1.The Chinese version of Chronic Illness Rejection and Discrimination Scale: reliability and validity in maintenance hemodialysis patients

BackgroundPerceived discrimination has been identified as a main risk factor for depression in maintenance hemodialysis patients. Chronic Illness Rejection and Discrimination Scale (CIRDS) is a measure for assessing perceived discrimination in individuals with chronic disease. However, the Chinese version of CIRDS for maintenance hemodialysis patients has not yet been established. ObjectiveTo translate CIRDS into Chinese version and evaluate its reliability and validity in maintenance hemodialysis patients, so as to provide an effective tool for assessing the perceived discrimination among maintenance hemodialysis patients. MethodsThe Brislin's model for translation, back-translation, cross-cultural adaptation and pre-experimentation was utilized to develop a Chinese version of CIRDS. A coherent of 250 maintenance hemodialysis patients attending Taihe Hospital Affiliated to Hubei Medical College, from July to October 2023 were selected as the research subjects. The formal scale was refined by employing item analysis, exploratory factor analysis and confirmatory factor analysis. The validity of the scale was evaluated using content validity and construct validity. The reliability of the scale was evaluated using Cronbach's α coefficient, test-retest reliability and split-half reliability. ResultsThe Chinese version of CIRDS consisted of 11 items, including 2 factors (perceived discrimination and perceived rejection). The scale-level content validity index (S-CVI) value was 0.898 and the item-level content validity index (I-CVI) values ranged from 0.875 to 1.000. Two common factors were extracted by exploratory factor analysis and explained 65.41% of the total variance. Confirmatory factor analysis also indicated that the model provided a good fit for the data. The Cronbach's α coefficient of the scale was 0.910, with Cronbach's α coefficients of 0.835 and 0.912 for the perceived discrimination and perceived rejection, respectively. The split-half reliability of the scale was 0.803, and the test-retest reliability was 0.920. ConclusionThe Chinese version of CIRDS has excellent reliability and validity, which can be used to evaluate the perceived discrimination in maintenance hemodialysis patients.
2.Design and practice of critical care humanistic nursing curriculum under the perspective of"great ideological and political education"

Objective To construct a critical care humanistic nursing curriculum under the perspective"great ideological and political education",and evaluate its effects in the training of intensive care unit(ICU)standardized training nurses.Methods A research team was established and organized by department of nursing of Zhongshan Hospital,Fudan University,which consisting of 6 critical care nursing managers and specialists,including 1 chief nurse,3 head nurses,1 master's graduate,and 1 doctoral graduate.The team was responsible for literature review,questionnaire design,expert recruitment,survey distribution and collection,data integration,and analysis.A preliminary questionnaire was developed through literature reviews on critical care humanistic nursing,brainstorming sessions,and clinical surveys.A two-round Delphi method was conducted to finalize the humanistic nursing training curriculum.Ideological and political elements,such as political identity,patriotism,moral cultivation,legal awareness,scientific spirit and professional quality were integrated into the program.From July 2023 to June 2024,newly enrolled ICU standardized training nurses at Zhongshan Hospital,Fudan University participated in the training.Training outcomes were assessed via theoretical examinations and pre-and post-training evaluations of humanistic competency.Results A total of 10 experts participated in the consultation,resulting in a training course consisting of two primaryl indicators and 11 secondary indicators.A total of 60 nurses participated in the training,achieving an average post-training theoretical score of 84.92±7.73.Significant improvements were observed across all dimensions of humanistic competency post-training compared to pre-training(humanistic care practice competency:33.95±4.50 vs.30.82±4.10,communication skills:20.27±2.68 vs.18.98±2.98,self-management:10.07±1.91 vs.9.20±2.02,ethical and legal competence:10.38±1.61 vs.9.68±2.02,psychological adaptability:14.20±2.31 vs.13.03±2.19,and total humanistic practice competency score:88.87±7.65 vs.81.71±8.55),with all differences statistically significant(all P<0.05).Conclusion The critical care humanistic nursing curriculum under the perspective of"great ideological and political education"can effectively improve the humanistic quality of nurses and enhance their ICU practical skills,so as to provide reference for the training of nurses in intensive care.
3.Comparative efficacy of early versus delayed reconstruction of anterior cruciate ligament ruptures under arthroscopy

