1.Construction of management index system for rational drug use of key monitoring drugs

OBJECTIVE To establish management index system for rational drug use of key monitoring drugs, and provide reference for the management of key monitoring drugs in the hospitals. METHODS First, the management index system for rational drug use of key monitoring drugs was drafted by collecting the evidence from related medical literature. Next, using a modified Delphi method, twenty experienced experts from the fields of pharmacy, medical practice, healthcare insurance, and finance were selected to participate in two rounds of questionnaire consultations. Based on the expert enthusiasm coefficient, authority coefficient, degree of opinion concentration, and degree of coordination, the final indicators were determined to establish a management index system for rational drug use of key monitored drugs in medical institutions. RESULTS The expert enthusiasm coefficients reached 100% in both rounds of consultation. In first-level, second-level and third-level indicators, the authority coefficients of experts were 0.89, 0.86 and 0.87, and coordination coefficients of the experts in importance score were 0.300 (P< 0.05), 0.125 (P<0.05) and 0.139 (P<0.05), respectively. The average score for the importance of all indicators reached over 3.5, in which the full score ratio ranged from 35% to 100%. Except that the variation coefficient of a third-level indicator “number of specifications purchased for key monitored drugs” was 0.26, the variation coefficients of rest indicators were less than or equal to 0.25. Based on the results of expert consultation, final version of the management index system established in this study, including two first-level indicators (drug procurement and use, and rational drug use), five second-level indicators (such as the accessibility, cost-effectiveness) and twenty third-level indicators (such as the number of specifications purchased for key monitored drugs, the increase in the cost of key monitored drugs). CONCLUSIONS The management index system established in this study possesses high reliability and strong operability, and may provide a reference for the management of key monitoring drugs in the hospitals.
2.Oxoglutarate dehydrogenase-L regulates mitochondrial metabolism reprogramming of renal tubular epithelial cells to alleviate renal fibrosis

Objective To investigate the role and underlying mechanisms of oxoglutarate dehydrogenase-L(OGDHL)in renal fibrosis.Methods Twelve male wild-type C57BL/6J mice(8 weeks old,weighting about 25 g)were randomly divided into control group and model group(n=6).A mouse model of renal fibrosis was established by unilateral ureteral obstruction(UUO).HE staining,immunofluorescence assay,and immunohistochemical and Masson staining were applied to assess the extent of renal fibrosis.Human kidney-2 proximal tubule cell line(HK-2)were randomly divided into blank plasmid group(Vector),OGDHL overexpression group(OGDHL OE),blank plasmid+TGF-β1 treatment group(TGF-β1+Vector),TGF-β1 treatment and OGDHL overexpression group(TGF-β1+OGDHL OE).Transmission electron microscopy(TEM)was employed to observe mitochondrial morphology.RT-qPCR,Western blotting and immunofluorescence assay were used to detect the expression levels of OGDHL and mitochondrial metabolism-related genes.Adenosine triphosphate(ATP)level,oxygen consumption rate(OCR),extracellular acidification rate(ECAR)and other mitochondrial metabolism-related indicators were detected with corresponding reagent kits and Agilent Seahorse.Results UUO operation resulted in obvious thinning of the renal cortex,significant hydronephrosis.HE staining revealed that tubular atrophy accompanied by dilated tubular lumens in the kidneys.Immunofluorescence,immunohistochemical and Masson staining showed obvious renal fibrosis after UUO operation.In the mouse model of renal fibrosis,the expression of OGDHL was decreased by 82.9%at mRNA level(P<0.001)and 81.9%at protein level(P<0.001),the production of mitochondrial ATP was reduced by 0.970±0.151 μmol/L(P<0.01),and notable damaged mitochondrial structure was observed,with maximum breathing capacity decreased by 25.260±1.920 pmol/min(P<0.001),expression levels of fatty acid oxidation-related genes were declined by 50.2%(P<0.05),while glycolysis was enhanced,with the genes related to glycolysis increased by 2.5 times when compared with those in the control group(P<0.05).After OGDHL overexpression,mitochondrial morphological damage was significantly improved,ATP production was increased by 0.980±0.090 μmol/L(P<0.05),with significantly alleviated energy metabolism disorder and renal fibrosis(P<0.05).Conclusion OGDHL regulates mitochondrial metabolic reprogramming of renal tubular epithelial cells to alleviate fibrosis,suggesting that OGDHL may be a new therapeutic target for renal fibrosis.

