1.A case report of pediatric liver transplantation complicated with median arcuate ligament compression syndrome

Median arcuate ligament compression syndrome (MALS) presents with atypical clinical manifestations. MALS is rarely reported in pediatric patients but is recognized as an independent risk factor for postoperative hepatic artery thrombosis in liver transplant recipients. We report a case of a pediatric liver transplant recipient with hepatolenticular degeneration, cirrhosis, and acute liver failure. Despite undergoing artificial liver support therapy, the patient showed no significant improvement in liver function and subsequently underwent liver transplantation. Intraoperatively, weak arterial pulsation and the absence of a pulsatile waveform in the hepatic artery anastomosis raised suspicion of MALS. The condition was successfully managed by releasing and transecting the median arcuate ligament, along with ligation of the splenic and left gastric arteries. To optimize transplantation outcomes, meticulous preoperative imaging assessment, particularly focusing on characteristic findings in CT angiography, is essential. Additionally, individualized surgical planning and intraoperative adjustments based on ultrasound monitoring and arterial pulsation assessments are critical for ensuring successful transplantation.
2.Application of loose-leaf textbooks in nursing risk education for nursing interns

Objective To evaluate the efficacy of loose-leaf textbooks in nursing risk education for nursing interns.Methods A random number table was used to divide 120 nursing interns in the geriatric oncology department into intervention and control groups,with 60 in each group.The control group received standard clinical nursing instruction,while the intervention group was taught using loose-leaf teaching materials in addition to standard clinical instruction.Occupational risk cognition,pa-tient safety perception,theoretical scores,clinical skills scores,and teaching satisfaction were compared between the two groups.Results The intervention group showed significantly higher scores in occupational risk cognition,patient safety perception,theo-retical achievement,clinical skills achievement,and teaching satisfaction compared to the control group(P<0.05).Conclu-sion Loose-leaf textbooks enhance nursing interns' learning of occupational risks,improve patient safety perception,boost theo-retical and clinical skills performance,and increase teaching satisfaction.
3.Clinical Study on Treatment of Cervical Spondylotic Radiculopathy by Ultrasound-Guided Nerve Block Combined with Arc Edge Needle-Scalpel

Objective To observe the clinical effect of ultrasound-guided nerve block combined with arc edge needle-scalpel in the treatment of cervical spondylotic radiculopathy.Methods 160 cases were randomly divided into 4 groups from A to D,40 cases in each group.Group A was given nerve block,group B was given ultrasound-guided nerve block,group C was given arc edge needle-scalpel,and group D was given ultrasound-guided nerve block combined with arc edge needle-scalpel for 4 weeks.The McGill pain scale,cervical dysfunction index,cervical motion(angle of cervical left and right rotation,anterior flexion,posterior extension and lateral flexion),myoelectric-evoked potentiall(median nerve and ulnar nerve conduction velocity,latency),arterial blood flow velocity(average vertebral artery and basal artery flow velocity,end-diastolic blood flow velocity)were observed.The serum levels of neuron-specific enolase(NSE),β-endorphin(β-EP),lipoprotein-associated phospholipase A2(LpPLA2),6-ketoprostaglandin E1α(6-keto-PGE1α)were measured.The clinical efficacy and safety of each group were observed.Results The response rate in group D was taller than that in group A and group B significantly(χ2=6.605,P=0.013;χ2=4.073,P=0.044).Compared with the other three groups,the McGill pain,NDI,NSE,LpPLA2 and 6-keto-PGE1α in group D decreased significantly(P<0.05),the β-EP increased significantly(P<0.05),and the angles of left and right cervical rotation,anterior flexion,posterior extension and lateral flexion increased significantly(P<0.05),and median nerve and ulnar nerve conduction velocity,vertebral artery and basilar artery average blood flow velocity,end diastolic blood flow velocity increased significantly(P<0.05),and median nerve and ulnar nerve latency decreased significantly(P<0.05).The safety index of group D was higher than that in group A significantly(χ2=5.641,P=0.018).Conclusion Ultrasound-guided nerve block combined with arc edge needle-scalpel can relieve neck and shoulder pain,improve cervical spine function and reduce complications in patients with cervical spondylotic radiculopathy.

Volumes 9 and 10 of Sun Simiao's Supplement to Important Formulas Worth a Thousand Gold Pieces record part of Trea-tise on Cold Damage,which contain Sun Simiao's unique thoughts on six-meridian treatment.Sun Simiao's treatment of Taiyang me-ridian emphasizes the classification of prescriptions and syndromes,advocates the correct treatment methods,and establishes three guidelines;Yangming meridian emphasizes the differentiation of cold and heat syndromes;except Taiyang meridian,all meridians em-phasize the differentiation of outline syndromes.It has made important contributions to the inheritance and development of the six-me-ridian thought of Treatise on Cold Damage in later generations.

9.Analysis of the impact of graft to recipient body weight ratio on the efficacy of living donor liver transplantation in infants with biliary atresia

Objective:To explore the effects of different graft to recipient body weight ratio (GRWR) on the efficacy of living donor liver transplantation (LDLT) in infants with biliary atresia (BA).Methods:Clinical data of 175 BA infants, including 98 males and 77 females, age at transplantation was 5.40 (4.77, 6.33) months, who underwent LDLT at the Department of Hepatobiliary Surgery, Children’s Hospital of Chongqing Medical University from May 2018 to December 2022 were retrospectively analyzed. They were divided into the routine GRWR group (2%≤GRWR≤ 4%, n=121) and high GRWR group (GRWR>4%, n=54). The preoperative general condition, intraoperative condition and postoperative recovery of children in the two groups were compared. Survival analysis was performed by using Kaplan-Meier method, and log-rank test was used to compare survival differences. Results:A total of 16 (9.14%, 16/175) children underwent unplanned surgery after LDLT. There were statistically significant differences in weight at LDLT and graft weight between children in the routine GRWR and high GRWR groups (both P<0.05). There were no statistically significant difference between the two groups in terms of operative time, intraoperative bleeding, and postoperative complication rates (all P>0.05). There were no hepatic artery thrombosis or stenosis occurred in the children of both groups after LDLT. The cumulative survival rates of the children in the routine GRWR group were 97.5% and 95.5% at 1 and 3 years after LDLT, respectively, and 96.3% and 94.2% at 1 and 3 years after LDLT in the high GRWR group, and the difference in cumulative survival rates between the two groups was not statistically significant ( P=0.692). Conclusion:The use of liver grafts with GRWR >4% is also safe and effective for LDLT in infants with BA, which supposes that may not be necessary to reduce the transplanted liver volume in children LDLT with high GRWR.

The"common medicines for various diseases"in the preface of Collective Commentaries on the Classic of Materia Medica has been highly praised by medical practitioners for its unique"disease-medicine"outline and clinical practicality,and has continued to be included and developed in later works on materia medica and prescriptions,laying the foundation for the research and develop-ment of"specialized prescriptions for specialized diseases"and"specialized medicines for specialized diseases".Tao Hongjing used"the root of major diseases"in Shen Nong's Classic of the Materia Medica as the basic framework,referred to the prescriptions and med-icines in Treatise on Cold Damage and Miscellaneous Diseases,and expanded his own and other medical experience to create the"com-mon medicines for various diseases"system,relating and summarizing the representative diseases and main therapeutic effects of medi-cines,and creating a"disease-medicine"outline system.The construction of the"common medicines for various diseases"system has opened up the precedent of"specialized prescriptions for specialized diseases"and"specialized medicines for specialized diseases",providing new prescription ideas for clinical differentiation and treatment.