Objective:To compare the efficacy of early versus delayed reconstruction of anterior cruciate ligament (ACL) ruptures under arthroscopy.Methods:A retrospective cohort study was conducted to analyze the clinical data of 260 young adults with ACL ruptures admitted to 940th Hospital of Joint Logistic Support Force of PLA from January 2022 to June 2024, including 171 males and 89 females, aged 18-45 years [(25.6±5.9)years]. Left knee was involved in 127 patients, while the right in 133 patients. All the patients underwent arthroscopic ACL reconstruction with autologous tendon grafts, of whom 130 patients were treated within 3 months after injury (early reconstruction group) but other 130 treated at 3 months after injury (delayed reconstruction group). The operative duration and intraoperative bleeding were compared between the two groups. The incidence of medial meniscus (MM) and lateral meniscus (LM) tears and the incidence of corresponding types of tears were recorded intraoperatively in the two groups. Tegner score and Lysholm score preoperatively, at 3 months postoperatively, and at the last follow-up were detected. Visual analogue scale (VAS) scores at 3 months postoperatively and at the last follow-up and postoperative complication rate were compared between the two groups.Results:All the patients were followed up for 6-15 months [(9.1±3.2)months]. There were no significant differences in operative duration or intraoperative bleeding between the two groups ( P>0.05). The incidence of MM tears in the early reconstruction group was 22.3% (29/130), lower than 34.6% (45/130) in the delayed reconstruction group ( P<0.05). There was no significant difference in the incidence of LM tears between the two groups ( P>0.05). The incidence of MM bucket-handle tears in the early reconstruction group was 2.3% (3/130), lower than 9.2% (12/130) in the delayed reconstruction group ( P<0.05), while no significant difference in the incidence of other types of meniscus tears was found between the two groups ( P>0.05). There were no significant differences in Tegner score or Lysholm score preoperatively between the two groups ( P>0.05). At 3 months postoperatively and at the last follow-up, the Tegner scores in the early reconstruction group were (7.4±1.3)points and (8.6±0.7)points, higher than (6.4±1.5)points and (7.9±0.6)points in the delayed reconstruction group and the Lysholm scores were (82.1±7.1)points and (90.7±3.8)points in the early reconstruction group, higher than (79.5±6.8)points and (86.3±4.0)points in the delayed reconstruction group ( P<0.01). There were no significant differences in VAS scores between the two groups at 3 months postoperatively or at the last follow-up ( P>0.05). The postoperative complication rate was 8.5% (11/130) in the early reconstruction group and 12.3% (16/130) in the delayed reconstruction group ( P>0.05). Conclusion:For young patients with ACL rupture, arthroscopic reconstruction within 3 months after injury can reduce the incidence of MM tear and bucket-handle tear and improve knee function without increasing the incidence of other complications when compared with delayed reconstruction at 3 months after injury.
4.Effect of hip abductor muscle exercise on three-dimensional gait and collapse rate in patients with non-traumatic femoral head necrosis

BACKGROUND:Non-traumatic necrosis of the femoral head is a difficult joint disease,and preserving one's own femoral head is of great significance for young patients.Currently,there is a lack of regular and unified functional exercise plans.OBJECTIVE:To investigate the effect of hip abductor muscle exercise on the three-dimensional gait and collapse rate of patients with non-traumatic femoral head necrosis,and to provide theoretical basis for effective rehabilitation of patients with non-traumatic femoral head necrosis.METHODS:Totally 81 non-traumatic and non-surgical patients with femoral head necrosis(81 hips)admitted to the Third Affiliated Hospital of Guangzhou University of Chinese Medicine from June 2020 to June 2022 were included.Patients were randomly divided into a control group(n=40)and a hip abductor muscle exercise group(n=41).The control group received routine physical therapy and medication treatment.The hip abductor muscle exercise group underwent hip abductor muscle exercise on the basis of the control group.The gait status(hip joint range of motion,step length difference,and ground contact time difference)of two groups of patients was evaluated using the Tecnobody balance assessment system before and 12 weeks after treatment.The BIODEX-S4 isokinetic muscle strength testing system was used to test the peak force distance of hip abduction isokinetic muscle strength.Gluteus medius width ratio was compared between two groups.The collapse rate was compared between two groups of patients 1 year after treatment.RESULTS AND CONCLUSION:(1)There was no statistically significant difference between the hip abductor muscle exercise group and the control group in terms of age,gender,side profile,body mass index,and etiology(P>0.05).(2)Before treatment,there was no significant statistical difference in the range of motion of the affected hip joint between the two groups(P>0.05).After treatment,the range of motion of the affected hip joint in both groups improved compared to before treatment(P<0.05),and the range of motion of the affected hip joint in the hip abductor muscle exercise group was significantly higher than that in the control group(P<0.05).(3)Before treatment,there was no significant statistical difference in the step length and touchdown time between the two groups(P>0.05).12 weeks after treatment,the difference in step length and touchdown time between the two groups decreased compared to before treatment(P<0.05),and the difference in step length and touchdown time between the hip abductor muscle exercise group after treatment was significantly greater than that of the control group(P<0.05).(4)Before treatment,there was no significant statistical difference in gluteus medius width ratio between the two groups(P>0.05).12 weeks after treatment,the gluteus medius width ratio of both groups increased compared to that before treatment(P<0.05),and the gluteus medius width ratio of the hip abductor muscle exercise group was significantly higher than that of the control group(P<0.05).(5)At 1 year after treatment,the comparison of femoral head collapse rates between the two groups showed significant statistical significance(P<0.05),and that in the hip abductor muscle exercise group(22%)was significantly lower than that in the control group(45%).(6)It is concluded that exercise of the hip abductor muscle effectively enhances hip joint function,improves gait,and reduces the rate of femoral head collapse in patients with non-traumatic femoral head necrosis.It is recommended to use hip abductor muscle exercise as one of the basic methods for hip preservation in non-traumatic femoral head necrosis.
5.Predictive Value of miRNAs Markers for Advanced Lung Squamous Cell Carcinoma.

