1.Hypoxia-induced USP22 affects the malignant biological behavior of esophageal squamous cell carcinoma by regulating HIF-1α

Objective To investigate the effect of ubiquitin specific peptidase 22 (USP22) on the occurrence and development of esophageal squamous cell carcinoma (ESCC) under hypoxic conditions, and its regulatory relationship with hypoxia-inducible factor-1α (HIF-1α). Methods Western blotting and quantitative polymerase chain reaction were used to detect the differences in USP22 protein and mRNA expression between normal esophageal epithelial cells HEEC and ESCC cell lines KYSE30, KYSE150, EC9706, and TE-1 under normoxic (5% CO2, 20% O2, 75% N2) and hypoxic (5% CO2, 1% O2, 94% N2) conditions. By transfecting USP22 plasmid or siUSP22, ESCC cells were divided into a normoxia control group, a normoxia+USP22 group, a normoxia+siUSP22 group, a hypoxia control group, a hypoxia+USP22 group, and a hypoxia+siUSP22 group. The proliferation and migration abilities of cells in each group were detected. The expression of USP22 and HIF-1α under hypoxic conditions after up-regulating or down-regulating USP22 was detected, and their regulatory relationship was verified. The interaction between USP22 and HIF-1α was verified by co-immunoprecipitation (Co-IP) technique. Results Compared with HEEC cells, the expression of USP22 in ESCC cells was significantly increased (P<0.05). Up-regulation of USP22 expression promoted the proliferation and migration of ESCC cells, while silencing USP22 inhibited the proliferation and migration of ESCC cells (P<0.05). Under hypoxic conditions, the expression of USP22 and HIF-1α increased, and with the up-regulation of USP22 expression, the expression of HIF-1α also significantly increased (P<0.05). Co-IP experiment confirmed the binding between USP22 and HIF-1α. Conclusion Up-regulation of USP22 expression promotes the proliferation and migration of ESCC cells. Hypoxia microenvironment can induce the increase of USP22 expression in ESCC. USP22 may participate in the regulation of the occurrence and development of ESCC by directly binding to HIF-1α.
2.Expression and clinical significance of HBV RNA in chronic hepatitis B patients with low-level viremia

ObjectiveTo investigate the expression characteristics of serum HBV RNA in patients with low-level viremia (LLV) and its value in the diagnosis of LLV. MethodsA total of 402 chronic hepatitis B (CHB) patients who attended Affiliated Hospital of Zunyi Medical University from December 2023 to May 2025 were enrolled, and according to their viral load, they were divided into complete virologic response (CVR) group (190 patients with HBV DNA <20 IU/mL) and LLV group (212 patients with an HBV DNA level of ≥20 IU/mL and <2 000 IU/mL). The two groups were analyzed in terms of age, sex, disease type, serum HBV RNA, HBeAg, alanine aminotransferase (ALT), aspartate aminotransferase (AST), and total bilirubin (TBil), and according to HBeAg status, the patients in the LLV group were further divided into HBeAg-negative group with 140 patients and HBeAg-positive group with 72 patients. The chi-square test was used for comparison of categorical data between two groups, and the Mann-Whitney U test was used for comparison of continuous data between two groups. A multivariate Logistic regression analysis was used to investigate the influencing factors for LLV in CHB patients, and a Spearman’s rank correlation analysis was used to analyze the correlation of HBV RNA with HBV DNA, ALT, AST, and TBil in the LLV group. The receiver operating characteristic (ROC) curve was plotted to evaluate the efficacy of HBV RNA in the diagnosis of LLV. ResultsCompared with the CVR group, the LLV group had a significantly higher serum level of HBV RNA [3 (1 — 5) log10 copies/mL vs 2 (1 — 3) log10 copies/mL, Z=-2.346, P=0.019] and a significantly higher proportion of patients with hepatitis B cirrhosis (31.13% vs 22.11%, χ2=4.155, P=0.042) or hepatocellular carcinoma (9.91% vs 4.74%, χ2=3.876, P=0.049). The multivariate Logistic regression analysis showed that HBV RNA (odds ratio=1.163, 95% confidence interval: 1.058 — 1.278, P=0.002) was an independent risk factor for the onset of LLV in CHB patients. Among the patients with LLV, HBeAg-positive patients had a significantly higher level of HBV RNA than HBeAg-negative patients [6 (4 — 7) log10 copies/mL vs 2 (1 — 3) log10 copies/mL, Z=-9.962, P<0.001]. The correlation analysis showed that HBV RNA level had no significant correlation with HBV DNA, ALT, AST, or TBil in the patients with LLV (all P>0.05). The ROC curve analysis showed that HBV RNA had an AUC of 0.567 for the diagnosis of LLV (P=0.021), with an optimal cut-off value of 4.5 log10 copies/mL, a sensitivity of 30.7%, and a specificity of 85.8%. ConclusionSerum HBV RNA level is an independent risk factor for the development of LLV in CHB patients, and there is a significant increase in the expression of HBV RNA in HBeAg-positive patients. Therefore, it may serve as a potential biomarker for clinical risk assessment.
3.Efficacy of integrated traditional Chinese and Western medicine in diabetic macular edema

