1.Prognostic value of CT combined with DCE-MRI parameters in predicting poor short-term prognosis of patients with colorectal cancer after radical surgery

Objective:To investigate the predictive value of computed tomography (CT) combined with dynamic enhanced magnetic resonance imaging (DCE-MRI) parameters in patients with colorectal cancer after radical surgery.Methods:A total of 180 patients with colorectal cancer admitted to the Second Hospital of Hebei Medical University from Dec. 2021 to Dec. 2023 were retrospectively selected as the study objects. All patients were treated with radical resection of colorectal cancer and followed up for 12 months. They were divided into good prognosis group ( n=129) and poor prognosis group ( n=51) according to whether tumor recurrence and metastasis occurred. All patients were examined by CT and DCE-MRI. Clinical data, CT and DCE-MRI parameters were compared between the two groups. Cox regression was used to analyze the influencing factors of short-term adverse prognosis in patients with colorectal cancer, and ROC curve was drawn to analyze the value of SUV max, K trans, K ep and Ve in single or combined prediction of short-term adverse prognosis in patients with colorectal cancer. Results:SUV max, K trans, K ep and Ve in the poor prognosis group were higher than those in the good prognosis group ( P<0.05) . Cox regression analysis showed that high levels of SUV max ( HR=2.818, 95% CI= 1.808-4.393) , K trans ( HR=516.829, 95% CI=6.745-30603.733) , K ep ( HR=117.756, 95% CI= 4.598-3015.614) and Ve ( HR=9453.000, 95% CI= 63.534-1406482.337) were independent risk factors for short-term adverse prognosis in patients with colorectal cancer ( P<0.05) . The AUC value of SUVmax, K trans, K ep and Ve combined predicted short term adverse prognosis in colorectal cancer patients was higher than that of single detection ( Z=3.126, 4.359, 4.368, 3.987, P<0.05) . Conclusion:CT combined with DCE-MRI parameters have high predictive value for short term poor prognosis of patients with colorectal cancer after radical surgery.
2.Stress analysis of computer aided design/computer aided manufacture post-core materials with different elastic moduli

BACKGROUND:Post and core restoration is a common choice for tooth defects,but the repair effects of various post and core materials are different. OBJECTIVE:To evaluate the stress distribution at the post and core,tooth root,and bonding agent site of post and core models made of different elastic modulus post and core materials using finite element method. METHODS:A three-dimensional root canal treated maxillary central incisor model was built using three-dimensional modeling software,which was restored with a full ceramic crown.The post and core materials in the restoration used nanoceramic resin(elastic modulus=12.8 GPa),composite resin(elastic modulus=16 GPa),hybrid ceramic(elastic modulus=34.7 GPa),glass ceramic(elastic modulus=95 GPa),titanium alloy(elastic modulus=112 GPa),and zirconia(elastic modulus=209.3 GPa).The model was fixed in cortical bone.A 100 N concentrated force of 45° from the long axis of the tooth was applied to 1/3 of the crown and tongue side of the central incisor.The stress distribution of the post and core,dentin,and tooth-root bonding agent in the model was repaired by the maximum principal stress criterion. RESULTS AND CONCLUSION:(1)When the post and core materials with higher elastic modulus was used,the post-core stress in the repair model was more concentrated.When the elastic modulus of the post and core materials(nanoceramic resin and composite resin)was close to dentin,the stress distribution of the post and core was more uniform.The stress distribution of dentin in all restoration models was similar regardless of post and core materials.When the post and core with higher elastic modulus was used,more stress concentration was shown at the post and root bonding agent in the repair model.(2)The maximum stress values at the post and core,tooth root,and the bonding agent site of post and tooth root in the nanoceramic resin model were 31.00,33.21,and 0.51 MPa,respectively.The maximum stress values at the post and core,tooth root,and the bonding agent between the post and tooth root in the composite resin model were 36.84,33.14,and 0.59 MPa,respectively.In the mixed ceramic model,the maximum stress values at the post and core,tooth root,and the bonding agent between the post and tooth root were 64.05,32.83,and 1.00 MPa,respectively.In the glass ceramic model,the maximum stress values at the post and core,tooth root,and the bonding agent between the post and tooth root were 112.30,32.69,and 1.73 MPa,respectively.In the titanium alloy model,the maximum stress values of the post and core,tooth root,and the bonding agent between the post and tooth root were 120.00,32.17,and 1.86 MPa,respectively.In the zirconia model,the maximum stress values of the post and core,tooth root,and the bonding agent between the post and tooth root were 148.80,31.85,and 2.28 MPa,respectively.(3)The higher the elastic modulus of the post and core material,the higher the maximum stress at the post and core during restoration.The elastic modulus of the post and core material had no significant effect on the maximum stress of the dental bonding agent and dentin.
3.Patients waiting for a liver transplantation from the perspective of psychological resilience:a qualitative study

