1.Analysis of management indicators for type 2 diabetes mellitus patients in Urumqi City from 2017 to 2021

Objective:
To investigate the implementation and evaluate the effectiveness of health management services for patients with type 2 diabetes mellitus (T2DM) in Urumqi City from 2017 to 2021, so as to provide the reference for enhancing T2DM patients management.
Methods:
Data on the rates of record establishment, health management and standardized management for T2DM patients, and blood glucose control rate in managed patients in Urumqi City from 2017 to 2021 were collected through the quarterly reports on basic public health service management from the Weining Health Platform System. The trends in the four management indicators, and the differences between urban and rural areas were analyzed.
Results:
The rates of record establishment, health management and blood glucose control rate in managed patients in Urumqi City were 46.94%, 38.37% and 59.92%, respectively, showing upward trends from 2017 to 2021 (all P<0.05). The rate of standardized management was 75.89%, showing a downward trend (P<0.05). The rate of record establishment was higher in urban areas than in rural areas (47.76% vs. 40.56%, P<0.05). The rates of standardized management and blood glucose control in managed patients were lower in urban areas than in rural areas (75.18% vs. 81.46%, 58.93% vs. 67.64%, both P<0.05). The rate of health management was 38.39% in urban areas and 38.24% in rural areas, with no statistically significant difference (P>0.05). The trends in the rates of record establishment, health management and standardized management in both urban and rural areas were consistent with those in the overall population.
Conclusions
From 2017 to 2021, the rates of record establishment, health management and blood glucose control in managed patients in Urumqi City showed upward trends, while the rate of standardized management exhibited a downward trend. There were urban-rural differences in the rates of record establishment, standardized management and blood glucose control in managed patients.
2.Data Mining on Medication Patterns of Formulas Containing the Herbal Pair of Bupleuri Radix and Scutellariae Radix

Objective To analyze the composition patterns and indications of formulas containing the herbal pair of Bupleuri Radix and Scutellariae Radix(abbreviated as herbal pair Chaihu-Huangqin).Methods With Zhong Yi Fang Ji Da Ci Dian(Encyclopedia of Chinese Medicinal Formulas)as the data source,formulas containing herbal pair Chaihu-Huangqin were screened.Excel 2019,SPSS Modeler,and Cytoscape were employed to investigate the composition patterns and indications of the formulas containing the herbal pair.Results A total of 928 formulas containing the herb pair were identified,involving 293 herbs.Beyond Bupleuri Radix and Scutellariae Radix,high-frequency herbs of Glycyrrhizae Radix et Rhizoma,Ginseng Radix et Rhizoma,Angelicae Sinensis Radix,Paeoniae Radix Alba,Pinelliae Rhizoma,Gardeniae Fructus,and Poria are the primary ingredients of formulas containing herbal pair Chaihu-Huangqin.The herbs as the ingredient combinations were predominantly cold in nature and were bitter,pungent,or sweet in flavors,primarily acting on the liver and lung meridians.Association rule analysis revealed that herbal pair Chaihu-Huangqin was frequently use together with herbal groups of"Pinelliae Rhizoma-Ginseng Radix et Rhizoma-Glycyrrhizae Radix et Rhizoma"and"Poria-Ginseng Radix et Rhizoma-Glycyrrhizae Radix et Rhizoma".Formulas containing herbal pair Chaihu-Huangqin were indicated for 184 kinds of diseases,in particular for exogenous febrile diseases,heat syndrome,and ocular disorders.Complex network analysis and dosage analysis indicated that common combined herbs of Pinelliae Rhizoma,Ginseng Radix et Rhizoma,Gardeniae Fructus,and Puerariae Lobatae Radix were indicated for exogenous febrile diseases;common combined herbs of Paeoniae Radix Rubra,Coptidis Rhizoma,and Gardeniae Fructus were indicated for heat syndrome;and common combined herbs of Plantaginis Semen,Cimicifugae Rhizoma,and Cassiae Semen were indicated for ocular disorders.The dosage ratio of Bupleuri Radix-Scutellariae Radix consistently maintained a 1∶1 ratio across all above indications.Conclusion The herbal pair Chaihu-Huangqin functions to harmonize shaoyang meridian and demonstrates broad clinical applicability.The data mining study has revealed its dominant indications and combinatorial patterns,which will provide evidence-based insights for modern clinical practice.
3.Obesity-driven oleoylcarnitine accumulation in tumor microenvironment promotes breast cancer metastasis-like phenotype.