Objective To evaluate the efficacy of combining the replenishing qi and nourishing yin empirical formula with anti-vascular endothelial growth factor (VEGF) in diabetic macular edema (DME). Methods A retrospective analysis was conducted on 59 patients diagnosed with DME at Tongji Hospital of Tongji University or Shanghai Nanxiang Hospital, Jiading District from June 2019 to December 2022. Among them, 29 cases received intravitreal injection of ranibizumab (anti-VEGF group), while 30 cases received both intravitreal injection of ranibizumab and oral administration of the replenishing qi and nourishing yin empirical formula (combined treatment group). The best corrected visual acuity (BCVA), central macular thickness (CMT), and the traditional Chinese medicine (TCM) syndrome score were compared between the two groups before treatment and at 4, 8, 12, 16, 20, and 24 weeks after treatment. Results Compared with before treatment, BCVA significantly improved (P<0.05) and CMT significantly decreased (P<0.05) at different time points after treatment in both groups. At 16, 20, and 24 weeks after treatment, BCVA in the combined treatment group was superior to that in the anti-VEGF group (P<0.01). At 12, 16, 20, and 24 weeks after treatment, CMT in the combined treatment group was lower than that in the anti-VEGF group (P<0.01). Starting from week 8 after treatment, the TCM syndrome scores in the combined treatment group were lower than those in the anti-VEGF group (P<0.01). Conclusions The replenishing qi and nourishing yin empirical formula could improve the efficacy of anti-VEGF therapy in DME patients, indicating that integrating traditional Chinese and Western medicine has certain clinical application value in treating DME.
4.Relationship between ambulatory blood pressure changes and health-related behaviors and sleep quality in hypertensive patients

Objective To explore the relationship between ambulatory blood pressure changes and health-related behaviors as well as sleep quality in hypertensive patients. Methods A retrospective study was conducted on 684 patients who underwent 24-hour ambulatory blood pressure monitoring at Chengdu Fifth People's Hospital between January 2023 and April 2025. The cohort included 502 hypertensive patients and 182 non-hypertensive patients. Clinical data from both groups of patients were collected. Their health-related behaviors and sleep quality were investigated. Pearson correlation analysis was conducted to explore the relationship between ambulatory blood pressure changes and health-related behaviors as well as sleep quality in hypertensive patients. Results The differences in dSBP, dDBP, nSBP, nDBP, 24-hour SBP, 24-hour DBP, 24-hour SBP CV, and 24-hour DBP CV levels between the two groups were statistically significant (P<0.05). The scores of the Active Health Behavior Scale (AHBS) in the hypertension group were lower than those in the non-hypertension group, while the scores of the Pittsburgh Sleep Quality Index (PSQI) in the hypertension group were higher than those in the non-hypertension group (P<0.05). AHBS showed a negative correlation with dSBP, dDBP, nSBP, nDBP, 24 h SBP, 24 h DBP, 24 h SBP CV, and 24 h DBP CV (P<0.05), while PSQI showed a positive correlation with dSBP, dDBP, nSBP, nDBP, 24 h SBP, 24 h DBP, 24 h SBP CV, and 24 h DBP CV (P<0.05). Conclusion Health-related behaviors and sleep quality can influence ambulatory blood pressure through various mechanisms, including neural, endocrine, and vascular functions.
5.Expression of HBV RNA in hepatitis B virus-associated hepatocellular carcinoma

