultimately provide a solid scientific foundation for developing innovative therapies to delay or even reverse IDD.

Recently, there has been an increasing risk of importation of tropical diseases into China and the resultant re-transmission in the country with the in-depth implementation of the “Belt and Road” Initiative, which poses a serious threat to the national public health security. To effectively respond to the cross-border transmission risk of tropical diseases and facilitate the process towards tropical disease control and elimination in China and the countries along the “Belt and Road” Initiative, this article analyzes the current status and governance risks of major imported tropical diseases, cross-border joint prevention and control polices implemented for tropical diseases and challenges in the establishment of the joint prevention and control system for tropical diseases in China, and discusses the establishment and implementation path of the joint prevention and control system for tropical diseases in countries along the “Belt and Road” Initiative. This path covers the establishment of cross-border cooperation mechanisms, research and development and pilot production of Chinese public health products, and implementation of key cross-border tropical disease prevention and control projects. The establishment of this system will further improve Chinese prevention and control capabilities for key cross-border tropical diseases, build a demonstrative prevention and control model for tropical diseases, and promote international technical exchanges and cooperation of tropical diseases.
