1.Effects of cochlear implantation on vestibular function in patients with large vestibular aqueduct syndrome.

Objective:This study aimed to compare the effects of cochlear implantation(CI) on vestibular function in patients with large vestibular aqueduct syndrome(LVAS) and in patients with extremely severe deafness with normal inner ear structure. Methods:A total of 28 LVAS patients and 28 patients with normal inner ear structure who suffered from extremely severe deafness were selected. The parameters of caloric tests, bone conduction evoked cervical vestibular-evoked myogenic potentials(cVEMP), bone conduction evoked ocular vestibular-evoked myogenic potentials(oVEMP) and video head impulse tests(v-HIT) were compared between the two groups before and after CI. The data were analyzed using SPSS 26.0 software. Results:There was no significant difference in the results of the preoperative caloric test, v-HIT, and oVEMP(P1, N1) between the LVAS group and the control group(P>0.05). Compared to the control group, the LVAS group exhibited a shorter cVEMP P1[(13.41±0.71)ms vs (16.28±0.89)ms, P<0.000 1], shorter N1[(19.83±0.54)ms vs (28.18±1.56)ms, P<0.000 1], higher amplitude[(123.60±83.80)μV vs (73.92±79.85)μV, P=0.049 4] and higher oVEMP amplitude[(16.60±13.87)μV vs (9.96±10.47)μV, P=0.028 5] before CI. The abnormal rate of caloric test increased in both groups after CI(25.00% vs 57.14%, P=0.028 8, 32.14% vs 82.14%, P=0.000 3, respectively). There was no significant difference in the v-HIT parameters in both groups before and after the operation. As for the LVAS group, there was no statistically significant difference in cVEMP and oVEMP induction rates before and after operation. In the control group, there was a decrease in cVEMP induction rate(96.42% vs 64.28%, P=0.005 2) and oVEMP induction rate(96.42% vs 57.14%, P=0.000 9) after CI. LVAS group showed a shorter cVEMP P1[(13.41±0.71)ms vs (10.30±0.60)ms, P<0.000 1]; shorter cVEMP N1[(19.86±0.53)ms vs (18.97±1.33)ms, P=0.004 7]; decreased amplitude[(124.50±84.86)μV vs (64.35±61.57)μV, P=0.001 0] and shorter oVEMP amplitude[(15.92±13.03)μV vs (9.16±9.20)μV, P=0.009 9] after CI. The oVEMP N1 in the control group was longer than that before operation[(11.73 ± 0.91)ms vs (13.35 ± 2.60)ms, P=0.019 6], whereas there was no significant difference in other VEMP parameters after CI. Conclusion:Before CI, there was no significant difference in the results of the caloric test and v-HIT between the LVAS group and the control group, but the LVAS group exhibited increased sensitivity to acoustic stimulation-induced myogenic potentials. After CI, the function of the semicircular canal was impaired in both groups in the low-frequency area, and remained largely unaffected in the high-frequency area. Additionally, the function of the otolith in the LVAS group was less affected than that in the control group after CI, which may be related to the fact that the enlarged vestibular aqueduct of the LVAS patients acted as the third window of the inner ear.

2.MRI combined with serum carbohydrate antigen 125 and human epididymis protein 4 for differential diagnosis of type Ⅰ and Ⅱ epithelial ovarian cancers