Objective To investigate the expression of HBV RNA in hepatitis B virus-associated hepatocel-lular carcinoma(HBV-HCC).Methods A total of 259 patients with chronic hepatitis B(CHB)and 41 HBV-HCC patients treated in the outpatient or inpatient department of the Affiliated Hospital of Zunyi Medical University from December 2023 to January 2025 were enrolled as subjects.The differences in age,sex,and levels of HBV RNA,HBV DNA,ALT,AST,and TBIL between the two groups were compared.Differences in viral load between HBV RNA and HBV DNA within each group were compared.Correlations between HBV RNA and HBV DNA,ALT,AST,and TBIL within each group were analyzed.Diagnostic efficacy of HBV RNA for HBV-HCC was determined using receiver operating characteristic(ROC)curve analysis.Results The HBV-HCC group had significantly higher HBV RNA load,HBV RNA positivity rate,ALT,AST and TBIL levels and proportion of male patients and older age compared to the CHB group(P<0.05).Scatter plot analysis revealed a higher HBV DNA load compared to the HBV RNA load in the CHB group(P<0.000 1).Correlation analysis showed that HBV RNA was positively correlated with HBV DNA in both groups(P<0.05).In the CHB group,HBV RNA was positively correlated with ALT,AST and TBIL(P<0.001).The ROC curve showed that the area under the curve(AUC)for HBV RNA was 0.6923(P<0.000 1),with an optimal cut-off value of≥2.5 log10 copies/mL for diagnosing HBV-HCC.Conclusion Serum HBV RNA levels demonstrate high sensitivity and specificity for identifying HBV-HCC and can serve as a reliable indicator for predicting the occurrence of HBV-HCC.
6.Effects of 12.5% carbohydrate solution and MCT solutions at various concentrations on gastric emptying in healthy volunteers

Objective:To observe the effects of 12.5% carbohydrate (CHO) solution and medium chain triglycerides (MCT) solutions at various concentrations on gastric emptying, in order to determine the optimal MCT-CHO combination for enhanced recovery after surgery.Methods:This study was a prospective study. Ten healthy volunteers were selected to ingest 400 ml of the following 5 solutions every day: water, 12.5% CHO, 1% MCT, 2% MCT and 4% MCT. According to the above results, the optimal concentration of MCT solution was 2% MCT, showing comparable gastric emptying rate compared with 12.5% CHO solution. On this basis, combinations of MCT and CHO solutions were determined to be 30 energy % (EN%) MCT+70 EN% CHO and 20 EN% MCT+80 EN% CHO according to the energy distribution ratio. Then the volunteers were given 400 ml of the following 4 solutions every day: 12.5% CHO, 2% MCT, 30 EN% MCT+70 EN% CHO (equivalent to 2% MCT+8% CHO), and 20 EN% MCT+80 EN% CHO (equivalent to 4.4% MCT+10% CHO). Gastric emptying during fasting (T m) and immediately (T 1), 30 min (T 2), 60 min (T 3), 90 min (T 4) and 120 min after ingestion (T 5) were observed by antral ultrasonography. The degree of thirst, hunger, and anxiety was assessed by Visual Analogue Scale, and the taste was rated. Results:The gastric emptying rate in descending order was 20%EN MCT+80%EN CHO group>30%EN MCT+70%EN CHO group≈2%MCT group≈12.5%CHO group. There was no significant difference in gastric emptying rate between 30 EN% MCT+70 EN% CHO group and 12.5% CHO group ( P>0.05). The scores of thirst, hunger and anxiety in 30 EN% MCT+70 EN% CHO group at T 1 to T 5 were significantly lower than those at T m ( P<0.05). 30 EN% MCT+70 EN% CHO group showed the highest taste score. Conclusion:30 EN% MCT+70 EN% CHO solution has similar gastric emptying rate compared with 12.5% CHO solution. It can relieve thirst, hunger, anxiety and other subjective feelings caused by fasting, with tastes better than 12.5% CHO solution.
7.A novel anatomical model of the lumbar sympathetic nerve for subarachnoid-related studies via a dorsal approach