Objective To explore the psychological feelings and coping resources of patients waiting for a liver transplantation,in order to provide references for implementing psychological nursing care.Methods Using purposive sampling,15 patients waiting liver transplantation surgery from June to November 2024 at a tertiary hospital in Qingdao were selected as study subjects.Semi-structured in-depth interviews were conducted,and data were analyzed using interpretative phenomenological analysis to extract themes.Results The psychological feelings of patients waiting for a liver transplantation included 2 main themes with 6 sub-themes,namely,the complex and contradictory psychological feelings of liver transplant patients during the waiting period(expectations for transplanta-tion and future outlook,multiple psychological burdens intertwining,introspection and growth),and mobilizing psychological resilience resources to deal with complex psychological feelings(core resources,internal resources,external resources).Conclusion Clinical medical staff should implement psychological care for patients waiting for a liver transplantation based on the complex and contradictory psychological feelings they experience.Also,they should target the coping resources for psychological feelings to construct coping strategies,thereby comprehensively improving patients' psychological health during the waiting period.The coping strategies include promoting patients' health behaviors,improving their psychological status,and providing multidimensional social support for them.
4.Clinical observation of intradermal needles and moxibustion combined with bladder irrigation in the treatment of post-stroke urinary retention

Objective:To observe the clinical efficacy of intradermal needles and moxibustion combined with bladder irrigation in the treatment of post-stroke urinary retention and its effect on the patient's quality of life and depressive mood.Methods:A total of 80 patients with post-stroke urinary retention were divided into an observation group and a control group by the random number table method,with 45 patients in each group.Both groups received the same basic treatment.In the control group,bladder irrigation was added;in the observation group,intradermal needles and moxibustion were added in addition to the treatment in the control group.After 2 weeks of continuous treatment,the clinical efficacy and the occurrence of adverse events were compared between the two groups,and the changes in the traditional Chinese medicine(TCM)symptom score,urinary function,quality of life,and depressive mood were observed.Results:The total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,the TCM symptom score,residual urine volume,subjective quality of life profile score,and self-rating depression scale score of both groups were significantly reduced(P<0.05),and those of the observation group were significantly lower than those of the control group(P<0.05).Single maximal voiding volume,maximum cystometric capacity,and intravesical pressure were significantly increased in both groups(P<0.05)and were significantly higher in the observation group than in the control group(P<0.05).No needle shock,needle breakage,or allergic reaction happened in the two groups.Conclusion:Based on conventional treatment,the efficacy of intradermal needles and moxibustion combined with bladder irrigation in the treatment of post-stroke urinary retention was better than that of bladder irrigation.It was effective in reducing clinical symptoms,improving urinary function,improving quality of life and depression,and had a good safety profile.

8.Summary of best evidence for non-pharmacological management of postoperative delirium in adult liver transplant recipients

Objective:To summarize the best evidence for non-pharmacological management of postoperative delirium in adult liver transplant recipients, providing reference for postoperative delirium management in liver transplant recipients.Methods:Evidence on non-pharmacological management of postoperative delirium in adult liver transplant recipients was retrieved through computer in databases such as UpToDate, British Medical Journal (BMJ) Best Practice, Embase, PubMed, Cochrane Library, Joanna Briggs Institute Evidence-Based Health Care Center Database, Guidelines International Network, China Biology Medicine disc, China National Knowledge Infrastructure, WanFang Data, and VIP. The search period was from database establishment to January 31, 2023. The types of literature included guidelines, clinical decision-making, best practices, systematic review, summary of evidence, and expert consensus. Two researchers conducted quality evaluation and evidence extraction on the included literature.Results:A total of 10 articles were included, involving 1 clinical decision-making, 4 guidelines, 3 systematic reviews, and 2 expert consensuses. A total of 25 best pieces of evidence for non-pharmacological management of postoperative delirium in adult liver transplant recipients were summarized from four aspects, including screening, evaluation, non-pharmacological prevention, and non-pharmacological treatment.Conclusions:The best evidence on non-pharmacological management of postoperative delirium in adult liver transplant recipients summarized provides a reference for postoperative delirium management in liver transplant recipients.
9.Mediating effect of individual resilience between work stress and work engagement among nurses in Emergency Department