Objective To observe the value of MRI combined with serum carbohydrate antigen 125(CA125)and human epididymis protein 4(HE4)for differential diagnosis of type Ⅰ and Ⅱ epithelial ovarian cancers(EOC).Methods Totally 87 EOC patients were retrospectively enrolled.According to pathology,35 cases of type Ⅰ EOC were taken as type Ⅰ group,while 52 cases of type Ⅱ EOC were taken as type Ⅱ group.Conventional MRI manifestations and apparent diffusion coefficient(ADC)value of lesions,as well as CA125 and HE4 were compared between groups,and their efficacy for differential diagnosis of type Ⅰ and Ⅱ EOC were analyzed.Results Significant differences of conventional MRI manifestations of lesions,including composition,mural nodules,peritoneal diffusion and lymph node metastasis,of ADC value of lesions,also of patients'CA125 and HE4 were found between groups(all P<0.05).The area under the curve(AUC)of conventional MRI manifestations and ADC value of lesions,patients'CA125 and HE4 for distinguishing typeⅠ and type Ⅱ EOC was 0.694,0.730,0.670 and 0.708,respectively,while of the combination of the above four was 0.865,higher than that of each one alone(Z=3.008,2.138,3.005,2.746,all P<0.05).Conclusion MRI combined with CA125 and HE4 was helpful for differential diagnosis of type Ⅰ and Ⅱ EOC.
3.Research on the application of non-contact physiological and psychological detection in the analysis of long-term simulated weightlessness effects

Objective Explore a non-contact physiological and psychological detection model based on facial video in simulations of weightlessness effects,research new methods for non-contact heart rate and negative mood state detection in long-term simulations of weightlessness effect analysis.Methods Construct a non-contact physiological and psychological data collection system for fusion analysis of visible light and thermal infrared videos.Collect physiological and psychological data of volunteers in the"Earth Star-Ⅱ"90-day head-down bed rest experiment.A non-contact heart rate detection model based on GCN facial multi-region feature fusion and a non-contact negative mood state detection model considering data reliability were constructed,and the effectiveness of the models were validated with finger clip heart rate and POMS-SF scale as labels.Results The experimental results show that the average difference in the Bland-Altman plot of the non-contact heart rate detection model is-1.26 bpm,and 96.3%of value error detection data falls within the 95%confidence interval,indicating a high consistency between the model detected heart rate and the finger clip heart rate.The non-contact negative mood state detection model achieves an accuracy of>0.85 for detecting tension,depression,anger,and fatigue.Features such as heart rate,AU06,eye gaze,and head pose were observed to be important to mood state detection.Conclusion Non-contact physiological and psychological detection methods not only can be utilized for long-term physiological analysis in simulations of weightlessness effects,but also provide a novel technical approach for on-orbit astronauts health assurance during long-term space flight in the future.
4.Accelerate the Transformation of Oncology Pharmacists’ Services and Promote the Development of Oncology Pharmacy

The column named “Oncology Pharmacy Management and Services” is introduced, which discussed in depth from the two parts of “frontier progress of anti-tumor drugs” and “comprehensive clinical evaluation of anti-tumor drugs”, aiming to provide new ideas for the exploration of oncology pharmaceutical services, so as to further accelerate the transformation of oncology pharmaceutical care and promote the high-quality development of oncology pharmacy.
5.Clinical features and management of primary malignant renal perivascular epithelioid cell tumor

Objective To summary the pathological and clinical characteristics,treatments and prognosis of malignant renal perivascular epithelioid cell tumor (PEComa).Methods Between August 2013 and June 2017,8 cases of malignant renal PEComa were analysed respectively.Of all the patients,there were 4 males and 4 females,aged 27-65 years with the average of 46.3 years old.Three cases were detected in routine examination occasionally,three cases complained of intermittent back pain with fever,one presented with swollen and painful right hand and right foot,and one case presented with hematuria.CT or MRI examinations indicated the malignant lesions before the surgery.Clinical staging was performed with 2 cases of T1N0M0,1 case of T1 N0M1,2 cases of T2N0M0,1 case of T3M0M0,and 2 cases of T4N0M1.Three cases underwent radical nephrectomy,1 case underwent radical nephrectomy plus renal artery embolization,2 cases underwent partial nephrectomy,and 2 cases underwent nephrectomy plus inferior vena cava tumor thrombectomy.One case was treated with ifosfamide plus epirubicin after operation due to multiple distant metastases and 1 case was treated with oxaliplatin before the sugery because of excessive tumor diameter.None of the rest received any adjuvant therapy.Results Postoperative histopathological examination showed multiple nodules in reddish gray and yellow color,with soft texture and partial incomplete capsule.Microscopically,there were large atypical cell components,some of which were spindle shaped,with disordered cell arrangement,some of which were associated with a large amount of necrotic tissue,and abundant light eosinophil cytoplasm.Tissue components can be found thick-wall blood vessels,smooth muscle-like cells,and adipose tissue.Immunohistochemistry showed 4 cases were positive of HMB-45,4 of Melan-A,7 of SAM.Seven cases were negative of CK,6 of S-100 75% (6/8) and Ki67 (10%-70%).Two cases died of multiple metastases (lung,bone,liver),1 case survived with tumor recurrence,with a follow-up from 14 to 60 months (mean 29 months).Conclusions Primary malignant renal PEComa is rare in practice with relative large diameter.The diagnosis depends on pathological findings,and radical nephrectomy is the first choice of treatment.It is easy to recur and metastasize after the operation.
6.Primary melanoma of the urinary system: a report of 5 cases and review of the literature