Obesity is a significant risk factor for cancer and is associated with breast cancer metastasis. Nevertheless, the mechanism by which alterations in systemic metabolism affect tumor microenvironment (TME) and consequently influence tumor metastasis remains inadequately understood. Herein, we found that perturbations in circulating metabolites induced by obesity promote metastasis-like phenotypes in breast cancer. Oleoylcarnitine (OLCarn) concentrations were elevated in the serum of obese mice and humans. Administration of exogenous OLCarn induces metastasis-like characteristics in breast cancer cells. Mechanistically, OLCarn directly interacts with the Arg176 site of adenylate cyclase 10 (ADCY10), leading to the activation of ADCY10 and enhancement of cAMP production. Mutations at Arg176 prevent OLCarn from binding to ADCY10, disrupting the ADCY10-mediated activation of cyclic adenosine monophosphate (cAMP) signaling pathway. This activation promotes transcription factor 4 (TCF4)-dependent kinesin family member C1 (KIFC1) transcription, thereby driving breast cancer metastasis. Conversely, the neutralization of both ADCY10 and KIFC1 through knockdown or pharmacological inhibition abrogates the oncogenic effects mediated by OLCarn. Hence, obesity-induced systemic environmental changes lead to the aberrant accumulation of OLCarn within the TME, making it a potential therapeutic target and biomarker for breast cancer.
4.Genomic characteristics and phylogenetic analyses of enteroaggregative Escherichia coli infection in diarrhea outpatients in Pudong New Area, Shanghai

ObjectiveTo investigate the whole genomic characteristics and phylogenetic relationships of clinical isolates of enteroaggregative Escherichia coli (EAEC) in diarrhea outpatients in Pudong New Area, Shanghai. MethodsBased on the diarrheal disease surveillance network in Pudong New Area, Shanghai, whole-genome sequencing was performed on a total of 55 EAEC strains isolated from fecal samples of the diarrhea outpatients from January 2015 to December 2019. The genome analyses based on raw sequencing data encompassed genome size, coding genes, dispersed repeat sequences, genomic islands, and protein coding regions, and pan-genome analyses were conducted simultaneously. Contigs sequences assays were performed to analyze molecular characteristics including serotypes, antibiotic resistance genes, and virulence factors. The phylogenetic clusters and multilocus sequence typing (MLST) were identified, and a phylogenetic tree was constructed. ResultsEAEC exhibited an open pan-genome. The predominant serotype of EAEC in diarrhea outpatients in Pudong New Area was O130:H27, and the carriage rate of β-lactam resistance genes was the highest (67.27%, 37/55). A total of 29 virulence factors and 106 virulence genes were identified, phylogenic group B1 was the predominant group, and clonal group CC31 was the dominant clonal group. The strain distribution was highly heterogeneous. ConclusionThe genomic characteristics of EAEC displayed significant strain polymorphism. It is necessary to develop effective strategies for differential diagnosis and improve detection capabilities for infection with EAEC of different serotypes and genotypes.
5.Study on the influence of field angle on the results of EPID dose verification in vivo