Objective:To explore the relationship between individual resilience, work stress, and work engagement among nurses in Emergency Department.Methods:A questionnaire survey was conducted on 350 nurses from six Class Ⅲ Grade A hospitals in Shandong Province from April to September 2022, using the General Information Questionnaire, the Chinese version of the Stressor Scale for Emergency Nurse, the Chinese version of the Utrecht Work Engagement Scale, and the Medical Staff Composition Resilience Scale. Pearson correlation was used to analyze the correlation between nurse work stress, work engagement, and individual resilience. The structural equation model was constructed using a mediating effect testing program to analyze the relationships between variables.Results:A total of 350 questionnaires were distributed, and 330 valid questionnaires were collected, with an effective response rate of 94.28%. The scores of nurse work engagement, work stress, and individual resilience were (54.26±10.08), (77.36±25.48), and (67.76±12.33), respectively. Work stress was negatively correlated with work engagement and individual resilience (all P<0.01), while individual resilience was positively correlated with work engagement ( P<0.01). Individual resilience was a mediating variable between work stress and work engagement among nurses in Emergency Department, with a mediating effect accounting for 31.38% of the total effect. Conclusions:Most nurses of Emergency Department have moderate levels of work stress, work engagement, and individual resilience. Individual resilience partially mediates the relationship between work stress and work engagement among nurses in Emergency Department. Nursing managers should attach importance to the organizational culture atmosphere and develop corresponding measures to enhance the individual resilience of nurses and encourage them to be energetic and enthusiastic in their work.
10.Sweat secretion and biochemical indexes in Yin-deficient ovariectomized rats

Objective To assess the sweat secretion of Yin-deficient ovariectomized rats and investigate the changes in biochemical indexes.Methods Eighteen SD female rats were randomly divided into a sham operation group,model group,and positive control group of six rats each.The rats in the sham operation group underwent a sham operation,and those in the model group and positive control group underwent bilateral ovariectomy.L-Thyroxine(92 mg/kg)was given once a day for 7 consecutive days starting on the 7th postoperative day to establish a Yin-deficient ovariectomized model.The rats in the positive control group were orally administered Qinggu San Tang(7.3 g/kg)once a day,while those in the sham operation group and model group were orally administered an equal amount of distilled water once a day for a total of 14 days.Sweat secretion from the plantar region of the foot was measured using the Wada-Takagaki reagent coloring method.At the end of the experiment,blood was taken from the abdominal aorta and the tissue of the paw pads was separated.The serum levels cyclic adenosine monophosphate(cAMP),cyclic guanosine monophosphate(cGMP),luteinizing hormone(LH),gonadotropin-releasing hormone(GnRH),and estradiol(E2)were measured by enzyme-linked immunosorbent assay.Western Blot was used to determine the expression levels of M3 R,β2 AR,and aquaporin-5(AQP5)in the paw pad.Results The three main findings of this study were as follows.(1)Compared with the rats in the sham operation group,those in the model group were more irritable and aggressive,and their body weights decreased while their average temperature and sweat secretion significantly increased.(2)Serum cAMP level and cAMP/cGMP ratio increased,the LH and GnRH levels significantly increased,and the E2 level decreased.(3)M3 R expression was down-regulated and β2 AR and AQP5 expression was up-regulated in the paw pads of the rats.After 2 weeks of positive control treatment,the serum cAMP level and cAMP/cGMP ratio significantly decreased and the LH and GnRH levels decreased;however,no statistically significant difference was observed in the serum E2 level.The expression levels of M3 R were increased-regulated in paw pads of the rats,and reduced expression of β2 AR and AQP5.Conclusions Sweat secretion significantly increased in this"combined disease and evidence"model of perimenopausal syndrome kidney yin deficiency established by desiccation combined with thyroxine.The underlying mechanism may be related to the changes in cGMP,cAMP,and key proteins M3 R,β2 AR,and AQP5 in sweat glands that regulate sweat secretion.