BACKGROUND:
Lung cancer is one of the leading causes of cancer-related mortality worldwide, with above 80% of cases be non-small cell lung cancer (NSCLC), among which lung squamous cell carcinoma (LUSC) occupies a significant proportion. Although comprehensive cancer therapies have considerably improved the overall survival of patients, patients with advanced LUSC have a poorer prognosis. Therefore, there is a need for a biomarker to predict the progress of advanced LUSC in order to improve prognosis through early diagnosis. Previous studies have shown that miRNAs are differentially expressed in lung cancer tissues and play roles as potential oncogenes or tumor suppressors. The aim of this study is to identify differentially expressed miRNAs between early-stage and advanced-stage LUSC, and to establish a set of miRNAs that can predict the progress of advanced LUSC.
METHODS:
Clinical data and miRNA-related data of LUSC patients were downloaded from The Cancer Genome Atlas (TCGA) database. Bioinformatic methods were applied to analyze the data. Receiver operating characteristic (ROC) curves were plotted, and various online tools were used to predict target genes, with subsequent analysis of the potential biological mechanisms of these genes.
RESULTS:
A total of 58 differentially expressed miRNAs were identified between the experiment group and the control group. Seven miRNAs were selected for potential construction of a miRNA biomarker through LASSO regression, and based on the area under the curve (AUC) values of each miRNA, four of these miRNAs (miR-377-3p, miR-4779, miR-6803-5p, miR-3960) were ultimately chosen as biomarkers for predicting advanced LUSC. The AUC under the ROC curve for the combined four miRNAs was 0.865. Enrichment analysis showed that these target genes were involved in several pathways, including cancer-related pathways, mitogen-activated protein kinase (MAPK) signaling pathway, serine/threonine kinase, and tyrosine kinase signaling pathways.
CONCLUSIONS
The combined use of miR-377-3p, miR-4779, miR-6803-5p and miR-3960 provides a good predictive ability for the progress of advanced LUSC patients, with an AUC of 0.865.

6.Transabdominal bowel ultrasound for monitoring efficacy of vedolizumab treatment in patients with moderate-to-severe ulcerative colitis