Performance appraisal of public hospitals have given a guidance for the development of public hospitals at all levels.A Class A tertiary hospital reviewed the problems in the development of the hospital at the present stage and focused on the following four aspects:①insufficient fine management;②No clear orientation of discipline development;③The bottleneck of the improvement of medical operation efficiency;④New challenges in the reform of payment mode.The tertiary hospital launched a fine management practice in May 2022,in order to solve the problems by taking the Department of Surgery as a pilot area,laying the foundation for fine management through information system construction,improving the efficiency of medical operation through management process optimization,improving the overall competitiveness of disciplines through the construction of sub-specialty and Discipline Alliance and adjusting the performance appraisal index system to play the role of performance incentives.The measures effectively improve the overall capacity and efficiency of hospital medical services and help the hospital to achieve high-quality development.
5.Discussion on multiple primary malignancy ICD-10 coding

Objective To analyze the coding quality of multiple primary malignant tumors in a tertiary hospital,identify existing problems,and improve the accuracy of coding for multiple primary malignant tumors.Methods We collected 393 medi-cal records from a tertiary hospital from June 1,2021,to June 30,2023,with ICD-10 code C97 for the discharge diagnosis.By carefully reading the medical records,we analyzed the reasons for coding errors in multiple primary malignant tumors with code C97.Results Among the 393 medical records,there were 34 cases with coding errors in C97,including 11 cases where meta-static malignant tumors were erroneously coded as multiple primary malignant tumors,9 cases where malignant tumors of the same organ system with non-adjacent sites and the same histological type were erroneously coded as multiple primary malignant tumors,7 cases where distant metastasis or local invasion was erroneously coded as multiple primary malignant tumors,4 cases where be-nign tumors were mistakenly coded as malignant tumors,and 3 cases where suspected malignant tumors were coded as confirmed cases.Conclusion To improve the quality and efficiency of C97 coding,coders need to have a clear understanding of the defini-tion of multiple primary malignant tumors,read the medical records carefully and completely,determine the location and histolog-ical type of the tumors accurately,and strengthen communication with clinical physicians.
6.Reforming public hospital procurement management model by establishing a procurement department

Objective Procurements are important economic activities in public hospitals.This paper aims to strengthen internal control of procurements and standardize procurement management and to improve operational efficiency and achieve high-quality development.Methods On the basis of analyzing the situation and shortcomings of the existing decentralized procure-ment management model,we utilized the five-aspect theory of"personnel,machinery,material,method,and environment",which is commonly used in enterprise quality management,to design a comprehensive procurement management model by engag-ing the actual needs.Results An independent procurement department was established in the hospital.With the department,procurement management was improved through professional training,separation of procurement and management,and construc-tion of information technology.Conclusion The coordinated procurement management model with an independent procurement de-partment can ensure the legality and compliance of procurement work,and realize planned procurement projects,efficient procure-ment processes,high-quality procurement results,and savings in procurement amounts.This assists hospitals in achieving high-quality development and providing a reference for scientifically strengthening procurement management in peer public hospitals.
7.The establishment and practice of the school-supervisor-postgraduate "three-dimensional interaction" mode of academic norms and ethics education for medical postgraduates

This study established a school-supervisor-postgraduate "three-dimensional interaction" mode of academic norms and ethics education for medical postgraduates based on their training characteristics. The mode was evaluated and improved in practice. The results showed that after the implementation of the mode, graduate students significantly improved their awareness of how to standardize the implementation of experiments [(79.86% vs. 89.47%); P=0.021] and how to write formal experimental records [(84.72% vs. 92.76%); P=0.028]. In addition, there were significant increases in the pass rate of pre defense (85.42% vs. 94.08%; P=0.014) and blind review (84.03% vs. 93.42%; P=0.010). The "three-dimensional interaction" mode truly helps to achieve a win-win-win situation between postgraduate supervisors and the school, and is worth further practice and promotion.
8.Ideological and political teaching design for clinical research curriculum