Objective The study attempted to establish a less invasive model for dissecting and monitoring the sympathetic nerve in rats via a dorsal approach for subarachnoid block-related studies.Methods A traditional abdominal approach model and a new dorsal approach model were established in SD rats,and the modeling time of the two models was observed.The stability of the new model was evaluated by measuring blood pressure(BP),heart rate(HR),percentage change in lumbar sympathetic nerve activity(LSNA change%),norepinephrine(NE),and nitric oxide(NO)content after subarachnoid injection of bupivacaine.Results(1)Building a new model:the time required to create new models for the dorsal approach(DA)group was shorter than that for the traditional abdominal approach(VA)group,as shown by the result(P<0.0001).(2)Evaluation of the new model:compared with the NS group,the MAP and SBP were lower at T2(5 min after injection of bupivacaine into the subarachnoid space)and T3(10 min)(P<0.05);the LSNA change%was significantly different(P<0.05);the concentration of NE was lower at T3(P<0.05).Conclusions The study presents a novel lumbar sympathetic anatomy model using the dorsal approach for subarachnoid-related investigations,which was effectively employed to examine the impact of subarachnoid block anesthesia on lumbar sympathetic nerve activity.

9.Development and validation of a risk prediction model for early postoperative delirium in lung transplant patients

Objective To understand the current situation and influencing factors of delirium in lung transplant patients in the early postoperative period,and to construct and verify a risk prediction model.Methods The convenience sampling method was used to select patients who were admitted to a tertiary general hospital in Jiangsu Province for lung transplantation from June 2023 to November 2024.Lasso regression was used to screen variables,and logistic regression analysis was used to explore the influencing factors of early postoperative delirium in lung transplantation patients,and the risk prediction model was constructed and nomogram was drawn.The area under the working curve(AUC)of the subjects and the Hosmer-Lemeshow test were used to evaluate the discrimination and calibration of the model.Internal validation of the model was performed by repeated sampling 1000 times using Bootstrap method.Results A total of 228 lung transplant patients were included in this study,of which 76 developed delirium in the early postoperative period,with an incidence of 33.33%.Age≥51.5 years,hypertension combined with diabetes mellitus,the Lung Allocation System(LAS)score ≥ 75.63,the duration of mean arterial pressure less than 70 mmHg≥ 12.50 min,intraoperative red blood cell transfusion≥750 ml,and midazolam dosage≥102.50 mg were the independent risk factors for early postoperative delirium in lung transplantation(P<0.05).The AUC of the model was 0.771;the optimal cut-off value was 0.334;the sensitivity and specificity were both 0.724.The results of Hosmer-Lemeshow test showed that x2=5.677,P=0.683;the internal verification showed that the AUC of the model was 0.737,and the absolute error of the actual and predicted values of the calibration curve was 0.023,and the calibration curve was close to the ideal curve.Conclusion Age,hypertension combined with diabetes mellitus,LAS score,duration of mean arterial pressure less than 70 mmHg,amount of intraoperative red blood cell transfusion,and midazolam dosage are the influencing factors for early postoperative delirium in lung transplant patients.The risk prediction model constructed in this study has good predictive performance and can be used as a risk prediction tool for early postoperative delirium in lung transplant patients,helping to timely and accurately identify high-risk groups of delirium,helping to reduce the incidence of early postoperative delirium,and improve patient prognosis.
10.Chinese expert consensus on preoperative localization techniques for pulmonary small nodules in video-assisted thoracoscopic surgery

Preoperative localization of pulmonary small nodules can effectively improve the safety and success rate of video-assisted thoracoscopic surgery(VATS),but there are still controversies regarding the selection of relevant techniques.Interventional Oncology Committee of the Chinese Anti-Cancer Association organized domestic interventional radiology experts engaged in treatment of pulmonary malignancies,based on clinical experience and evidence-based medical evidence to formulate this Chinese expert consensus on preoperative auxiliary localization techniques for VATS of pulmonary small nodules.