Objective To investigate the effect of field angle on the results of in vivo dose validation of electronic portal imaging device(EPID)in patients.Methods Design the mold test and analyze the influence of different mold thicknesses and different frame angles on the 2D γ pass rate.Twenty-three patients who un-derwent radiotherapy in the Department of Radiotherapy of Jinhua Municipal Central Hospital from January to June 2023 were selected as the research object.In vivo dose verification was carried out during treatment to obtain 2D γ pass rate using same-day sector beam CT(FBCT)of planned CT and executive image guided(IG-RT)as reference images,and the influence of field angle on pass rate was analyzed.Results When the frame angle was unchanged,the area of the shooting field was larger than 17 cm×17 cm,and the 2D γ passing rate decreased with the increase of the thickness.The frame angle had no effect on the 2D γ pass rate when the mold thickness was constant.In clinical treatment data,the passage rate of 2D γ near 0°/180° was higher than that near 90°/270°(P<0.05),and the passage rate near 90°/270° in the FBCT group was higher than that in the IGRT group(P<0.05).The median passing rate of 3 mm 2D γ was 97.97%in 3%of the 23 patients.The non-IGRT group was 96.81%,the IGRT group was 97.89%,the FBCT group was 98.94%.There was a statistically significant difference in 2D γ passing rate between the non-IGRT group and the IGRT group(Z=-5.083,P<0.05),and there was a statistically significant difference in 2D γ passing rate between the IGRT group and the FBCT group(Z=-10.657,P<0.05).Conclusion Clinically,the difference of pass rate in vi-vo dose verification at different rack angles is mainly due to the difference of images within and between ses-sions.Using same-day FBCT as the reference image for in-vivo dose verification can improve the accuracy of pass rate and eliminate the influence of image difference between sessions.
6.Kobophenol A inhibits LPS-induced macrophage M1 polarization via Prdx6

Objective :
To explore the effects and mechanisms of Kobophenol A ( KPA) on lipopolysaccharide ( LPS) -induced M1 macrophage polarization,and to provide a theoretical basis for the treatment of inflammatory immune diseases and the development of new drugs.
Methods:
The M1 macrophage polarization model of RAW264. 7 was established by LPS induction,and the peroxiredoxin 6 ( Prdx6) knockdown model was constructed using the Prdx6 inhibitor MJ33 and Prdx6-siRNA.RAW264. 7 cells,a mouse macrophage cell line,were treated with various concentrations of KPA. Cell viability was assessed using the CCK-8 assay.The expression levels of Prdx6 and M1 macrophage polarization-related proteins,including inducible nitric oxide synthase ( iNOS) and cyclooxygenase-2 ( COX-2) ,were detected by Western blot and immunofluorescence staining.The expression levels of Prdx6 and M1 macrophage polarization-related genes iNOS,interleukin-6 ( IL-6) ,and tumor necrosis factor α ( TNF-α) ,were measured by RT-qPCR. Flow cytometry was employed to detect the expression of cluster of differentiation 86 ( CD86) ,a marker of M1 macrophages.
Results:
Compared with the LPS-induced M1 macrophage polarization model , KPA significantly reversed the morphological changes of M1 macrophage polarization in RAW264. 7 macrophages and decreased the expression of M1 macrophage polarization-related proteins iNOS,COX- 2,CD86 and related genes iNOS,IL-6,TNF-α ( all P<0. 05) .In addition,LPS significantly downregulated the expression of Prdx6 in RAW264. 7 macrophages,while KPA upregulated the expression of Prdx6.Moreover,treatment with the Prdx6 inhibitor MJ33 significantly upregulated the expression of iNOS,a marker of M1 macrophage polarization,in RAW264. 7 macrophages,whereas treatment with KPA significantly downregulated the expression of iNOS ( all P<0. 05) .
Conclusion
KPA inhibits LPS-induced M1 polarization of RAW264. 7 macrophages by upregulating the expression of Prdx6.

8.Antibody-platinum(Ⅳ)prodrugs conjugates for targeted treatment of cutaneous squamous cell carcinoma