Objective To observe the value of transabdominal bowel ultrasound for monitoring therapeutic efficacy of vedolizumab in patients with moderate-to-severe ulcerative colitis.Methods Totally 47 ulcerative colitis patients with Mayo endoscopic score(MES)≥2 and treated with vedolizumab were retrospectively included.Transabdominal bowel ultrasound examinations were performed at baseline,at the end of induction therapy,also 3 and 6 months after maintenance therapy,while colonoscopy was performed at baseline and 6 months after maintenance therapy.According to colonoscopy results 6 months after maintenance therapy,the patients were divided into improved group(MES≤1 or MES reduction≥1,n=25)and non-improved group(n=22),and ultrasonic findings of sigmoid colon were compared between and within groups.Results At baseline,no significant difference of ultrasonic findings of sigmoid colon was observed between groups(all P>0.05),whereas differences of bowel wall thickness at the end of induction therapy,of Limberg grade and abnormal perienteric lymph nodes 3 months after maintenance therapy,as well as bowel wall stratification 6 months after maintenance therapy were noticed between groups(all P<0.05).Within improved group,compared with those in baseline,bowel wall thickness improved at all time points after the beginning of treatment,Limberg grade improved 3 and 6 months after maintenance therapy,while bowel wall stratification,abnormal perienteric lymph node and perienteric fat edema improved 6 months after maintenance therapy(all P<0.05).Meanwhile,no significant change of ultrasonic findings of sigmoid colon was observed in non-improved group during the above periods(all P>0.05).Conclusion Transabdominal bowel ultrasound could be used to accurately monitor the therapeutic efficacy of vedolizumab in patients with moderate-to-severe ulcerative colitis.
7.Impact of internet-based patient decision aid on prenatal screening and diagnostic decision-making: a Meta-analysis

Objective:To systematically evaluate the impact of internet-based patient decision aid (PtDA) on decision-making for prenatal screening and diagnosis among pregnant women.Methods:Randomized controlled trials (RCTs) on the application of internet-based PtDA for prenatal screening and diagnosis among pregnant women were searched in PubMed, Embase, Web of Science, CINAHL, Cochrane Library, China National Knowledge Infrastructure, Wanfang Date, VIP, and China Biology Medicine disc. The search period was from inception to August 11, 2024. Two researchers independently screened the literature, extracted data, and assessed the quality of included studies. Meta-analysis was performed using RevMan 5.4 software.Results:A total of 10 studies involving 3 838 pregnant women were included. The meta-analysis showed that internet-based PtDA significantly reduced decision conflict ( P<0.05), improved decision knowledge ( P<0.01), and decreased decision anxiety ( P<0.05) among pregnant women but had no significant impact on decision regret ( P>0.05) . Conclusions:Internet-based PtDA effectively reduce decision conflict and anxiety while improving decision knowledge during prenatal screening and diagnostic decision-making. Future studies should focus on developing culturally appropriate decision aids tailored to the Chinese context and explore the enhanced role of nurses in prenatal screening counseling.
8.Meta-synthesis of qualitative studies on the reasons for delayed medical consultation in cervical cancer patients

Objective:To systematically analyze the reasons for delayed medical consultation in cervical cancer patients, providing a reference basis for promoting timely medical consultation and early treatment for cervical cancer patients.Methods:A computer-based search was conducted across various databases, including China National Knowledge Infrastructure, Wanfang Data, VIP, China Biology Medicine disc, PubMed, Web of Science, Embase, CINAHL, and Cochrane Library, for qualitative studies on delayed medical consultation in cervical cancer patients. The search covered all available records up to June 30, 2024. The Joanna Briggs Institute centre for evidence-based health care qualitative research quality assessment criteria was used to assess the quality of the included studies, and an integrative synthesis method was applied to combine the findings.Results:A total of 11 studies were included, with 36 individual findings. These findings were grouped into 13 new categories. A total of three major synthesized results were identified: insufficient awareness of cervical cancer, barriers to medical consultation behavior, and the influence of social factors.Conclusions:Strengthening the promotion of cervical cancer health knowledge, providing psychosocial support for patients, and establishing robust social support systems are essential strategies to facilitate timely medical consultation and early treatment for cervical cancer patients.
9.Experiences of patients with premature ovarian insufficiency: a Meta-synthesis of qualitative research

Objective:To systematically evaluate and integrate qualitative research on the experiences of patients with premature ovarian insufficiency (POI) , so as to provide a basis for developing targeted interventions.Methods:The qualitative research on the experiences of patients with POI was electronically searched in Wanfang Data, VIP, China National Knowledge Infrastructure, China Biology Medicine disc, PubMed, Cochrane Library, Embase, CINAHL, and Web of Science. The search period was from database establishment to July 19, 2024. The included articles were evaluated with the quality evaluation criteria for qualitative research of the Joanna Briggs Institute Evidence-Based Health Care Center. Aggregative synthesis was used to integrate the literature.Results:A total of 11 articles were included. The 38 themes were distilled and summarized into 12 new categories, culminating in four integrated outcomes of unfamiliar bodies, psychological shock and calming, the double-edged sword of family and society, and hormone therapy.Conclusions:Patients with POI face multiple pressures from illness, psychology, family and society. Improving the experience of patients with POI requires a concerted effort by healthcare professionals, families, and even society.