Objective To discuss the clinical diagnosis,treatment and prognosis of malignant melanoma of urinary system.Methods The clinical data of 5 patients with primary malignant melanoma of urinary system were retrospectively analyzed.There were 2 cases of primary melanona of the urethra,3 cases of primary malignant melanoma of the bladder.The diameter of the tumor ranged from 0.9 to 5.1 cm with an average of 3.1 cm.Results Two cases of urethral patients underwent radical resection of urethra.Among 3 cases of bladder,1 cases were in poor condition,and underwent laparoscopic partial cystectomy.In 1 young men,radical resection was refused and only transurethral resection of the bladder tumor was performed.Radical resection of bladder was done in 1 cases.Postoperative pathology showed that the tumor cells of 4 cases were fusiform under microscope,1 case was polygonal.5 cases showed melanin in the cytoplasm and diffuse proliferation of tumor cells,with obvious heterogeneity,cell proliferation index Ki-67 10%-30%.During the follow-up period of 7-30 months (median 19 months),3 patients died of metastasis.Conclusions Malignant melanona of urinary system is rare,with high malignancy and poor prognosis.Targeted therapy and immune therapy has become a new treatment option,which could improve the prognosis of patients.
7.Utilization Investigation of EGFR-TKI in the Patients with Lung Cancer in 11 Hospitals of Zhejiang Province during 2009 and 2015

Objective: To investigate the utilization status of epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI) in 11 hospitals of Zhejiang province from 2009 to 2015, and to analyze the use rationality.Methods: The doctor's advice in 40 days annually was collected in 11 hospitals of Zhejiang province from 2009 to 2015, and the drug consumption, frequency of utilization (DDDs), defined daily cost (DDC) and drug utilization index (DUI) were analyzed for the patients with lung cancer treated with EGFR-TKI.Results: Icotinib, erlotinib and gefitinib were the three prevalent EGFR-TKIs used in Zhejiang province, and icotinib started to be used in clinics in 2013.The overall cost of EGFR-TKIs increased year by year during 2009 and 2015, and the total amount of sales increased by 4.67 times in 2015 when compared with that in 2009.Generally, the DDDs value of erlotinib showed a decreasing trend, however, that of icotinib and gefitinib rose year by year during 2009 and 2015.Erlotinib had the highest DDC followed by gefitinib and icotinib.The mean value of DUI of the three targeted drugs was about 1.Conclusion: The utilization of EGFR-TKI is reasonable in 11 hospitals of Zhejiang province with increasing comsuption.
8.Study of effects of micheliolide on ovarian cancer cell lines and its mechanism