Antibody-drug conjugates(ADCs)are a new type of targeting antibodies that conjugate with highly toxic anticancer drugs via chemical linkers to exert high specificity and efficient killing of tumor cells,thereby attracting considerable attention in precise oncology therapy.Cetuximab(Cet)is a typical antibody that offers the benefits of good targeting and safety for individuals with advanced and inoperable cutaneous squamous cell carcinoma(cSCC);however,its anti-tumor activity is limited to a single use.Cisplatin(CisPt)shows good curative effects;however,its adverse effects and non-tumor-targeting ability are major drawbacks.In this study,we designed and developed a new ADC based on a new cytotoxic platinum(Ⅳ)prodrug(C8Pt(Ⅳ))and Cet.The so-called antibody-platinum(Ⅳ)prodrugs conjugates,named Cet-C8Pt(Ⅳ),showed excellent tumor targeting in cSCC.Specifically,it accurately delivered C8Pt(Ⅳ)into tumor cells to exert the combined anti-tumor effect of Cet and CisPt.Herein,metabolomic analysis showed that Cet-C8Pt(Ⅳ)promoted cellular apoptosis and increased DNA damage in cSCC cells by affecting the vitamin B6 metabolic pathway in tumor cells,thereby further enhancing the tumor-killing ability and providing a new strategy for clinical cancer treatment using antibody-platinum(Ⅳ)prodrugs conjugates.
9.Development and performance testing of a novel transcatheter tricuspid valve interventional device

Objective To develop a novel transcatheter tricuspid valve replacement device and test its performance. Methods The transcatheter tricuspid valve stent consisted of double-layer self-expanding nitinol stent, biotissue-derived bovine pericardial leaflets, and PTFE woven. The delivery system, mainly consisting of a handle control unit and a delivery sheath, was sent to the correct position via right atrium or jugular vein. The sheath had a visualization feature, and the handle control unit could realize the functions of stable release and partial recovery of the interventional valve. In addition, this study performed animal survival experiments on the basis of in vitro experiments. A large-white pig was used as the experimental animal. Cardiopulmonary bypass was established through median thoracotomy, then the right atrium was opened, and the interventional valve was released under direct vision without cardiac arrest. Approximately 1 month after interventional valve implantation, the maneuverability and stability of the interventional tricuspid device were evaluated by autopsy. Results Through the animal experiment, the interventional valve was successfully released, and the anchoring was satisfactory. Postoperative transthoracic echocardiography showed that the interventional valve opened and closed well, the flow rate of tricuspid valve was 0.6 m/s, and there was no obvious tricuspid regurgitation. One month after the operation, we dissected the large-white pig and found the interventional valve was not deformed or displaced, the leaflets were well aligned, and there was thrombus attachment in the groove between the inner and outer layers of the interventional valve. Conclusion Animal experiment shows that the novel device can stably and firmly attach to the tricuspid annulus, with good anchoring effect, and effectively reduce paravalvular leakage.
10.The protective effect and mechanism of Taraxasterol on Erastin induced ferroptosis in chondrocytes

Objective To investigate the role of Taraxasterol(TAR)on ferroptosis in chondrocytes induced by Erastin.Methods The C28/I2 chondrocyte line was treated with Erastin to construct the ferroptosis model of chon-drocytes in vitro and the experiments were divided into Control,Erastin,TAR,and TAR+Erastin groups.Cell via-bility was detected by the CCK-8 assay.Cytotoxicity was detected by the lactate dehydrogenase(LDH)kit and the Calcein/PI cytokinesis kit.Flow cytometry was used to detect lipid reactive oxygen species(ROS).The intracellular glutathione(GSH)content was detected by GSH kit.Mitochondrial membrane potential was detected by JC-1 stai-ning and RH123 staining.ACSL4 and GPX4 protein expression and the key indicators of ferroptosis were detected by Western blot.Results TAR restored the decreased cell viability of C28/I2 chondrocytes induced by Erastin treatment as well as reduced Erastin-induced cytotoxicity(P<0.01).Compared with the control group,the level of intracellular lipid ROS increased(P<0.01)and the content of GSH decreased(P<0.01)after treatment with Erastin,while TAR could reduce the production of lipid ROS(P<0.01)and increase the content of GSH(P<0.01).TAR restored mitochondrial membrane potential in C28/I2 chondrocytes ferroptosis,decreased ACSL4 pro-tein expression(P<0.01)and increased GPX4 protein expression(P<0.01).In addition,TAR restored the re-duced cell viability caused by IL-1 β treatment.Conclusion TAR can inhibit Erastin induced ferroptosis in C28/I2 chondrocytes,which may be related to the regulation of ACSL4 and GPX4 protein expression.