The integration of ideological and political education into medical courses is an important part of China's medical education reform in the new era.The construction of ideological and political education in clinical research curriculum is of great significance for the cultivation of clinical research talents with both ability and political integrity.Based on the clinical research curriculum offered by the Second Clinical College of Wuhan University,this paper introduced the necessity of ideological and political curriculum in clinical research curriculum.Through the learning situation analysis of the eight-year program students majoring in clinical medicine,the ideological and political teaching objectives were formulated,and the ideological and political elements and cases were selected in combination with the teaching content of clinical research curriculum.Combined with the characteristics of the medical professional methodology course,the diversified,innovative student-centered teaching methods were designed integrating the theoretical teaching,flipped classroom,scenario simulation teaching,problem-based learning,case-based learning and team based learning,and added the consideration of moral education dimension in the teaching evaluation,to carry out the ideological and political teaching design of clinical research curriculum.These measures promoted the organic integration of knowledge teaching,ability training and value building,and provided reference for the ideological and political education in clinical research curriculum.

Objective To monitor the susceptibility of clinical isolates to antimicrobial agents in tertiary hospitals in major regions of China in 2022.Methods Clinical isolates from 58 hospitals in China were tested for antimicrobial susceptibility using a unified protocol based on disc diffusion method or automated testing systems.Results were interpreted using the 2022 Clinical &Laboratory Standards Institute(CLSI)breakpoints.Results A total of 318 013 clinical isolates were collected from January 1,2022 to December 31,2022,of which 29.5%were gram-positive and 70.5%were gram-negative.The prevalence of methicillin-resistant strains in Staphylococcus aureus,Staphylococcus epidermidis and other coagulase-negative Staphylococcus species(excluding Staphylococcus pseudintermedius and Staphylococcus schleiferi)was 28.3%,76.7%and 77.9%,respectively.Overall,94.0%of MRSA strains were susceptible to trimethoprim-sulfamethoxazole and 90.8%of MRSE strains were susceptible to rifampicin.No vancomycin-resistant strains were found.Enterococcus faecalis showed significantly lower resistance rates to most antimicrobial agents tested than Enterococcus faecium.A few vancomycin-resistant strains were identified in both E.faecalis and E.faecium.The prevalence of penicillin-susceptible Streptococcus pneumoniae was 94.2%in the isolates from children and 95.7%in the isolates from adults.The resistance rate to carbapenems was lower than 13.1%in most Enterobacterales species except for Klebsiella,21.7%-23.1%of which were resistant to carbapenems.Most Enterobacterales isolates were highly susceptible to tigecycline,colistin and polymyxin B,with resistance rates ranging from 0.1%to 13.3%.The prevalence of meropenem-resistant strains decreased from 23.5%in 2019 to 18.0%in 2022 in Pseudomonas aeruginosa,and decreased from 79.0%in 2019 to 72.5%in 2022 in Acinetobacter baumannii.Conclusions The resistance of clinical isolates to the commonly used antimicrobial agents is still increasing in tertiary hospitals.However,the prevalence of important carbapenem-resistant organisms such as carbapenem-resistant K.pneumoniae,P.aeruginosa,and A.baumannii showed a downward trend in recent years.This finding suggests that the strategy of combining antimicrobial resistance surveillance with multidisciplinary concerted action works well in curbing the spread of resistant bacteria.

Objective To investigate the effects of microgravity environment on hibernating myoblasts.Methods Hibernating myoblasts were cultured under real and simulated microgravity conditions for 10 days.RNA-seq analysis and immunofluorescence are used to analysis the impact of microgravity environment on cell growth and gene expression of myoblasts.Results Under the microgravity conditions,genes associated with proliferation were upregulated.Under simulated microgravity,there were more and higher proportion of Ki67 positive cells compared to normal gravity conditions.Conclusion The microgravity environment promotes the proliferation of hibernating myoblasts.