Objective To study the roles of micheliolide on ovarian cancer cells. Methods Firstly, human ovarian cancer cell lines HeyA8, SKOV3 and A2780/DDP were treated with different concentration of micheliolide (0.25, 0.5, 1, 2.5, 5, 10, 20, 50 μmol/L) for 72 hours, then methyl thiazolyl tetrazolium (MTT) assay was used wo detect the growth of the human ovarian cancer cell lines and the stongest inhibited cell line were selected for the following test. Secondly, after HeyA8 cell line was treated with different concentration (5, 10, 20μmol/L) of micheliolide for 24 hours, the HeyA8 cell apoptosis was measured byflow cytometry. Thirdly, the expression of RelA mRNA in HeyA8 cell was detected through real-time PCR, the expressions of nuclear factor κB(NF-κB)signal pathway related protein RelA and the activited cysteinyl aspartate specific proteinase (caspase-9) were detected by western blot analysis. Results (1) The growth of HeyA8, SKOV3 and A2780/DDP cells were all significantly inhibited after being treated with different concentration of micheliolide for 72 hours and the roles of inhibition were all concentration dependant (P<0.05). The half maximal inhibitory concentration (IC50) of HeyA8, SKOV3 and A2780/DDP were (9.8±2.2), (12.0±2.1) and (12.8±1.8)μmol/L, respectively. We chose HeyA8 cell to do the following expreriments because of its best inhibited effect. (2) After HeyA8 cell was treated with micheliolide of different concentrations, as the concentration increased (20 and 0μmol/L, for example), the apoptosis rate of HeyA8 cell raised from (7.2±1.0)%to (17.4±1.1)%, the percentage of survived cells reduce from (92.8 ± 1.3)% to (82.6 ± 1.4)%,and the relative mRNA level of RelA decreased from 1.00 ± 0.13 to 0.18 ± 0.00 (P<0.01); furthermore, the expression of RelA protein was weaken and the activited caspase-9 protein expression was increased gradually. Conclusions Micheliolide plays a significantly inhibited role in HeyA8, SKOV3 and A2780/DDP cells. The inhibited role of micheliolide inovarian cancer cells might through inhibiting nuclear factor-kappa B (NF-кB) signaling pathway, and inducing the expression of activited caspase-9 protein to promoting apoptosis of HeyA8 cell.
9.Application of feedforward control in nursing risk management in Department of Geriatrics

Objective To explore the effect of feedforward control on nursing risk management in Department of Geriatrics. Methods We retrospectively analyzed the nursing risk factors in 842 cases of elderly patients from August 2012 to July 2013. Aiming at the risk factors, the management method of feedforward control was used in 848 elderly patients from August 2013 to July 2014, and we evaluated the objective indexes before and after the feedforward control. Results After the deployment of feedforward control, the incidence of nursing adverse events was significantly reduced from 42 cases to 7 cases (χ2 =26. 296,P<0. 01). Compared with the control group, patients satisfaction scores were remarkably increased from (93. 13 ± 1. 76) score to (98. 25 ± 1. 49) score, (t= -7. 415,P<0. 01). Conclusions The implementation of feedforward control and management helps to prevent and control nursing risk, which is conducive to improving the quality of nursing and is an effective method to guarantee the safety of inpatient.
10.Impacts of personalized nursing intervention on medical compliance of patient with rheumatoid arthritis

Objective To explore the impacts of the personalized nursing interventions on medical compliance of patients with rheumatoid arthritis ( RA) . Methods A total of 60 patients with RA were randomly classified into experimental group ( 30 cases ) and control group ( 30 cases ) . The patients of control group received routine medicine treatment and specialist nursing care. In the experimental group, personal profiles were established and all patients of the experimental group applied personalized nursing interventions based on control group. The rehabilitation status and medical compliance were observed before and after interventions in two groups, and treatment efficacy was evaluated by American College of Rheumatology ( ACR) and compared and analyzed between two groups. Results After 12 weeks nursing interventions, the improvement of ACR20 in the experimental group was better than that of the control group (χ2 =4. 344, P <0. 05 ); early morning stiffness, joint swelling and tenderness of the experimental group ameliorated compared with the control group (t= -4. 524,-8. 283,-5. 842;P<0. 01); the difference of medical compliance was statistical significant between two groups (χ2 =10. 76,P < 0. 05). Conclusions For patients with RA, personalized nursing interventions can effectively improve their medical compliance and clinical efficacy.
